Prevalence of Helicobacter pylori vacA,cagA,dupA and oipA Genotypes in Patients with Gastric Disease

Gastric diseases such as chronic gastritis and gastric cancer are most commonly caused by virulence factors of Helicobacter pylori (H. pylori), such as the vacA, cagA, dupA and oipA genes. Therefore, this study investigated the prevalence and the combination of these virulence factors from patients with gastric diseases. The endoscopic biopsies were obtained from 516 patients with gastric symptoms, 101 of which were from patients with normal gastric tissue, 365 of which were from patients with chronic gastritis, and 50 of which were from patients with gastric cancer. H. pylori and the virulence factors were detected by PCR. The oipA gene exhibited an increased risk for chronic gastritis (p = 0.0296), and the vacA gene demonstrated a risk for gastric cancer from chronic gastritis (p = 0.0002). Based on the combination of the virulence factors, cagA, vacA, dupA and oipA genes exhibited a high prevalence in patients with chronic gastritis and gastric cancer. The cagA+/dupA+ genotype demonstrated a significant correlation in patients with normal gastric mucosa (p = 0.0278). In the chronic gastritis group, a significant association was observed between the cagA+ and the vacA s1m1 genotypes (p cagA+/dupA+ genotypes (p = 0.0183), the dupA+/oipA+ genotypes (p cagA, vacA, dupA, and oipA genes, which contributed to the risk of developing gastroduodenal diseases. Furthermore, this is the first study to reveal a high prevalence of the oipA gene in H. pylori isolates in Brazil.

Influence of Water Content on the Quality of Pigeonpea Seeds

The species Cajanus cajan L. where pigeonpea is from presents a harvest potential in tropical regions for it is used as human and animal feed, besides being used as green fertilizer. However, the harvest area of this Fabaceae is still insignificant, due to especially the lack of quality seeds. In this context, it is possible to affirm that drying and store are portrayed as important steps for obtaining superior quality seeds. The aim of this study is to evaluate the physiologic quality of dwarf pigeonpea seeds, with different water content during storage. A factorial scheme 3 × 6 was adopted in the delimitation completely randomized, with four replications. Treatments were constituted by the combination of lots of seeds containing three different water contents (11%, 14% and 16%), submitted to a 10-month storage period, with evaluations every two months (0, 2, 4, 6, 8 and 10). Seeds were stocked in a bag type kraft under normal lab conditions, that is, no control. Physiologic quality of the seeds was evaluated through the following tests: Germination Pattern Test—GPT, first count of germination, accelerated aging and electric conductivity. It can be concluded that pigeonpea with hard seeds containing water content at 11% existing superior physiologic quality throughout the 10-month storage, under no controlled condition, certainly promoted by the less intense breathing from the reserves cumulated in the seed lot. Pigeonpea seeds storage with water content superior to 14% promotes a sharp decrease of physiologic quality, due to an increase in metabolic activity.

Prioritizing Alarms in Real-Time： Intelligent Alarms Advanced System

During the operation of complex process, such as oil production or refming, abnormal situations may occur, leading to an alarm flooding. Alarm flooding is the signalling of a large number of alarms in a few minutes, in such a way that it is impossible for the operator to attend to all alarms. On these occasions, it is usual that the operator leaves the alarm summary list and gets an analysis of the plant through the screens of the DCS （digital control system）, seeking to understand the situation. The alarm summary list ceases to be a useful tool. In such cases, the operator might have the aid of a filter that would present the highest priority alarms and other information associated with them, enabling him to gain a better knowledge of the situation. This paper describes the interface of a system aimed to help the operator to have a more comprehensive knowledge of the process （a better situational awareness） during process upsets that cause alarm flooding, recovering the utility of the alarm layer to the safety of industrial processes.

Adrenal Insufficiency by Adrenoleukodystrophy

The X-linked adrenoleukodystrophy (ALD) is a severe neurodegenerative disorder due to mutations in the ABCD1 gene. Objective: To report a case of a 19-year-old man with adrenal insufficiency due to adrenoleukodystrophy. Method: Case report and literature review. Result: A previously healthy 19-year-old male patient was admitted to the emergency room with nausea and vomiting for 5 days, who progressed to abdominal pain, severe asthenia, and fever (38.5°C). He referred progressive darkening of the skin, oral mucosa, tongue and nail bed of the hands and feet, observed in the last 6 years. Emergency laboratory evaluation showed severe hyponatremia and hyperkalemia, which, together with decreased plasma cortisol, directed the investigation to causes of adrenal insufficiency. High ACTH (Adrenocorticotropic hormone) and very long chain fatty acid levels closed the diagnosis. Discussion: ALD is characterized by progressive demyelination in the central and peripheral nervous system and adrenal insufficiency consequence to the accumulation of very long chain fatty acids (VLCFA) in the adrenal. The overall incidence of ALD is 1:17,000. Adrenal insufficiency may be the first symptom of ALD in boys and adults. The diagnosis is based on the measurement of VLCFA plasma levels. Allogeneic bone marrow transplantation is the only treatment that provides a permanent cure when the procedure is performed at an early stage of brain demyelination, i.e. when patients are asymptomatic, although brain magnetic resonance imaging (MRI) is abnormal. Treatment of Addison’s disease is obligatory, but does not change the course of neurological symptoms.

Three Different Doses of Intrathecal Morphine as Part of a Multimodal Regimen for Post-Cesarean delivery Analgesia: A Randomized Double-Blinded Trial

Brazil is one of the countries that carry out cesarean delivery in the world. The pain after cesarean delivery (CD) is one of the main concerns of parturient. Intrathecal Morphine (ITM) is widely used for the treatment of pain after CD;however, the optimal dose with minimal side effects is to be established. Objectives: To compare the analgesia and side effects resulted by three different doses of ITM as part of a multimodal regimen for post-cesarean delivery analgesia. Methods: One hundred ninety-four patients were sampled in this randomized, double-blinded, three-arm study (group 0.075: ITM 0.75 mg;group 0.1: ITM 0.1 mg and group 0.15: ITM 0.15 mg). All patients received intravenous dexamethasone, dipyrone and ketoprofen. The primary endpoint was pain intensity measured at rest and on movement, while the secondary endpoint was overall satisfaction with pain relief, the incidence of nausea and vomiting, intensity of pruritus, and the use of additional tramadol, antipruritic and antiemetic use within 24 hours after postoperation. Results: The three groups showed similar pain intensity at rest and on movement. The pain intensity on movement was significantly higher than at rest in the groups (p 0.001). There were no significant differences among the groups regarding secondary endpoint. Conclusions: Our findings suggest that in cases of an elective CD, 0.075 mg of ITM produces postoperative analgesia of similar quality that provided by 0.1 or 0.15 mg ITM. The patients should have a multimodal analgesia approach that includes: ITM, anti-inflammatory agents and access to additional systemic analgesic if necessary.

Infrared Spectroscopy of Titania Sol-Gel Coatings on 316L Stainless Steel

Sol-gel titania films were deposited on 316L stainless steel using titanium isopropoxide as a chemical precursor. Dip-coating was performed at withdrawal speeds of 6 mm/min, 30 mm/min, and 60 mm/min. deposited gel films were heat treated in air at 80℃, 100℃, 300℃, and 400℃. The structural evolution of the coatings was evaluated by infrared reflection-absorption spectroscopy. The influence of the withdrawal speed and the heat treatment temperature on the structure of the films was studied by varying the reflectance incidence angle during the infrared experiments and by Glow Discharge Spectrometry. Free functional groups were detected. The results indicate the formation of bidendate bridging coordination of carboxylic acid to titanium. Titanium atoms can also be pentacoordinated according to the processing conditions of the films. We observed a tendency of increasing amounts of OH groups with decreasing reflectance incidence angle. The film hardness was measured via Knoop microindenation hardness test.

Due to high competition of the agriculture machines sector, the auto parts suppliers are investing in research to promote the development and reduce the costs of the products, passing these results to their clients who are the automakers. It was identified the need to redesign the regulation sieve system harvester, which is a compound assembly in the harvester＇s grain cleaning system, with the objective to reduce the costs and improve the ergonomic aspect, keeping the same efficiency from the original project. The improvement was developed by studying the original design and identifying possible project fails, such as components and manufacturing process which could be eliminated. For this, it was needed to do a research about rules and project considerations that must be applied in the products development and ergonomic too, specifically about geometric forms and anthropometric measures. From the redesign of this system, it is verified that it is possible by a study to identify possible improvements, and to develop a new system by a simple project, improving the ergonomic, reducing the cost and keeping the same efficacy from the original project.

Raising of Horn Fly Haematobia irritans(L.)(Diptera:Muscidae)in Laboratory by Means of Egg and Larva Inoculation

Several studies have required Haematobia irritans (L.) raising in laboratory. The present study assessed two methods of inoculating immature forms of H. irritans to obtain adults. In 2007, 15 Nellore steers (Bos indicus) (L.) were used for the collection of feces free of anthelmintic treatment and flies to produce for eggs and larva. For method I, 30 eggs were incubated in square filter paper (5 × 5 cm) and deposited on bovine feces (500 g) where they were kept until hatching and spontaneous penetration of larvae (L1) into the fecal mass. After 24 h, eggs were analyzed under a stereoscope microscope (40×) for the number of larvae that instinctively penetrated the feces. In method II, larvae were obtained only by natural egg hatching. At birth, 30 larvae were collected and individually inoculated, directly onto the fecal plate by employing a moistened brush. The tests were carried out at controlled temperature (28°C ± 2°C) and saturated humidity (80%) until the emergence of flies with both methods. The number of emerged flies was considered in the result. Using method I, 276 (76.7%) flies emerged from 360 inoculated eggs, while using method II, 283 (78.6%) flies emerged from 360 inoculated larvae. There was no significant difference (P = 0.7821) between methods for the number of flies;however, the proportion between males and females by means of larva inoculation was different from 1:1 (P = 0.0146). Results indicated that both methods led to a satisfactory production of flies and egg inoculation provided an easier establishment.

Brain Abscess after COVID-19: Case Report

As the COVID-19 pandemic progresses, complications and unusual presentations of the disease have been described. Among them, the involvement of distinct parts of the neuroaxis. We report a rare case of brain abscess in elderly after SARS-Cov-2 infection readmitted in our health unit. Patient was treated with ceftriaxone, metronidazole and vancomycin with good clinical and therapeutic response. The satisfactory conduct of the case was only possible by the involvement of a multiprofessional team, which sought early diagnosis, surgical intervention and adequate duration of treatment.
