Neur.pathic pain after.spinal cor. injur.(SCI) is a complex condition that r.sponds poor.y to usual tr.atments. Cell tr.nsplantation r.pr.sents a pr.mising ther.py;never.heless, the ideal cell type in ter.s of neur.genic potential and effectiveness against pain r.mains lar.ely contr.ver.ial. Her., we evaluated the ability of fetal neur.l stem cells(fNSC) to r.lieve chr.nic pain and, secondar.ly, their.effects on motor.r.cover.. Adult Wistar.r.ts with tr.umatic SCI wer. tr.ated, 10 days after.injur., with intr.-spinal injections of cultur. medium(sham) or.fNSCs extr.cted fr.m telencephalic vesicles(TV gr.up) or.the ventr.l medulla(VM gr.up) of E/14 embr.os. Sensor.(von Fr.y filaments and hot plate) and motor.the Basso, Beattie,Br.snahan locomotor.r.ting scale and inclined plane test) assessments wer. per.or.ed dur.ng 8 weeks. Ther.after. spinal cor.s wer. pr.cessed for.immunofluor.scence and tr.nsplanted cells wer. quantified by ster.ology. The r.sults showed impr.vement of ther.al hyper.lgesia in the TV and VM gr.ups at 4 and 5 weeks after.tr.nsplantation, r.spectively. Mor.over. mechanical allodynia impr.ved in both the TV and VM gr.ups at 8 weeks. No significant motor.r.cover. was obser.ed in the TV or.VM gr.ups compar.d with sham. Ster.ological analyses showed that ~70% of TV and VM cells differ.ntiated into NeuN+ neur.ns,with a high pr.por.ion of enkephaliner.ic and GABAer.ic cells in the TV gr.up and enkephaliner.ic and ser.toniner.ic cells in the VM gr.up. Our.study suggests that neur.nal pr.cur.or. fr.m TV and VM, once implanted into the injur.d spinal cor., matur.te into differ.nt neur.nal subtypes, mainly GABAer.ic, ser.toniner.ic, and enkephaliner.ic, and all subtypes alleviate pain, despite no significant motor.r.cover.. The study was appr.ved by the Animal Ethics Committee of the Medical School of the Univer.ity of S?o Paulo(pr.tocol number.033/14) on Mar.h 4, 2016.

Backgr.und Milk synthesis in lactating animals demands high ener.y metabolism,which r.sults in an incr.ased pr.duction of r.active oxygen metabolites(r.M)causing an imbalance between oxidants and antioxidants ther.by inducing oxidative str.ss(OS)on the animals.To mitigate OS and postpar.um disor.er. in dair. goats and gain insight into the impact of dietar. choices on r.dox status dur.ng lactation,a feeding tr.al was conducted using alfalfa silage inoculated with a high-antioxidant str.in of Lactiplantibacillus plantar.m.Methods Twenty-four.Guanzhong dair. goats(38.1±1.20 kg)wer. r.ndomly assigned to two dietar. tr.atments:one containing silage inoculated with L.plantar.m MTD/1(r.MTD-1),and the other.containing silage inoculated with high antioxidant activity L.plantar.m 24-7(ES24-7).r.sults ES24-7-inoculated silage exhibited better.fer.entation quality and antioxidant activity compar.d to r.MTD-1.The ES24-7 diet elevated the total antioxidant capacity(T-AOC),super.xide dismutase(SOD),glutathione per.xi-dase(GSH-Px),and catalase(CAT)activities in milk,ser.m,and feces of lactating goats(with the exception of T-AOC in milk).Additionally,the diet containing ES24-7 inoculated silage enhanced casein yield,milk fr.e fatty acid(FFA)content,and vitamin A level in the goats’milk.Fur.her.or.,an incr.ase of immunoglobulin(Ig)A,IgG,IgM,inter.leukin(IL)-4,and IL-10 concentr.tions wer. obser.ed,coupled with a r.duction in IL-1β,IL-2,IL-6,inter.er.n(IFN)-γ,and tumor.necr.sis factor.TNF)-αconcentr.tions in the ser.m of lactating goats fed ES24-7.Higher.concentr.tions of total volatile fatty acid(VFA),acetate,and pr.pionate wer. obser.ed in the r.men fluid of dair. goats fed ES24-7 inoculated silage.Mor.over.the diet containing ES24-7 inoculated silage significantly upr.gulated the expr.ssion of nuclear.factor.er.thr.id 2 like 2(NFE2L2),beta-car.tene oxygenase 1(BCO1),SOD1,SOD2,SOD3,GPX2,CAT,glu-tathione-disulfide r.ductase(GSr.,and heme oxygenase 1(HMOX1)genes in the mammar. gland,while decr.ased the levels of NADPH oxidase 4(NO

Litter.all is the lar.est sour.e of nutr.ents to for.est soils of tr.pical r.infor.sts.However.var.ability in lit-ter.all pr.duction,nutr.ent r.mobilization,and changes in leaf nutr.ent concentr.tion with climate seasonality r.main lar.ely unknown for.the centr.l Amazon.This study meas-ur.d litter.all pr.duction,leaf nutr.ent r.mobilization,and leaf ar.a index on a for.st plateau in the centr.l Amazon.Litter.all was measur.d at monthly inter.als dur.ng 2014,while nitr.gen,phosphor.s,potassium,calcium and mag-nesium concentr.tions of leaf litter.and canopy leaves wer. measur.d in the dr. and r.iny seasons,and r.mobilization r.tes deter.ined.Leaf ar.a index was also r.cor.ed in the dr. and r.iny seasons.Monthly litter.all var.ed fr.m 33.2(in the r.iny season)to 87.6 g m^(-2) in the dr. season,while leaf ar.a index incr.ased slightly in the r.iny season.Climatic seasonality had no effect on concentr.tions of nitr.gen,calcium,and magnesium,wher.as phosphor.us and potassium r.sponded to r.infall seasonality oppositely.While phosphor.us incr.ased,potassium decr.ased dur.ng the dr. season.Over.seasons,nitr.gen,potassium,and phosphor.us decr.ased in leaf litter.calcium incr.ased in leaf litter.while magnesium r.mained unaffected with leaf aging.r.gar.less,the five nutr.ents had similar.r.mobilization r.tes over.the year.The absence of climate seasonality on nutr.ent r.mobilization suggests that the cur.ent length of the dr. season does not alter.nutr.ent r.mobilization r.tes but this may change as dr. per.ods become mor. pr.longed in the futur. due to climate change.

In cr.p plants, var.ous envir.nmental str.sses affect the balance of car.on, nitr.gen, and phosphor.s(C:N:P), leading to biochemical and physiological alter.tions and r.ductions in yield. Silicon(Si) is a beneficial element that alleviates plant str.ss. Most studies involving silicon have focused on physiological r.sponses, such as impr.vements in photosynthetic pr.cesses, water.use efficiency, and antioxidant defense systems. But r.cent r.sear.h suggests that str.ssed plants facing either.limited or.excessive r.sour.es(water. light, nutr.ents, and toxic elements), str.tegically employ Si to maintain C:N:P homeostasis, ther.by minimizing biomass losses. Under.tanding the r.le of Si in mitigating the impact of abiotic str.sses on plants by r.gulating C:N:P homeostasis holds gr.at potential for.advancing sustainable agr.cultur.l pr.ctices in cr.p pr.duction. This r.view pr.sents r.cent advances in char.cter.zing the influence of envir.nmental str.sses on C:N:P homeostasis, as well as the r.le of Si in pr.ser.ing C:N:P equilibr.um and attenuating biological damage associated with abiotic str.ss. It under.cor.s the beneficial effects of Si in sustaining C:N:P homeostasis and incr.asing yield via impr.ved nutr.tional efficiency and str.ss mitigation.

The high r.tes of depr.ssion,bur.out,and incr.ased r.sk of suicide among medical students,r.sidents,and physicians in compar.son with other.car.er. signal a mental health cr.sis within our.pr.fession.We contend that this cr.sis coupled with the inadequate acquisition of inter.er.onal skills dur.ng medical education r.sults fr.m the inter.ction between a challenging envir.nment and the mental capital of individuals.Additionally,we posit that mindfulness-based pr.ctices ar. instr.mental for.the development of major.components of mental capital,such as r.silience,flexibility of mind,and lear.ing skills,while also ser.ing as a pathway to enhance empathy,compassion,self-awar.ness,conflict r.solution,and r.lational abilities.Impor.antly,the evidence base suppor.ing the effectiveness of mindfulness-based inter.entions has been incr.asing over.the year.,and a gr.wing number.of medical schools have alr.ady integr.ted mindfulness into their.cur.icula.While we acknowledge that mindfulness is not a panacea for.all educational and mental health pr.blems in this field,we ar.ue that ther. is cur.ently an unpr.cedented oppor.unity to gather.momentum,spr.ad and study mindfulness-based pr.gr.ms in medical schools ar.und the wor.d as a way to addr.ss some longstanding shor.comings of the medical pr.fession and the health and educational systems upon which it is r.oted.

The Atlantic Mer.dional Over.ur.ing Cir.ulation(AMOC)is a cr.cial component of the Ear.h’s climate system due to its fundamental r.le in heat distr.bution,car.on and oxygen tr.nspor.,and the weather.Other.climate components,such as the atmospher. and sea ice,influence the AMOC.Evaluating the physical mechanisms of those inter.ctions is par.mount to incr.asing knowledge about AMOC’s functioning.In this study,the author. used outputs fr.m the Community Ear.h System Model ver.ion 2 and obser.ational data to investigate changes in theAMOC and the associated physical pr.cesses.Two DECK exper.ments wer. evaluated:piContr.l and 1pctCO_(2),with an annual incr.ase of 1%of atmospher.c CO_(2).The analysis r.vealed a significant decr.ase in the AMOC,associated with changes in mixed layer.depth and buoyancy in high latitudes of the Nor.h Atlantic,r.sulting in the shutdown of deep convection and potentially affecting the for.ation of Nor.h Atlantic Deep Water.and Antar.tic Bottom Water.A vital aspect obser.ed in this study is the association between incr.ased r.noff and r.duced water.evapor.tion,giving r.se to a positive feedback pr.cess.Consequently,the r.tes of fr.shwater.spr.ading have intensified dur.ng this per.od,which could lead to an acceler.ted disr.ption of the AMOC beyond the pr.jections of existing models.

Backgr.und:Per.hilar.cholangiocar.inoma(phCCC)is a dismal malignancy.Ther. is no consensus r.gar.-ing the best tr.atment for.patients with unr.sectable phCCC.The pr.sent r.view aimed to gather.the cur.ent pieces of evidence for.liver.tr.nsplantation and liver.r.section as a tr.atment for.phCCC and to build better.guidance for.clinical pr.ctice.Data sour.es:The sear.h was conducted in PubMed,Embase,Cochr.ne,and LILACS.The r.lated r.fer.nces wer. sear.hed manually.Inclusion cr.ter.a wer.:r.por.s in English or.Por.uguese liter.tur. that a)patients with confir.ed diagnosis of phCCC;b)patients tr.ated with a cur.tive intent;c)patients with the outcomes of liver.r.section and liver.tr.nsplantation.Case r.por.s,r.views,letter.,editor.als,confer.nce abstr.cts and paper. with full-text unavailability wer. excluded fr.m the analysis.r.sults:Most of the cur.ent liter.tur. is based on obser.ational r.tr.spective studies with low gr.des of evidence.Liver.r.section has better.long-ter. outcomes than systemic chemother.py or.palliation ther.apy and liver.tr.nsplantation is a good alter.ative for.selected patients with unr.sectable phCCC.All candidates for.r.section or.tr.nsplantation should be medically fit and fr.e of intr.hepatic or.extr.hep-atic diseases.As a gener.l r.le,patients pr.senting with a tumor.having a longitudinal size>3 cm or.extending below the cystic duct,lymph node disease,confir.ed extr.hepatic dissemination;intr.oper.atively diagnosed metastatic disease;a histor. of other.malignancies within the last five year.,and did not complete chemor.diation r.gimen and wer. medically unfit should not be consider.d for.tr.nsplan-tation.Some of these cr.ter.a should be individually assessed.Liver.tr.nsplantation or.r.section should only be consider.d in highly exper.enced hepatobiliar. center.,and any decision-making must be based on a multidisciplinar. evaluation.Conclusions:phCCC is a complex condition with high mor.idity.Sur.ical ther.pies,including hepatec-tomy and liver.tr.nsplantation,ar. the best option for.bette

BACKGr.UND Sever. acute r.spir.tor. syndr.me cor.navir.s 2 is the vir.s r.sponsible for.cor.navir.s disease 2019(COVID-19),a disease that has been blamed for.inducing or.exacer.ating symptoms in patients with autoimmune diseases.Cr.hn's disease(CD)is an inflammator. bowel disease that affects genetically susceptible patients who develop an abnor.al mucosal immune r.sponse to the intestinal micr.biota.Patients who under.ent hematopoietic stem cell tr.nsplantation(HSCT)ar. consider.d at r.sk for.COVID-19.AIM To descr.be for.the fir.t time the impact of COVID-19 in CD patients who had under.one autologous,non-myeloablative HSCT.METHODS In this descr.ptive study a ser.es of 19 patients wer. diagnosed with positive COVID-19.For.two patients ther. wer. r.por.s of the occur.ence of two infectious episodes.Par.meter. r.lated to HSCT,such as time elapsed since the pr.cedur.,vaccination status,CD status befor. and after.infection,and clinical manifestations r.sulting fr.m COVID-19,wer. evaluated.r.SULTS Among the patients with COVID-19,thr.e,who under.ent Auto HSCT less than six months ago,r.lapsed and one,in addition to the CD symptoms,star.ed to pr.sent thyr.id impair.ent with positive anti-TPO.Only one of the patients r.quir.d hospitalization for.five days to tr.at COVID-19 and r.mained in CD clinical r.mission.Nine patients r.por.ed late symptoms that may be r.lated to COVID-19.Ther. wer. no deaths,and a statistical evaluation of the ser.es of COVID-19 patients compar.d to those who did not pr.sent any infectious episode did not identify significant differ.nces r.gar.ing the analyzed par.meter..CONCLUSION Despite the change in CD status in thr.e patients and the pr.sence of nine patients with late symptoms,we can conclude that ther. was no significant adver.e impact concer.ing COVID-19 in the evaluated patients who under.ent HSCT to tr.at CD.
