Prehospital critical care drug-therapy and 30-day mortality in patients with acute respiratory disease

BACKGROUND: Prehospital medication is a reality, and the role of these therapies must be explored to assess their validity, especially for acute respiratory diseases, which are usually associated with increased morbidity. The aim of this study was to examine the association of prehospital medication use with mortality in patients with acute respiratory ***: A prospective, multicenter, emergency medical service(EMS) delivery cohort study was carried out in adults with unselected respiratory diseases managed by EMS who were transferred to the emergency department. From January 1, 2019, to October 31, 2023, six advanced life support units, thirty-eight basic life support units, and four hospitals in Spain participated in the study. Demographic data, vital signs, use of mechanical ventilation, prehospital respiratory diagnosis, and prehospital medication were collected. The primary outcome was 30-day in-hospital ***: A total of 961 patients were included, with a mortality rate of 17.5%(168 patients). Age, an increasing number of comorbidities, the use of invasive mechanical ventilation(IMV), the use of major analgesics, hypnotics, and bicarbonate were risk factors. In contrast, elevated systolic blood pressure and Glasgow Coma Scale scores were found to be protective factors against mortality. The predictive capacity of the model reached an area under the curve(AUC) of 0.857(95% confidence interval [95% CI] 0.827–0.888).CONCLUSION: Our data revealed that IMV, major analgesics, hypnotics and bicarbonate administration were associated with elevated mortality. Adding prehospital drug therapy information to demographic variables and vital signs could improve EMS decision-making, allowing a better characterization of patients at risk of clinical worsening.

Direct seawater splitting has emerged as a popular and promising research direction for synthesising clean,green,non-polluting,and sustainable hydrogen energy without depending on high-purity water in the face of the world’s shortage of fossil ***,efficient seawater splitting is hindered by slow kinetics caused by the ultra-low conductivity and the presence of bacteria,microorganisms,and stray ions in ***,producing hydrogen on an industrial scale is challenging due to the high production *** present review addresses these challenges from the catalyst point of view,namely,that designing catalysts with high catalytic activity and stability can directly affect the rate and effect of seawater *** the ion transfer perspective,designing membranes can block harmful ions,improving the stability of seawater *** the energy point of view,mixed seawater systems and self-powered systems also provide new and low-energy research systems for seawater ***,ideas and directions for further research on direct seawater splitting in the future are pointed out,with the aim of achieving low-cost and high-efficiency hydrogen production.

Unlike other major crops,little research has been performed on tomato to reduce the generation time for speed *** evaluated several agronomic treatments to reduce the generation time of tomato in the‘M82'(determinate)and‘Moneymaker'(indeterminate)varieties and evaluated the best combination in conjunction with embryo *** container sizes with volumes of 0.2 L(XS),0.45 L(S),0.8 L(M),1.3 L(L),and6 L(XL),were evaluated in the first experiment under the autumn *** found that plants grown in XL containers exhibited better development and required less time from sowing to anthesis(DSA)and from anthesis to fruit ripening(DAR).In the second experiment,using XL containers in the autumn-winter cycle,we evaluated the effects of cold priming at the cotyledonary stage,water stress,P supplementation,and K supplementation on generation *** to the control,we found that cold priming significantly reduced the number of leaves,plant height to first the inflorescence,and DSA(2.7 d),whereas K supplementation reduced the DAR(8.8 d).In contrast,water stress and P supplementation did not significantly affect any of the measured traits like DAR,DSA or fruit *** validate these data,in a third experiment with XL containers in the spring-summer cycle,the combination of cold priming and K supplementation was tested,confirming the significant effect of this combination on the reduction of generation time(2.9 d for DSA and 3.9 d for DAR)compared to the *** rescue during the cell expansion cycle(average of 22.0 d and 23.3 d after anthesis for‘M82'and‘Moneymaker',respectively)allowed the shortening of the generation time by 8.7 d in‘M82'and 11.6 d in‘Moneymaker'compared to the in planta fruit *** combination of agronomic treatments with embryo rescue can effectively increase the number of generations per year from three to four for speed breeding of tomato.

The hazard produced by natural phenomena on infrastructure and urban populations has been widely studied in the last 50 years. Researchers have recognised that the real danger posed by these phenomena depends on their extreme values. Most researchers focus on the extremes of natural phenomena considered in isolation, one variable at a time. However, what is relevant in hazard studies is coincident extremes of several climatic variables, i.e., the presence of compound extremes. The peak value of these extremes seldom coincides, but off-peak values located in the tail of the distributions are often concurrent and can lead to catastrophic events. What is essential in hazard studies is to calculate the probabilistic distribution of the extremes of coincident climatic variables. The presence of correlations between these variables complicates the problem. This paper presents a computationally efficient and robust mathematical methodology to solve the problem. The procedure is based on the convolution of the distributions of the climatic variables. Once the probabilistic distribution of the compound variables is found, it is possible to calculate the curves of the return period, which is the indicator of importance in hazard and risk studies. This compound Return Period is computed using the Statistics of Extreme Values. To illustrate the problem, the case of a cyclone landing close to a low-gradient coastal city is discussed, and its probability of flooding and recurrence period is calculated. We show that the failure to correctly model the correlation between variables can result in overestimating the Return Period curve, consequently increasing mitigation costs.

Hepatocellular carcinoma remains a significant cause of mortality worldwide,particularly among patients with liver *** most cases,surveillance in cirrhotic patients is neglected,leading to a diagnosis when the neoplasm is at an advanced *** this context,Zhou et al carried out a network metaanalysis to demonstrate the effectiveness of hepatic arterial infusion chemotherapy,concluding that it is a superior approach compared to sorafenib and transarterial chemoembolization in the treatment of advanced hepatocellular ***,the meta-analysis in question lacks methodological rigor,preventing the authors from making more definitive ***,we understand that transarterial chemoembolization,when properly indicated,is a highly effective therapeutic option,and that sorafenib,given the results of new therapies based on immune checkpoint inhibitors,is no longer the recommended drug for the treatment of these ***,we believe the use of hepatic arterial infusion chemotherapy is increasingly limited and lacks strong scientific support.

In December 2023,I was part of a doctoral viva at the University of Edinburgh,and one colleague reminded us of the novelty effect notion in *** a nutshell,this term refers to individuals or institutions quickly embracing a process,idea or product,because it is perceived and experienced as innovative and beneficial,but as time passes by,the novelty becomes normalized and interest begins to *** example,this effect has been examined in relation to digital technologies in education(e.g.,Jeno et al.,2019;Tsay et al.,2020).The discussion that ensued in the viva lingered in my head for a few days as it prompted me to reflect on the influence that content and language integrated learning(CLIL)continues to have around the *** other words,research and practice have demonstrated that CLIL has overcome the novelty effect,and this special issue attests to it.

Herein,we present the electrocatalytic four-electron hydrogenation of oxalic acid into glycolic acid using black TiO_(2)as an *** acid is an abundant compound found in several sources of organic *** results showed a high selectivity of black TiO_(2)toward glycolic acid,with the formation of glyoxylic acid being the rate-limiting step (glyoxylic acid is the two-electron intermediate).The highest Faradaic efficiency (FE) of 69.6%±8.3% was achieved at 10.2 mA cm^(-2)in 4 h of electrolysis using an H-type cell operated at room temperature,with 50.2%±3.8% of oxalic acid conversion (degradation kinetic constant k=0.0042±0.0001 min-1),58.8%±7.0%of reaction yield and 1.2±0.18 g L-1of glycolic acid production.A theoretical model of black TiO_(2)coming from anatase TiO_(2)was implemented by introducing Ti3+defects,which gave black TiO_(2)the theoretical capability to easily transform oxalic acid into glycolic acid as experimentally *** reaction mechanism was supported and described in detail by density functional theory calculations,which revealed that surface Ti^(3+)states were the main catalytic *** is the first time that a detailed step-by-step mechanism at the atomic level has been proposed for this electrocatalytic reaction,which represents a valuable contribution to the understanding of this process of high energy/environmental *** is also the first time that black TiO_(2)has been used as an electrocatalyst for this sustainable process.

Background: The effects of radiation therapy can impact patients’ quality of life, leading to treatment interruptions and therefore sub-optimal outcomes. The main aim was to evaluate the preliminary effects of Ocoxin cream® in the prevention of radiation therapy. Methods: Fifty patients were enrolled in an observational, longitudinal, prospective, single-centre clinical trial in the Department of Radiation Therapy at the National Institute of Oncology and Radiobiology in Havana, Cuba. The Radiation Therapy Oncology Group (RTOG) toxicity criteria were used to classify the radiation therapy, and the Dermatology Life Quality Index (DLQI) instrument was used to assess quality of life. Results: Patients who met the inclusion criteria were enrolled consecutively and were treated with teletherapy using a 60Co source with 1.25 MeV energy. 70.0% of the patients were head and neck cancer patients. No grade 3 - 4 radiation therapy was reported, nor were there any interruptions in radiation treatment due to skin toxicity. Radiation therapy was observed in only 14.0% of patients, and of these, only two patients developed grade 2 toxicity. The perception of quality of life before vs. after radiation therapy remained within an average range of “no effect” (at the start of radiation therapy) to “small effect” (at the end of radiation therapy). Conclusions: This clinical study is the first report of the use of Ocoxin cream® in cancer patients and demonstrates that it is able to prevent radiation therapy and reduce the severity of toxicity of treatment with ionising radiation.
