Electrocardiographic changes during induced therapeutic hypothermia in comatose survivors after cardiac arrest

AIM: To assess the safety of therapeutic hypothermia(TH) concerning arrhythmias we analyzed serial electrocardiograms(ECG) during ***: All patients recovered from a cardiac arrest with Glasgow ssion were treated with induced mild TH to 32-34℃. TH was obtained with cool fluid infusion or a specific intravascular device. Twelvelead ECG before,during,and after TH,as well as ECG telemetry data was recorded in all patients. From a total of 54 patients admitted with cardiac arrest during the study period,47 patients had the 3 ECG and telemetry data available. ECG analysis was blinded and performed with manual caliper by two independent cardiologists from blinded copies of original ECG,recorded at 25 mm/s and 10 mm/m V. Coronary care unit staff analyzed ECG telemetry for rhythm disturbances. Variables measured in ECG were rhythm,RR,PR,QT and corrected QT(QTc by Bazett formula,measured in lead v2) intervals,QRs duration,presence of osborn's J wave and U wave,as well as sT segment displacement and T wave amplitude in leads Ⅱ,v2 and ***: Heart rate went down an average of 19 bpm during hypothermia and increased again 16 bpm with rewarming(P oth). There was a nonsignificant prolongation of the PR interval during TH and a significant decrease with rewarming(P = 0.041). QRs duration significantly prolonged(P = 0.041) with TH and shortened back(P sented a mean prolongation of 58 ms(P significant shortening with rewarming of 22.2 ms(P = 0.017). osborn or J wave was found in 21.3% of the patients. New arrhythmias occurred in 38.3% of the patients. Most frequent arrhythmia was non-sustained ventricular tachycardia(19.1%),followed by severe bradycardia or paced rhythm(10.6%),accelerated nodal rhythm(8.5%) and atrial fibrillation(6.4%). No life threatening arrhythmias(sustained ventricular tachycardia,polymorphic ventricular tachycardia or ventricular fibrillation) occurred during TH.

Dynamical Donwscaling for Railroad Areas in Eastern Amazon and southeastern Brazil:Current Climate and Near-Future Projections

We performed a dynamic downscaling using REGCM4 regional model driven by MPI global model for current (1990/2012) and near-future (2015/2039) climate in order to characterize the seasonal rainfall regimes throughout the railroad areas in eastern Amazon and southeastern Brazil. The analysis of observational data for the current climate indicated the existence of pronounced spatial variations in rainfall regime across railroad regions during both the rainy and dry seasons. Although models have presented generalized underestimation, the regional model showed improvements on spatial representation and intensity of the rainfall in comparison with global model results. We reported the future projections taking into account the correction of simulated rainfall by the values of the biases found in each respective seasonal regime, so that the results are expressed by percentage changes of the future (2015/2037) relative to the current climate patterns. For the railroad in eastern Amazon, projections indicate a weak decrease of rainfall of about -15% in the rainy season (January to May), however during the dry season (June to october) are expected drastic reductions between -70% and -90% in south (Carajás in Pará state) and north (sao luis in Maranhao state) portions. Conversely, for the railroad in southeast Brazil, model projections point out for an increased rainfall regime during the rainy season (october to February) around +30% to +40% in the east part of the region over the Espírito santo state.

sintering of Pellets, Compound by Fly Ash, Clay and Charcoal in a Fixed Bed Combustion Reactor

Normally, industries in general, produce waste at its majority toxic, such as fly ash, for example, which damage the environment. The aim of this paper is to investigate the evolution of the temperature in a combustion reactor, full of pellets manufactured with fly ash, clay and charcoal powder, after sintering, to obtainment synthetic aggregates for use in civil construction. The pellets were produced in a cement mix. For realization of the work, a co-current combustion reactor was made in order to analyze the temperatures profile and investigate if the values of these temperatures would be sufficient to initiate the process of sintering of the pellets. Temperatures reached in the reactor varied in the range of 800 ℃-1,290 ℃. These values are sufficient to initiate the process of sintering of the pellets. For the experiment realized, parameters such as inlet velocity of the fluid （air）, diameters of the pellets and size of charcoal crushed in a disk mill were varied and the effect of variations of these parameters were analyzed for the experiment. The historical temperatures were recorded by a data acquisition instrument and subsequently plotted for analysis.

AIM: To investigate the clinical impact of capsule endoscopy(CE) after an obscure gastrointestinal bleeding(oGIB) episode, focusing on diagnostic work-up, followup and predictive factors of rebleeding. METHoDs: Patients who were referred to Hospital del Mar(Barcelona, spain) between 2007 and 2009 for oGIB who underwent a CE were retrospectively analyzed. Demographic data, current treatment with non-steroid antiinflammtory drugs or anticoagulant drugs, hemoglobin levels, transfusion requirements, previous diagnostic tests for the bleeding episode, as well as CE findings(significant or non-significant), work-up and patient out-comes were analyzed from electronic charts. Variables were compared by χ 2 analysis and student t test. Risk factors of rebleeding were assessed by Log-rank test, Kaplan-Meier curves and Cox regression model. REsULTs: There were 105 patients [45.7% women, median age of 72 years old(interquartile range 56-79)] and a median follow-up of 326 d(interquartile range 123-641) included in this study. The overall diagnostic yield of CE was 58.1%(55.2% and 63.2%, for patients with occult oGIB and overt oGIB, respectively). In 73 patients(69.5%), oGIB was resolved. Multivariate analysis showed that hemoglobin levels lower than 8 g/dL at diagnosis [hazard ratios(HR) = 2.7, 95%CI: 1.9-6.3], patients aged 70 years and above(HR = 2.1, 95%CI: 1.2-6.1) and significant findings in CE(HR = 2.4, 95%CI: 1.1-5.8) were independent predictors of rebleeding. CoNCLUsIoN: one third of the patients presented with rebleeding after CE; risk factors were hemoglobin levels s or the presence of significant lesions.

Hyperglycemia promotes overexpression of sR-BII isoform of the scavenger receptor class B type I in type 2 diabetes mellitus: A study in Juana Koslay City, san luis, Argentina

The scavenger receptor class B type I (sR-BI) is a high-density lipoprotein (HDL) receptor involved in reverse cholesterol transport. some studies reported the association to be stronger in the presence of diabetes. The full length gene encoding sR-BI is comprised in 13 exons that are alternatively spliced to produce two major transcripts: the full length sR-BI and the splice variant sR-BII, in which exon 12 is skipped. Considering that type 2 diabetes status is characterized by changes in the concentration of plasma lipids, modifications in lipoprotein size and composition, which may be important modulators of the sR-BI expression;the aims of the study were to examine the influence of sR-BI polymorphism (rs838895) on lipid profile and sR-BI mRNA expression in a population of diabetic patients living in Juana Koslay City. Blood samples were drawn from controls (n = 40) and Type 2 diabetic patients (n = 66) and DNA and total RNA were obtained. sR-BI mRNA expression was measured by RT-PCR and sR-BI polymorphism was detected by Tetra Primer ARMsPCR. Compared to controls, diabetic patients had higher fasting serum glucose, glycated hemoglobin, triglycerides, total cholesterol, lowdensity lipoprotein cholesterol, and lower highdensity lipoprotein cholesterol. sR-BI mRNA expression was lower in T2DM when compared to controls, suggesting that the hyperglycemia presents in T2DM patients down-regulates sR-BI mRNA expression. Interestingly, we found that decreased sR-BI expression resulted in markedly increased plasma LDL concentrations in T2DM subjects, and the overexpression of sRBII isoform is responsible for the markedly increased plasma LDL-c concentrations. The polymorphism (rs838895) did not modify the mRNA level of sR-BI in leucocytes from control and diabetic patients. This study provides novel evidence suggesting that hyperglycemia may affect reverse cholesterol transport by controlling sRBI expression in diabetic patients. As climatic changes and human uses intensify,resource managers and other decision makers are taking actions to either avoid or respond to ecosystem tipping points,or dramatic shifts in structure and function that are often costly and hard to *** indicates that explicitly addressing tipping points leads to improved management *** on theory and examples from marine systems,we distill a set of seven principles to guide effective management in ecosystems with tipping points,derived from the best available *** principles are based on observations that tipping points(1)are possible everywhere,(2)are associated with intense and/or multifaceted human use,(3)may be preceded by changes in earlywarning indicators,(4)may redistribute benefits among stakeholders,(5)affect the relative costs of action and inaction,(6)suggest biologically informed management targets,and(7)often require an adaptive response to *** suggest that early action to preserve system resilience is likely more practical,affordable,and effective than late action to halt or reverse a tipping *** articulate a conceptual approach to management focused on linking management targets to thresholds,tracking early-warning signals of ecosystem instability,and stepping up investment in monitoring and mitigation as the likelihood of dramatic ecosystem change *** approach can simplify and economize management by allowing decision makers to capitalize on the increasing value of precise information about threshold relationships when a system is closer to tipping or by ensuring that restoration effort is sufficient to tip a system into the desired regime.
