Short- and long-term efficacy of endoscopic balloon dilation in Crohn's disease strictures

AIM:To evaluate short- and long-term efficacy of endoscopic balloon dilation in a cohort of consecutive patients with symptomatic Crohn's disease (CD)-related strictures. METHODS:Twenty-six CD patients (11 men; median age 36.8 year, range 11-65 years) with 27 symptomatic strictures underwent endoscopic balloon dilation (EBD).Both naive and post-operative strictures, of any length and diameter, with or without associated fistula were included. After a clinical and radiological assessment, EBD was performed with a Microvasive Rigiflex through the scope balloon system. The procedure was considered successful if no symptom reoccurred in the following 6 mo. The long-term clinical outcome was to avoid surgery. RESULTS:The mean follow-up time was 40.7 ± 5.7 mo (range 10-94 mo). In this period, forty-six EBD were performed with a technical success of 100%. No procedure-related complication was reported. Surgery was avoided in 92.6% of the patients during the entire follow-up. Two patients, both presenting ileocecal strictures associated with fistula, failed to respond to the treatment and underwent surgical strictures resection. Of the 24 patients who did not undergo surgery, 11 patients received 1 EBD, and 13 required further dilations over time for the treatment of relapsing strictures (7 patients underwent 2 dilations, 5 patients 3 dilations, and 1 patient 4 dilations). Overall, the EBD success rate after the first dilation was 81.5%. No difference was observed between the EBD success rate for naive (n = 12) and post-operative (n = 15) CD related strictures (p > 0.05). CONCLUSION:EBD appears to be a safe and effective procedure in the therapeutic management of CD-related strictures of any origin and dimension in order to prevent surgery.

Beneﬁcial microbes can improve soil health by promoting soil structure,nutrient cycling,and disease *** addition,a wide array of rhizospheric microbes are responsible for producing metabolically active compounds including various types of plant growth ***,microbial biodiversity studies could contribute to the improvement of agricultural practices in deprived areas,such as the pampean semiarid *** vast majority of studies conducted on endophytic microorganisms have focused on intensive crop legume *** contrast,little attention has been paid to microorganisms of native legumes,whose ecology is not directly affected by human *** this study,endophytic microorganisms isolated from root nodules of a selected native legume of the genus Rhynchosia were *** isolates were studied with a focus on their plant growth-pro-moting rhizobacteria(pGpR)*** the edaphic characteristics of the pampean semiarid region,the isolates obtained were evaluated for their ability to grow under three salt stress conditions(50,100,and 200 mM NaCl)and four different pH values(6,7,8,and 9).Based on their pGpR activities,the selected strains were phylogenetically grouped using *** results showed great variability among the isolates in terms of the characteristics *** legumes manifested a wide endophytic variability and remarkable perfor-mance in pGpR *** conclude that they could be used as potential bioinoculants for legume cultivation,an excellent alternative to the use of chemical fertilizers that currently pollute the environment.

The analysis and design of the extended state observer （ESO） involves a continuous non-smooth structure, thus the study of the ESO dynamic requires mathematical tools of the nonlinear systems analysis. This paper establishes the sufficient conditions for absolute stability of the ESO. Based on this study, a methodology to estimate several nonlinear functions in dy- namics systems is proposed.

在汉语国际教育领域中,汉字是非常重要的教学内容。对于非汉字文化圈的罗马尼亚大学生,特别是大学才开始学习汉语的学生来说,汉字习得是一大难点。笔者通过短期的助教工作以及自己汉语学习的经验发现,罗马尼亚初中学生汉语课堂教学进度慢,课程数量少,而且学校对学生汉语方面的学习要求比较低。与之相反的是大学生每个星期要上较多的汉语课,课堂教学进度快,学校对学生汉语方面的学习要求高,学生需要在短期内掌握大量的新知识。虽然汉语专业的大学生有很多不同的课型,但没有专门教汉字的课程,可参考的汉字教学和学习资料也非常欠缺,怎样学好汉字成了困扰罗马尼亚学生的一个大问题。基于上述原因,本文致力于探究罗马尼亚大学生汉字学习方面的学习难点,本文通过问卷调查了大学生汉字学习各个方面的情况,并通过访谈从汉语教师的角度进一步理解了汉字教学的现状。问卷调查的结果显示,罗马尼亚大学生存在汉字学习策略不恰当、受母语负迁移的影响、对汉字课堂教学的忽视、汉字文化水平较低等问题。除此之外,从教师访谈的内容可以发现,罗马尼亚大学生写不好汉字的原因还包括教师汉字教学时间有限、未深入解释汉字内涵、不重视汉字文化及其演变方面的教学。为了解决罗马尼亚大学生汉字学习过程中遇到的问题,本文建议大学生先花时间理解汉字的结构和意义后再进行汉字书写练习,而不是通过反复抄写机械地学习字形。此外,为了提高汉字教学的有效性,本文针对罗马尼亚汉语专业设置的课型设计了可参考的汉字教学建议。

In recent years, the transcription factor ABI4 has emerged as an important node of integration for externaland internal signals such as nutrient status and hormone signaling that modulates critical transitions during the growthand development of plants. For this reason, understanding the mechanism of action and regulation of this protein rep-resents an important step towards the elucidation of crosstalk mechanisms in plants. However, this understanding hasbeen hindered due to the negligible levels of this protein as a result of multiple posttranscriptional regulations. To betterunderstand the function and regulation of the ABI4 protein in this work, we performed a functional analysis of severalevolutionarily conserved motifs. Based on these conserved motifs, we identified ortholog genes of ABI4 in differentplant species. The functionality of the putative ortholog from Theobroma cacao was demonstrated in transient expres-sion assays and in complementation studies in plants. The function of the highly conserved motifs was analyzed aftertheir deletion or mutagenesis in the Arabidopsis ABI4 sequence using mesophyll protoplasts. This approach permitted usto immunologically detect the ABI4 protein and identify some of the mechanisms involved in its regulation. We identi-fied sequences required for the nuclear localization （Ap2-associated motif） as well as those for transcriptional activationfunction （LRp motif）. Moreover, this approach showed that the protein stability of this transcription factor is controlledthrough protein degradation and subcellular localization and involves the Ap2-associated and the pEST motifs. We dem-onstrated that the degradation of ABI4 protein through the pEST motif is mediated by the 26S proteasome in responseto changes in the sugar levels.

AIM:To study the association between genetic ancestry,non-alcoholic fatty liver disease(NAFLD) metabolic characteristics in two cohorts of patients,from Brazil and portugal. METHODS:We included 131 subjects from Brazil [(n = 45 with simple steatosis(S. Steatosis) and n = 86 with nonalcoholic steatohepatitis(NASH)] and 90 patients from portugal(n = 66,S. Steatosis; n = 24,NASH). All patients had biopsy-proven NAFLD. In histologic evaluation NAFLD activity score was used to assess histology and more than 5 points defined NASH in this study. patients were divided into two groups according to histology diagnosis:simple steatosis or non-alcoholic statohepatitis. Genetic ancestry was assessed using real-time polymerase chain reaction. Seven ancestry informative markers(AT3-I/D,LpL,Sb19.3,ApO,FYNull,pV92,and CKMM) with the greatest ethnicgeographical differential frequencies(≥ 48%) were used to define genetic ancestry. Data were analyzed using R pROJECTS software. Ancestry allele frequencies between groups were analyzed by GENEpOp onlineand the estimation of genetic ancestry contribution was evaluated by ADMIX-95 software. The 5% alpha-error was considered as significant(p ple,NASH was significantly more frequent among the elderly patients with diabetes(NASH 56 ± 1.1 years old vs S. Steatosis 51 ± 1.5 years old,p = 3.7 x 10-9),dyslipidemia(NASH 63% vs S. Steatosis 37%,p = 0.009),higher fasting glucose levels(NASH 124 ± 5.2 vs S. Steatosis 106 ± 5.3,p = 0.001) and Homeostatic Model of Assessment index > 2.5 [NASH 5.3(70.8%) vs S. Steatosis 4.6(29.2%) p = 0.04]. In the portuguese study population,dyslipidemia was present in all patients with NASH(p = 0.03) and hypertension was present in a larger percentage of subjects in the S. The present article comprehensively reviews the thoracic outlet syndrome (TOS), its etiology and assessment, with reference to the usual conservative treatment and surgical therapy, highlighting new definitions and reporting obsolete tests. The syndrome affects about 8% of the population, with higher prevalence in females, and rarely affects the children. Conservative, non-surgical treatment produces good results, and it becomes important to know which specific areas need to be treated. Currently, there is no rehabilitation scheme, either osteopathic or chiropractic, considered as a valid tool for a precise localization of the pathologic condition, probably due to the lack of a manual assessment process to evaluate bones and muscular components of the thoracic outlet. In this situation, the manual treatment performed is usually generic. The present article proposes a manual evaluation that can be used to identify the anatomical area which needs to be treated. This hypothesis of assessment should be carried out after the clinician has ruled out the necessity of surgery with instrumental examinations.
