Introduc.ion: Helic.bac.er pylori infec.ion is a real health problem worldwide. It is the most c.mmon c.ronic.bac.erial infec.ion in the world, and is partic.larly prevalent in developing c.untries. Objec.ive: To determine the frequenc. of Helic.bac.er pylori infec.ion and to study the epidemiologic.l, c.inic.l and endosc.pic.c.arac.eristic. assoc.ated with this infec.ion in Brazzaville. Patients and Methods: This was a desc.iptive c.oss-sec.ional study c.nduc.ed from January to November 2020, i.e. 11 months. This work foc.sed on 100 symptomatic.patients over 18 years old referred for upper GI endosc.py. Gastric.biopsies for biologic.l study by urease test and molec.lar study by real time Pc. tec.nique were taken. Results: With a mean age of 46.32 ± 15.20 years, the frequenc. of Hp infec.ion was 91%, with a female predominanc. of 53%. The sex ratio was 0.92. The average age was 46.32 ± 15.20 years. c.rriage of the infec.ion was more important in households with more than 3 persons, in patients c.nsuming public.tap water and in those using both types of sanitary fac.lities. Endosc.py was indic.ted for epigastralgia in 93.1% of c.ses. About 56.14% of the infec.ed patients had normal muc.sa versus 12.28% with ulc.rated lesions and 22.81% with gastritis. c.nc.usion: The prevalenc. of Helic.bac.er pylori infec.ion is signific.nt in c.ngo, justifying early detec.ion in order to improve management.

Introduc.ion: Viral hepatitis B (VHL) is a public.health problem, partic.larly in sub-Sahara Afric.. The aim of this study was to assess vac.ination c.verage against HBV in c.ildren in Brazzaville. Patients and Methods: This was a c.oss-sec.ional analytic.l study c.nduc.ed in Brazzaville health c.ntres from January to September 2019. It involved c.ildren aged between six months and six years who rec.ived a vac.ination against HBV. Sampling was exhaustive and based on stratified sampling. Results: The overall prevalenc. of c.ildren vac.inated against HBV in Brazzaville was 96.2%. It was insuffic.ent in the Talangai health distric. (79%). The pentavalent vac.ine was administered to 97.7% of c.ildren, 85% of whom had rec.ived all three doses. The reasons for inc.mplete vac.ination were parents’ ignoranc. of HVB (85.6%) and of vac.ination (14.3%). c.nc.usion: Although the prevalenc. of vac.inated c.ildren is high in Brazzaville, it is still insuffic.ent in some health distric.s, partic.larly Talangai, bec.use parents are unaware of the disease and of vac.ination. Pentavalent is the only vac.ine available in the national vac.ination programme, whic. is why an effec.ive national vac.ination polic. needs to be put in plac.. .

Ingestion of c.ustic. by c.ildren is serious bec.use of the inc.ease in their frequenc. and the diffic.lties due to their management. Objec.ive: To improve the management of c.ustic.lesions in Pediatric.Department at the University Hospital of Brazzaville. Patients and Methods: Prospec.ive study was c.nduc.ed from January 2014 to Dec.mber 2015 in the Pediatric.Departments of the UHB and c.nters of digestive endosc.py in Brazzaville. All c.ildren who ingested a c.ustic.produc. and hospitalized were inc.uded after obtaining parental’s c.nsent. The studied parameters were: Age, sex, nature and quantity of the c.ustic. the family’s attitude, c.inic.l signs, endosc.pic.results ac.ording to Zagar’s c.assific.tion, therapeutic.modalities and evolution. Results: 8292 c.ildren were hospitalized and 68 bec.use of the ingestion of c.ustic. a frequenc. of 0.8%. The average age was 23.6 months [2 months - 16 years]. The male sex was predominant (57.4%). Ingestion was ac.idental in 88.2% and voluntary in 11.8%. Fac.ors favoring ingestion were storage methods (81.5%) and parental’s inattention or negligenc. (10.8%). Bleac. was the most inc.iminated c.ustic.(54.4%) followed by c.ustic.soda (29.4%). The estimated quantity ingested was assumed to be minimal in 61.7%. Unsuitable ac.s were prac.ic.d by the family before the medic.l c.nsultation in 66.1%. The dominant symptoms were digestive (47%) and neurologic.l (25%). The average time to perform endosc.py was 48 hours in 88.2%. Endosc.py revealed lesions in 51.7%, the different stages were I (12 c.ses), II (16 c.ses) and III (3 c.ses). The mains fac.ors of gravity were inappropriate gestures (85%) and the non-respec. of the fasting (9%). Proton pump inhibitors were used in 50%. The evolution was favorable for in 73.5%. c.nc.usion: Ingestion of c.ustic. by c.ildren is most often ac.idental favored by the inadequate pac.aging and storage. Prevention is about educ.ting people in order to reduc. their frequenc..

Bac.ground: In c.ngo, there is no pediatric.gastroenterology department and the ac.ivity of c.lonosc.py has never been evaluated. The aim of this study is to report the indic.tions and the results of pediatric.c.lonosc.py in Brazzaville. Methods: A retrospec.ive study was c.nduc.ed from January 2016 to August 2017 by analyzing reports of c.lonosc.py performed in the gastroenterology department in Brazzaville. Sex, presc.ibers, indic.tions and results of c.lonosc.py were the variables study. Results: over the 19-month period, 489 c.lonosc.pies have been performed, in whic. 13 (2.7%) c.lonosc.pies were performed in c.ildren under 17 years, with an average age of 9.8 ± 4.6 (8 males and 6 females) and the extreme ranging from 1 - 17 years. The sex ratio (H/F) was 1.6. The presc.ibers were gastroenterologist (n = 4), pediatric.an (n = 4), surgeons (n = 2), general prac.itioners (n = 2), pediatric.students (n = 1). c.ronic.bowel inflammatory diseases and polypec.omy was the most frequent c.nditions for c.lonosc.py indic.tion. Seven (7) c.lonosc.pies were normal. 4 c.ses were rec.o-polyps, and 2 c.ses were ulc.rative c.litis. c.nc.usion: pediatric.c.lonosc.py is a powerful diagnostic.and therapeutic.means. The pediatric.ans should think about it to improve the management of c.ildren with digestive symptoms that meet the indic.tions of pediatric.c.lonosc.py.

Hepatitis B virus (HBV) infec.ion is a major public.health problem in the world. In c.ngo, studies on the prevalenc. of HBV infec.ion are c.mmon but HDV infec.ion is rare. Objec.ive: c.ntribute to improve the management and prevention of HBV and HDV infec.ions. Patients and Methods: This was a desc.iptive and c.oss-sec.ional study c.nduc.ed from January to August 2016 in the health servic.s of the various departments of c.ngo and at the national public.health laboratory in Brazzaville. All patients with HBsAg surfac. antigen who were at least 18 years old and who c.nsented to the study were inc.uded. The HBs antigen was first tested by a rapid test then c.nfirmed by ELISA. All HBsAg positive patients were subsequently subjec.ed to a sec.nd ELISA test for Delta antibody. The tec.niques of extrac.ion and c.nventional and spec.fic.gene amplific.tion were c.rried out on plasma for the identific.tion of the genotypes of the two viruses. The data analysis was done with EpiInfo V7 software. The proportions were c.mpared using the c.i-square test or the Fisher test at the signific.nc. level of 5%. Results: 1618 subjec.s were inc.uded, of whom 180 were positive for HBsAg, a frequenc. of 11.12%. The sex ratio (M/F) was 0.77%, the mean age was 40.2 ± 15.6 years with extremes ranging from 18 to 88 years. Of the 180 HBsAg positive, 22 were anti-VHD positive, a frequenc. of 12.22%. HBV DNA was amplified in 83.8% (151/180), HDV RNA in 63.6% (14/22). For HBV, three genotypes were identified: E (46.9%), A (43.7%) and D (9.4%), however for VHD three genotypes D1, D5, D8 were identified. c.nc.usion: c.-infec.ion of hepatitis B and Delta viruses is c.mmon in c.ngo. The management and prevention of hepatitis B must take into ac.ount the reality of this c.-infec.ion.

The Helicobacter pylori (Hp) infection is a public health problem, especially in developing countries. However, the responsibility of Hp for the occurrence of recurrent abdominal pain is still unclear. Objectives: To determine the prevalence of Helicobacter pylori (Hp) infection in children with recurrent abdominal pain (RAP) in Brazzaville and to establish the relationship between Hp infection and RAP. Patients and Methods: We conducted a cross-sectional case-control study, between March and September 2018, including 106 children with RAP and 218 children without RAP, aged 2 to 17 years old, received in health centres in Brazzaville. All the children were subjected to detection of Hp fecal antigen by qualitative immuno-chromatography in the faeces. Results: The prevalenc. of Hp infec.ion among c.ildren with RAP in Brazzaville was 43.4%. The female sex was most represented in both populations, with a sex ratio of 0.84 in the infec.ed population. c.mpared with c.ntrols, these were c.ildren with a mean age of 8.13 ± 4.57 years. Infec.ed c.ildren with RAP were unsc.ooled in 66.7% (n = 31) of c.ses (p < 0.05). Those using potty were 63% (n = 29). Introduc.ion: c.rrhosis is a serious pathology that leads to c.mplic.tions whose management remains diffic.lt in our c.untry. This study aimed to evaluate the evolutionary aspec.s of c.rrhosis at the Brazzaville University Hospital. Patients and methods: this was a retrospec.ive study over a period of 18 months (from January 2015 to July 2016), performed in the Gastroenterology Department of the Brazzaville University Hospital. All patients regularly followed for dec.mpensated c.rrhosis were inc.uded. The studied variables were the epidemiologic.l, c.inic.l and parac.inic.l c.arac.eristic., the oc.urrenc. of c.mplic.tions, the survival, and the c.uses of death. Univariate analysis was used to determine prognostic.fac.ors based on the c.ild Pugh sc.re. Results: There were 43 patients, inc.uding 32 men and 11 women, with a mean age of 52 ± 9.5 years. Viral hepatitis B was the most c.mmon etiology (39.5%). c.ild Pugh stage c.was found in 60.5%. Oesophageal varic.s were present in 93% of c.ses. Survival at 18 months was 72%. In univariate analysis, two variables were signific.ntly assoc.ated with dec.eased survival, inc.uding c.ild Pugh stage c.and esophageal varic.s (grade II and III). c.nc.usion: c.rrhosis remains a worrying pathology bec.use the diagnosis is often made at the stage of often serious c.mplic.tions putting at risk of vital prognosis.

Introduc.ion: Human immunodefic.enc. virus (HIV) infec.ion is a public.health problem of c.nc.rn. Anti-retroviral therapy (ART) is assoc.ated with multiple side effec.s. This study aimed at identifying the different hepatic.manifestations of antiretroviral therapy and the responsible molec.les. Patients and Methods: This was an eight months period prospec.ive desc.iptive study, from January 1st to August 31st, 2015, c.nduc.ed in the Department of Gastroenterology and Internal Medic.ne at the Brazzaville University Teac.ing Hospital. Study partic.pants were treatment-na?ve HIV patients who were initiated on ART treatment during the study period. Patients with liver disease, liver c.tolysis prior to initiation of therapy, and those with alternative therapy that may c.use hepatotoxic.ty were exc.uded. The sample size was 110 patients. Results: The age was ranging from 25 to 70 years with a mean age of 47.5 ± 7.5 years. During the six months of follow-up, the alarming hepatic.signs were observed in 26.36% of c.ses (n = 29) in the 3rd month of treatment. There was no observed alarming sign in the 6th month of follow-up. The c.tolytic.pattern was observed in 54.55% of c.ses (n = 60) in the 3rd month. The c.olestatic.pattern was observed in 6.36% of c.ses (n = 7) in the 3rd month. Triple therapy c.mbination of Zidovudine, Lamivudine and Nevirapine (AZT + 3Tc.+ NVP) was the most used in 57.27% (n = 63) with a statistic.lly signific.nt p value to the oc.urrenc. of c.tolytic.pattern (p c.nc.usion: Drug induc.d liver toxic.ty oc.urs in a signific.nt number of patients starting ART. The prevalenc. of hepatic.events was high at the third month of treatment and the triple therapy of Zidovudine, Lamivudine and Nevirapine (AZT + 3Tc.+ NVP) was the most inc.iminated.

Objec.ive: To determine the plac. of two identific.tion tests for Helic.bac.er pylori infec.ion available in c.ngo. Materials and Methods: This was a c.mparative study c.rried out in two digestive endosc.py c.nters in Brazzaville from 1 January to 31 May 2018. Symptomatic.patients referred for upper gastrointestinal endosc.py were inc.uded systematic.lly. The frequenc. of infec.ion was determined from two identific.tion tests, namely the rapid urease test in gastric.biopsies and the detec.ion of antigen (Ag) for the germ in the stool. The c.iterion for judging the presenc. of the germ in eac. patient was the positivity of at least one of the two tests. The Mc.EMAR X2 test (p c.mparison of averages. Results: During the study period, 137 c.nsenting patients were inc.uded, inc.uding 62 men and 75 women. The overall inc.denc. of Helic.bac.er pylori (Hp) infec.ion was 79.6% (109/28). Of the 137 patients, 18 were urease positive only;6 were looking for Hp Ag in the stool, and 85 were in the two tests. The frequenc. of infec.ion was 75.2% (103/137) with the rapid urease test and 66.4% (91/137) with the Hp Ag test in stool. The rapid urease test proved more reliable in the diagnosis of Helic.bac.er pylori infec.ion than the stool antigen test.

Introduc.ion: Undernutrition is a c.ndition frequently enc.untered in pediatric.. It leads to an inc.eased morbidity and mortality regardless of the underlying c.ndition or other risk fac.ors suc. as age. In c.untries with limited resourc.s suc. as ours, the diagnosis of undernutrition is often limited to the c.inic.l presentation, and the c.ntribution of biology is not often taken into ac.ount. Objec.ives: To establish the relationship between anthropometric.parameters and biologic.l markers in the diagnosis and c.assific.tion of undernutrition and to assess the risk of infec.ious c.mplic.tions during undernutrition in c.ildren. Materials and Methods: A c.oss-sec.ional study c.nduc.ed in Brazzaville among undernourished c.ildren aged 1 - 59 months between Oc.ober 2018 and April 2019. c.inic.l diagnosis was based on WHO growth c.arts. The c.P, orosomuc.id, albumin and transthyretin were obtained using the c.bas c.11 analyzer, whic. enabled the c.lc.lation of Prognostic.Inflammatory and Nutritional Index (PINI). The c.mparison of the means of the biologic.l markers used the Student’s t-test, the risk of infec.ious c.mplic.tions the c.i-square. The c.rrelation of the diagnostic.value of Z-sc.re weight/height and PINI was also investigated. The signific.nc. level was set at 0.05. Results: Of the 95 c.ildren enrolled 63 (66.3%) were c.inic.lly severely malnourished, inc.uding 26 ac.te (41.3%) and 37 c.ronic.(58.7%). The PINI revealed severe undernutrition in 85 c.ildren (89.4%) inc.uding 50 ac.te (58.8%) and 35 c.ronic.(41.2%). c.P and orosomuc.id were statistic.lly higher in severe ac.te undernutrition (p c.nc.usion: Anthropometric.parameters have a front-line advantage for assessing and c.assifying undernutrition. However, biologic.l markers of undernutrition with PINI should be systematized in the diagnosis and management of undernutrition.
