Optimal stem cell source for allogeneic stem cell transplantation for hematological malignancies

Hematopoietic stem cell transplant(HSCT) is a standard treatment for many hematological *** different sources of stem cells, namely bone marrow(BM), peripheral blood stem cells(PBSC) and cord blood(CB) can be used for HSCT, and each has its own advantages and disadvantages. Randomized controlled trials(RCTs) suggest that there is no significant survival advantage of PBSC over BM in Human Leukocyte Antigen-matched sibling transplant for adult patients with hematological malignancies. PBSC transplant probably results in lower risk of relapse and hence better disease-free survival, especially in patients with high risk disease at the expense of higher risks of both severe acute and chronic graft-versus-host disease(GVHD).In the unrelated donor setting, the only RCT available suggests that PBSC and BM result in comparable overall and disease-free survivals in patients with hematological malignancies; and PBSC transplant results in lower risk of graft failure and higher risk of chronic *** level evidence is not available for CB in comparison to BM or PBSC. The risks and benefits of different sources of stem cells likely change with different conditioning regimen, strategies for prophylaxis and treatment of GVHD and manipulation of grafts. The recent success and rapid advance of double CB transplant and haploidentical BM and PBSC transplants further complicate the selection of stem cell source. Optimal selection requires careful weighing of the risks and benefits of different stem cell source for each individual recipient and donor. Detailed counseling of patient and donor regarding risks and benefits in the specific context of the patient and transplant method is essential for informed decision making.

Crohn's disease in adults: Observations in a multiracial Asian population

AIM： To determine the demography and clinical presentation of CD and secondly to determine any differences in the prevalence between the different ethnic groups in a multiracial Asian population. METHODS： Patients with CD who were seen in 2001- 2003 in the University of Malaya Medical Centre （UMMC） were enrolled in this study. Prevalence of disease was calculated for the group as a whole and by race with hospital admissions per ethnic group as the denominator. RESULTS： Thirty-four patients were diagnosed to have CD. Basic demographic data of patients; male：female 17：17; mean age 29.1 years （±13.5 years）; ethnic group： Malays 5 （14.7%）, Chinese 12 （35.3%） and Indians 17 （50%）. Twenty-six （76.5%） were diagnosed under the age of 40 and 8 （23.5%） were diagnosed over the age of 40. Location of the disease was as follows： ileocolonic 13 （38.2%）, terminal ileum only 9 （26.5%）, colon only 8 （23.5%）, and upper gastrointestinal 4 （11.8%）. Sixteen （47.1%） had penetrating disease, 9 （26.5%） had stricturing disease and 9 （26.5%） had nonpenetrating and non-stricturing disease. The hospital admission prevalence of CD was 26.0 overall, Indians 52.6, Chinese 6.9, and Malays 9.3 per 10^5 admissions per ethnic group. The difference between Indians and Malays： [OR 5.67 （1.97, 17.53）P〈 0.001] was statistically significant but not between the Indians and the Chinese [OR 1.95 （0.89, 4.35） P= 0.700]. The difference between the Chinese and the Malays was also not statistically significant. [OR 2.90 （0.95, 9.42）P= 0.063]. CONCLUSION： The clinical presentation of CD is similar to the Western experience. Although the overall prevalence is low, there appears to be a clear racial predominance among the Indians.

Sensitivity analysis of parameters and contrasting performance of ground filtering algorithms with UAV photogrammetry-based and LiDAR point clouds

Most ground filtering algorithms are primarily designed for airborne LiDAR point cloud processing and their successful use in identifying ground points from photogrammetric point clouds remains *** compared six ground filtering algorithms implemented in Metashape,ArcGIS,CloudCompare,LAStools,and *** used UAV photogrammetry-based(acquired under leaf-off conditions)and airborne LiDAR(leaf-on)point clouds of the same area to:(i)compare accuracy of generated DTMs;(ii)evaluate the effect of vegetation density and terrain slope on filtering accuracy;and(iii)assess which algorithm parameters have the greatest effect on the filtering *** results show that the performance of filtering algorithms was affected by the point cloud type,terrain slope and vegetation *** results were generally better for LiDAR(RMSE 0.13–0.19 m)than for photogrammetric(RMSE 0.19–0.23 m)point *** behavior in varying vegetation and terrain conditions was consistent for LiDAR point ***,when applied on photogrammetric point clouds,the algorithms’behavior was inconsistent,especially in areas of steep slope(except for the Progressive Triangulated Irregular Network in LAStools).Parameters related to the selection of the initial minimum elevation ground points were the most influential in all algorithms and point clouds.

This article aims to show that virtual water has historically been an adaptation strategy that enabled some arid regions to develop a prosperous economy without putting pressure on their scarce water *** water is referred to as the total amount of water that is consumed to produce goods and *** an example,in arid central Iran,the deficiency in agricultural revenues was offset by more investment in local industries that enjoyed a perennial capacity to employ more *** revenues of local industries weaned the population from irrigated agriculture,since most of their raw materials and also food stuff were imported from other regions,bringing a remarkable amount of virtual *** virtual water not only sustained the region’s inhabitants,but also set the stage for a powerful polity in the face of a rapid population growth between the 13th and 15th centuries *** resultant surplus products entailed a vast and safe network of roads,provided by both entrepreneurs and ***,it became possible to import more feedstock such as cocoons from water-abundant regions and then export silk textiles with considerable *** article concludes that a similar model of virtual water can remedy the ongoing water crisis in central Iran,where groundwater reserves are overexploited,and many rural and urban centers are teetering on the edge of socio-ecological *** holds an urgent lesson on sustainability for our today’s policy that stubbornly peruses agriculture and other high-water-demand sectors in an arid region whose development has always been dependent on virtual water.
