Persistent increase and improved survival of stage I lung cancer based on a large-scale real-world sample of 26,226 cases

Background:Lung cancer prevails and induces high mortality around the world.This study provided real-world information on the evolution of clinicopathological profiles and survival outcomes of lung cancer,and provided survival information within stage I subtypes.Methods:Patients pathologically confirmed with lung cancer between January 2009 and December 2018 were identified with complete clinicopathological information,molecular testing results,and follow-up data.Shifts in clinical characteristics were evaluated usingχ^(2)tests.Overall survival(OS)was calculated through the Kaplan-Meier method.Results:A total of 26,226 eligible lung cancer patients were included,among whom 62.55%were male and 52.89%were smokers.Non-smokers and elderly patients took increasingly larger proportions in the whole patient population.The proportion of adenocarcinoma increased from 51.63%to 71.80%,while that of squamous carcinoma decreased from 28.43%to 17.60%.Gene mutations including EGFR(52.14%),KRAS(12.14%),and ALK(8.12%)were observed.Female,younger,non-smoking,adenocarcinoma patients and those with mutated EGFR had better survival prognoses.Importantly,this study validated that early detection of early-stage lung cancer patients had contributed to pronounced survival benefits during the decade.Patients with stage I lung cancer,accounted for an increasingly considerable proportion,increasing from 15.28%to 40.25%,coinciding with the surgery rate increasing from 38.14%to 54.25%.Overall,period survival analyses found that 42.69%of patients survived 5 years,and stage I patients had a 5-year OS of 84.20%.Compared with that in 2009-2013,the prognosis of stage I patients in 2014-2018 was dramatically better,with 5-year OS increasing from 73.26%to 87.68%.Regarding the specific survival benefits among stage I patients,the 5-year survival rates were 95.28%,93.25%,82.08%,and 74.50%for stage IA1,IA2,IA3,and IB,respectively,far more promising than previous reports.Conclusions:Crucial clinical and pathological changes have been observed in the pas

A thorough interrogation of the immune landscape is crucial for immunotherapy strategy selection and prediction of clinical responses in non-small-cell lung cancer(NSCLC)patients.Single-cell RNA sequencing(scRNA-seq)techniques have prompted the opportunity to dissect the distinct immune signatures between lung adenocarcinoma(LUAD)and lung squamous cell carcinoma(LUSC),the two major subtypes of NSCLC.Here,we performed scRNA-seq on 72,475 immune cells from 40 samples of tumor and matched adjacent normal tissues spanning 19 NSCLC patients,and drew a systematic immune cell transcriptome atlas.Joint analyses of the distinct cellular compositions,differentilly expressed genes(DEGs),cell-cell interactions,pseudotime trajectory,transcriptomic factors and prognostic factors based on The Cancer Genome Atlas(TCGA),revealed the central roles of cytotoxic and effector T and NK cells and the distinct functional macrophages(Mo)subtypes in the immune microenvironment heterogeneity between LUAD and LUSC.The dominant subtype of Mo was FABP4-Mp in LUAD and SPP1-Mo in LUSC.Importantly,we identifieda novel lymphocyte-related Mo cluster,which we named SELENOP-Mo,and further established its antitumor role in both types,especially in LUAD.Our comprehensive depiction of the immune heterogeneity and definition of Mp clusters could help design personalized treatment for lung cancer patients in clinical practice.

Lung cancer is one of the most leading causes of death throughout the world,and there is an urgent requirement for the precision medical management of it.Artificial intelligence(AI)consisting of numerous advanced techniques has been widely applied in the field of medical care.Meanwhile,radiomics based on traditional machine learning also does a great job in mining information through medical images.With the integration of AI and radiomics,great progress has been made in the early diagnosis,specific characterization,and prognosis of lung cancer,which has aroused attention all over the world.In this study,we give a brief review of the current application of AI and radiomics for precision medical management in lung cancer.

The learning curve for transurethral enucleation with bipolar energy for benign prostate hyperplasia:a single-surgeon experience of 494 patients

This study was performed to investigate the learning curve of transurethral enucleation with bipolar energy(TUEB)for benignprostatic hyperplasia.The study involved 494 consecutive patients who underwent TUEB for benign prostatic hyperplasia fromAugust 2018 to March 2022 by one surgeon(SJJ,Seoul National University Bundang Hospital,Seongnam,Korea).The patientswere followed up at 1 week,1 month,3 months,and 6 months postoperatively.To evaluate the learning curve of TUEB,perioperativeparameters including the enucleation ratio(enucleated tissue weight/transitional zone volume),TUEB efficiency(enucleatedtissue weight/operation time),and enucleation efficiency(enucleated tissue weight/enucleation time)were analyzed.Functionaloutcomes and postoperative complications were also assessed,including the International Prostate Symptom Score(IPSS),IPSSquality-of-life(QoL)score,and uroflowmetry outcomes.The patients’median age was 72(interquartile range[IQR]:66–78)years,and the estimated prostate volume and transitional zone volume were 63.0(IQR:46.0–90.6)ml and 37.1(IQR:24.0–60.0)ml,respectively.The enucleation ratio,TUEB efficiency,and enucleation efficiency were 0.60(IQR:0.46–0.54)g ml−1,0.33(IQR:0.22–0.46)g min−1,and 0.50(IQR:0.35–0.72)g min−1,respectively,plateauing after 70 cases.The functional outcomes,including total IPSS,IPSS QoL score,and uroflowmetry outcomes,significantly improved at 6 months after TUEB(all P<0.05),but without significant differences over the learning curve.Sixty-five(13.2%)patients developed complications after TUEB,21.5%of whom experienced major complications(Clavien–Dindo grade≥3).The rate of major complications declined as the number ofTUEB cases increased(P=0.013).Our results suggest that the efficiency of TUEB stabilized within 70 procedures.

Elasto-inertial microfluidic separation of microspheres with submicron resolution at high-throughput

Elasto-inertial microfluidic separation offers many advantages including high throughput and separation resolution.Even though the separation efficiency highly depends on precise control of the flow conditions,no concrete guidelines have been reported yet in elasto-inertial microfluidics.Here,we propose a dimensionless analysis for precise estimation of the microsphere behaviors across the interface of Newtonian and viscoelastic fluids.Reynolds number,modified Weissenberg number,and modified elastic number are used to investigate the balance between inertial and elastic lift forces.Based on the findings,we introduce a new dimensionless number defined as the width of the Newtonian fluid stream divided by microsphere diameter.The proposed dimensionless analysis allows us to predict whether the microspheres migrate across the co-flow interface.The theoretical estimation is found to be in good agreement with the experimental results using 2.1-and 3.2-μm-diameter polystyrene microspheres in a co-flow of water and polyethylene oxide solution.Based on the theoretical estimation,we also realize submicron separation of the microspheres with 2.1 and 2.5μm in diameter at high throughput,high purity(>95%),and high recovery rate(>97%).The applicability of the proposed method was validated by separation of platelets from similar-sized Escherichia coli(E.coli).

The Persistence and Zonal Scale of Atmospheric Dipolar Modes

This study investigates the relationship between the persistence and the zonal scale of atmospheric dipolar modes(DMs). Results from the daily data of ERA5 and the long-term output of an idealized atmospheric model show that the atmospheric DMs with a broader(narrower) zonal scale dipolar structure possess a longer(shorter) persistence. A detailed vorticity budget analysis indicates that the persistence of a hemispheric-scale DM(1/1 DM) and a regional or sectoral DM(1/8 DM) in the model both largely rely on the persistence of the nonlinear eddy forcing. Linear terms can indirectly reduce the persistence of the anomalous nonlinear eddy forcing in a 1/8 DM by modifying the baroclinicity via the arousal of anomalous vertical motions. Therefore, the atmospheric DMs with a broader(narrower) zonal scale possess a longer(shorter) persistence because the effects of the linear terms are less(more) pronounced when the atmospheric DMs have better(worse) zonal symmetry. Further analyses show that the positive eddy feedback effect is weak or even absent in a 1/8DM and the high-frequency eddy forcing acts more like a concomitant phenomenon rather than a leading driving factor for a 1/8 DM. Thus, the hemispheric-scale DM and the regional or sectoral DMs are different, not only in their persistence but also in their dynamics.

In this paper,the problem of identifying autoregressive-moving-average systems under random threshold binary-valued output measurements is considered.With the help of stochastic approximation algorithms with expanding truncations,the authors give the recursive estimates for the parameters of both the linear system and the binary sensor.Under reasonable conditions,all constructed estimates are proved to be convergent to the true values with probability one,and the convergence rates are also established.A simulation example is provided to justify the theoretical results.

Background The poultry industry needs effective antibiotic alternatives to control outbreaks of necrotic enteritis(NE)caused by Clostridium perfringens.Methods The aim of this study was to investigate the effects of dietary supplementation with Macleaya cordata extract(MCE)on the immune function and gut microbiota of broilers with NE.A total of 2881-day-old broiler chicks were randomly assigned to a 2×2 factorial arrangement with two concentrations of dietary MCE supplementation(0 or 350 mg/kg of diet)and two disease challenge statuses(control or NE).Results The results revealed that NE significantly increased the feed conversion rate(FCR),mortality,intestinal lesion score,the levels of IL-1β,IL-17 and IFN-γ/IL-4 in serum and IL-17/IL-10 in the jejunal mucosa,m RNA levels of TLR2,IFN-γand p Ig R in the jejunum,and Clostridium perfringens concentrations in the cecum.NE significantly decreased the body weight(BW),body weight gain(BWG),jejunal villus height,V/C,m RNA level of AMPK-α1 in jejunum,IL-4 level in the jejunal mucosa and lactic acid bacteria abundance in the cecum.MCE significantly increased BW,BWG,jejunal villus height,V/C,m RNA levels of occludin,ZO-1 and AMPK-α1 in the jejunum,the levels of Ig A and Ig G in serum and IL-10 in the jejunal mucosa and m RNA levels of NF-κB,IL-10 and MHC-II in the jejunum.Additionally,MCE significantly decreased the FCR,mortality,intestinal lesion score,jejunal crypt depth,the levels of IFN-γand IL-17 in serum and IL-17/IL-10 in the jejunal mucosa,Clostridium perfringens concentrations in the cecum,and m RNA levels of IL-17/IL-10 in the jejunum.Moreover,NE significantly increased the abundance of bacteria that are associated with inflammation,obesity and depression(Alistipes,Barnesiella,Intestinimonas,RF39 and UCG-005)and significantly decreased the abundance of short-chain fatty acid(SCFA)-producing bacteria(Anaerotruncus,Butyricicoccus and Bacteroides)in the cecum.MCE significantly increased the abundance of SCFA-producing bacteria(Streptococcus,Ruminococcus_torques_group

Self-healing Ga-based liquid metal/alloy anodes for rechargeable batteries

With the rapid development of electronics,electric vehicles,and grid energy storage stations,higher requirements have been put forward for advanced secondary batteries.Liquid metal/alloy electrodes have been considered as a promising development direction to achieve excellent electrochemical performance in metal-ion batteries,due to their specific advantages including the excellent electrode kinetics and self-healing ability against microstructural electrode damage.For conventional liquid batteries,high temperatures are needed to keep electrode liquid and ensure the high conductivity of molten salt electrolytes,which also brings the corrosion and safety issues.Ga-based metal/alloys,which can be operated at or near room temperature,are potential candidates to circumvent the above problems.In this review,the properties and advantages of Ga-based metal/alloys are summarized.Then,Ga-based liquid metal/alloys as anodes in various metal-ion batteries are reviewed in terms of their self-healing ability,battery configurations,working mechanisms,and so on.Furthermore,some views on the future development of Ga-based electrodes in batteries are provided.
