Multitarget stool DNA tests increases colorectal cancer screening among previously noncompliant Medicare patients

AIM To determine the uptake of noninvasive multitarget stool DNA (mt-s DNA) in a cohort of colorectal cancer(CRC) screening non-compliant average-risk Medicare *** This cross sectional primary care office-based study examined mt-s DNA uptake in routine clinical practice among 393 colorectal cancer screening non-compliant Medicare patients ages 50-85 ordered by 77 physicians in a multispecialty group practice (USMD Physician Services, Dallas, TX) from October, 2014-September, 2015. Investigators performed a Health Insurance Portability and Accountability Act compliant retrospective review of electronic health records to identify mt-s DNA use in patients who were either > 10 years since last colonoscopy and/or > 1 year since last fecal occult blood test. Test positive patients were advised to get diagnostic colonoscopy and thereafter patients were characterized by the most clinically significant lesion documented on histopathology of biopsies or excisional tissue. Descriptive statistics were employed. Key outcome measures included mt-s DNA compliance and diagnostic colonoscopy compliance on positive *** Over 12 mo, 77 providers ordered 393 mt-sD NA studies with 347 completed (88.3% compliance). Patient mean age was 69.8 (50-85) and patients were 64% female. Mt-sD NA was negative in 85.3%(296/347) and positive in 14.7%(51/347). Follow-up colonoscopy was performed in 49 positive patients (96.1% colonoscopy compliance) with two patients lost to follow up. Index findings included: colon cancer(4/49, 8.2%), advanced adenomas (21/49, 42.9%), non-advanced adenomas(15/49, 30.6%), and negative results (9/49, 18.4%). The positive predictive value for advanced colorectal lesions was 51.0% and for any colorectal neoplasia was 81.6%. Layered metal dichalcogenides (LMDs) neuromorphic memristor devices offer a promising alternative toconventional von Neumann architectures, addressing speedand energy efficiency constraints. However, challenges remainin controlling resistive switching and operating voltage incrystalline LMD memristors due to environmental stabilization issues, which hinder neural network hardware development. Herein, we introduce an optimization method formemristor operation by controlling oxidation through ozonetreatment, creating a SnO_(x)/SnS_(2) resistive layer. These optimized memristors demonstrate low switching voltages (~1 V),rapid switching speeds (~20 ns), high switching ratios (10^(2)),and the ability to emulate synaptic weight plasticity. Crosssectional transmission electron microscopy and energy-dispersive X-ray spectroscopy identified defects and Ti conductive filaments in the resistive switching layer, contributingto uniform switching and minimized operating variation. Thedevice achieved 90% accuracy in MNIST handwritten recognition, and hardware-based image convolution was successfully implemented, showcasing the potential of SnO_(x)/SnS_(2)memristors for neuromorphic applications.

High efficiency synthesis of HKUST-1 under mild conditions with high BET surface area and CO2 uptake capacity

This study focuses on the development of a hydrothermal method for the rapid synthesis of good quality copper benzene-1,3,5-tricarboxylate（referred to as HKUST-1）with high yield under mild preparation conditions to address the issues associated with reported *** synthesis conditions and activation methods were studied to understand their influence on the properties of *** was found that mixing the precursors at50°C for 3 h followed by activation via methanol refluxing led to the formation of a product with the highest BET specific surface area of 1615 m2/g and a high yield of 84.1%.The XRD and SEM data illustrated that the product was highly *** sample was also tested on its capacity in *** results showed strong correlation between surface area of the sample and its CO2uptake at 1 bar and 27°*** HKUST-1 prepared in this study demonstrated a high CO2uptake capacity of 4.2 mmol/*** is therefore concluded that this novel and efficient method can be used in the rapid preparation of HKUST-1 with high surface area and CO2uptake capacity.

Quantification of intermuscular and intramuscular adipose tissue using magnetic resonance imaging after neurodegenerative disorders

Ectopic adiposity has gained considerable attention because of its tight association with metabolic and cardiovascular health in persons with spinal cord injury（SCI）. Ectopic adiposity is characterized by the storage of adipose tissue in non-subcutaneous sites. Magnetic resonance imaging（MRI） has proven to be an effective tool in quantifying ectopic adiposity and provides the opportunity to measure different adipose depots including intermuscular adipose tissue（IMAT） and intramuscular adipose tissue（Intra MAT） or intramuscular fat（IMF）. It is highly important to distinguish and clearly define these compartments, because controversy still exists on how to accurately quantify these adipose depots. Investigators have relied on separating muscle from fat pixels based on their characteristic signal intensities. A common technique is plotting a threshold histogram that clearly separates between muscle and fat peaks. The cut-offs to separate between muscle and fat peaks are still not clearly defined and different cut-offs have been identified. This review will outline and compare the Midpoint and Otsu techniques, two methods used to determine the threshold between muscle and fat pixels on T1 weighted MRI. The process of water/fat segmentation using the Dixon method will also be outlined. We are hopeful that this review will trigger more research towards accurately quantifying ectopic adiposity due to its high relevance to cardiometabolic health after SCI.

Management of pembrolizumab-induced steroid refractory mucositis with infliximab:A case report

BACKGROUND Pembrolizumab is an anti-programmed death receptor 1(PD-1)that was shown to have a tolerable safety profile with 17%of grade 3-4 drug-related adverse events,notable response rate of 16%with median duration of response of 8 mo,and median overall survival of 8 *** mucositis is a very rare complication with only two cases of grade 4 mucositis reported,and both cases had good response to intravenous methylprednisolone and subsequent oral prednisone *** report the first case of pembrolizumab-induced severe mucositis that was refractory to steroid *** SUMMARY An 80-year-old woman with a past medical history of recurrent right cheek nodular melanoma status post resection and new right lung metastatic melanoma on immunotherapy presented with dysphagia and odynophagia for 2 *** initially received 2 doses of ipilimumab 1 year ago with good outcome,but treatment was discontinued after developing severe diarrhea and *** was then initiated 4 mo after disease *** improvement was noted after 3 ***,after 6 cycles of pembrolizumab,patient developed odynophagia and *** of symptoms was noted after discontinuation of pembrolizumab and initiation of steroids.3 mo later,patient developed pharyngeal swelling with hoarseness and new oxygen requirement due to impending airway obstruction while being on prednisone tapering regimen,finally ended up with intubation and *** analysis of left laryngeal and epiglottis tissue showed granulation tissue with acute on chronic inflammation,negative for malignancy and *** achieved marked improvement after 2 doses of infliximab of 5 mg/kg every 2 wk while continuing on prednisone tapering *** We report the first case of pembrolizumab-induced grade 4 mucositis that had limited recovery with prolonged steroid course but had rapid response with addition of *** patient had recurrent mucositis symp

Validation of the Farsi version of the Death Obsession Scale among nurses

Objectives:Obsession is one of the important aspects in death *** objective of this study was to estimate the reliability,validity,and factorial structure of the Farsi version of the Death Obsession Scale(DOS).Methods:A convenience sample of 106 Iranian nurses from two hospitals at Tehran city,Iran was *** completed the DOS,Death Concern Scale,Collett-lester Fear of Death Scale,Death Anxiety Scale,Reasons for Death Fear Scale,and Death Depression ***:Cronbach's a for the DOS was 0.95,and 2-week test-retest reliability was *** DOS correlated 0.48,0.46,0.47,0.39 and 0.44 with the last mentioned scales,respectively(P<0.01),indicating good construct and criterion-related *** components analysis of the DOS identified three factors accounted for 74.82%of the *** 1 labeled“Death rumination”(34.78%of the variance),Factor 2 labeled“Death dominance”(29.65%of the variance),and Factor 3 labeled“Death idea repetition”(10.38%of the variance).Conclusions:The DOS has good validity and reliability,and it could be recommended for use in clinical and research settings to assess the death obsession in Iranian nurses.
