Background: Currently, surgical resection represents the only curative treatment for pancreatic cancer(PC), however, the majority of tumors are no longer resectable by the time of diagnosis. The aim of this study was to describe time trends and distribution of pancreaticoduodenectomies(PDs) performed for treating PC in Brazil in recent years. Methods: Data were retrospectively obtained from Brazilian Health Public System(namely DATASUS) regarding hospitalizations for PC and PD in Brazil from January 2008 to December 2015. PC and PD rates and their mortalities were estimated from DATASUS hospitalizations and analyzed for age, gender and demographic characteristics. Results: A total of 2364 PDs were retrieved. Albeit PC incidence more than doubled, the number of PDs increased only 37%. Most PDs were performed in men(52.2%) and patients between 50 and 69 years old(59.5%). Patients not surgically treated and those 70 years or older had the highest in-hospital mortality rates. The most developed regions(Southeast and South) as well as large metropolitan integrated municipalities registered 76.2% and 54.8% of the procedures, respectively. LMIM PD mortality fluctuated, ranging from 13.6% in 2008 to 11.8% in 2015. Conclusions: This study suggests a trend towards regionalization and volume-outcome relationships for PD due to PC, as large metropolitan integrated municipalities registered most of the PDs and more stable mortality rates. The substantial differences between PD and PC increasing rates reveals a limiting step on the health system resoluteness. Reduction in the number of hospital beds and late access to hospitalization, despite improvement in diagnostic methods, could at least in part explain these findings.

Trypanosoma rangeli and T.cruzi are both parasitic unicellular species that infect humans.Unlike T.cruzi,the causative agent of Chagas disease,T.rangeli is an infective and non-pathogenic parasite for humans,but pathogenic for vectors from the Rhodnius genus.Because both species can coexist in different hosts and overlap their infective cycles but very little is known about the infection of T.rangeli in mammalian cells,we decided to characterize both the development of this parasite in cell culture and the effect of therapeutic agents with potential trypanocidal action on it.We found that T.rangeli exhibits a cycle of infection in Vero cells similar to that for T.cruzi and that the repurposed drug,17-AAG,and the natural extract Artemisia sp.essential oil produce a toxic effect on epimastigotes showing a trypanocidal action from the fifth day of culture.Both treatments also affected the infection of trypomastigotes and reduced the capacity of replication of amastigotes of T.rangeli.Since T.cruzi/T.rangeli coinfection cases have been reported,the finding of drugs with potential activity against both species could be significant in the future.Furthermore,studies of susceptibility of both species to drugs could also help to know the different mechanisms of pathogenicity in humans displayed by T.cruzi that are absent in T.rangeli.

Passive strategies for acclimatization of buildings have been studied by several authors in many countries, especially the evaporative and radiant cooling techniques. Fiber cement tiles are very common in popular constructions due to their low cost. However, they have over twice of the value thermal transmittance indicated to this bioclimatic zone according to Brazilian guidelines. The objective is to present an alternative to reduce high temperatures on fiber cement tiles. In this paper, the monitoring of passive cooling of roofing during the spring season in a city with subtropical climate is described. Single and combined reflective and evaporative cooling systems were studied in different environmental conditions. Internal surface temperatures of tiles were monitored together with weather variables. Results show a decrease of about 6 oC, 9 oC, 10 oC and 11 oC as compared to the original tiles according to environment conditions and the combined passive cooling techniques. These results allow for the conclusion that the use of passive cooling techniques opens up new possibilities to attenuate the internal surface temperatures of tiles and to consequently decrease the roofing solar heat gain into buildings, thus, providing less air cooling energy consumption.

BACKGROUND Occult hepatitis C virus(HCV) infection(OCI) may be associated with extrahepatic diseases and it is known that the patients with chronic kidney disease(CKD) who are on hemodialysis(HD) present a higher prevalence of this type of infection than the general population, with a worse clinical outcome.However, there are no data in the literature to assess the presence of OCI in patients prior to the initiation of renal replacement therapy(RRT). Therefore, this study aimed to evaluate the occurrence and epidemiological aspects of OCI in patients with Predialysis CKD. We hypothesize that this infection could occur before RRT initiation.AIM To research the status in predialysis patients when HD patients have high prevalence of OCI.METHODS A cross-sectional study was conducted between 2015 and 2017. Adults with creatinine clearance < 60 mL/min·1.73 m^2(predialysis patients) were recruited to the study. Pregnant and postpartum women, patients with glomerulopathies,and patients showing positivity for serological markers of hepatitis B virus(HBV), HCV or human immunodeficiency virus infection were excluded. Patients were diagnosed with OCI according to test results of anti-HCV antibody negativity and HCV RNA positivity in either ultracentrifuged serum or, if serumnegative, in peripheral blood mononuclear cells.RESULTS Among the 91 total patients included in the study, the prevalence of OCI was 16.5%. Among these 15 total OCI patients, 1 was diagnosed by 14 ultracentrifuged serum results and 14 were diagnosed by peripheral blood mononuclear cell results. Compared to the non-OCI group, the OCI patients presented higher frequency of older age(P = 0.002), patients with CKD of mixed etiology(P = 0.019), and patients with markers of previous HBV infection(i.e.,combined positivity for anti-hepatitis B core protein antibody and anti-hepatitis B surface protein antibody)(P = 0.001).CONCLUSION Among predialysis patients, OCI involved the elderly, patients with CKD of mixed etiology, and patients with previous HBV infection.

The article presents a case study about thermal comfort in two public squares located in the coastal region of southeastern Brazil, within a post-occupational assessment context where it was sought to estimate the thermal sensation and perception of the users generating systematized knowledge about the built environment. The objective is to instruct future interventions for improvement in the living condition where it is particularly important for the success of the activities predicted there. Surveys on thermal comfort along with goers of the squares were held on three consecutive days in April 2013 and February 2014 with microclimate monitoring of the places adopting the predictive PET (physiological equivalent temperature) index calculated based on the bioclimatic model Rayman. The obtained data set generated valuable information about the pattern of thermal comfort allowing understanding the analytical correlations between PET index, climate variables and the perception of comfort. The article seeks to contribute towards the recognition of bioclimatic specificities in the project process aimed to the improvement of the environmental and social performance of public squares and aspects that are relevant to the urban planners.

Natural Ventilation by Air Captors and Extractors Sheds in Hospitals in Brazil： Wind Tunnel Measurements

Natural ventilation is an efficient design strategy for the passive cooling of buildings, especially in tropical countries such as Brazil. Among the ventilation strategies, sheds can be highlighted. These structures consist of roof openings that work as air captors or extractors depending on their location in relation to the prevailing wind directions. The hospitals of the Sarah Network, designed by the Brazilian architect Joao Filgueiras Lima, Lele, are worldwide known for using these elements to improve natural ventilation. This paper analyses the natural ventilation performance of sheds for air collecting and extracting in two Sarah hospitals located in the cities of Salvador and Rio de Janeiro. In each building, the sheds were analyzed for air extracting and collecting. The analyses were carried out by reduced physical models in an atmospheric boundary layer wind tunnel. The wind velocity was measured at external and internal points of the buildings, using hot-wire anemometers. The results show that the wards in Rio de Janeiro hospital are 17% more ventilated than the ones in the Salvador hospital. However, this difference occurs not only because of the collecting sheds but also because of set of openings and the configuration of the covering in hospitals in Rio de Janeiro.

Breast cancer is the most frequent malignant neoplasm and the leading cause of cancer death in women in the world. Early detection techniques have been proposed on the basis of bioelectrical measurements. The aim of this study was to evaluate analytically and experimentally the inductive phase shift as a function of multi-frequency induced currents in breast cancer conditions. Virtual simulation by biophysical models and experimental measurements by magnetic induction in agar phantoms were developed to estimate the inductive phase shift as a function of the bulk electrical properties in typical breast volumes with tumors in specific positions. The analytical and experimental results are qualitatively consistent and provide evidence that the inductive phase shift has a potential clinical value to detect cancerous tumors toward inside a typical volume of breast tissue.

Trichinella spiralis (T. spiralis), which is a cosmopolitan nematode that infects humans among other species, presents a complex host-parasite relationship that hinders the development of tools to eradicate the parasitosis. The aim of this research was to analyze the host response during a primary infection with T. spiralis in five genetically different mouse lines of the CBi-IGE stock. Adult males from the CBi+, CBi−, CBi, CBi/L and CBi/C lines were infected with 1, 2 or 4 L1 larvae per g of body weight. In the chronic stage, the number of parasites per g of tissue (relative larval load, rLL) showed a significant host genotype-dose interaction, since it did not increase in the same way in the five genotypes. At the lowest dose, both CBii−and CBi/L mice were resistant while CBi+, CBi/C, and CBi were susceptible. At the highest dose, only CBi/L remained resistant, and CBi+ was the most susceptible. The reproductive capacity index of adult worms (RCI = rLL/infective dose) evinced only a genotype effect, allowing rating each line as resistant or susceptible regardless of dose. Animals receiving 2 L1 larvae were also sacrificed in the intestinal phase (6 and 13 days p-i) to determine the number of adult parasites (nAP) recovered in a small intestine segment, and female fecundity (Ff). No differences in nAP were observed among genotypes on day 6 p-i. nAP decreased between days 6 and 13 p-i, this reduction being different among genotypes and significant only in CBi/L and CBi/C. Ff decreased in CBi/L and CBi/C on day 13 p-i. At the time of infection, serum cytokine baseline values showed a Th1 orientation for genotype CBi/L (high IFN-γ and IL-2) and a Th2 for CBi+ (high IL-4 and IL-10).The variability in the response observed in this murine model suggests its potential usefulness to gain insight into the mechanisms that regulate host-parasite relationship.
