Cecum Dieulafoy＇s Lesion Presenting with Recurrent Lower Gastrointestinal Bleeding： Case Report

Introduction： Gastrointestinal Dieulafoy＇s lesion is a rare entity, of unknown etiology, and corresponds to an arterial malformation at the submucosal space that can be a source of life-threatening hemorrhage. We report a case of a Cecum Dieulafoy＇s bleeding lesion that was managed endoscopically with a favorable outcome. Case report： Female, 70-year-old, diagnosed with type 2 diabetes, hypothyroidism, and chronic heart failure associated with rheumatic mitral stenosis, submitted to biological valve replacement in 2006 and with permanent atrial fibrillation using vitamin K antagonist anticoagulant for thrombosis prophylaxis. Her background includes a stroke in 2004 without any permanent disability. In 2016, the patient experienced voluptuous and painless lower gastrointestinal bleeding with severe acute anemia, requiring hospital admission, fluid resuscitation and blood transfusion. Urgent colonoscopy revealed a small reddish vascular malformation at the cecum with oozing active bleeding, about 3 mm in size. Initially argon plasma coagulation was performed with satisfactory and immediate hemostasis. One week later, she recurred with hematochezia. The lesion at the cecum was reassessed and it was possible to notice a large caliber vessel surrounded by a normal appearance mucosa, compatible with Dieulafoy＇s lesion and it was treated with an endoscopic clip placement with a good long-term response. Conclusion： Endoscopy is the method of choice for diagnosis of Dieulafuy＇s lesion and may provide efficient treatment with mechanical hemostasis such as endoclip placement with a high success rate.

Study of Factors That Influence the Sustainability of Agricultural Development in the Bobonaro municipality, Timor-Leste

The aim of the study is to assess the factors that influence the sustainability of agricultural development. The study was carried out in the municipality of Bobonaro for about four months, from July to October 2021. Slovin’s method was used to determine the sample size, with a desired percentage of 10% of the total population. Thus, 154 producers were interviewed as respondents and key persons in the area of agriculture. The variables interviewed are the main factors of agricultural productivity and the factors that affect sustainable agricultural development. The results of the descriptive statistical analysis of the data showed that about 94.87% of the respondents’ productive area is in operation, with an average of 1.84 ha per respondent and the abandoned area around 0.11 ha per respondent. The average value of rice production is 2.38 ton per ha and corn 1.07 ton per ha. About 87% of producers raised the main types of animals such as cattle, pigs, goats with an average density of 2 to 5 animals per establishment. About 61% of producers have access to the market at a normal price for agricultural products. The factors that hinder less successful agricultural development, such as: those of nature with the greatest impact are climate changes, which influence changes in cultivation sessions and in the characteristics of productive soils, availability of water for irrigation and animal feed. On the socioeconomic side, it includes the willingness or interest of young people who want to work as farmers, less investment and the impact of the Covid-19 pandemic. It is concluded that natural and socioeconomic impacts can reduce agricultural productivity, so that this will be a major challenge for the development of the agricultural sector in the future.

Carbon, Water, Ecological Footprints, Energy and Nutritional Densities of Omnivore and Vegan Culinary Preparations

This study aimed to evaluate energy and nutritional densities, water, carbon and ecological footprints, cost, of omnivorous and vegan main courses served in a university restaurant in the city of Rio de Janeiro. A cross-sectional, analytical study was conducted with 40 main dish-type preparations, 20 of which are omnivorous and 20 vegans, served at lunch, to assess energy and nutritional densities, carbon, water and ecological footprints and cost of preparations. We propose a healthy and sustainable preparation index (HSPI) to evaluate from the list, the best preparation options considering the nutritional quality combined with the impact that the food causes on the environment. Preparations with the highest HSPI were considered the best options because they have a good relation between the nutritional profile and the environmental impact. Results: Regarding energy (ED), nutritional (ND) densities and water, carbon and ecological footprints, omnivorous preparations presented much higher values when compared to the vegan ones. The omnivorous preparations had the highest average cost (R$ 3.44). Regarding the HSPI, vegan preparations showed better rates than omnivorous preparations. Food services should promote healthy and sustainable choices by offering menus with low energy density preparations, high nutritional density, and low environmental impact, considering local realities and customer needs. Conclusion: This study was able to evaluate the best preparation options, considering the nutritional profile and the food impact on the environment, using health and sustainable indicators. Obtaining indicators of preparations regarding healthiness and sustainability, in practice, translates environmental aspects in menu planning, which contribute to changes in food consumption patterns in food services, in addition to contributing to the reduction of the environmental impact. Visible Losses to the mechanical Harvesting of Ratoon Sugarcane Using the Harvester John Deere 3520

The aim of this study was to evaluate the visible loss of sugar cane and the damage to the knuckles, using the John Deere 3520 harvester on three different travel speeds (3.0;4.0 and 5.0 km·h-1), in DIC with five repetitions in Campos dos Goytacazes, Rio de Janeiro/Brazil. Each treatment was composed by six lines of harvested cane stumps, with a length of 290 m each. In these six lines, the remains of sugar cane left in the field were collected by placing the sampling frame in two central lines every 50 m, and separating 40 m of edge. The sampling area was surrounded by 2 m wide and 10 m long, totaling 20 m2. To calculate the damage to stumps, a visual methodology by [1], was used, which classifies the damage grade, ranging from 1 to 4. Fifty stumps were assessed randomly for each speed, in a sampling area of 1800 m2. The data were submitted to ANOVA and Tukey test at 5%, in order to compare the effect of different speeds on the losses and damages. There was no significant difference regarding the loss or damage by comparing the different speeds. Therefore, it is more advantageous to use the speed of 5.0 km·h-1, harvesting more in less time and causing the same level of damage.

Allelopathic Effect of the Coffee Residue in Emergency and Urochloa brizantha Growth

Weed management is important for the increase of agricultural crops, since they have a negative influence on yield and harvest quality. To optimize the management it is possible to use the coffee infusion residues, which has caffeine contents, a substance that has an allelopathic effect to some plants. Thus, the goal was to evaluate the allelopathic effect of coffee residues in the emergence and growth of Urochloa brizantha under product doses, gathering and application seasons in pre- and post-emergence. The experiment was carried out at UFLA, in a greenhouse with potting, with Urochloa brizantha seeds, cultivar Piata, in DBC with three replicates in the factorial (3 × 4 × 2): time of gathering (A, B, C);concentration of ground (0, 20, 50 and 100 (% W/V)) and pre- and post-emergence application. The height, fresh and dry weight, Emergence Speed Index—ESI and phytotoxicity were evaluated. It was verified the significance for pre-emergence treatments, reducing plant height, increasing phytotoxicity and reducing fresh weight. For the ESI, it was identified that only for gathering season obtained a significant result. In the phytotoxicity evaluations, it obtained interaction for the season of application with residue gathering season. For product doses and applications in pre- and post-emergence interaction was observed for phytotoxicity. It is concluded that the residue has allelopathic effect reducing growth and increasing phytotoxicity up to a maximum dose of 55.83 and 86.76 for pre- and post-emergence applications of Urochloa brizantha.

Partial purification,immobilization and preliminary biochemical characterization of lipases from Rhizomucor pusillus

Lipases have important applications in biotechnological processes, motivating us to produce, purify, immobilize and perform a biochemical characterization of the lipase from Rhizomucor pusillus. The fungus was cultivated by solid state fermentation producing lipolytic activity of about 0.5 U/mL(4U/g). A partial purification by gel filtration chromatography in Se-phacryl S-100 allowed obtaining a yield of about 85% and a purification factor of 5.7. Our results revealed that the purified enzyme is very stable with some significant differences in its properties when compared to crude extract. The crude enzyme extract has an optimum pH and temperature of 7.5 ° C and 40 ° C, respectively. After purification, a shift of the optimum pH from 7 to 8 was observed, as well as a rise in optimumtemperature to 60 ° C and an increase in stability. The enzyme was immobilized on CNBr-Agarose and Octyl-Agarose supports, having the highest immobilization yield of 94% in the second resin. The major advantage of immobilization in hydrophobic media such as Octyl is in its hyper activation, which in this case was over 200%, a very interesting finding. Another advantage of this type of immobilization is the possibility of using the derivatives in biotechnological applications, such as in oil enriched with omega-3 as the results obtained in this study display the hydrolysis of 40% EPA and 7% DHA from sardine oil, promising results compared to the literature.

Conservative Surgery for Subungual melanoma In Situ Using matriderm^(■)

Background: Standard treatment for subungual melanoma is wide local excision including digit amputation in order to obtain safety margins. Level of amputation has been discussed in order to achieve appropriate oncological results along with preservation of limb function wherein local excision of subungual melanoma in situ without amputation is an option. methods: We report two cases of subungual melanoma in situ treated by using a conservative approach. Both cases were treated with local excision with the resected tumor defect covered with an artificial dermis, matriderm?. One case was of a 65-year-old male with a left thumb subungual melanoma in situ and another of a 41-year-old female with the same type of melanoma in the left hallux. Full-thickness skin graft was used for reconstruction after 7 weeks from the first surgery in the latter, and in the former case re-epithelization occurred within 90 days after surgery. To date, no clinical signs of recurrence have been found, and finger/toe functions were completely preserved with a minimized scar and little graft bed sensitivity. Follow-up time is two years and ten months for the left thumb case, and one year and six months for the left hallux subungual melanoma in situ. Conclusion: We present two cases of subungual melanoma in situ treated with digit sparing surgery including excision of the periosteum followed by the use of an artificial dermal template to guarantee coverage of the tumor bed defect, providing good functional and cosmetic outcome.
