Hepatic encephalopathy(HE) remains a diagnosis of exclusion due to the lack of specific signs and symptoms. Refractory HE is an uncommon but serious condition that requires the search of hidden precipitating events(i.e.,portosystemic shunt) and alternative diagnosis. Hypothyroidism shares clinical manifestations with HE and is usually considered within the differential diagnosis of HE. Here,we describe a patient with refractory HE who presented a large portosystemic shunt and post-ablative hypothyroidism. Her cognitive impairment,hyperammonaemia,electroencephalograph alterations,impaired neuropsychological performance,and magnetic resonance imaging and spectroscopy disturbances were highly suggestive of HE,paralleled the course of hypothyroidism and normalized after thyroid hormone replacement. There was no need for intervention over the portosystemic shunt. The case findings support that hypothyroidism may precipitate HE in cirrhotic patients by inducing hyperammonaemia and/or enhancing ammonia brain toxicity. This case led us to consider hypothyroidism not only in the differential diagnosis but also as a precipitating factor of HE.

Benthic Diatom Communities of a Large Mediterranean River under the Influence of a Thermal Effluent

The influence of a thermal discharge caused by the cooling system of a nuclear power station on benthic diatom communities was assessed at the lower Ebro River (in Spain), and the information generated could be useful to understand the effects of increasing temperature on large Mediterranean rivers. Surveys conducted at sites before and after the effluent and collected from natural and artificial substrate were analyzed and, Non-metrical Multidimensional Scaling (NMDS), Similarity Percentage Analysis (SIMPER) and 1-way Analysis of Similarities (ANOSIM) were performed to assess changes in community structure. The relationship between diatom assemblages and environmental variables was assessed with a multivariate distance-based linear regression model (DISTLM) and the model was visualized through a redundancy analysis (dbRDA). NMDS ordination was obtained with a stress of 0.18 and 0.17 for natural and artificial substrates, respectively. ANOSIM showed significant differences between Control and Impacted sites (p 0.05). Simper analysis showed that the mean dissimilarity between Control and Impacted sites was of 42.22% for natural substrate and of 39.97% for artificial substrate. DISTLM selected a set of explanatory variables (dissolved oxygen, Tº?difference, total phosphorus, pH and chlorophyll) with a 67.24% of fitted variation. Diatoms showed sensitivity to thermal changes, even though when these did not exceed 3℃. The factors that seemed to influence benthic assemblages the most were seasonal variation and the thermal increase caused by the nuclear power station.

Purpose:This study was aimed to analyze the mediation role of cardiorespiratory fitness(CRF)on the association between fatness and cardiometabolic risk scores(CMRs)in European ***:A cross-sectional study was conducted in adolescents(n=525;46%boys;14.1±1.1 years old,mean ±SD)from 10 European cities involved in the Healthy Lifestyle in Europe by Nutrition in Adolescence *** was measured by means of the shuttle run test,while fatness measures included body mass index(BMI),waist to height ratio,and fat mass index estimated from skinfold thicknesses.A clustered CMRs was computed by summing the standardized values of homeostasis model assessment,systolic blood pressure,triglycerides,total cholesterol/highdensity lipoprotein cholesterol ratio,and ***:Linear regression models indicated that CRF acted as an important and partial mediator in the association between fatness and CMRs in 1217-year-old adolescents(for BMI:coefficients of the indirect role b=0.058(95%confidence interval(95%CI):0.023-0.101),Sobel test z=3.11(10.0%mediation);for waist to height ratio:b=4.279(95%CI:2.242-7.059),z=3.86(11.5%mediation);and for fat mass index:b=0.060(95%CI:0.020-0.106),z=2.85(9.4%mediation);all p<0.01).Conclusion:In adolescents,the association between fatness and CMRs could be partially decreased with improvements to fitness levels;therefore,CRF contribution both in the clinical field and public health could be important to consider and promote in adolescents independently of their fatness levels.

The effects of a cyanobacterial extract （CE） on Helicobacter pylori biofilm formation onto hydrophobic and hydrophilic abiotic surfaces and the expression of luxS, flaA, omp18, lpxD and ureA genes associated to biofilm were studied. NCTC11638 reference strain and HP796, a resistant clinical isolate, were grown in Mueller-Hinton broth supplemented with 5% fetal calf serum （FCS） or 1% CE. The ability to form biofilm, viability, morphological changes and gene expression of adhered H. pylori cells were determined. The strains were able to form biofilm on both surfaces with the nutritional supplements analyzed. H. pylori conserved a characteristic bacillary morphology and viability with CE. Cells attachment was higher with CE than FCS regardless of strains and surfaces. The most remarkable increase in gene expression was observed with the ompl8 gene using the CE supplement, indicating the important participation of outer membrane proteins in biofilm establishment. The clinical isolate showed similar and even greater gene expression than the reference strain. The results obtained indicated that the nutrients provided by CE favored biofilm formation with retained pathogenicity that under certain conditions can occur in natural aquatic environments.
