Yield of capsule endoscopy in obscure gastrointestinal bleeding:A comparative study between premenopausal and menopausal women

AIM To evaluate differences in capsule endoscopy(CE) performed in the setting of obscure gastrointestinal bleeding(OGIB) among premenopausal women(PMW) and menopausal women(MW).METHODS Retrospective, single-center study, including female patients submitted to CE in the setting of OGIB between May 2011 and December 2016. Patients were divided into 2 groups according to age, considering fertile age as ≤ 55 years and postmenopausal age as > 55 years. The diagnostic yield(DY), the rebleeding rate and the time to rebleed were evaluated and compared between groups. Rebleeding was defined as a drop of Hb > 2 g/dL or need for transfusional support or presence of melena/*** A hundred and eighty three female patients underwent CE for OGIB, of whom 30.6%(n = 56) were PMW and 69.4%(n = 127) were MW. The DY was 30.4% in PMW and 63.8% in MW. The most common findings were angiodysplasias in both groups(PMW: 21.4%, MW: 44.9%)(P = 0.003). In PMW, only 1.8% required therapeutic endoscopy. In 17.3% of MW, CE findingsled to additional endoscopic treatment. Rebleeding at 1, 3 and 5 years in PMW was 3.6%, 10.2%, 10.2% and 22.0%, 32.3% and 34.2% in MW. Postmenopausal status was significantly associated with higher DY(P < 0.001), TY(P = 0.003), rebleeding(P = 0.031) and lower time to rebleed(P = 0.001).CONCLUSION PMW with suspected OGIB are less likely to have significant findings in CE. In MW DY, need for endoscopic treatment and rebleeding were significantly higher while time to rebleed was lower.

The use of triple therapy for hepatitis C not only increases the rate of sustained virological responses compared with the use of only interferon and ribavirin(RBV) but also leads to an increased number of side effects. The subject of this study was a 53-year-old male who was cirrhotic with hepatitis C virus genotype 1 A and was a previous null non-responder. We initially attempted retreatment with boceprevir(BOC), Peginterferon and RBV, and a decrease in viral load was observed in the 8th week. In week 12, he presented with disorientation, flapping, fever, tachypnea, arterial hypotension and tachycardia. He also exhibitedleucopenia with neutropenia. Cefepime and filgrastim were initiated, and treatment for hepatitis C was suspended. A myelogram revealed hypoplasia, cytotoxicity and maturational retardation. After 48 h, he developed bilateral inguinal erythema that evolved throughout the perineal area to the root of the thighs, with exulcerations and an outflow of seropurulent secretions. Because we hypothesized that he was suffering from Fournier's Syndrome, treatment was replaced with the antibiotics imipenem, linezolid and clindamycin. After this new treatment paradigm was initiated, his lesions regressed without requiring surgical debridement. Triple therapy requires knowledge regarding the management of adverse effects and drug interactions; it also requires an understanding of the importance of respecting the guidelines for the withdrawal of treatment. In this case report, we observed an adverse event that had not been previously reported in the literature with the use of BOC.

Obesity has become a major global health challenge and it is a risk factor for the development of several comorbid conditions. Additionally, obesity has considerable economic consequences. Obesity is a multifactorial condition that arises from independent influences of genetic and social-environmental factors on food intake and physical activity. It has been difficult to establish clear associations between weight status and the intake of single foods or food groups. In most people, the predisposition to obesity has a polygenic basis, which means that obesity will develop if an individual has several polygenic variants that increase body weight. The FTO gene was the first GWAS-identified obesity-susceptibility gene and since then other polygenic variants that are associated with BMI and dietary intake have also been identified. However, this is still an active area of research as more polygenetic variants await discovery.

Introduction: In the Americas, Brazil contributes 91.63% of the total cases and the state of Pará still has high endemia for leprosy. Objective: To analyze the performance of a rapid test for the diagnosis and epidemiological surveillance of leprosy in endemic areas. Methods: The sample consisted of 70 MB multibacillary leprosy (MB) patients, 63 paucibacillary (PB) patients, and 80 intradomiciliary consanguineous contacts (ICSCO) of patients. A rapid test with a 15-minute reading was applied using two prototypes: prototype 1, double test with trisaccharide antigen (NT-P-BSA) at 1a. line (83.2 ng/test) and disaccharide antigen (ND-O-BSA) at 2a. (83.2 ng/test), both with a flow of 0.08 μL/mm with a 10 μC membrane, anti-IgM conjugate with a flow of 0.040 μL/mm and a Tris-Triton and prototype 2 runner buffer with MIX antigen (trisaccharide + disaccharide) in the same concentrations and conditions of prototype 1. Results: The comparison of the MIX test positivity rate and the disaccharide or trisaccharide doublet test across all samples was statistically significant, demonstrating that the MIX test had higher seropositivity rates compared to the ND-O-BSA or NT-P-BSA. It was demonstrated that the MIX test showed a good performance, with 25.39% of the PB patients negative for the disaccharide and trisaccharide duplet test, but positive for MIX. Conclusions: These data suggest the potential for further optimizing the performance by adding other synthetic antigens to the MIX antigens.

BACKGROUND Hepatitis C virus(HCV)infection is responsible for a chronic liver inflammation,which may cause end-stage liver disease and hepatocellular *** E(protein:ApoE,gene:APOE),a key player in cholesterol metabolism,is mainly synthesized in the liver and APOE polymorphisms may influence HCV-induced liver *** To determine whether APOE alleles affect outcomes in HCV-infected patients with liver cirrhosis following orthotopic liver transplantation(OLT).METHODS This was a cohort study in which 179 patients,both genders and aged 34-70 years,were included before or after(up to 10 years follow-up)*** injury severity was assessed using different criteria,including METAVIR and models for endstage liver *** polymorphisms were analyzed by quantitative real-time polymerase chain *** The APOE3 allele was the most common(67.3%).In inflammation severity of biopsies from 89 OLT explants and 2 patients in pre-transplant,the degree of severe inflammation(A3F4,0.0%)was significantly less frequent than in patients with minimal and moderate degree of inflammation(≤A2F4,16.2%)P=0.048,in patients carrying the APOE4 allele when compared to *** addition,a significant difference was also found(≤A2F4,64.4%vs A3F4,0.0%;P=0.043)and(A1F4,57.4%vs A3F4,0.0%;P=0.024)in APOE4 patients when compared to APOE3 *** fibrosis degree of the liver graft in 8 of 91 patients and the lack of the E4 allele was associated with more moderate fibrosis(F2)(P=0.006).CONCLUSION Our results suggest that the E4 allele protects against progression of liver fibrosis and degree of inflammation in HCV-infected patients.

Fractures of the zygomaticomaxillary complex are among the most common face traumas. Based upon the complexity and great variety of reported diagnoses and treatments, the proposal of this study was to evaluate, clinically and radiographically, unilateral zygomatic fractures treated through internal rigid fixation with miniplates and screws of 1.5 mm. Material and Method: 15 patients with unilateral fractures of the zygomaticomaxillary complex were analyzed, and compared with 15 patients without fractures so that a comparative analysis of the area and the perimeter of the orbital cavities could be made, as well as the distance from the nasal point to the zygomatic prominence between both groups. Results: In the radiographic analysis, the both groups presented similarity in the perimeter and in the area of the orbital cavities. Concerning the distance from the nasal point to the zygomatic prominence, only the operated group showed a significant difference between the sides, even though clinically the observation of the asymmetry had been absent or discreet. Conclusion: The treatment of unilateral fractures of the zygomaticomaxillary complex with the use of plates and screws of the 1.5 mm system proved to be effective, showing good esthetic results and low complication rates.

The present study evaluates the performance of three numerical weather forecasting models: Global Forecast System (GFS), Brazilian Regional Atmospheric Modelling System (BRAMS) and ETA Regional Model (ETA), by means of the Mean Error (ME) and the Root Mean Square Error (RMSE), during the most rainy four months period (January to April 2012) on Eastern Amazonia. The models displayed errors of superestimation and underestimation with respect to the observed precipitation, mainly over center-north of Pará and all of Amapá, where the precipitation is higher. Among the analyzed models, GFS shows the best performance, except during January and March, when the model to underestimated precipitation, possibly due to the anomalously high values recorded.

The aim of this study was to compare transgenic and conventional soybean cultivars in regard to yield performance and agronomic traits. Six soybean cultivars were tested: three conventional cultivars and their three isogenic transgenic derivatives, in two locations, Lavras and Patos de Minas in Minas Gerais, Brazil, in the 2012/13 crop season. data on yield (kg·ha-1), plant height (cm), and tolerance to lodging were collected. The cultivars had different responses. The cultivar BRS/MG46 Conquista had lower yields than its isogenic derivative BRS Valiosa RR, but this was not seen with the conventional cultivar M-SOY 6101, which produced more than its isogenic derivative M-SOY 7211 RR. Between the isogenic cultivars BRS/MG68 Vencedora and BRS Favorita RR, there were no significant differences. Nevertheless, the contrast between the mean values of the transgenic and conventional cultivars shows that the conventional cultivars had mean values greater than the transgenic cultivars.

Morphology Characterization and Kinetics Evaluation of Pitting Corrosion of Commercially Pure Aluminium by Digital Image Analysis

The pit morphology and growth kinetics of commercially pure aluminium in naturally aerated NaCl solutions were studied using an image processing method based on reflected light microscopy. In order to distinguish between pits and pre-existing cavities, metallographic examination and statistical analysis were carried out before and after corrosion testing. The results show that the pit shapes and sizes are more dependent on the immersion time than the chloride concentration. Pits are predominantly hemispherical, but they undergo reasonable geometric transitions associated with increased immersion time and occur without significant depth variation. The role of chloride ions is more closely associated with the pit nucleation phenomenon.
