Objective: To compare d.gital and.ultrasonographic cervical examination for the pred.ction of preterm d.livery in patients hospitalized.for preterm labor. Stud. d.sign: Fiftynine patients were includ.d. The Bishop score was evaluated.upon ad.ission, weeks gestational age. Ultrasonographic measurement of cervical length was d.ne within 24 h after entry. d.livery before 37 weeks gestational age was the primary end.oint. Attend.ng obstetricians were blind.d.to the results of echography. Results: Preterm d.livery rate was 39%(23/59). The risk of preterm d.livery was significantly increased.when the Bishop score was greater than or equal to 6 (OR=4.45 [1.41-14.01]) or when ultrasonographic cervical length was less than or equal to 27 mm (OR=4.04 [1.32-12.3]), but d.gital examination was the only ind.pend.nt risk factor in multivariate analysis. Sensitivity, specificity, positive and.negative pred.ctive value for d.gital examination and.ultrasonography were respectively 74, 61, 55 and.79%, 70, 64, 55 and.77%. Combination of d.gital examination and.ultrasonography d.d.not yield.better results. Conclusion: In our series, pred.ction of preterm d.livery was not improved.by ultrasonography compared.to d.gital examination. The size of the cervical shift observed.in most patients hospitalized.for preterm labor may rend.r ultrasonography less relevant in id.ntifying patients anticipated.to d.liver prematurely.

The goals of this stud. were to d.termine whether anal sphincter d.sfunction in spina bifid. d.velops d.ring fetal life or after birth and.whether it reflects the severity of spina bifid. and.therefore can be used.as a criterion to select the cases that could.benefit from in utero surgery. Total protein and.d.gestive enzyme activities [γ-glutamyl transpeptid.se (GGTP), aminopeptid.se M (AMP), and.alkaline phosphatase isoenzymes includ.ng the intestinal form (iALP)] were assayed.retrospectively in amniotic fluid.from 80 myelomeningocele spina bifid. cases without unrelated.associated.malformation (gestational age 14-33 wk). A normal enzyme activity profile was observed.in 46 of the 80 cases. Two abnormal profiles were observed. 1) bilious vomiting, characterized.by abnormally high GGTP and.AMP activities but normal iALP, and.2) d.gestive enzyme leakage, characterized.by abnormally high activities of GGTP, AMP, and.iALP, typical of anal incontinence. No relation was observed.between these enzyme activity profiles and.the d.fferent second.ry signs of spina bifid. or the level of the d.mage. In concluson, anal sphincter d.sfunction in spina bifid. revealed.by amniotic fluid.d.gestive enzyme activities occurred.before 24 wk in fetal life in 28.7%of cases. This criterion may be ind.cative of the severity of spina bifid. and.therefore perhaps could.be used.to select cases that are suited.to in utero surgery. It could.also be used.to establish the potential benefit of this surgery in fecal incontinence.

This paper d.scribes a novel approach which offers the potential to red.ce d.signs to a small number of highly integrated.mod.les which may then be linked.using simple, regular interconnecting structures. A number of important ad.antages accrue for systems built in this way. An example is given of the use of this system in the d.velopment of an embed.ed.computing system, and.the benefits that result are d.scussed.

The objective of this stud. was to d.termine the frequency of oligohyd.amnios in term pregnancies d.ring the Paris heat wave of 2003 (August 4-14). A compari son of this frequency to a historical control group examined.d.ring the same per iod.in 2002. The frequency of oligohyd.amnios in normal term pregnancies was 17. 5%d.ring the heat wave that affected.Paris in August 2003. This frequency was s ubstantially higher than the rate of oligohyd.amnios in normal term pregnancies in August 2002 (4.4%).
