Background: Hepatocellular adenoma is a rare liver tumor that may require surgical treatment in cases of hemorrhage or suspicion of malignant lesions. Aim: To analyze data from patients who underwent hepatectomy for hepatocellular adenoma (HCA) in Rio de Janeiro, Brazil. Methods: From January 2005 to March 2019, sixty-nine patients with HCA underwent hepatectomy at centers in Rio de Janeiro. They were included in the analysis patients undergoing hepatectomy with pathological diagnosis of hepatocellular adenoma and excluded patients with hepatectomy with anatomopathological diagnosis other than hepatocellular adenoma, mainly nodular focal hyperplasia. Data related to patients, tumor and surgery were analyzed retrospectively. Results: Sixty patients (87%) were female and nine were male. Among women, 83% had a history of contraceptive use;among men, only one had an androgen intake history. Overall mean age was 36.4 years (15 - 49), with men older than women (33.9 ± 8.14 years vs. 40.4 ± 6.27 years;P = 0.02). Forty one patients reported abdominal pain, associated or not to other symptoms;32% had an episode of hemorrhage;28 were asymptomatic with an incidental radiological finding. In total, 45 patients presented only one lesion and overall mean size was 8.1 cm (2 - 31);tumors were larger among men (mean size 12.9 ± 9.86 cm vs. 7.7 ± 4.58 cm;P = 0.009). Twenty one surgeries were laparoscopic. Hepatocellular carcinoma (HCC) was identified in three specimens and the incidence was higher among men (22.2% vs. 1.6%;P = 0.042) and in tumors larger than 20 cm (66.6% vs. 0.02%). There was no perioperative mortality. Two of the three patients with HCC died with extrahepatic recurrence;the remaining patient is well after 36 months. Overall mean follow-up time was 14.2 months (2 - 76). Conclusion: Male patients with HCA were older and had larger tumors when compared to females. Introduction In Brazil,approximately 5%are born with a congenital disorder,potentially fatal without *** study aims to evaluate the relationship between gastrointestinal congenital malformation(GICM)mortality,health indicators,and socioeconomic factors in *** GICM admissions(Q39-Q45)between 2012 and 2019 were collected using national *** demographics,socioeconomic factors,clinical management,outcomes,and the healthcare workforce density were also accounted *** Surgical Workforce density and the number of neonatal intensive care units in a region were extracted from national datasets and combined to create a clinical index termed'NeoSurg'.Socioeconomic variables were combined to create a socioeconomic index termed SocEcon'.Simple linear regression was used to investigate if the temporal changes of both indexes were *** correlation between mortality and the different indicators in Brazil was evaluated using Pearson's correlation *** Over 8 years,Brazil recorded 12804 GICM *** Southeast led with 6147 cases,followed by the Northeast(2660),South(1727),North(1427),and Midwest(843).The North and Northeast reported the highest mortality,lowest NeoSurg,and SocEcon Index ***,mortality rates declined across regions from 7.7%(2012)to 3.9%(2019),a 51.7%*** North and Midwest experienced the most substantial reductions,at 63%and 75%,*** significantly correlated with the indexes in nearly all regions(p<0.05).Conclusion Our study highlights the correlation between social determinants of health and GICM mortality in Brazil,using two novel indexes in the pediatric *** findings provide an opportunity to rethink and discuss new indicators that could enhance our understanding of our country and could lead to the development of necessary solutions to tackle existing challenges in Brazil and globally.

Background: Neuroendocrine neoplasms are those that develop from a neuroendocrine cell. They most commonly affect the lungs, gastrointestinal tract, and pancreas, being rare conditions in the female genital tract. When present, these neoplasms often manifest with nonspecific signs and symptoms such as pain, itching, swelling, single-focus lesions, bleeding, and enlargement of inguinal lymph nodes, in addition to the presence of progressively enlarging vulvar nodules. Consequently, the diagnostic investigation involves histopathological examination and confirmation through immunohistochemistry. Objective: To present a comprehensive understanding of this rarely studied pathology. The primary objective is to provide valuable insights that could aid in the future development of universally applicable treatment guidelines. Case Presentation: A 57-year-old female, with no prior comorbidities, menopause at 36, who presented with a left vulvar nodule accompanied by intense pain and swelling, later diagnosed with small cell neuroendocrine carcinoma in the vulva. Conclusion: This case report highlights the importance of enhancing our knowledge regarding small cell neuroendocrine carcinoma in the vulva, given its scarcity in medical literature. The information presented here underscores the need for standardized diagnostic and treatment approaches, paving the way for future consensus on managing this uncommon but challenging neoplasm.

Chikungunya fever(CF)is caused by an arbovirus whose manifestations are extremely diverse,and it has evolved with significant severity in recent *** clinical signs triggered by the Chikungunya virus are similar to those of other ***,fever starts abruptly and reaches high levels,followed by severe polyarthralgia and myalgia,as well as an erythematous or petechial maculopapular rash,varying in severity and *** 40%to 60%of affected individuals report persistent arthralgia,which can last from months to *** symptoms of CF mainly represent the tissue tropism of the virus rather than the immunopathogenesis triggered by the host's immune *** main mechanisms associated with arthralgia have been linked to an increase in T helper type 17 cells and a consequent increase in receptor activator of nuclear factor kappa-Βligand and bone *** review suggests that persistent arthralgia results from the presence of viral antigens post-infection and the constant activation of signaling lymphocytic activation molecule family member 7 in synovial macrophages,leading to local infiltration of CD4+T cells,which sustains the inflammatory process in the joints through the secretion of pro-inflammatory *** term"long chikungunya"was used in this review to refer to persistent arthralgia since,due to its manifestation over long periods after the end of the viral infection,this clinical condition seems to be characterized more as a sequel than as a symptom,given that there is no active infection involved.
