Very large hidden genetic diversity in one single tumor:evidence for tumors-in-tumor

Despite the concern of within-tumor genetic diversity,this diversity is in fact limited by the kinship among cells in the tumor.Indeed,genomic studies have amply supported the ’Nowell dogma’ whereby cells of the same tumor descend from a single progenitor cell.In parallel,genomic data also suggest that the diversity could be> 10-fold larger if tumor cells are of multiple origins.We develop an evolutionary hypothesis that a single tumor may often harbor multiple cell clones of independent origins,but only one would be large enough to be detected.To test the hypothesis,we search for independent tumors within a larger one(or tumors-in-tumor).Very high density sampling was done on two cases of colon tumors.Case 1 indeed has 13 independent clones of disparate sizes,many having heavy mutation burdens and potentially highly tumorigenic.In Case 2,despite a very intensive search,only two small independent clones could be found.The two cases show very similar movements and metastasis of the dominant clone.Cells initially move actively in the expanding tumor but become nearly immobile in late stages.In conclusion,tumors-in-tumor are plausible but could be very demanding to find.Despite their small sizes,they can enhance the within-tumor diversity by orders of magnitude.Such increases may contribute to the missing genetic diversity associated with the resistance to cancer therapy.

BACKGROUND Methotrexate(MTX)is the usual first-line treatment for rheumatoid arthritis(RA).Long-term use of MTX has been associated with liver steatosis(LS)and liver fibrosis(LF).AIM To determine if LS in patients treated with MTX for RA is associated with MTX cumulative dose(MTX-CD),metabolic syndrome(Mt S),body mass index(BMI),the male sex,or LF.METHODS A single-center,prospective study of patients receiving MTX for RA was performed from February 2019 to February 2020.The inclusion criteria were patients aged 18 years or older diagnosed with RA by a rheumatologist and being treated with MTX(without limitation on the duration of treatment).The exclusion criteria were previous diagnosis of liver disease(hepatitis B or C virus infection,known nonalcoholic fatty liver disease),alcohol consumption greater than 60 g/d in males or 40 g/d in females,human immunodeficiency virus infection on antiretroviral therapy,diabetes mellitus,chronic renal failure,congestive heart failure,or BMI greater than 30 kg/m^(2).Patients receiving leflunomide in the 3 years prior to the study were also excluded.Transient elastography(Fibro Scan,Echosens?,Paris,France)was used for fibrosis determination(LF>7 Kp A)and computer attenuation parameter(CAP)for LS(CAP>248 d B/m).Demographic variables,laboratory data,MTX-CD(>4000 mg),Mt S criteria,BMI(>25),transient elastography,and CAP scores were collected from all patients.RESULTS Fifty-nine patients were included.Forty-three were female(72.88%),and the mean age was 61.52 years(standard deviation:11.73).When we compared MTX-CD≤4000 mg(26 patients;14 with LS and 12 without)with>4000 mg(33 patients;12 with LS and 21 without),no statistical differences were found(P=0.179).We compared CAP scores stratified by Mt S,BMI,sex,and LF.There were no significant differences in CAP scores based on the presence of Mt S[CAP/Mt S:50 no Mt S(84.75%);9 Mt S(15.25%);P=0.138],the male sex(CAP/sex:8 male/18 female LS;8 male/25 female no LS;P=0.576),or LF[CAP/fibrosis:53 no LF(89.83%);6 LF(10.17%);P=0.239].LS determ

Evaluation of massive transfusion protocol practices by type of trauma at a level I trauma center

OBJECTIVE:To evaluate massive transfusion protocol practices by trauma type at a level I trauma center.;METHODS:A retrospective analysis was performed on a sample of 76 trauma patients with MTP activation between March 2010 and January 2015 at a regional trauma center. Patient demographics, transfusion practices, and clinical outcomes were compared by type of trauma sustained.;RESULTS:Penetrating trauma patients who required MTP activation were significantly younger, had lower injury severity score (ISS), higher probability of survival (POS), decreased mortality, and higher Glasgow Coma scale (GCS) compared to blunt trauma patients. Overall, the mortality rate was 38.16%. The most common injury sustained among blunt trauma patients was head injury (36.21%), whereas the majority of the penetrating trauma patients sustained abdominal injuries (55.56%). Although the admission coagulation parameters and timing of coagulopathy were not significantly different between the two groups of patients, a significantly higher proportion of penetrating trauma patients received high plasma content therapy relative to blunt trauma patients (p < 0.01).;CONCLUSIONS:Despite the use of the same MTP for all injured patients requiring massive transfusion, significant differences existed between blunt trauma patients and penetrating trauma patients. These differences in transfusion characteristics and outcomes following MTP activation underscore the complexity of implementing MTPs and warrant vigilant transfusion practices to improve outcomes in trauma patients.

BACKGROUND Ampullary adenocarcinoma(AAC)is a rare neoplasm that accounts for only 0.2%of all gastrointestinal cancers.Its incidence rate is lower than 6 cases per million people.Different prognostic factors have been described for AAC and are associated with a wide range of survival rates.However,these studies have been exclusively conducted in patients originating from Asian,European,and North American countries.AIM To evaluate the histopathologic predictors of overall survival(OS)in South American patients with AAC treated with curative pancreaticoduodenectomy(PD).METHODS We analyzed retrospective data from 83 AAC patients who underwent curative(R0)PD at the National Cancer Institute of Peru between January 2010 and October 2020 to identify histopathologic predictors of OS.RESULTS Sixty-nine percent of patients had developed intestinal-type AAC(69%),23%had pancreatobiliary-type AAC,and 8%had other subtypes.Forty-one percent of patients were classified as Stage I,according to the AJCC 8 th Edition.Recurrence occurred primarily in the liver(n=8),peritoneum(n=4),and lung(n=4).Statistical analyses indicated that T3 tumour stage[hazard ratio(HR)of 6.4,95%confidence interval(CI)of 2.5-16.3,P<0.001],lymph node metastasis(HR:4.5,95%CI:1.8-11.3,P=0.001),and pancreatobiliary type(HR:2.7,95%CI:1.2-6.2,P=0.025)were independent predictors of OS.CONCLUSION Extended tumour stage(T3),pancreatobiliary type,and positive lymph node metastasis represent independent predictors of a lower OS rate in South American AAC patients who underwent curative PD.

Cytomegalovirus (CMV) infection in inmunocompetent hosts generally is asymptomatic or may present as a mononucleosis syndrome but rarely can lead to severe organ complications. We report a case of simultaneous hepatic and pericardic CMV infection in a 36-year old immunocompetent man. He was admitted to coronary unit with fever, chest pain radiated to shoulders, changes on electrocardiogram with diffuse ST elevation and modest laboratory elevations in the MB fraction of creatine kinase (CK-MB) of 33.77 μg/L (0.1-6.73), serum cardiac troponin T of 0.904 ng/mL (0-0.4), creatine kinase of 454 U/L (20-195) and myoglobin of 480.4 μg/L (28-72). Routine laboratory test detected an elevation of aminotransferase level: alanine aminotransferase 1445 U/L, aspartate aminotransferase 601 U/L. We ruled out other causes of hepatitis with normal results except IgM CMV. The patient was diagnosed with myopericarditis and hepatitis caused by cytomegalovirus and started symptomatic treatment with salicylic acid. In few days the laboratory findings became normal and the patient was discharged.

Characterization and Classification of Agricultural Production Systems in the Galapagos Islands (Ecuador)

The low level of agricultural productivity within the production systems in the insular region of Galapagos is caused by the lack of sustainable technologies accordingly to the insular conservation regime. As a consequence, producers face low productivity and economic losses, which result in low levels of incomes and hasten the change from agricultural to tourist activities. With consequent abandonment of productive lands, the presence of invasive species and pathogens has become a growing threat to the biodiversity of the Galapagos National Park. For this reason, it is necessary to generate and disseminate technologies adapted to the prevalent production systems in the islands. Primary data were collected through face-to-face surveys in a sample of 208 producers from 4 islands during the period from July to November 2018. Multivariate analyses (principal components and cluster) were applied to characterize and classify typical agricultural production systems. The results show three types of systems, differentiated mainly by the technology used in the management of agricultural production, that influence the yields of crops and of animal products such as beef and milk, which cannot satisfy the demand for food on the islands, which has created the dependence on imports to cover the production gap. The definition of the types of agricultural production systems can help in identifying sustainable production alternatives that integrate local social and environmental needs. This is one of the very few technical descriptions of farms on the islands and should be used by policymakers to plan interventions.

Morphometric, Physicochemical, Thermal, and Rheological Properties of Rice (<i>Oryza sativa</i>L.) Cultivars <i>Indica ×Japonica</i>

The anther culture technical was applied to produce haploid lines of rice (Oryza sativa L.). The hybrids (K/ A92VM061, K/A92VM067, K/A92VM0611, K/A92VM719, K/A92VM720 and K/A92VM721) were obtained in order to generate new varieties from Indica and Japonica cultivars. Morphometric parameters of the grains were evaluated by image analysis. Flours were prepared from the whole rice grains and physicochemical, thermal and rheological properties, X-ray diffraction pattern and evaluation of color using the CIELAB system were assessed. The hybrids lines showed long (061, 611, 721), medium (719 and 720) and short (067) grains. The rice samples presented lipids (2.6% - 3.2%), protein (11% - 15%), total dietary fiber (8.4% - 10.2%), total starch (65% - 74%) and apparent amylose (5% - 32%) contents. Gelatinization temperature (Tp) was found in the range of 66.1℃ - 79.4℃ with enthalpy (ΔH) value between 3.4 - 8.1 J/g. The retrogradation parameters (temperature and ΔH) were lower than those for gelatinization in all samples. The rice samples presented A-type X-ray diffraction pattern. Rice pastes showed a non-Newtonian behavior and the brightness (L*) characterize the color of the samples. Hybrid rice grains presented morphometric properties more similar to Japonica than Indica variety. Rice hybrid had higher protein content than Indica variety. Apparent amylose, viscosity and gelatinization temperature varied significantly among hybrids and varieties.

Laser system design for table-top X-ray light source

We present possible conceptual designs of a laser system for driving table-top free-electron lasers based on terahertz acceleration. After discussing the achievable performances of laser amplifiers with Yb:YAG at cryogenic and room temperature and Yb:YLF at cryogenic temperature, we present amplification modules with available results and concepts of amplifier chains based on these laser media. Their performances are discussed in light of the specifications for the tasks within the table-top light source. Technical and engineering challenges, such as cooling, control, synchronization and diagnostics, are outlined. Three concepts for the laser layout feeding the accelerator are eventually derived and presented.
