Indirect calorimetry in obese female subjects: Factors influencing the resting metabolic rate

AIM: To evaluate selected factors influencing resting energy expenditure(REE) in obese female ***: Seventy seven 61 obese Caucasian women [mean age of 52.93 ± 13.45 years, and mean body mass index(BMI) of 41.78 ± 11.54 kg/m2] were enrolled; measurements of resting metabolic rate(RMR) by a ventilated, open-circuit system, indirect calorimeter were performed after an overnight fast. Body composition as well as medications, physical parameters, blood samples, disease pattern, and smoking were considered. RESULTS: RMR was significantly associated with body weight(r = 0.732, P < 0.001), body height(r = 0.401,P = 0.008), BMI(r = 0.504, P < 0.001), waist circumference(r = 0.602, P < 0.001), mid-upper arm circumference(r = 0.417, P = 0.006), mid-upper arm muscle circumference(r = 0.344, P = 0.028), total body water(r = 0.339, P = 0.035), body temperature(r = 0.409, P = 0.007), smoking(P = 0.031), serum T4 levels(r = 0.331, P = 0.036), obstructive sleep apnoea syndrome(OSAS; P = 0.023), impaired glucose tolerance(IGT; P = 0.017) and impaired glycaemic status, including hyperinsulinism, IGT and diabetes mellitus(P = 0.003).CONCLUSION: Future research should be prompted to optimize the procedure of indirect calorimetry to achieve clinical benefits in obese subjects.

Prevention Strategies of Contrast Medium Induced Nephropathy (CIN): A Review of the Current Literature

Contrast medium induced nephropathy is the third most common cause of renal failure for inpatients and represents the 10% of all acute kidney injury occurring during hospital-stay. It is associated with prolonged hospitalization, cost increase and, above all, an unfavourable short- and long-term prognosis. Here, the authors discuss about the contrast medium induced nephropathy prevention strategies, from the identification of patients at risk and drugs potentially nephrotoxic, to the hydration with possible administration of drugs that appeared to be, in some contexts, nephron-protective, and finally we analyze the radiological procedure aimed at the correct choice of type and administration modality of the contrast medium according to current literature.

Quantitative Evaluation of Inflammatory Response in Rats Submitted to the Abdominal Wall Surgery Using Peritoneum of Paca Preserved in 300% Supersaturated Solution of Sucrose or 98% Glycerin

This study aimed to quantitative evaluate the implantation of paca’s peritoneum, a new biomaterial option, preserved in 300% supersaturated sugar solution or 98% glycerin on the abdominal wall of Wistar rats. 60 males, weighing between 150 and 200 g, are organized into the following experimental groups: control group (GI);group peritoneum preserved in 300% supersaturated sugar solution (GII);and group peritoneum preserved in 98% glycerin (GIII). Each group had 20 animals. Five rats from each group underwent euthanasia at four different moments: 7th, 15th, 30th, and 60th post-operatory day for histological and stereological evaluations of the graft/native tissue interface. On histological examination, one found out an extensive presence of inflammatory infiltrate at the early periods (7th and 15th day) and an extensive presence of connective tissue at the late moments (30th and 60th day). Through stereological analysis, one found out that the number of mononuclear cells decreased throughout the evaluation moments. One concluded that the paca’s peritoneum as a biological membrane preserved in 98% glycerin, when implanted on the abdominal wall of rats, microscopically allowed one to obtain, through stereological analyses, a lower inflammatory response than the paca’s peritoneum preserved in 300% supersaturated sugar solution.

BACKGROUND: Epidural analgesia initiated early in labor (when the cervix is less than 4.0 cm dilated) has been associated with an increased risk of cesarean delivery. It is unclear, however, whether this increase in risk is due to the analgesia or is attributable to other factors. METH- ODS: We conducted a randomized trial of 750 nulliparous women at term who were in spontaneous labor or had spontaneous rupture of the membranes and who had a cervical dilatation of less than 4.0 cm. Women were randomly assigned to receive intrathecal fentanyl or systemic hydromorphone at the first request for analgesia. Epidural analgesia was initiated in the intrathecal group at the second request for analgesia and in the systemic group at a cervical dilatation of 4.0 cm or greater or at the third request for analgesia. The primary outcome was the rate of cesarean delivery. RESULTS: The rate of cesarean delivery was not significantly different between the groups (17.8 percent after intrathecal analgesia vs. 20.7 percent after systemic analgesia; 95 percent confidence interval for the difference, - 9.0 to 3.0 percentage points; P=0.31). The median time from the initiation of analgesia to complete dilatation was significantly shorter after intrathecal analgesia than after systemic analgesia (295 minutes vs. 385 minutes, P<0.001), as was the time to vaginal delivery (398 minutes vs. 479 minutes, P<0.001). Pain scores after the first intervention were significantly lower after intrathecal analgesia than after systemic analgesia (2 vs. 6 on a 0- to- 10 scale, P<0.001). The incidence of one- minute Apgar scores below 7 was significantly higher after systemic analgesia (24.0 percent vs. 16.7 percent, P=0.01). CONCLUSIONS: Neuraxial analgesia in early labor did not increase the rate of cesarean delivery, and it provided better analgesia and resulted in a shorter duration of labor than systemic analgesia.
