AIM To determine the sensitivity and specificity of liver stiffness measurement(LsM) and serum markers(sM) for liver fibrosis evaluation in chronic hepatitis *** Between 2012 and 2014,81 consecutive hepatitis C virus(HCV) patients had METAVIR score from liver biopsy compared with concurrent results from LsM [transient elastography(TE) [Fibros can~/ARFI technology(Virtual Touch~)] and sM [FIB-4/aspartate aminotransferase-toplatelet ratio index(APRI)].The diagnostic performance of these tests was assessed using receiver operating characteristic *** optimal cut-off levels of each test were chosen to define fibrosis stages F ≥ 2,F ≥ 3 and F = *** Kappa index set the concordance *** Fifty point six percent were female and the median age was 51 years(30-78).Fifty-six patients(70%) weretreatment-na?*** optimal cut-off values for predicting F ≥ 2 stage fibrosis assessed by TE were 6.6 kP a,for acoustic radiation force impulse(ARFI) 1.22 m/s,for APRI 0.75 and for FIB-4 *** F ≥ 3 TE was 8.9 kP a,ARFI was 1.48 m/s,APRI was 0.75,and FIB-4 was *** F = 4,TE was 12.2 kP a,ARFI was 1.77 m/s,APRI was 1.46,and FIB-4 was *** APRI could not distinguish between F2 and F3,P = *** negative predictive value for F = 4 for TE and ARFI was 100%.Kappa index values for F ≥ 3 METAVIR score for TE,ARFI and FIB-4 were 0.687,0.606 and 0.654,*** demonstrates strong concordance between all three screening methods,and moderate to strong concordance between them and APRI(Kappa index = 0.507).CONCLUsION Given the costs and accessibility of LsM methods,and the similarity with the outcomes of sM,we suggest that FIB-4 as well as TE and ARFI may be useful indicators of the degree of liver *** is of particular importance to developing countries.

Crustal seismicity in northwestern Mendoza Province in Argentina, corresponding to the transition zone between the Chilean-Pampean flat subduction zone（26.5-33.5°s） and the southern Central Andes normal subduction zone to the south, is studied in order to i） identify its relationship with the mapped structure, ii）determine deformational mechanisms and iii） constrain the geometry of the fold and thrust belt in the lower crust. Through this, we aim to determine which are the structures that contribute to Andean construction, east of the Frontal Cordillera in Argentina and at the western Principal Cordillera in Chile. Data from a temporary local seismic network are reprocessed in order to achieve a precise location of hypocenters and, whenever possible, to build focal mechanisms. Results are interpreted and compared with previous seismic studies and structural models. Analyzed seismicity is grouped around the eastern front of Frontal Cordillera, with hypocenters mainly at depths of 25-40 km. Contrastingly, earthquakes in the Principal Cordillera to the west are located at the axial Andean sector and Chilean slope, with depths shallower than 15 km. Obtained focal mechanisms indicate mainly strike-slip displacements, left lateral at Frontal Cordillera and right lateral at Principal Cordillera. Based on these observations, new possible structural models are proposed, where seismogenic sources could be either associated with inherited basement structures from the Cuyania-Chilenia suture; or correspond to deep-blind thrusts linked with a deeper-than-previously-assumed decollement that could be shared between Frontal Cordillera and western Precordillera. This deeper decollement would coincide in turn with the one determined from receiver function analysis for the eastern sierras Pampeanas in previous works, potentially implying a common decollement all through the fold and thrust belt configuration. The study region is seismically and tectonically characterized by the angle variations in the subduction of the Nazca plate. The results obtained from earthquakes location between 32° and 36°s latitude and67°-71°W longitude are presented in this work. The presence of a wedge of asthenospheric materials and the partial or total eclogitization of the subducted Nazca plate and its relation with isostatic cortex models published was analyzed. In addition, a gravimetric profile obtained from gravity forward modeling is presented at 33.5°s, proposing a new configuration at depths for the main tectonic components： Nazca plate, asthenospheric wedge and south American plate. Also, a new density scheme using recently published velocity models was obtained.

We investigate the effect of the westerly rotation of the lithosphere on the active margins that surround the Americas and find good correlations between the inferred easterly-directed mantle counterflow and the main structural grain and kinematics of the Andes and sandwich arc *** the Andes,the subduction zone is shallow and with low dip,because the mantle flow sustains the slab;the subduction hinge converges relative to the upper plate and generates an uplifting doubly verging *** sandwich Arc is generated by a westerly-directed sAM(south American) plate subduction where the eastward mantle flow is steepening and retreating the subduction *** this context,the slab hinge is retreating relative to the upper plate,generating the backarc basin and a low bathymetry single-verging accretionary *** Central America,the Caribbean plate presents a more complex scenario:(a) To the East,the Antilles Arc is generated by westerly directed subduction of the sAM plate,where the eastward mantle flow is steepening and retreating the subduction zone.(b) To the West,the Middle America Trench and Arc are generated by the easterly-directed subduction of the Cocos plate,where the shallow subduction caused by eastward mantle flow in its northern segment gradually steepens to the southern segment as it is infered by the preexisting westerly-directed subduction of the Caribbean *** the frame of the westerly lithospheric flow,the subduction of a divergent active ridge plays the role of introducing a change in the oceanic/continental plate’s convergence angle,such as in NAM(North American)plate with the collision with the Pacific/Farallon active ridge in the Neogene(Cordilleran orogenic type scenario).The easterly mantle drift sustains strong plate coupling along NAM,showing at Juan de Fuca easterly subducting microplate that the subduction hinge advances relative to the upper *** lower/upper plate convergence coupling also applies along strike to the neigh

Earthquakes with magnitudes M ≤ 3.5 were registered in the Andes backarc between ~32.5°s 33.75°s within a temporary experiment from November 2002 to March 2003. Data were collected from 15 seismological broad band stations, deployed above flat subduction section and also above the transitional to normal section of the Nazca plate. seismic events were located mostly in part of the southern Precordillera and Cerrilladas Pedemontanas of Mendoza Province. Focal mechanism solutions were obtained, for selected data between15 kmand35 kmdepths, from P wave first motion using FOCMEC software. A band trending NW-NNW of normal focal mechanism earthquakes is located just by the thrust front towards West, and covers the southern tip of the southern Precordillera and the Western side of the Cerrilladas Pedemontanas, Mendoza Province, Argentina. Thrust focal mechanism solutions obtained in the present work for events with magnitude less than or equal to 3.5 also show that the thrust front south of ~33.1°s is located in the same place as Mw ≥ 3.6 earthquakes. The most significant findings in this work are these signals of extensional regimen which appear in a compressional subduction tectonic regimen. Nevertheless the short temporal experiment precludes firm interpretations about this particular phenomenon. Probably it is related to a temporary stress relaxation. The normal earthquakes are likely associated to normal faults, and/or to their subsidiaries, that were inverted by contraction and now re-inverted at least transitorily as normal. These faults, which are near and to the East of the suture between Chilenia and Cuyania Paleozoic terranes, were originally involved in the Cuyo Triassic basin formation.

目的确定心脏MRT1图测量在定量评价弥漫性心肌纤维化中的价值,并与心内膜活检进行对照。材料与方法研究符合HIPAA原则,获得单位伦理委员会的批准。回顾同一单位5年内进行心内膜活检和心肌MRI检查的心肌病病人。47例病人(53%男性,平均年龄46.8岁)进行了诊断性心肌MRI检查和心内膜活检。13名健康志愿者(54%男性,平均年龄38.1岁)作为对照组进行了心脏MRI检查。在1.5TMR设备上团注0.2mmol/kg钆螯合物后,采用反转恢复Look-Locker序列进行心肌的T1图测量,采集时间为(10.7±2.7)min。采用梯度回波反转恢复序列评价钆对比剂的延迟强化。2名放射医师采用盲法评价,意见一致后得出心脏MRI的结论。对心内膜活检的样品进行Masson染色,采用自动分析软件对数字图像进行纤维化的定量测量。2名病理学家采用盲法进行组织病理学结果报告。统计学分析包括Mann-WhitneyU检验、方差分析和线性回归。结果心肌纤维化的中位数为8.5%(四分位距5.7%～14.4%)。对照组的T1值均大于无或伴明显的钆延迟强化病人[分别为(466±14)ms、(406±59)ms和(303±53)ms,Ps。结论心脏的MRT1图定量测量可无创性评价心肌病病人心内膜弥漫性纤维化的情况。原文载于Radiology,2012,265(3):724-732.
