indocyanine green-based fluorescence imaging in visceral and hepatobiliary and pancreatic surgery:State of the art and future directions

In recent years, the use of fluorescence-guided surgery(FGS) to treat benign and malignant visceral, hepatobiliary and pancreatic neoplasms has significantly increased. FGS relies on the fluorescence signal emitted by injected substances(fluorophores) after being illuminated by ad hoc laser sources to help guide the surgical procedure and provide the surgeon with real-time visualization of the fluorescent structures of interest that would be otherwise invisible. This review surveys and discusses the most common and emerging clinical applications of indocyanine green(ICG)-based fluorescence in visceral, hepatobiliary and pancreatic surgery. The analysis, findings, and discussion presented here rely on the authors' significant experience with this technique in their medical institutions, an up-to-date review of the most relevant articles published on this topic between 2014 and 2018, and lengthy discussions with key opinion leaders in the field during recent conferences and congresses. For each application, the benefits and limitations of this technique, as well as applicable future directions, are described. The imaging of fluorescence emitted by ICG is a simple, fast,relatively inexpensive, and harmless tool with numerous different applications in surgery for both neoplasms and benign pathologies of the visceral and hepatobiliary systems. The ever-increasing availability of visual systems that can utilize this tool will transform some of these applications into the standard of care in the near future. Further studies are needed to evaluate the strengths and weaknesses of each application of ICG-based fluorescence imaging in abdominal surgery.

AIM:To investigate in greater detail the efficacy and safety of sorafenib for the treatment of hepatocellular carcinoma(HCC)in patients with established cirrhosis.METHODS:From October 2009 to July 2012 patients with an established diagnosis of cirrhosis and HCC treated with sorafenib were consecutively enrolled.According to the Barcelona Clinic Liver Cancer(BCLC)classification,patients were in the advanced stage(BCLC-C)or in the intermediate stage(BCLC-B)but unfit or unresponsive to other therapeutic strategies.Treatment was evaluated performing a 4-phase computed tomography or magnetic resonance imaging scan every 2-3 mo,and analyzed according to the modified Response Evaluation Criteria in Solid Tumors.Sorafenib was administered at 800 mg/d,until radiological progression or occurrence of unacceptable adverse events(AEs).Univariate and multivariate analyses identified predictors of 16-wk clinical benefit and overall survival.RESULTS:Forty-four patients were enrolled,15 had intermediate HCC and 14 a Child-Pugh score of B7.AEs caused treatment interruption in 19 patients(43%),and median treatment duration was shorter in this subset(5 wk vs 19 wk,P<0.001)and in the BCLC-C subgroup(13 wk vs 40 wk,P=0.015).No significant differences in the reason for treatment interruption or in treatment duration were found comparing patients in Child-Pugh class A vs B or in patients older or younger than 70 years.After 16 wk of treatment,18 patients(41%)had stable disease or partial response.Patients with viral infection or BCLC-C were at higher risk of disease progression.ECOG,extrahepatic spread,macrovascular invasion,alpha-fetoprotein or alkaline phosphatase levels at admission were independent predictors of overall survival.CONCLUSION:In patients with cirrhosis and HCC treated with sorafenib,AEs are a common cause of early treatment withdrawal.Vascular invasion and extrahepatic spread condition early response to treatment and survival.Baseline biochemical parameters may be helpful to identify patients at higher risk of shorter

蛋黄中含有大量胆固醇脂,怀疑长期食用蛋黄可能与高脂血症密切相关。本试验旨在研究食用蛋清或蛋黄对雄性Sprague Dawley(SD)大鼠血脂、大脑神经元发育和相关基因表达水平的影响。选取63只18日龄SD大鼠,平均分为7组,每组3个重复,(每个重复3只),适应性喂养一周后,组1至组6分别饲喂添加含1.6%、4.0%、7.2%蛋黄基础日粮以及添加2.8%、7.2%、13.2%蛋清基础日粮,组7为对照组饲喂基础日粮(含3%全蛋)。连续饲喂4周后分析大鼠体重、饲料消耗、血脂、肝功能和神经元发育等指标,以及海马神经元BDNF和trk B的表达水平。高蛋白组(13.20%)大鼠体重和采食量显著增加,其料重比为2.30±0.06,低于与对照组(3.03±0.11)。4%蛋黄组显著降低了大鼠最终体重和采食量,料重比升高至3.35±0.11,显著高于对照组。添加1.6%和7.20%蛋黄组大鼠体重和采食量均有下降。与对照组相比,添加蛋黄为7.20%的料重比较高(3.18±0.05),而1.6%蛋黄组的饲料重比(2.92±0.11)略低于对照组。高蛋白(7.2%和13.2%)含量饲料饲喂大鼠能显著增加其血清中TC、TG和LDLc含量,而HDL含量降低。4%蛋黄饲喂大鼠能降低血清TC、TG和LDL-c含量,增加HDL含量。各组间血清AST和ALT浓度无显著差异,即高水平蛋清或蛋黄食用不影响肝功能。与对照组相比,饲料中添加1.6%和4%蛋黄组大鼠海马CA1区神经元细胞形态完整、数量丰富。而7.2%蛋黄组大鼠海马CA1区神经元细胞形态结构有所损伤,完整细胞数量减少。仅饲喂蛋白组大鼠海马CA1区神经元有部分损伤,且细胞数量减少。BDNF和trk B作为BDNF的高亲和力受体,在蛋白摄入组表达量显著低于蛋黄摄入组(p<0.05)。长期食用蛋黄有助于减轻体重、增加血脂HDL水平,而长期食用蛋白有助于增加体重,且血脂HDL水平下降;食用4%蛋黄可促进海马CA1区神经元发育;食用蛋黄可通过增加BDNF和trk B基因表达量而增加大脑认知功能。

Performance evaluation of the incorporation of different wire meshes in between perforated current collectors and membrane electrode assembly on the Passive Direct methanol fuel cell

Passive Direct methanol fuel cells(DMFC)are more suitable for charging small capacity electronic devices.In passive DMFC,the fuel and oxidant are supplied by diffusion and natural convection process on the anode and cathode sides respectively.Current collectors(CC)play a vital importance in fuel cell performance.This paper presents the combined impact of perforated and wire mesh current collectors(WMCC)on passive DMFC performance.Three types of open ratios of perforated current collectors(PCC),such as 45.40%,55.40%and 63.40%and two types of wire mesh current collectors with open ratios of 38.70%and 45.40%were chosen for the experimental work.A combination of TaguchiL9 rule is considered.A combination of three PCC and two WMCC on both anode and cathode was used.Methanol concentration was varied from 1 mol·L^(-1)-5 mol·L^(-1)for nine combinations of PCC and WMCC.From the experimental results,it is noticed that the combination of PCC and WMCC with an open ratio of 55.40%and 38.70%incorporated passive DMFC produced peak power density at 5 mol·L^(-1)of methanol concentration.The passive DMFC performance was evaluated in terms of maximum power density and maximum current density.The combined current collectors of PCC and WMCC open ratios of 55.40%+38.70%have more stable voltage than single PCC of open ratio 63.40%at 4 mol·L^(-1)of methanol concentration.

There is a surge in the burden of cardiovascular disease(CVD)in Sub-Saharan African(SSA)countries.Over the last 50 years there has been a rapid growth in the consumption of ultra-processed food products high in salt,fat,and glycemic load in SSA.There is overwhelming evidence linking UPFP consumption to an increased risk of coronary artery disease,cerebrovascular disease,overall CVD,and all-cause mortality.Front-of-pack labeling(FOPL)represents an important intervention to guide consumers towards healthier diets and ultimately contribute to the prevention of diet-related adverse health outcomes including CVD.This paper discusses the challenges in the development and implementation of FOPL systems in SSA,and proposes some key elements that would be critical for developing an effective FOPL system in SSA countries.

Multiple Detector-Row CT in Gastric Cancer Staging: Prospective Study

The aim of this study was to evaluate the accuracy of multiple detector computed tomography (MDCT) in the preoperative staging of gastric cancer, prospectively comparing CT findings with pathological findings at surgery, in a single-center study. A total of 19 consecutive patients with primary cancer recruited between March and July 2014 were submitted to preoperative MDCT staging according to a standard protocol. All diagnostic procedures were performed by dedicated radiologists who were unaware of the final pathological results. Subsequently, 16 patients underwent surgical treatment and 15 were finally included in the study. The primary tumor was detected at CT in all 15 cases. CT results for T staging were in agreement with pathological findings in 12 of 15 cases, with overall accuracy of 80%. Stage-specific sensibility was high for advanced stages (sensibility for T1, T3, and T4 resulted 60%, 85.7%, and 100%, respectively), while earlier stages showed higher specificity (specificity for T1, T3, and T4 resulted 100%, 75%, and 91.7%, respectively). Overall N staging accuracy was 86.7%, with 13 of 15 patients correctly staged. Stage-specific sensibility was 75% for N0 and 100% for N3, while specificity was 100% for N0 and lower for advanced stages. Accuracy for peritoneal involvement was 100%. Our findings show a good performance of the diagnostic protocol performed with MDCT tested in this study.

本研究采用分段盐析法和柱层析法从巢湖新鲜蓝藻中提取纯化藻蓝蛋白。首先,使用硫酸铵、柠檬酸三铵、柠檬酸钠和硫酸钠对藻蓝蛋白粗提液进行两步分段盐析纯化,并利用紫外-可见分光光度法对藻蓝蛋白溶液进行纯度及回收率分析,以此确定了硫酸铵、柠檬酸三铵、柠檬酸钠和硫酸钠的最佳用量。其结果为:硫酸铵、柠檬酸三铵、柠檬酸钠和硫酸钠在第一和第二盐析过程中的最佳摩尔浓度分别为:硫酸铵(NH)SO为1.0 mol/L和1.7 mol/L;柠檬酸三铵CHNO为0.7mol/L和1.3 mol/L;柠檬酸钠CHNaO为0.5 mol/L和0.9 mol/L;柠檬酸钠CHNO7为1.0 mol/L和1.3 mol/表示硫酸钠NaSO。经过两步分段盐析,发现随着无机盐摩尔浓度的增高,杂质去除量增大到一定程度后会去除藻蓝蛋白,故纯度和收率都先提高再降低。最优结果表明,使用(NH)SO纯化后,藻蓝蛋白纯度达2.0以上,藻蓝蛋白回收率较高。为了进一步提高藻蓝蛋白的纯度,用(NH)SO进行第二次盐析后,利用离子交换色谱法进一步纯化藻蓝蛋白,研究了pH、离子强度、洗脱速度与进样浓度对纯化效果的影响。其最佳条件为:纤维素A-500,洗脱液pH值7.0,样品浓度1-2 mol/L,加样速度2cm/min,洗脱液NaCl浓度0.25mol/L,最终得到纯度在4.0以上的试剂级藻蓝蛋白。

Complications Following Surgery for Gastric Cancer: Analysis of Prospectively Collected Data

Background and Aims: The complication rate after surgery for gastric cancer varies according to the particular definition of morbidity, so it’s necessary to report them using a standardized method, the Clavien-Dindo system. The purpose of this study was to prospectively analyze all post-gastrectomy complications in patients with gastric adenocarcinoma according to the severity grade using Clavien-Dindo system, in order to identify risk factors for postoperative complications and their prognostic significance on survival. Methods: This study is based on data from 90 consecutive patients who underwent gastrectomy for gastric neoplasia between January 2010 and February 2014 at the same unit. 15 patients were excluded (benign tumors, GISTs, missing data). Complications were categorized according to the Clavien-Dindo classification (uncomplicated patients vs patients classified ≥Grade I). The following risk factors were studied: age, BMI, sex, operation method, extent of resection, duration of surgery, transfusions, TNM staging, and lymph node ratio. Multivariate logistic regression was used to evaluate the association between risk factors and presence of complications. To assess the effect on overall survival, after selection of covariates using backward elimination, the Cox proportional hazard model was applied. Results: Among these patients, 49 (65.3%) developed complications, stratified as follows: Grade I, 6 (8%);Grade II, 24 (32%);Grade III, 6 (8%);Grade IV, 13 (17.3%). The laparoscopic technique (OR = 0.050;95% CI = 0.005 - 0.550, p = 0.0143) and no transfusions (OR = 0.219;95% CI = 0.058 - 0.827, p = 0.0251) were found to reduce the incidence of postoperative complications in the multivariate analysis. With regard to the survival analysis, lymph node ratio, malnutrition, extended resection and presence of complications were significant predictors of reduced survival in the multivariate analysis. This paper concerns the study of the numerical approximation for the following initialboundary value problem{ut-uzx=f（u）,t∈（0,1）,t∈（0,T） u（0,t）=0,t∈（0,1）,t∈（0,T） u（x,0）=u0（x）,x∈（0,1）where f（s） is a positive, increasing, C1 convex function for the nonnegative values of s, f（0）〉0, f∞ds/f（s）〈∞, u0∈C1（[0, 1]）, u0（0） = 0, u＇0（1）=0. We find some conditions under which the solution of a semidiscrete form of the above problem blows up in a finite time and estimate its semidiserete blow-up time. We also prove the convergence of the semidiscrete blow-up time to the theoretical one. A similar study has been also undertaken for a discrete form of the above problem. Finally, we give some numerical results to illustrate our analysis.
