Nonalcoholic fatty liver disease(NAFLD)has recently become an emerging health problem worldwide(1).The pathogenetic mechanisms involved in the development and progression of NAFLD are due to genetic predisposition that expresses a metabolic profile associated with high energy food intake(2).The most accurate estimate of the worldwide prevalence of NAFLD is 24–25%of the general population,and ranges from 5–18%in Asia to 20–30%in the Western countries(1,3).Nowadays,the reason for this variability is not clear ***,it is plausible that genetic factors could play a major role in pathogenesis and the advances in genomics,transcriptomics,and proteomics have highlighted new pathogenic *** fact,increasing literature data support the role of single nucleotide polymorphisms(SNPs),and in particular the SNPs of genes involved in insulin signaling,lipid homeostasis,and oxidative stress,not only in the susceptibility to develop NAFLD,but also in the severity of liver damage and in the etiology of multisystemic metabolic disorders(4).The development of the Genome Wide Association Study technology has allowed the identification of many SNPs involved in the onset of NAFLD,since they can change the stages of development,the rate of progression,and the efficacy of treatment(5).Recently,Tricòet *** the clinical and genetic features associated with pediatric NAFLD in a prospective study in a large multiethnic cohort of obese adolescents(6).A total of 503 subjects,identified as"The Yale Pediatric NAFLD cohort",were enrolled,including 191(38.0%)Caucasians,134(26.6%)African Americans,and 178(35.4%)Hispanics.

We have read with a great interest the review published by Singh et al, on the treatment options in alcoholic and non-alcoholic fatty liver disease, including various new targeted therapies that are currently under investigation. Recently, we described the health effects of the Mediterranean diet associated to an antioxidant complex rich in silymarin, to improve in overweight patients anthropometric parameters, glucose and lipid metabolism and intra-hepatic fat accumulation.

Acupuncture accelerates recovery after general anesthesia: a prospective randomized controlled trial

BACKGRoUND： Acupuncture anesthesia was created in the 1950＇s in China and continues to be used there today during most major surgeries. It is widely used in China for such complex operations as brain heart, and abdominal surgery. It is popular in China because it is economical, practical, and beneficial to the patients. With acupuncture anesthesia there is less bleeding during surgery and there is also quicker post-operative recovery. oBJECTIVE： This randomized prospective study aims at comparing the effect of two acupoints （Yongquan, KI1 and Renzhong, DU26） with sham acupuncture and no acupuncture on the time to recovery of consciousness after general anesthesia by means of the Bispectral Index monitor （BIS） deSIGN, SETTING, PARTICIPANTS AND INTERVENTIoNS： This is a prospective randomized controlled study. We randomly assigned 50 patients to 5 groups during recovery from surgical anesthesia. Four groups had acupuncture on KI1 （group A）, DU26 （groups B）, both KI1 and DU26 （group C）, and sham points （group D）, and one had no acupuncture （group E）. MAIN oUTCoME MEASURES： Bispectral Index （BIS）, time to spontaneous eye opening, time to tracheal extubation, and time to following commands were measured as the main outcome measures RESULTS： Time to spontaneous eye opening differed among groups （P=0.002）, as well as time to tracheal extubation （P〈0.000 1） and time to following commands （P=0.000 6）. BIS values differed significantly among groups both 5 and 10 min after the end of anesthesia （P〈0.000 1 and P=0.000 4, respectively）. BIS values of groups D and E were lower than those of the other groups and those of group C were higher. The same pattern was observed also 15 and 30 rain after the end of anesthesia although the difference among groups was not significant at these time points （P=0.164 and P=0.104 respectively）. Metastatic gastric cancer remains an incurable disease,with a relative 5-year survival rate of 7%-27%.Chemotherapy,which improves overall survival(oS) and quality of life,is the main treatment *** has demonstrated that the best survival results obtained in earlier randomized studies were achieved with three-drug regimens containing a fluoropyrimidine,an anthracycline,and cisplatin(ECF).Although there has been little progress in improving median oS times beyond the 9-mo plateau achievable with the standard regimens,the availability of newer agents has provided some measure of optimism.A number of new combinations incorporating docetaxel,oxaliplatin,capecitabine,and S-1 have been explored in randomized *** combinations,such as epirubicin-oxaliplatin-capecitabine,have been shown to be as effective as(or perhaps more effective than) ECF,and promising early data have been derived for S-1 in combination with *** factor that might contribute to extending median oS is the advancement whenever possible to second-line cytotoxic ***,the biggest hope for signif icant survival advances in the near future would be the combination of new targeted biological agents with existing chemotherapy f irst-line regimens.

In recent years, the use of fluorescence-guided surgery(FGS) to treat benign and malignant visceral, hepatobiliary and pancreatic neoplasms has significantly increased. FGS relies on the fluorescence signal emitted by injected substances(fluorophores) after being illuminated by ad hoc laser sources to help guide the surgical procedure and provide the surgeon with real-time visualization of the fluorescent structures of interest that would be otherwise invisible. This review surveys and discusses the most common and emerging clinical applications of indocyanine green(ICG)-based fluorescence in visceral, hepatobiliary and pancreatic surgery. The analysis, findings, and discussion presented here rely on the authors' significant experience with this technique in their medical institutions, an up-to-date review of the most relevant articles published on this topic between 2014 and 2018, and lengthy discussions with key opinion leaders in the field during recent conferences and congresses. For each application, the benefits and limitations of this technique, as well as applicable future directions, are described. The imaging of fluorescence emitted by ICG is a simple, fast,relatively inexpensive, and harmless tool with numerous different applications in surgery for both neoplasms and benign pathologies of the visceral and hepatobiliary systems. The ever-increasing availability of visual systems that can utilize this tool will transform some of these applications into the standard of care in the near future. Further studies are needed to evaluate the strengths and weaknesses of each application of ICG-based fluorescence imaging in abdominal surgery.

Repair of sustained liver injury results in fibrosis(*** accumulation of extracellular matrix proteins),and ultimately the complete distortion of parenchymal architecture of the liver,which we call *** and staging of fibrosis is thus a mainstay in the management of chronic liver diseases,since many clinically relevant decisions,such as starting treatment and/or monitoring for complications including hepatocellular carcinoma,may depend on *** gold standard for fibrosis staging is liver biopsy,the role of which,however,is questioned nowadays because of cost,hazards and poor acceptance by *** the other hand,imaging techniques and/or measurement of direct and indirect serum markers have not proved to be completely satisfactory under all circumstances as alternatives to liver *** progress in this field is nowmore crucial than ever,since treatments for established fibrosis appear on the *** dissection of the pathways involved in the pathophysiology of liver diseases has put forward several novel candidate biomarkers of liver fibrosis,such as growth arrest-specific6,Mac-2-binding protein,osteopontin,placental growth factor,growth/differentiation factor 15 and hepatocyte growth *** molecules have been suggested to have potential to complement or substitute methods currently used to stage liver ***,we review the pros and cons for their use in this setting.
