Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the ***: We searched mEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, mEDLINE database and Pubmed platform were screened again using the same terms, from June 2019 until November ***: Of 556 initial articles, seven were included in this review. most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among ***: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Time-Series Embeddings from Language models:A Tool for Wind Direction Nowcasting

Wind direction nowcasting is crucial in various sectors,particularly for ensuring aviation operations and *** this context,the TELmo(Time-series Embeddings from Language models)model,a sophisticated deep learning architecture,has been introduced in this work for enhanced wind-direction *** by using three years of data from multiple stations in the complex terrain of an international airport,TELmo incorporates the horizontal u(east-west)and v(north-south)wind components to significantly reduce forecasting *** a day with high wind direction variability,TELmo achieved mean absolute error values of 5.66 for 2-min,10.59 for 10-min,and 14.79 for 20-min forecasts,processed within a swift 9-ms/step *** degree-based analysis,in comparison,yielded lower performance,emphasizing the effectiveness of the u and v *** contrast,a Vanilla neural network,representing a shallow-learning approach,underperformed in all analyses,highlighting the superiority of deep learning methodologies in wind direction *** is an efficient model,capable of accurately forecasting wind direction for air traffic operations,with an error less than 20°in 97.49%of the predictions,aligning with recommended international *** model design enables its applicability across various geographical locations,making it a versatile tool in global aviation meteorology.

Twisted and Absolute Degrees of maps into the Projective Plane

For each based map f : X → RP2from a closed surface into the real projective plane, we compute its absolute and twisted degrees and describe the action of the fundamental group of RP2over the based homotopy class of f. We emphasize the finding that for any nonorientable closed surface,there exists only one based homotopy class of maps from it into RP2whose maps have twisted degree zero and absolute degree nonzero – which shows that, unlike the absolute degree, the twisted degree is not able to detect the strong surjectivity in this setting. In all the other scenarios, the absolute degree of each map is either equal to the twisted degree or its absolute value – and so the twisted degree detects strong surjectivity.

Colorectal cancer(CRC)is one of the most prevalent malignancies worldwide,being the third most commonly diagnosed malignancy and the second leading cause of cancer-related deaths *** the progress in screening,early diagnosis,and treatment,approximately 20%-25%of CRC patients still present with metastatic disease at the time of their initial ***,the burden of disease is still expected to increase,especially in individuals younger than 50 years old,among whom early-onset CRC incidence has been *** and early detection are pivotal to improve CRC-related *** is well established that CRC screening not only reduces incidence,but also decreases deaths from *** screening strategies have proven effective in decreasing both CRC incidence and mortality,though variations in efficacy have been reported across the ***,uncertainties persist regarding the optimal screening method,age intervals and ***,adherence to CRC screening remains globally *** recent years,emerging technologies,notably artificial intelligence,and non-invasive biomarkers,have been developed to overcome these ***,controversy exists over the actual impact of some of the new discoveries on CRC-related outcomes and how to effectively integrate them into daily *** this review,we aim to cover the current evidence surrounding CRC *** will further critically assess novel approaches under investigation,in an effort to differentiate promising inno-vations from mere novelties.

Increasing dietary fiber intake for type 2 diabetes mellitus management: A systematic review

BACKGROUND Type 2 diabetes is a chronic,non-communicable disease with a substantial global impact,affecting a significant number of *** etiology is closely tied to imbalanced dietary practices and sedentary ***,increasing die-tary fiber(DF)intake has consistently demonstrated health benefits in numerous studies,including improvements in glycemic control and weight *** To investigate the efficacy of DF interventions in the management of type 2 diabetes mellitus(T2Dm).mETHODS A systematic literature review was conducted to explore the association between DF intake and the management of *** the inclusion and exclusion criteria,a total of 26 studies were included in this *** The main strategies implied to increased DF intake were:High DF diet plus acarbose(2 studies);DF supplements(14 studies);and high DF diets(10 studies).Overall,most studies indicated that increased DF intake resulted in im-provements in glycemic control and weight management in T2Dm *** DF represents a valuable strategy in the treatment of type 2 diabetes,improving health *** intake offers the potential to improve quality of life and reduce complications and mortality associated with ***,through supplements or enriched foods,DF contributes significantly to the control of several markers such as HbA1c,blood glucose,triglycerides,low-density lipoprotein,and body weight.

Histological Assessment of Bone Regeneration in the maxilla with Homologous Bone Graft:A Feasible Option for maxillary Bone Reconstruction

Bone biomaterials have been increasingly used to reconstruct maxillary atrophic ***,the aim of this study was to evaluate bone reconstruction in the maxilla using a homologous cortico-cancellous FFB(lyophilized)graft and verify its *** individuals were included from 2014 to *** first surgery was performed to install homologous bone blocks in the *** period of the second intervention varied between 5 months and 15 days to 11 months(≈7.93 months).The biopsies were taken from the central region of the matured graft during the surgery for implant *** patients presented clinical and radiographic conditions for the installation of dental *** was a 100%of survival *** histological assessment showed that the homologous block bone graft was an osteoconductive biomaterial,with connective tissue present,and newly formed bone juxtaposed on its *** were bone trabeculae with osteocytes and active osteoblasts with connective tissue in the mineralization process;the remodeling process can be found through the reverse lines.A limited focus of necrosis with fibrosis was detected,with small resorption and areas of inflammatory infiltrate,but without clinical *** homologous block bone graft can be considered a feasible option to substitute the autogenous bone graft(gold standard),with predictable clinical and favorable histological *** patients had a shorter surgical period,low morbidity,and an unlimited amount of biomaterial available at an accessible cost.
