Safety and feasibility of same day discharge after per oral endoscopic pyloromyotomy in refractory gastroparesis: a pilot study

Background:Gastroparesis is a debilitating medical condition with limited treatment options.Gastric per-oral pyloromyotomy(G-POEM)has emerged as a promising treatment option with remarkable short-term clinical success shown in multiple studies.While the post-procedure protocol is not standardized across many centers,the majority of the centers observes these patients in the hospital after the procedure for monitoring.In this single-center prospective study,we evaluated the safety and feasibility of same day discharge after the G-POEM procedure.Methods:All the patients with refractory gastroparesis undergoing G-POEM from October 2019 to March 2020 were enrolled.A total of 25 patients were enrolled in the procedure.Based on the pre-defined criteria,patients were either discharged on the same day after the procedure or admitted to the hospital for further observation.The patient and procedure-related data were extracted from the chart review.Univariate analysis was performed(chi-squared test)on categorical variables after organizing categorical variables as numeric counts or percentages.The student t test was performed on continuous variables after reporting as mean and standard deviation.for analysis with a smaller sample size,fisher exact and Mann–Whitney tests were used.Results:A total of 25 patients were enrolled.The technical success of G-POEM was 100%and clinical success was 80%(20/25)at 1-month follow-up.Of the 25 patients,9 patients(36%)were discharged on the same day according to the procedure from the recovery unit.Of the remaining 16 patients who were admitted to the hospital post-procedure,10(40%)were admitted due to procedure-related causes while other admissions were either pre-planned or due to social reasons.The average Charlson comorbidity index was lower in the same day discharge group(Pf patients requiring double myotomy was higher in the same day discharge group(Pf G-POEM in the study cohort was 12%(3/25)with all complications being mild wi

Typology and Agroecology of Agroecosystems in Vegetation Dynamics in the Ecotones of the Mbam and Inoubou

Agroforestry systems strongly characterize the Cameroonian agrarian landscape. Agroforests are among those structuring the ecotones of Mbam and Inoubou in the Central Cameroon region. Numerous works on agroecosystems of Central and South Cameroon, few have come out contribution of the structure of these traditional systems in the dynamics of the vegetation of these forest-savannah ecotones. The present contribution has the overall objective of demonstrating the structural efficiency of agroforests the dynamics of ecotone vegetation, but also in the conservation of biodiversity. To do this, a participatory analysis was carried out with 56 farmers distributed in the five villages of Makénéné. Botanical inventories supplemented socio-economic household surveys. The data collected was subjected to various analyzes (univariate test, analysis of variance, multivariate test, PCA, CAH). The results reveal that 55.2% of agroforests are less than 15 years old and those with an area greater than 1500 m2 predominate (33%). They are mainly young with generally small surface areas. The horizontal structure reveals that the largest diameter classes are those of [20 - 30 cm[ and [10 - 20 cm[ with a very low rate of basal area. Agroforests with trees over 10 m high are dominant in five villages of Makénéné. Two types of structural profiles characterize the agroforestry flora of the area, namely intensive pluristratified home gardens on savannah and intensive pluristratified agroforests under forest-savannah transition vegetation. Principal Component Analysis (PCA) and Ascending Hierarchical Clustering (HAC) show three Agroforest Clusters each. The PCA distribution reveals that woody biomass (Y) is strongly correlated with tree diameter (DBH) and significantly with height (Cluster 2). Theoretical Study of Benzothiazole and Its Derivatives: Molecular Structure, Spectroscopic Properties, NBO, MEP and TD-DfT Analyses

Benzothiazole (BTH) and its derivatives are organic molecules with biologic actions. Because of their many applications, they are produced on a massive scale and used in a number of environmental compartments. Their discharge into water produces environmental problems, exposing our environment to public health problems. A solution that can contribute to their deterioration is becoming a necessity. for this reason, a conceptual analysis of the reactivity of benzothiazole and four of its compounds was undertaken in order to investigate certain aspects of their biodegradability. A theoretical investigations of the compounds studied were conducted in the gas and water phases with the most widely used density functional theory method, Becke-3-Parameter-Lee-Yang-Parr (B3LYP) with 6-31G+ (d, p) basis. Reactivity study calculated global indices of reactivity revealed that 2-SCH3_BTH is the most reactive. Dipole moment values analysis reveals that 2-NH2_BTH is the most soluble in water, while the lipophilicity shows that 2-NH2_BTH is the most hydrophilic compound. Thermodynamic parameters values reflect that reactions are respectively exothermic and spontaneous. By analyzing an Electrostatic Molecular Potential (EMP) map, researchers can pinpoint reactive sites on a molecule and anticipate its reactivity. This assessment is further enhanced by incorporating global and local reactivity descriptors. Additionally, an exploration of frontier molecular orbitals offers valuable insights into the molecule’s charge transfer characteristics. Moreover, a combined examination of internal and external molecular interactions unveils hyperconjugative interactions arising from charge delocalization, as elucidated through natural bond orbital (NBO) analysis.

In Central African Republic, edible caterpillars of Imbrasia gender are income of rural populations, present an opportunity to improve food security and belonging to the category of Non-wood forest products. The goal of this study is to document the ecology of caterpillars of genre Imbrasia in relation to its host plants in Boukoko. The questionnaire was addressed to the actors of the collection and consumption of caterpillars in this Commune, to their local knowledge concerning this food resource as well as the plants of their locality on which caterpillars are collected. The survey took place in September to October 2022 and as a general rule, the 120 respondents were elderly people whose age range varied from 35 to 70 years old. Plant samples are collected in their natural environment. Based on local names;herbaria are mounted, plants photographed and samples collected. The types of edible caterpillars have been inventoried in the field in collaboration with the local populations who consume them. Based on local names, the caterpillars are captured manually from morphological traits and then stored in bottles containing 70% ethanol. The specimens were identified. The data was entered into the Excel spreadsheet which also made it possible to produce histograms with the frequencies of the plant species most valued by the different caterpillars. A Principal Component Analysis (PCA) was then performed using R software to highlight the correlation between caterpillars and host plants. The main economic importance trees hosting edible caterpillars of Imbrasia gender in Boukoko region are: Entandrophragma cylindricum, Pentaclethra macrophylla, Maesopsis eminii, Albizia ferruginea, Macaranga spinosa, Amphimas ferrugineus, Pentaclethra macrophylla, Racosperma auriculiforme, Eribroma oblonga. PTEN in liver diseases and cancer

The phosphoinositide 3-kinase (PI3K)/phosphatase and tensin homolog (PTEN)/Akt axis is a key signal transduction node that regulates crucial cellular functions, including insulin and other growth factors signaling, lipid and glucose metabolism, as well as cell survival and apoptosis. In this pathway, PTEN acts as a phosphoinositide phosphatase, which terminates PI3Kpropagated signaling by dephosphorylating PtdIns(3,4)P2 and PtdIns(3,4,5)P3. However, the role of PTEN does not appear to be restricted only to PI3K signaling antagonism, and new functions have been recently discovered for this protein. In addition to the well-established role of PTEN as a tumor suppressor, increasing evidence now suggests that a dysregulated PTEN expression and/or activity is also linked to the development of several hepatic pathologies. Dysregulated PTEN expression/activity is observed with obesity, insulin resistance, diabetes, hepatitis B virus/hepatitis C virus infections, and abusive alcohol consumption, whereas mutations/deletions have also been associated with the occurrence of hepatocellular carcinoma. Thus, it appears that alterations of PTEN expression and activity in hepatocytes are common and recurrent molecular events associated with liver disorders of various etiologies. These recent f indings suggest that PTEN might represent a potential common therapeutic target for a number of liver pathologies.

Soil Organic Carbon and Nitrogen Dynamics in Arabica Coffee Agroforestry Systems in the Noun Division, West Cameroon

Agroforestry systems (AfSs) offer viable solutions to climate change because of the below-ground biomass (BGB) that is maintained by the soil. Therefore, spatially explicit estimation of their BGB is crucial to account for emission reduction efforts. A study to assess soil organic carbon (SOC) and nitrogen dynamics in Arabica coffee agroforests was conducted in two subdivisions (foumbot and Kouoptamo) of the Noun Division in western Cameroon. The methodological approach involved the collection of 150 soil samples taken at different depths: 0 - 10, 10 - 20 and 20 - 30 cm. Depending on the depth, the SOC stock is 27.93 ± 1.13 tC/ha at 10 cm depth, 22.37 ± 1.47 tC/ha at 20 cm and 20.79 ± 0.31 tC/ha at 30 cm. According to the age classes of the Arabica coffee systems (ACA), the C/N ratio in our study area averaged 26.94 ± 13.60 for the (5 - 20) year old systems in foumbot and 60.64 ± 48.80 for the (20 - 35) year old systems in Kouoptamo. Depending on the depth, at 10 cm this ratio is higher in Kouoptamo than in foumbot with a maximum value of 57 and 38 respectively for the two subdivisions. In view of the results obtained, it would be important to analyse the types of microorganisms responsible for the decomposition of organic matter which is linked to soil organic carbon.

Body fluid analog chlorination:Application to the determination of disinfection byproduct formation kinetics in swimming pool water

Disinfection by-products(DBPs)are formed in swimming pools by the reactions of bather inputs with the disinfectant.Although a wide range of molecules has been identified within DBPs,only few kinetic rates have been reported.This study investigates the kinetics of chlorine consumption,chloroform formation and dichloroacetonitrile formation caused by human releases.Since the flux and main components of human inputs have been determined and formalized through Body fluid Analogs(BfAs),it is possible to model the DBPs formation kinetics by studying a limited number of precursor molecules.for each parameter the individual contributions of BfA components have been quantified and kinetic rates have been determined,based on reaction mechanisms proposed in the literature.With a molar consumption of 4 mol Cl2/mol,urea is confirmed as the major chlorine consumer in the BfA because of its high concentration in human releases.The higher reactivity of ammonia is however highlighted.Citric acid is responsible for most of the chloroform produced during BfA chlorination.Chloroform formation is relatively slow with a limiting rate constant determined at 5.50×10^-3 L/mol/sec.L-histidine is the only precursor for dichloroacetonitrile in the BfA.This DBP is rapidly formed and its degradation by hydrolysis and by reaction with hypochlorite shortens its lifetime in the basin.Reaction rates of dichloroacetonitrile formation by L-histidine chlorination have been established based on the latest chlorination mechanisms proposed.Moreover,this study shows that the reactivity toward chlorine differs whether L-histidine is isolated or mixed with BfA components.

Evaluating the Role of Governance in the Management of Prunus africana (Hook.f.) Kalkman in the Adamawa Region of Cameroon

One of the problems at the root of the poor management of forest resources in Cameroon is the lack of good governance. This study aims to evaluate the governance put in place by the government of Cameroon to ensure better management of Prunus africana. It takes a critical look at the three pillars of forest governance in relation to the management of the resource, which are: policy, legal, institutional and regulatory frameworks (1);planning and decision-making process (2);and finally the implementation, enforcement and compliance of regulations (3). The assessment was conducted using the guide recommended by fAO-PROfOR. Individual and focus group interviews were conducted with stakeholders involved in the P. africana management process. The results in the form of assessment scores revealed poor governance in the management of the resource. This is due to non-compliance with Pillars 2 and 3 on the planning and decision-making process as well as the implementation and enforcement of regulations through weaknesses in the principles of transparency, accountability, the effectiveness of management measures and stakeholder participation. These are the points on which management strategies should be based for better management of P. africana in the production forests of the Adamawa region.

Plant Biodiversity and Structure of Robusta Coffee (Coffea canephora var. robusta) Agroforests in Cameroon

Agroforests are man-made ecosystems in which crops are associated with a main perennial species like Robusta Coffee Agroforests (RCAs), which is counted among the main modes of perennial crops production in Cameroon. Despite the rich ecosystem services provided, the diversity of Associated Species (AS) found in these RCAs and the structure of the landscapes they form remain little known in Cameroon. The current study aimed to inventory AS and characterize the structure of RCAs in four sites (Ayos, Malantouen, Melong and Nkongsamba) belonging to three robusta coffee production basins in Cameroon. A systematic inventory with dendrometric measurements of the wood AS and coffee trees was carried out on 120 one-hectare plot unit, i.e. 30 plots per site. The results showed that 102 AS belonging to 83 genera and 41 families were identified in these RCAs. The RCAs of Ayos in the dense rainforest zone with bimodal rainfall pattern were the most diverse with 71 species, followed by those of Melong and Nkongsamba with respectively 39 and 33 species respectively in the dense rainforest zone with monomodal rainfall pattern, and Malantouen with 33 species in the high savannahs of the west. Structurally, average coffee tree and AS densities founded ranged from 1208 - 1456 plants/ha and 71 - 214 stems/ha and those of basal area from 7.29 - 17.40 m2/ha for coffee trees and 7.97 - 16.14 m2/ha for AS in function of site. Basis on the vertical stratification, the proportion of the 3 - 6 m stratum, which is mainly represented by introduced AS, varied from 38% - 62% depending on the site. The results of this study showed that RCAs contribute to the conservation of plant biodiversity, given the specific richness identified in these ecosystems.

Experiences of Using Alternative Raw Materials in Manufacturing of Refractory Materials

The article describes the experiences of alternative materials for the manufacturing of the refractory materi- als in the company P-D Refractories CZ a. s. The atten- tion is focused on energy by-products （EBP）. Energy by-products are generated during burning and desul- phurization in thermal power plants. Classical high-tem- perature -fly ash （class f.fly ash according to ASTM C618） is the most important and .fly ash from .fluidized technology （ class C-fly ash） is the second group. In the Czech Republic, power plants produce about 14 million tons of energy by-products every year. Utilization of these products in ceramic technology means a reduction of raw material costs and also it helps to reduce adverse environ- mental impact. Class f-fly ash （ ffA ） and cinder from high temperature combustion （CD） were used in light- weight insulation fireclay bricks. We can use these mate- rials as a grog and a lightening agent for materials with bulk density over 900 kg · m-3 and classification tem- perature up to 1 150 ℃. Class C.fly ash （CfA） is be- ing tested in a wide range of the refractory materials. for example, it can be used in lightweight fireclay bricks, fireclay bricks for stoves, acid-resistant fireclay bricks or refractory castables. The range of potential products, where EBP could be used, is very wide and energy by-products have become an important raw mate- rial source.
