背景:本研究旨在评估智利南部Temuco地区成人胃食管反流病(GERD)患病率,并分析与GERD相关的人口统计学因素。方法:本研究开展于2017年,采用一种已被证明有效的GERD问卷,对智利南部Temuco地区的普通人群进行横断面调查。Temuco城区有245,317名居民,城区分为四个区域,代表着不同的社会经济水平。按52.8%的GERD患病率、3.0%的准确率、95.0%的可信区间及1.15的设计效应估算所需样本量。通过地区抽样建立组群,计算GERD患病率,并通过两变量及多因素分析评估其影响因素。结果:从四个区域共计纳入了1,069名研究对象(女性占47.9%,中位年龄40岁)进行问卷调查。GERD患病率为44.8%。最常诉及的症状是反流。1/3的研究对象采用了药物来控制症状,按照问卷评估,这些人均被认为患有GERD,尽管其中68%的人从未就医。GERD的发生与年龄(P<0.001)和性别显著相关(P=0.001)。结论:在本项基于人群的研究中,GERD患病率高达44.8%,其发生与年龄和性别有关。

X-ray diffractograms, optic and electronic microscopy were used to study the structural changes on extruded orange pulp using a Brabender® laboratory single-screw extruder (GNF 1014/2, 20:1, L/D). The results showed that most of the cellulose would be in the amorphous state, with small crystalline areas in the angular region of 2θ = 14.5°. The evaluated extrusion conditions did not affect the crystallinity of the raw cellulose, maintaining the size of the crystalline regions in the angular region of 2θ = 22.5°. Neutral detergent solubilized the crystalline areas of potassium citrate and cellulose, formed during the extrusion process. However, these conditions did not affect the crystallinity of the raw cellulose. In addition, no change was observed in the crystallinity of pectin. The electron microphotographs allowed for the estimation of the heterogeneity of orange pulp and assessment of the difference of resistance between the walls of the buds and juice vesicles. Thus, it was demonstrated that during the extrusion process most of the porosity and the cellular structure of the endocarp remained unaltered.

Biliary tract cancer, or cholangiocarcinoma, comprises a heterogeneous group of malignant tumors that can emerge at any part of the biliary tree. This group is the second most common type of primary liver cancer. Diagnosis is usually based on symptoms, which may be heterogeneous, and nonspecific biomarkers in serum and biopsy specimens, as well as on imaging techniques. Endoscopy-based diagnosis is essential, since it enables biopsy specimens to be taken. In addition, it can help with locoregional staging of distal tumors. Endoscopic retrograde cholangiopancreatography is a key technique for the evaluation and treatment of malignant biliary tumors. Correct staging of cholangiocarcinoma is essential in order to be able to determine the degree of resectability and assess the results of treatment. The tumor is staged based on the TNM classification of the American Joint Committee on Cancer. The approach will depend on the classification of the tumor. Thus, some patients with early-stage disease could benefit from surgery;complete surgical resection is the cornerstone of cure. However, only a minority of patients are diagnosed in the early stages and are suitable candidates for resection. In the subset of patients diagnosed with locally advanced or metastatic disease, chemotherapy has been used to improve outcome and to delay tumor progression. The approach to biliary tract tumors should be multidisciplinary, involving experienced endoscopists, oncologists, radiologists, and surgeons.

AIM：To compare the effect of phacoemulsification on intraocular pressure（IOP）in patients with primary open angle glaucoma（POAG）and pseudoexfoliation glaucoma（PXG）.METHODS：A retrospective comparative case series conducted at the Glaucoma Department at the Association to Prevent Blindness in Mexico.The study enrolled consecutive patients having phacoemulsification with intraocular lens（IOL）implantation and a diagnosis of POAG or PXG.Data about IOP values and number of glaucoma medications used was collected at baseline,1,3,6 and12mo postoperatively.RESULTS：The study enrolled 88 patients（88 eyes）.After phacoemulsification,there was a statistically significant reduction in IOP values and glaucoma medications use compared to baseline in both POAG and PXG patients（P〈0.001）.In the POAG group,a 20%decrease in IOP values was evidenced,and a 56.5%reduction in the number of medications used at the one-year follow-up.The PXG group showed a 20.39%,and a 34.46%decrease in IOP and number of medications used,respectively.A significant difference in the meanΔIOP（postoperative changes in IOP）was evidenced between groups（P=0.005）.The reduction of the postsurgical IOP mean values in both groups,the POAG group showed a greater reduction in IOP values compared to the PXG group.CONCLUSION：In both types of glaucoma,phacoemulsification cataract surgery can result in a significant IOP reduction（20%）over a 12mo follow-up period.The number of medications used is also significantly reduced up to 12mo after surgery,especially in the PXG group.

Disinfection of Primary Municipal Wastewater Effluents Using Continuous UV and Ozone Treatment

UV radiation and ozonation were investigated as disinfection alternatives for the wastewater treatment plant. The inactivation of total and fecal coliforms using ozone and ultraviolet radiation as separate treatments was evaluated. Different ozone concentrations (3 to 40 mg O3/L) were applied and UV fluencies ranging from 8.5 to 12 mJ/cm2 at different pH values (from 5 to 9) were tested. Best results were obtained for ozone doses near 20 mg/min with removals of 72% and 78% of fecal and total coliforms, respectively. The ozone also was capable of oxidizing organic matter in the effluent measured as COD (the highest removal obtained was 36% for 20 mg O3/min). Maximum bacterial resistance was observed at pH 7 in both cases. The UV light offered a high bacterial inactivation (over 80%) and the lowest bacterial inactivation was observed at pH 7. Finally, we obtained the electric energy per order (EEO, kWh/m3/order), defined as the electric energy (kW-h) required to degrade a contaminant by one order of magnitude in a unit volume of contaminated water, being noteworthy that EEO values for the UV process resulted were lower than those determined for the process with ozone in all the water flow tested.

Proteus mirabilis, a motile Gram-negative bacterium, represents a common cause of complicated urinary tract infections. Autotransporters are a family of secreted proteins from Gram-negative bacteria that direct their own secretion across the outer membrane (type V autotransporter secretion mechanism). Serine protease autotransporters of Enterobacteriaceae (SPATEs) include adhesins, toxins, and proteases that can contribute to the virulence. Plasmid-encoded toxin (Pet) is the predominant protein in culture supernatants of enteroaggregative E. coli prototype strain 042 and has been extensively studied. Pet toxin is encoded on the 65-MDa adherence-related plasmid of EAEC 042 strain. In this work, Pet protein was found in the supernatant obtained from Proteus mirabilis RTX339 strain isolated from a psychiatric patient suffering complicated urinary tract infections (UTIs). The nucleotide sequence of pet gene was obtained using primers designed from E. coli 042 pet gene reported. The alignment of the sequence showed 100% identity with the pet gene reported. Is important to note that Proteus mirabilis RTX339 pet gene has chromosomal location. The chromosomal location of the gene was established since no plasmids were harbored by this strain.

Five studies were conducted in order to produce less waste and economize the application of a CdCl2–Na3C6H5O7– KOH–H3BO3/NaOH/KCl–(NH2)2CS definite aqueous system in the chemical bath deposition of CdS thin films, as well as to generate supporting information helpful to explain the effects of varying certain parameters established for the use of this formulation. These studies are related to the utility of the pH 10 borate buffer, to the selection of the appropriateOH–concentration, commercial alkali, and reaction temperature, and to the reusing of the residual solution. These studies were conducted by means of optical characterization, pH measurements, energy consumption tests, and chemical analysis;and all possible implications were analyzed in detail and widely supported by the literature. It is observed that the addition of 5.0 mL of pH 10 borate buffer in the reaction solutions only causes a slight reduction in the pH value and, therefore, in the thickness of the resulted films. It is observed that a decrease in CdCl2 concentration causes an increase in the pH values, and when such reagent concentration is varied it is necessary to determine the pH value at which the best CdS films can be obtained;this indicates the existence of an optimum pH of deposition, depending on the used reagent concentrations. It is demonstrated that the use of KOH and NaOH is interchangeable, and the advantages of NaOH are evaluated in the context of hygroscopicity, CO2 absorption, purity, and price. It is observed that the energy consumption when depositing a determined film is lower at high temperatures, and it is concluded that the time and energy savings at high temperatures are undoubtedly advantages in the deposition process of CdS thin films. A formulation to recycle the residual solution is presented, which is very important for the reducing of residual volume, and, therefore, in the minimizing of the environmental impact.
