Background The positive influence of most types of exercise has been reported repeatedly,but what the most effective exercise approaches are for improving health-related quality of life(HRQoL)in people with cancer remains unknown.The aim of this systematic review and network meta-analysis was to synthesize the evidence from intervention studies to assess the effects of different types of exercise on HRQoL during and after cancer treatment.Methods MEDLINE,SPORTDiscus,the Cochrane Library,Web of Science,and Scopus were searched for randomized controlled trials aimed at testing the effects of exercise interventions meant to improve HRQoL in people with cancer.Separate analyses were conducted for HRQoL as measured by general and cancer-specific questionnaires.We also evaluated whether the effects of exercise were different during and after cancer treatment in both the physical and mental HRQoL domains.Results In total,93 studies involving 7435 people with cancer were included.Network effect size estimates comparing exercise intervention vs.usual care were significant for combined exercise(0.35,95%confidence interval(95%CI):0.14–0.56)for HRQoL as measured by general questionnaires,and for combined(0.31,95%CI:0.13–0.48),mind–body exercise(0.54,95%CI:0.18–0.89),and walking(0.39,95%CI:0.04–0.74)for HRQoL as measured by cancer-specific questionnaires.Conclusion Exercise programs combining aerobic and resistance training can be recommended to improve HRQoL during and after cancer treatment.The scarcity and heterogeneity of these studies prevents us from making recommendations about other exercise modalities due to insufficient evidence.

Applications of finite element simulation in orthopedic and trauma surgery

Research in different areas of orthopedic and trauma surgery requires a methodology that allows both a more economic approach and the ability to reproduce different situations in an easy way. Simulation models have been introduced recently in bioengineering and could become an essential tool in the study of any physiological unity, regardless of its complexity. The main problem in modeling with finite elements simulation is to achieve an accurate reproduction of the anatomy and a perfect correlation of the different structures, in any region of the human body. Authors have developed a mixed technique, joining the use of a three-dimensional laser scanner Roland Picza captured together with computed tomography(CT) and 3D CT images, to achieve a perfect reproduction of the anatomy. Finite element(FE) simulation lets us know the biomechanical changes that take place after hipprostheses or osteosynthesis implantation and biological responses of bone to biomechanical changes. The simulation models are able to predict changes in bone stress distribution around the implant, so allowing preventing future pathologies. The development of a FE model of lumbar spine is another interesting application of the simulation. The model allows research on the lumbar spine, not only in physiological conditions but also simulating different load conditions, to assess the impact on biomechanics. Different degrees of disc degeneration can also be simulated to determine the impact on adjacent anatomical elements. Finally, FE models may be useful to test different fixation systems, i.e., pedicular screws, interbody devices or rigid fixations compared with the dynamic ones. We have also developed models of lumbar spine and hip joint to predict the occurrence of osteoporotic fractures, based on densitometric determinations and specific biomechanical models, including approaches from damage and fracture mechanics. Background Pediatric cancer survivors are at increased risk of muscle weakness and low areal bone mineral density(aBMD).However,the prevalence of muscle strength deficits is not well documented,and the associations of muscle strength with aBMD are unknown in this population.Therefore,this study aimed to investigate the prevalence of upper-and lower-body muscle strength deficits and to examine the associations of upper-and lower-body muscle strength with age-,sex,and race-specific aBMD Z-scores at the total body,total hip,femoral neck,and lumbar spine.Methods This cross-sectional study included 116 pediatric cancer survivors(12.1±3.3 years old,mean±SD;42.2%female).Upper-and lower-body muscle strength were assessed by handgrip and standing long jump test,respectively.Dual‑energy X‑ray absorptiometry was used to measure aBMD(g/cm2).Associations between muscle strength and aBMD were evaluated in multivariable linear regression models.Logistic regression was used to evaluate the contribution of muscle strength(1-decile lower)to the odds of having low aBMD(Z-score≤1.0).All analyses were adjusted for time from treatment completion,radiotherapy exposure,and body mass index.Results More than one-half of survivors were within the 2 lowest deciles for upper-(56.9%)and lower-body muscle strength(60.0%)in comparison to age-and sex-specific reference values.Muscle strength deficits were associated with lower aBMD Z-scores at all sites(B=0.133–0.258,p=0.001–0.032).Each 1-decile lower in upper-body muscle strength was associated with 30%–95%higher odds of having low aBMD Z-scores at all sites.Each 1-decile lower in lower-body muscle strength was associated with 35%–70%higher odds of having low aBMD Z-scores at total body,total hip,and femoral neck.Conclusion Muscle strength deficits are prevalent in young pediatric cancer survivors,and such deficits are associated with lower aBMD Z-scores at all sites.These results suggest that interventions designed to improve muscle strength in this vulnerable population may have the added ben

Wheat grain quality characteristics have experienced increasing attention as a central factor affecting wheat end-use products quality and human health.Nonetheless,in the last decades a reduction in grain quality has been observed.Therefore,it is central to develop efficient quality-related phenotyping tools.In this sense,one of the most relevant wheat features related to grain quality traits is grain nitrogen content,which is directly linked to grain protein content and monitorable with remote sensing approaches.Moreover,the relation between nitrogen fertilization and grain nitrogen content(protein)plays a central role in the sustainability of agriculture.Both aiming to develop efficient phenotyping tools using remote sensing instruments and to advance towards a field-level efficient and sustainable monitoring of grain nitrogen status,this paper studies the efficacy of various sensors,multispectral and visible red-greenblue(RGB),at different scales,ground and unmanned aerial vehicle(UAV),and phenological stages(anthesis and grain filling)to estimate grain nitrogen content.Linear models were calculated using vegetation indices at each sensing level,sensor type and phenological stage.Furthermore,this study explores the up-scalability of the best performing model to satellite level Sentinel-2 equivalent data.We found that models built at the phenological stage of anthesis with UAV-level multispectral cameras using red-edge bands outperformed grain nitrogen content estimation(R2=0.42,RMSE=0.18%)in comparison with those models built with RGB imagery at ground and aerial level,as well as with those built with widely used ground-level multispectral sensors.We also demonstrated the possibility to use UAV-built multispectral linear models at the satellite scale to determine grain nitrogen content effectively(R2=0.40,RMSE=0.29%)at actual wheat fields.

Soccer is a very popular team sport that involves a variety of physical actions,such as sprints,accelerations,sudden decelera-tions,and jumps,among others.Research has placed recreational soccer in a good position as a strategy to increase physical activity levels and promote health across the lifespan.In this regard,recreational(non-competitive)soccer promotes social interaction and has the potential to increase long-term adherence to physical activity,even in those without prior experience in this sport.In this narrative review,we address the effects of recreational soccer on various health outcomes across the lifespan(i.e.,from childhood to the elderly years)in both sexes.In addition,the beneficial effects of recreational soccer for people with prevalent pathologies,such as hypertension,type II diabetes mellitus,and cancer,are discussed.

High-throughput crop phenotyping,particularly under field conditions,is nowadays perceived as a key factor limiting crop genetic advance.Phenotyping not only facilitates conventional breeding,but it is necessary to fully exploit the capabilities of molecular breeding,and it can be exploited to predict breeding targets for the years ahead at the regional level through more advanced simulation models and decision support systems.In terms of phenotyping,it is necessary to determined which selection traits are relevant in each situation,and which phenotyping tools/methods are available to assess such traits.Remote sensing methodologies are currently the most popular approaches,even when lab-based analyses are still relevant in many circumstances.On top of that,data processing and automation,together with machine learning/deep learning are contributing to the wide range of applications for phenotyping.This review addresses spectral and red-green-blue sensing as the most popular remote sensing approaches,alongside stable isotope composition as an example of a lab-based tool,and root phenotyping,which represents one of the frontiers for field phenotyping.Further,we consider the two most promising forms of aerial platforms(unmanned aerial vehicle and satellites)and some of the emerging data-processing techniques.The review includes three Boxes that examine specific case studies.

Objective To determine whether topical applications of thiosulfinate-enriched Allium sativum extract(TASE)can accelerate acute cutaneous wound healing(WH)in a murine model.Methods Keratinocyte viability and in vitro wound closure were assessed in keratinocyte cultures.Effects of topical TASE(0.5μg/mL of allicin in 97%ethanol)on acute cutaneous WH were determined in a murine model of acute cutaneous wound.Twelve mice were alternately assigned to the vehicle-and TASE-treated groups(n=6 per group).Expression levels of mRNA for keratinocyte differentiation marker-related proteins(filaggrin,loricrin and involucrin)and lipid synthetic enzymes(elongation of very long chain fatty acids protein 4(ELOVL4),fatty acid synthase(FA2H),3-hydroxy-3-methyl-glutaryl-coenzyme A reductase(HMGCoA),and serine palmitoyltransferase(SPT))were assessed using real-time quantitative polymerase chain reaction on day 3 and 8 after wounding,while transepidermal water loss(TEWL)rates were measured in wounded areas.Results TASE accelerated WH both in vivo(40%vs.22%reduction in wound area,P<0.01)and in vitro(90%vs.65%reduction in wound area,P<0.01).Moreover,topical applications of TASE upregulated the expression levels of epidermal mRNA for ELOVL4,HMGCoA,SPT,filaggrin,loricrin and involucrin(P<0.05 vs.vehicle-treated controls)on day 3 after wounding.Likewise,TASE significantly lowered TEWL rates in comparison with vehicle alone on day 8(33.06±2.09 g/(m^2·h)vs.24.60±2.04 g/(m^2·h),P<0.01).Conclusions Topical applications of TASE stimulated keratinocyte proliferation and formation of epidermal permeability barrier function,leading to acceleration of acute cutaneous WH.Topical products containing TASE could be used to manage acute cutaneous WH.

Osteoporosis in men is a heterogeneous disease that has received little attention.However,one third of worldwide hip fractures occur in the male population.This problem is more prevalent in people over70 years of age.The etiology can be idiopathic or secondary to hypogonadism,vitamin D deficiency and inadequate calcium intake,hormonal treatments for prostate cancer,use of toxic and every disease or drug use that alters bone metabolism.Risk factors such as a previous history of fragility fracture should be assessed for the diagnosis.However,risk factors in men are very heterogeneous.There are significant differences in the pharmacological treatment of osteoporosis between men and women fundamentally due to the level of evidence in published trials supporting each treatment.New treatments will offer new therapeutic prospects.The goal of this work is a revision of the present status knowledge about male osteoporosis.

Instability of the lumbar spine due to disc degeneration. A finite element simulation

Low back pain, which most common cause is degenerative disc disease, currently represents a serious problem due to its socio-economic repercussions. Different factors are involved in the degenerative process, being the most common: the loads on the spine, repeated flexion movements and individual genetic characteristics. Nowadays, there is no agreement on whether instability is the main cause of low back pain, or only one reason among many, and on whether increased spinal mobility, associated with instability, occurs either locally or at the lumbar spine as a whole. In this work, the simulation of disc degeneration is based on a finite element model of lumbar spine. A parametric study based on mechanical properties was established, for each lumbar spine movement, by evaluating the disc degeneration in 10% steps, from healthy disc to maximum degeneration. The results show as general trend a progressive mobility increase as the disc degeneration level raises. As main conclusion, disc degeneration causes increased mobility at all vertebral levels, with moderate values for incipient degeneration and much higher values for advanced degeneration, affecting more severely to the levels closest to degenerated disk. The great mobility increase detected at L5 could explain the instability detected as a clinical symptom.

Probability of Osteoporotic Vertebral Fractures Assessment Based on DXA Measurements and Finite Element Simulation

Osteoporotic vertebral fractures represent major cause of disability, loss of quality of life and even mortality among the elderly population. Decisions on drug therapy are based on the assessment of risk factors for fracture, from bone mineral density measurements. The combination of biomechanical models with clinical studies could better estimate bone strength and support the specialists in their decision. A model to assess the probability of fracture, based on the Damage and Fracture Mechanics has been developed, evaluating the mechanical magnitudes involved in the fracture process from clinical bone mineral density measurements. The model is intended for simulating the degenerative process in the skeleton, with the consequent lost of bone mass and hence the decrease of its mechanical resistance which enables the fracture due to different traumatisms. Clinical studies were chosen, both in non-treatment conditions and receiving drug therapy, and fitted to specific patients according their actual bone mineral density measures. The predictive model is applied in a finite element simulation of the lumbar spine. The fracture zone would be determined according loading scenario (fall, impact, accidental loads, etc.), using the mechanical properties of bone obtained from the evolutionary model corresponding to the considered time. Bone mineral density evolution in untreated patients and in those under different treatments was analyzed. Evolutionary curves of fracture probability were obtained from the evolution of mechanical damage. The evolutionary curve of the untreated group of patients presented a marked increase of the fracture probability, while the curves of patients under drug treatment showed variable decreased risks, depending on the therapy type. The finite element model allowed obtaining detailed maps of damage and fracture probability, identifying high-risk local zones at vertebral body, which are the usual localization of osteoporotic vertebral fractures.
