Lymphedema in the mastectomy Postoperative Period:An Integrative Literature Review

Aims and objectives: To analyze the evidence available in the literature on the stages during which upper limb lymphedema after mastectomy occurs. Background: Among the adverse effects of breast cancer treatment, lymphedema is the most prevalent. Design: Integrative literature review. methods: The Latin American and Caribbean Literature on Health Sciences (Literatura Latino-Americana e do Caribe em Ciências da Saúde—LILACS), Pubmed, and Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases were used to select the pertinent studies. The identification data for these studies were summarized, and their methodological features and results were extracted. Results: Regarding the time elapsed since surgery, the highest prevalence of lymphedema corresponded to the late postoperative period, and regarding its severity, mild lymphedema was the most prevalent form. The prevalence was also the highest among the women who were subjected to radical mastectomy and radiotherapy. Conclusions: many women who are undergoing treatment for breast cancer use strategies for the prevention and control of lymphedema of the upper limb following mastectomy. Relevance to Clinical Practice: The findings of the present study might inform future studies aiming to assess strategies that can be started in the immediate and late postoperative stages to detect lymphedema early and prevent the increase of its prevalence during the late postoperative period after mastectomy.

mammary health is issue faced by public health nurses and the science follows new ways into this thematic area of learning course wares. The purpose of this study was to construct and to validate a distance course for mammary health promotion for nurses. methodological proposal has been composed of five stages: 1) analysis and planning, 2) modeling, 3) implementation, 4) evaluation and maintenance and 5) distribution. A total of nine experts as research population were consulted who were expert judges of nursing, pedagogy and web designer, three of each specific area, which were included in the study. The variables that had concordance index of 50% were: graphic designer pages favors learning, the user has easy navigation, the learning of the student experience, the environment provides interdisciplinary, content is built on process rather than there is a unique sequence, allows access to the available environment and holds a discussion on the subject. The distance learning course is intended to broaden and to enrich the living spaces and construct their own knowledge, favors interactivity, communication, autonomy and cooperation between the participants and provides access to educational technologies.

Unraveling oral psoriasis and its relationship with geographic tongue: A literature review

Differentiating between oral psoriasis and geographic tongue is difficult and controversial because some patients with geographic tongue do not necessarily have psoriasis. Furthermore, the number of clinical studies, reporting histopathological and genetic evidence for the definitive diagnosis of oral psoriasis, is limited. The aim of this literature review was to obtain data for supporting the diagnosis of oral psoriasis with particular emphasis on the relationship between psoriasis and geographic tongue. Based on the current data, it can be concluded that geographic tongue is the most common oral lesion in psoriasis, and histopathological, immunohistochemical, and genetic similarities have been observed between the two diseases. This review also emphasizes the importance of conducting oral examinations in patients with psoriasis and skin examinations in patients with geographic tongue.

Renal Function after major Uro-oncologic Surgery and Dexmedetomidine Infusion

objective: Acute kidney injury in major surgery is associated with increased postoperative mortality. This study aimed to evaluate renal function after major urologic surgery and intraoperative dexmedetomidine infusion. methods: Thirty oncologic patients with normal renal function scheduled for prostatectomy or nephrectomy, anesthetized with combined epidural and general anesthesia, were randomized to receive either intraoperative blind infusion of dexmedetomidine (Dexmedetomidine Group, n = 15, 0.5 μg/kg load dose plus 0.7 μg/kg/h) or 0.9% saline (Control Group, n = 15) until the end of surgery. Intraoperative and cumulative 24-hour diuresis, serum creatinine (SCr), calculated creatinine clearance (ClCr) and serum cystatin C (SCys) at postoperative days 1, 2 and 3 and 2 weeks after surgery were evaluated. Results: mean ± standard deviation values for intraoperative diuresis in Dexmedetomidine and Control Groups were 566 ± 396 mL and 298 ± 153 mL, respectively (p = 0.014). Cumulative 24-hour diuresis in Dexmedetomidine and Control Groups was 1947 ± 266 mL and 1748 ± 237 mL, respectively (p = 0.91). mean values of SCr, ClCr and SCys were not significantly different from their baseline values in both groups and no significant differences were seen between groups at any moment for two weeks (p > 0.05). Conclusion: According to the doses used in this study, despite an intraoperative increase in diuresis, intraoperative infusion of dexmedetomidine did not influence renal performance up to two weeks after major uro-oncologic surgery, as evaluated by SCr, ClCr and SCys.
