Safe and effective sofosbuvir-based therapy in patients with mental health disease on hepatitis C virus treatment

AIMTo study impact of baseline mental health disease on hepatitis C virus (HCV) treatment; and Beck’s Depression Inventory (BDI) changes with sofosbuvir- and interferon-based therapy. METHODSThis is a retrospective cohort study of participants from 5 studies enrolled from single center trials conducted at the Clinical Research Center of the National Institutes of Health, Bethesda, MD, United States. All participants were adults with chronic HCV genotype 1 infection and naïve to HCV therapy. Two of the studies included HCV mono-infected participants only (SPARE, SYNERGY-A), and 3 included human immunodeficiency virus (HIV)/HCV co-infected participants only (ERADICATE, PFINPK, and ALBIN). Patients were treated for HCV with 3 different regimens: Sofosbuvir and ribavirin in the SPARE trial, ledipasvir and sofosbuvir in SYNERGY-A and ERADICATE trials, and pegylated interferon (IFN) and ribavirin for 48 wk in the PIFNPK and ALBIN trials. Participants with baseline mental health disease (MHD) were identified (defined as either a DSM IV diagnosis of major depression, bipolar disorder, schizophrenia, generalized anxiety, and post-traumatic stress disorder or requiring anti-depressants, antipsychotics, mood stabilizers or psychotropics prescribed by a psychiatrist). For our first aim, we compared sustained virologic response (SVR) and adherence (pill counts, study visits, and in 25 patients, blood levels of the sofosbuvir metabolite, GS-331007) within each study. For our second aim, only patients with HIV coinfection were evaluated. BDI scores were obtained pre-treatment, during treatment, and post-treatment among participants treated with sofosbuvir-based therapy, and compared to scores from participants treated with interferon-based therapy. AIM: To determine the use and teratogenicity of herbal medicinal products (HMP). METHODS: A retrospective study was conducted in a University hospital to compare the difference in the pattern of use and fetal outcomes between pregnant women who took HMP and Western pharmaceutical products (WPP). RESULTS: From 1995 Jan to 2001 Dec, 61 and 372 women took HMP and WPP one month before or during their current pregnancies respectively. There was an increase in the prevalence of pregnant women who took HMP from 0 % in 1995 to 0.8 % in 2001. Among HMP users, 51.6 %, 82.8 % and 58.6 % of them had low monthly family income (<15 000), low education level (secondary education or below) and were unemployed respectively. In comparison to WPP, pregnant women used smaller number of HMP (1.4 vs 3.0, P<0.01) at a later gestation (4.8 weeks vs 3.1 weeks, P <0.01) and within a shorter duration (11.1 d vs 47.9 d, P<0.01). Hokkien Merchants and the Kian Teik Tong： Economic and Political Influence in Nineteenth- Century Penang and Its Region

This article explores nineteenth-century Penang＇s Hokkkien merchants and their secret society or hui-the Klan Teik Tong diande tang）--which had a variety of roles and an extensive network. It contextualizes the merchants＇ secret society as a transnational socioeconomic and political organization rather than as an overseas Chinese criminal group in the wider Penang area. By recovering Klan Teik Tong and its network, it can be shown how these merchants secured and mobilized labour, capital, and allies in a way that cut across linguistic, ethnic, class and state boundaries in order to establish control of coolies and the lucrative opium, tin, and rice businesses, in order to exert political influence in the colonial and indigenous milieus of the nineteenth-century Penang region. They established a social contract through their Klan Teik Tong relief activities and initiation rituals, and thus were able to recruit thousands of members who were mainly labourers. With such a substantial social force, the merchants launched organized violence against their rivals to attain dominance in opium revenue farming and tin mining businesses in Penang, Krabi, and Perak. The widespread and strategic location of the Klan Teik Tong in Burma also enabled the same merchants to monopolize the Penang-Burma rice trade. The versatility of the Klan Teik Tong＇s functions allowed them to operate as an alternative political order vis-a-vis the colonial and indigenous powers. This arrangement allowed the Hokkien merchants to gain significant political clout in confronting the Siamese and Dutch authorities.

Background:The associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)for hepatocellular carcinoma(HCC)with fibrosis/cirrhosis is often associated with limited growth of future liver remnant(FLR).We introduced a new procedure named transcatheter arterial embolization-salvaged ALPPS(TAE-salvaged ALPPS)which was shown to be especially suitable for HCC patients with cirrhosis or fibrosis who failed adequately to respond to conventional *** short-term efficacy and safety for the TAE-salvaged ALPPS on patients with HCC and fibrosis/cirrhosis were ***:Consecutive HCC patients who underwent TAE-salvaged ALPPS in our hospital between November 2016 and June 2020 were retrospectively *** new ALPPS procedure included conventional ALPPS stage-1 using associating liver partition and portal vein *** FLR failed to reach sufficient hypertrophy,TAE was carried out 2 weeks later followed by liver resection 3 weeks after ALPPS ***:Nine of 10 patients had a single tumor(median diameter 14.0 cm,range,5.2-17 cm).The remaining patient had multiple tumors(diameter of one tumor 14.0 cm,and two satellite foci 2.0 and 3.0 cm).R0 resection was achieved in all patients(100%)after a median of 21 *** patients had cirrhosis,1 had METAVIR grade-3 fibrosis,and 3 had METAVIR grade-2 *** median increase in FLR volume after TAE-salvaged ALPPS was 69.7%(34.4-143.9%).The absolute and relative kinetic growth rates(KGRs)were 9.9(7.1-17.3)mL/day and 3.4%(1.9-7.2%)/day,*** median absolute KGRs were 15.7,2.6,and 19.5 mL/day in the first,second,and third postoperative weeks after ALPPS stage-1,*** rapid increase in KGR on the third week was induced by *** overall postoperative morbidity rates were 50,0%(5/10),20.0%(2/10)and 70.0%(7/10)after ALPPS stage-1,TAE and ALPPS stage-2,*** 90-day mortality rate was 10.0%(1/10).The median overall survival was 40 ***:The new TA

Charge balance materials for homojunction and heterojunction OLED applications

In a homojunction device, a single organic layer assumes the multiple roles of hole, electron transportation, and emitter. Its ease in processing is highly desirable from the manufacturing point of view. In this paper, we shall describe the synthesis of a range of bipolar small molecules and conductive vinyl polymers for application in homojunction and heterojunction organic light emitting diodes（OLEDs）. The bipolar materials, in general, consist of three basic building blocks: an arylamine, a 1,3,4-oxadiazole, and a polycyclic aromatic moiety. The achievement of charge balance can be validated either by direct measurement of electron/hole mobility or indirectly via optimization of device properties.A series of conductive vinyl copolymers containing hole transporting N-（4-methoxyphenyl）-N-（4-vinylphenyl）naphthalen-1-amine（4MeONPA） and electron transporting 2-phenyl-5-（4-vinylphenyl）-1,3,4-oxadiazole（OXA）at different compositions was applied for heterojunction and homojunction OLEDs. For heterojunction devices employed the copolymers as the hole transporting layer and Alq3as the electron transporting and emitting layer,a maximum luminance and current efficiency of over 23000 cd/m2and 4.2 cd/A（PL of Alq3）, respectively, were achieved at the charge balance composition. Homojunction devices for the copolymers were demonstrated by the addition of rubrene as a dopant. The single layer devices at the optimal copolymer composition has ca 1500 cd/m2and 0.74 cd/A.

Background Synovium-derived stem cells （SDSCs） with higher chondrogenic potential are attracting considerable attention as a cell source for cartilage regeneration. We investigated the effect of bone morphogenetic protein 2 （BMP-2） on transforming growth factor beta3 （TGF-β3）-induced chondrogenesis of SDSCs isolated from human osteoarthritic synovium in a pellet culture system. Methods The clonogenicity, stem cell marker expression and multi-differentiation potential of isolated SDSCs were determined by colony forming unit assay, flow cytometry and specific staining including alizarin red S, Oil red O and alcian blue staining, respectively. SDSCs pellet was cultured in chondrogenic medium with or without TGF-β3 or/and BMP-2. At day 21, the diameter and the weight of the pellets were measured. Chondrogenic differentiation of SDSCs was evaluated by Safranin O staining, immunohistochemical staining of collagen type Ⅱ, sulfated glycosaminoglycan （sGAG） synthesis and mRNA expression of collagen type Ⅱ, aggrecan, SOX9, link-protein, collagen type X and BMP receptor Ⅱ. Results Cells isolated under the optimized culturing density （104/60 cm2） showed clonogenicity and multi-differentiation potential. These cells were positive （〉99%） for CD44, CD90, CD105 and negative （〈10%） for CD34 and CD71. SDSCs differentiated to a chondrocytic phenotype in chondrogenic medium containing TGF-β3 with or without BMP-2. Safranin O staining of the extracellular matrix was positive and the expression of collagen type Ⅱ was detected. Cell pellets treated with TGF-β3 and BMP-2 were larger in diameter and weight, produced more sGAGs, and expressed higher levels of collagen type Ⅱ and other chondrogenic markers, except COL10A1, than medium with TGF-β3 alone. Conclusions SDSCs could be isolated from human osteoarthritic synovium. Supplementation with BMP-2 significantly promoted the in vitro TGF-β3-induced chondrogenic differentiation of SDSCs.
