Benefits of the use of blood conservation in scoliosis surgery

AIM To investigate whether autologous blood transfusion(ABT) drains and intra-operative cell salvage reduced donor blood transfusion requirements during scoliosis *** Retrospective data collection on transfusion requirements of patients undergoing scoliosis surgery is between January 2006 and March 2010. There were three distinct phases of transfusion practice over this time: Group A received "traditional treatment" with allogeneic red cell transfusion(ARCT) in response to an intra- or postoperative anaemia(Hb < 8 g/d L or a symptomatic anaemia); Group B received intra-operative cell salvage in addition to "traditional treatment". In group C,ABT wound drains were used together with both intra-operative cell salvage and "traditional treatment".RESULTS Data from 97 procedures on 77 patients,there was no difference in mean preoperative haemoglobin levels between the groups(A: 13.1 g/d L; B: 13.49 g/d L; C: 13.66 g/d L). Allogeneic red cell transfusion was required for 22 of the 37 procedures(59%) in group A,17 of 30(57%) in group B and 16 of 30(53%) in group C. There was an overall 6% reduction in the proportion of patients requiring an ARCT between groups A and C but this was not statistically significant(χ2 = 0.398). Patientsin group C received fewer units(mean 2.19) than group B(mean 2.94)(P = 0.984) and significantly fewer than those in group A(mean 3.82)(P = 0.0322). Mean length of inpatient stay was lower in group C(8.65 d) than in groups B(12.83) or A(12.62).CONCLUSION When used alongside measures to minimise blood loss during surgery,ABT drains and intra-operative cell salvage leads to a reduced need for donor blood transfusion in patients undergoing scoliosis surgery.

Awareness of the adverse impact of air pollution on attention-related performance such as learning and driving is rapidly ***,there is still little known about the underlying neurocognitive *** an adapted dot-probe task paradigm and event-related potential(ERP)technique,we investigated how visual stimuli of air pollution influence the attentional allocation *** were required to make responses to the onset of a target presented at the left or right visual *** probable location of the target was forewarned by a cue(pollution or clean air images),appearing at either the target location(attention-holding trials)or the opposite location(attention-shifting trials).Behavioral measures showed that when cued by pollution images,subjects had higher response accuracy in attention-shifting *** analysis results revealed that after the cue onset,pollution images evoked lower N300 amplitudes,indicating less attentioncapturing effects of dirty *** the target onset,pollution cues were correlated with the higher P300 amplitudes in attention-holding trials but lower amplitudes in attention-shifting *** indicates that after visual exposure to air pollution,people need more neurocognitive resources to maintain attention but less effort to shift attention *** findings provide the first neuroscientific evidence for the distracting effect of air *** conclude with several practical implications and suggest the ERP technique as a promising tool to understand human responses to environmental stressors.

Association between baseline levels of muscular strength and risk of stroke in later life:The Cooper Center Longitudinal Study

Background:Muscular strength is an important component of physical *** evaluated the relationship between baseline muscular strength and risk of stroke among adults who were aged≥65 years during ***:We included 7627 healthy adults(mean age=43.9 years,86.0%male)underwent a baseline physical examination between 1980 and *** strength was determined by 1-repetition maximum measures for bench press and leg press and categorized into age-and sex-specific tertiles for each *** fitness(CRF)was assessed via a maximal treadmill exercise *** enrolled in fee-for-service Medicare from 1999 to 2019 were included in the *** between baseline strength and stroke outcomes were estimated using a modified Cox proportional hazards *** a secondary analysis,we examined stroke risk by categories of CRF where Quintile 1=low,Quintiles 2-3=moderate,and Quintiles 4-5=high CRF based on age and ***:After 70,072 person-years of Medicare follow-up,there were 1211 earliest indications of incident *** multivariable analyses,the hazard ratio(95%confidence interval(95%CI))for stroke across bench press categories were 1.0(referent),0.96(0.83-1.11),and 0.89(0.77-1.04),respectively(p trend=0.14).The trend across categories of leg press was also non-significant(p trend=0.79).Adjusted hazard ratio(95%CI)for stroke across ordered CRF categories were 1.0(referent),0.90(0.71-1.13),and 0.72(0.57-0.92)(p trend<0.01).Conclusion:While meeting public health guidelines for muscular strengthening activities is likely to improve muscular strength as well as many health outcomes in older adults,performing such activities may not be helpful in preventing ***,meeting guidelines for aerobic activity is likely to improve CRF and lower stroke risk.

Glucose measurement via Raman spectroscopy of graphene:Principles and operation

The sensitivity of graphene’s Raman characteristics to external perturbations can be exploited for sensing *** apply this principle,Raman spectroscopy of graphene in contact with a chemical/biological analyte is recorded to unravel a correlation between analyte’s physical/chemical/biological properties and electronic properties(Fermi energy and dopants level)of *** an example,we demonstrate the principle and operation of glucose measurement using graphene’s Raman spectroscopy,utilizing both affinity and enzymatic detection *** the affinity detection,we monitor Raman spectrum of graphene in contact with a glucose containing sample to quantify the attachment of glucose onto 1-pyrene boronic acid(PBA)functionalized *** is revealed that glucose covalent bond to PBA induces n-type doping in graphene and increases its Fermi level following the Hill–Langmuir *** the enzymatic detection,we show that graphene’s Fermi energy shifts due to the biocatalytic oxidation of glucose by immobilized glucose oxidase(GOx)*** a result,graphene becomes p-doped with increasing glucose *** attribute this sensitivity to a re-distribution of ionic charges within the electric double layer(EDL)of graphene upon introducing the glucose *** model explains both our affinity and enzymatic glucose detection *** glucose detection,our graphene Raman spectroscopy-based sensor(GRS)may extend its applications to a broader range of remote and non-invasive detection without the need to employ electronics such as field-effect transistors.

Predicting the Thermal Behavior in Functional Textile Fibers Having Embedded Electronics

In this paper,both steady-state and transient thermal simulations were performed on functional fibers having an embedded electronic chip acting as a heat *** were conducted for a range of different fiber materials and arbitrary fiber cross-sectional *** show that under steady-state heating conditions,the thermal response for any arbitrary fiber shape and fiber material system was convection dominated regardless of the effective thermal conductivity of the fiber,and that the corresponding temperature rise within the fiber can be predicted analytically allowing for the maximum temperature to be estimated for any known heat load and fiber *** the case of transient heating,we show that for pulsed power operation of the embedded electronic device,the maximum temperature reached in the fiber is always greater than the maximum temperature of the equivalent steady-state average ***,high peak powers can be safely achieved if the power-on pulse time and duty cycle are selected to limit the maximum temperature reached in the *** on the results from the transient simulations,a set of criteria was developed to determine whether the operating conditions would be:(1)allowable for the fiber system,thus requiring no transient simulations,(2)requiring a transient simulation to verify that the maximum temperature is acceptable,and(3)the operating conditions are too severe and device operation at these conditions are not practical.

Context: Antiretroviral therapies improve the prognosis of NeuroAIDS contrasting with a high frequency of Minor Neurocognitive Disorders (MND) even in aviremic subjects. Objective: The objective of this study is to present the neurological and cognitive profile of People Living with HIV (PLHIV) initiating antiretroviral treatment in Kinshasa in the era of Dolutegravir (DTG). Methods: This is a multicenter, cross-sectional study with a descriptive aim carried out in 16 HIV Outpatient Treatment Centers (OTC) in Kinshasa from October 4, 2021 to February 15, 2022. The International HIV Dementia Scale (IHDS) correlated with the Activities of Daily Living (IADL) scale facilitated the categorization of NeuroCognitive Disorders (NCD) of PLHIV evaluated after carrying out a summary neurological examination. Results: Of the 96 patients recruited, 56.3% were women with a sex ratio of 0.68. The average age was 40.1 ± 12.1 years. The secondary education level was the majority at 64.6%. Malaria (44.8%) and tuberculosis (32.3%) were more common as opportunistic infections. They were alcoholics in (30.2%). Their history was heart disease (15.6%), high blood pressure (18.8%);drug abuse (10.4%). The IHDS score was light in 55.2% of cases. The correlation between IHDS/IADL watches asymptomatic neurocognitive impairments (ANI) in 77.1%, almost all of subjects are found with normal overall functioning (94.8%) and a disturbed neurological examination in 53.1% of cases with a predominance of motor impairments in 79.1%. Conclusion: In view of these results, early and systematic screening of NCD and associated factors remains necessary in our context. .

Dispersal and its evolution play a key role for population persistence in fragmented landscapes where habitat loss and fragmentation increase the cost of between-habitat *** such contexts,it is important to know how variation in dispersal and other traits is structured,and whether responses to landscape fragmentation are aligned with underlying dispersal-trait correlations,or dispersal ***,therefore,studied trait variation in Erigone longipalpis,a European spider species specialist of(often patchy)salt *** collected spiders in two salt-marsh landscapes differing in habitat *** then reared lab-born spiders for two generations in controlled conditions,and measured dispersal and its association with various key *** longipalpis population densities were lower in the more fragmented *** this,we found no evidence of differences in dispersal,or any other trait we studied,between the two *** a dispersal syndrome was present at the among-individual level(dispersers were more fecund and faster growing,among others),there was no indication it was genetically driven:among-family differences in dispersal were not correlated with differences in other ***,we showed that the observed phenotypic covariations were mostly due to within-family *** hypothesize that the dispersal syndrome is the result of asymmetric food access among siblings,leading to variation in development rates and carrying over to adult *** results show we need to better understand the sources of dispersal variation and syndromes,especially when dispersal may evolve rapidly in response to environmental change.
