AIM To estimate prevalence and phenotypic associations of selected inflammatory bowel disease(IBd)-associated genetic variants among Sri Lankan patients. METHOdS A case study of histologically confirmed ulcerative colitis(UC) or Crohn's disease(Cd) patients with ≥ 1 year disease duration, who were compared to unrelated, gender-matched, healthy individuals as controls, was conducted at four major centers in Sri Lanka. Phenotypic data of the cases were obtained and all participants were genotyped for 16 selected genetic variants: IL12 B :rs1045431, IL23 R :rs11805303, ARPC2 :rs12612347, IRGM :rs13361189, IL26/IL22 :rs1558744, CdH1 :rs1728785, IL10 :rs3024505, FCGR2 A :rs3737240, PTGER4 :rs4613763, IL17 REL/PIM3 :rs5771069, HNF4 a :rs6017342, STAT3 :rs744166, SMURF1 :rs7809799, LAMB1 :rs886774, HLA-dRB5, dQA1, dRB1, dRA :rs9268853, MST1, UBA7, and APEH :rs9822268. The genotypes of all variants were in Hardy-Weinberg Equilibrium(P > 10^(-3)). To account for multiple hypothesis testing, P-values dered *** A total of 415 patients and 465 controls were recruited. Out of the single nucleotide polymorphisms(SNPs) tested, the majority were not associated with IBd in Sri Lankans. Significant positive associations were noted between rs886774(LAMB1-gene) and UC(odds ratio(OR) = 1.42, P = 0.001). UC patients with rs886774 had mild disease(OR = 1.66, P d remained in remission(OR = 1.48, P d between rs10045431(IL 12 B gene) and upper gastrointestinal involvement in Cd(OR = 4.76, P = 0.002). CONCLUSION This confirms the heterogeneity of allelic mutations in South Asians compared to Caucasians. Most SNPs and disease associations reported here have not been described in South Asians.

To evaluate patient preferences for sildenafil citrate or tadalafil （PdE-5 inhibitors available for the treatment of erectile dysfunction [Ed]） and assess potential reasons for these preferences. Methods： This open-label study was conducted on Korean men taking sildenafil, at least 6 weeks prior to study entry, for Ed. Following screening, patients continued sildenafil treatment for 4 weeks, then after a 1-week washout period, switched to tadalafil for 8 weeks. Patients then continued with their treatment of choice during an extension phase. Psychosocial factors （time concern, spontaneity, sexual self-confidence） were evaluated using Psychological and Interpersonal Relation- ship Scales （PAIRS）, while timing of dose to sexual attempt patterns were assessed from patient diaries. Results： The present study enrolled 160 Korean men （mean age 55 years） with prior median sildenafil use of 585 days. during the extension phase, 73.7% of patients elected to take tadalafil, whereas 26.3% chose sildenafil （P 〈 0.001）. After switching from sildenafil to tadalafil, mean PAIRS time concern scores decreased from 2.54 to 2.42 （P = 0.002）, with no statistically significant differences observed between the sildenafil and tadalafil assessment phases in sexual spontaneity and self-confidence scores. Sexual attempts made 〉 4 h to 〈 36 h post-dose occurred in 4.5% of patients during the sildenafil assessment phase compared with 17.5% during the tadalafil assessment phase. Conclusion： After experiencing both sildenafil and tadalafil, the majority of patients exhibited a preference for tadalafil. This preference might be influenced by psychosocial factors, such as decreased time concerns, and a broader window of opportunity available for sexual activity.

dear Editor, Antibiotic resistance has emerged as a major threat to global health;multi-drug resistant bacteria already kill more patients in the United States each year than HIV/AIdS, Parkinson's disease, emphysema and homicide combined (Laxminarayan et al., 2013).Among the most effective bacterial resistance mechanisms are β-lactamases, a family of enzymes that are divided into four distinct *** A, C and d (serine-β-lactamases, SBLs) use a catalytic site serine residue to initiate inactivation of the antibiotic, while Class B (metallo-β-lactamases, MBLs) relies on a Zn2+-activated hydroxide (Walsh et al., 2005;Bush and Jacoby,2010;Mitic et al., 2014;Lisa et al., 2017).Clinically relevant inhibitors of Class C and d SBLs are available and in use (e.g., clavulanic acid (CA), drawz et al., 2010), but for MBLs the search for such inhibitors has remained challenging (McGeary et al., 2017).

Early disease detection is extremely important in the treatment and prognosis of many diseases, especially cancer. Often, proteomic fingerprints and a pattern recognition algorithm are used to classify the pathological condition of a given individual. It has been argued that accurate classification of the existing data implies an underlying biological significance. Two fingerprint-based classifiers, decision tree and medoid classification algorithm, and a biomarker-based classifier were examined using a published dataset of mass spectral peaks from 81 healthy individuals and 78 individuals with benign prostate hyperplasia (BPH). For all three methods, classifiers were constructed using the original data and the data after permuting the labels of the samples (BPH and healthy). The fingerprint-based classifiers produced accurate results for the original data, though the peaks used in a given classifier depended upon which samples were placed in the training set. Accurate results were also obtained for the dataset with permuted labels. In contrast, only three unique peaks were identified as putative biomarkers, producing a small number of reasonably accurate biomarker-based classifiers. The dataset with permuted labels was poorly classified. Since fingerprint-based classifiers accurately classified the dataset with permuted labels, the argument for biological significance from a fingerprint-based classifier must be questioned.

BACKGROUNd Efficient detection of delirium and comorbid delirium-dementia is a key diagnostic *** of new,efficient delirium-focused methods of cognitive assessment is a key challenge for improved detection of neurocognitive disorders in everyday clinical *** To compare the accuracy of two novel bedside tests of attention,vigilance and visuospatial function with conventional bedside cognitive tests in identifying delirium in older hospitalized *** 180 consecutive elderly medical inpatients(mean age 79.6±7.2;51%female)referred to a psychiatry for later life consultation-liaison service with delirium,dementia,comorbid delirium-dementia and cognitively intact *** were assessed cross-sectionally with conventional bedside cognitive tests[WORLd,Months Backward test(MBT),Spatial span,Vigilance A and B,Clock drawing test and Interlocking Pentagons test]and two novel cognitive tests[Lighthouse test,Letter and Shape drawing test(LSd)-4].RESULTS Neurocognitive diagnoses were delirium(n=44),dementia(n=30),comorbid delirium-dementia(n=60)and no neurocognitive disorder(n=46).All conventional tests had sensitivity of>70%for delirium,with best overall accuracy for the Vigilance-B(78.3%),Vigilance-A(77.8%)and MBT(76.7%)*** sustained attention component of the Lighthouse test was the most distinguishing of delirium(sensitivity 84.6%;overall accuracy 75.6%).The LSd-4 had sensitivity of 74.0%and overall accuracy 74.4%for delirium *** tests allowed for enhanced sensitivity(>90%)and overall accuracy(≥75%)with the highest overall accuracy for the combination of MBT-Vigilance A and the combined Vigilance A and B tests(both 78.3%).When analyses were repeated for those with dementia,there were similar findings with the MBTVigilance A the most accurate overall combination(80.0%).Combining the Lighthouse-SA with the LSd-4,a fail in either test had sensitivity for delirium of 91.4 with overall accuracy of 74.4%.

在地震工程中,地震荷载的预测需了解沉积层地面运动的放大作用,它定义为沉积层场地与岩石场地地面运动大小的比值。在1994年北岭地震中,观测到的沉积层场地余震的(震级为3～4)弱地面运动放大作用是主震(震级为6.7)放大作用的2倍。这种现象的一种解释是沉积层振幅的放大(非线性)作用。然而,利用经验脉冲响应进行的地震模拟和用弱非均匀性、随机三维地壳速度弹性有限差分法的计算表明,线性波传播模型可以解释所观测到的(表现为非线性)沉积层场地的这种地震响应。随机三维速度变化同样可以再现观测到的峰值地面运动的对数-正态频散。确定性的波传播模型不足以定量地研究近源地面运动的定标律和频散。
