Effects of high-dose glucose-insulin-potassium on acute coronary syndrome patients receiving reperfusion therapy:a meta-analysis

BACKGROUND:This meta-analysis aimed to assess the efficacy of high-dose glucose-insulinpotassium(GIK) therapy on clinical outcomes in acute coronary syndrome(ACS) patients receiving reperfusion ***:We searched the PubMed,Web of Science,MEDLINE,Embase,and Cochrane Library databases from inception to April 26,2022,for randomized controlled trials(RCTs) that compared high-dose GIK and placebos in ACS patients receiving reperfusion *** primary endpoint was major adverse cardiovascular events(MACEs).RESULTS:Eleven RCTs with 884 patients were ultimately *** with placebos,high-dose GIK markedly reduced MACEs(risk ratio [RR] 0.57,95% confidence interval [95% CI]:0.35 to 0.94,P=0.03) and the risk of heart failure(RR 0.48,95% CI:0.25 to 0.95,P=0.04) and improved the left ventricular ejection fraction(LVEF)(mean difference [MD] 2.12,95% CI:0.40 to 3.92,P=0.02) at 6 ***,no difference was observed in all-cause mortality at 30 d or 1 ***,high-dose GIK was significantly associated with increased incidences of phlebitis(RR 4.78,95% CI:1.36 to 16.76,P=0.01),hyperglycemia(RR 9.06,95% CI:1.74 to 47.29,P=0.009) and hypoglycemia(RR 6.50,95% CI:1.28 to 33.01,P=0.02) but not reinfarction,hyperkalemia or secondary *** terms of oxidative stress-lowering function,high-dose GIK markedly reduced superoxide dismutase(SOD) activity but not glutathione peroxidase(GSH-Px) or catalase(CAT) ***:Patients with ACS receiving reperfusion therapy exhibited a reduction in MACEs and good oxidative stress-lowering eflcacy in response to high-dose ***,with a higher incidence of complications such as phlebitis,hyperglycemia,and ***,there were no observed survival benefits associated with high-dose *** trials with long-term follow-up are still needed.

Theoretical Study on Ionic Liquid Based on 1-Ethyl-3-MethylImidazolium Cation and Hexafluorophosphate or Tetrafluoroborate

The Hartree-Fock and DFT/B3LYP methods have been employed to investigate the electronic structures of 1-ethyl-3-methyl-imidazolium cation （EMIM＋）, BF4^-, PF6^-, EMIM＋BF4^-, and EMIM＋-PF6^- using the Gaussian-94 soft-package at 6-31＋G（d,p） basis set level for hydrogen, carbon, nitrogen, boron, phosphorus, and fluorine atoms. Comparison of the electronic structures of the lowest energy of EMIM＋- BF4^- and EMIM＋-PF6^- pairs, and single EMIM＋, BF4^- and PF6^- showed that the optimized EMIM＋-BF4^- and EMIM＋-PF6^- pair conformers were BF4^- and PF6^- outside the 5-ring plane between the ethyl group and the methyl group. The cohesion of C H. … F hydrogen bond between cation and anion is reinforced by charge assistance. The interaction energy between EMIM＋ and PF6 is 328.8 kJ/mol at the B3LYP level and 326.6 kJ/mol at the Hartree-Fock level, whereas that between EMIM＋ and BF4 is 353.5 kJ/mol at the B3LYP level and 350.5 kJ/mol at the Hartree-Fock level. The low energy interactions caused by bulky asymmetric EMIM＋, and charge dispersion of cation and anion give rise to the low melting point of ionic liquid EMIM＋-BF4^- and EMIM＋-PF6^-. The two hydrogen bonding models of single hydrogen bond formation, and the hydrogen transfer between C2 in EMIM＋ and F in BF4^- or PF6^- were principally depicted.

The anisotropy of suprathermal electrons in the Martian ionosphere

Suprathermal electrons are an important population of the Martian ionosphere, either produced by photoionization of atmospheric neutrals or supplied from the Solar Wind (SW). This study is dedicated to an in-depth investigation of the pitch angle distribution of suprathermal electrons at two representative energies, 19−55 eV and 124−356 eV, using the extensive measurements made by the Solar Wind Electron Analyzer on board the Mars Atmosphere and Volatile Evolution. Throughout the study, we focus on the overall degree of anisotropy, defined as the standard deviation of suprathermal electron intensity among different directions which is normalized by the mean omni-directional intensity. The available data reveal the following characteristics: (1) In general, low energy electrons are more isotropic than high energy electrons, and dayside electrons are more isotropic than nightside electrons;(2) On the dayside, the anisotropy increases with increasing altitude at low energies but remains roughly constant at high energies, whereas on the nightside, the anisotropy decreases with increasing altitude at all energies;(3) Electrons tend to be more isotropic in strongly magnetized regions than in weakly magnetized regions, especially on the nightside. These observations indicate that the anisotropy is a useful diagnostic of suprathermal electron transport, for which the conversion between the parallel and perpendicular momenta as required by the conservation of the first adiabatic invariant, along with the atmospheric absorption at low altitudes, are two crucial factors modulating the observed variation of the anisotropy. Our analysis also highlights the different roles on the observed anisotropy exerted by suprathermal electrons of different origins.

冠状动脉粥样硬化性心脏病患者血清miR-24-3p和MARK4水平表达与心功能的相关性研究

目的探讨动脉粥样硬化性心脏病(coronary atherosclerotic heart disease,CHD)患者血清中微小RNA(micro RNA,miR)-24-3p,微管亲和调节激酶4(microtubule affinity-regulating kinase 4,MARK4)水平与心功能的相关性。方法选取2021年8月~2023年4月间在四川省人民医院就诊的138例CHD患者为研究对象(CHD组),依据美国纽约心脏病学会(New York Heart Association,NYHA)分级,将CHD组患者分为Ⅱ级(n=39),Ⅲ级(n=68)和Ⅳ级(n=31)。另选取同期健康体检人员123例作为健康组。采用实时荧光定量PCR(qRT-PCR)法检测miR-24-3p及MARK4表达水平。检测CHD组及健康组人员心功能指标。采用Pearson法分析血清miR-24-3p,MARK4及与心功能指标间的相关性。结果与健康组比较,CHD组患者的MARK4(1.19±0.21 vs 1.00±0.20),左心室收缩末期内径(LVESd)、左心室舒张末期内径(LVEDd)、室间隔厚度(IVS)、左室舒张末期容积(LVEDv)、左室收缩末期容积(LVESv)及左心室质量(LV mass)水平升高(t=7.462,18.242,23.888,9.941,2.812,12.520,11.029),miR-24-3p[(0.62±0.11)vs(1.00±0.17)],LVEF及心输出量(CO)水平降低(t=20.821,10.212,18.188),差异具有统计学意义(均P<0.05)。随着心功能分级加重,MARK4表达水平(1.09±0.19,1.18±0.17,1.32±0.22),LVESd,LVEDd,IVS,LVEDv,LVESv及LV mass逐渐升高(F=13.025,10.606,11.920,57.956,29.680,21.253,12.954),miR-24-3p表达水平(0.84±0.15,0.61±0.11,0.37±0.08),LVEF,CO水平逐渐降低(F=139.227,9.720,13.411),差异具有统计学意义(均P<0.05)。Pearson相关性分析显示,CHD组患者血清中miR-24-3p与MARK4表达呈负相关(r=-0.540,P<0.05)。CHD组患者血清miR-24-3p相对表达量与LVESd,LVEDd,IVS,LVEDv,LVESv及LV mass呈负相关(r=-0.656,-0.636,-0.617,-0.576,-0.492,-0.687,均P<0.05),与LVEF及CO呈正相关(r=0.570,0.683,均P<0.05);MARK4相对表达量与LVESd,LVEDd,IVS,LVEDv,LVESv及LV mass呈正相关(r=0.503,0.542,0.508,0.624,0.516,0.560,均P<0.05),与LVEF及CO呈负相关(r=-0.594,-0.525,均P<0.05)。结论CHD患者血清中miR-24-3p表达降低,MARK4表达升高,且二者与心功能指标间存在显著相关性。

Buffer management optimization strategy for satellite ATM

ECFD （erroneous cell tail drop）, a buffer management optimization strategy is suggested which can improve the utilization of buffer resources in satellite ATM （asynchronous transfer mode） networks. The strategy, in which erroneous cells caused by satellite channel and the following cells that belong to the same PDU （protocol data Unit） are discarded, concerns non-real-time data services that use higher layer protocol for retransmission. Based on EPD （early packet drop） policy, mathematical models are established with and without ECTD. The numerical results show that ECTD would optimize buffer management and improve effective throughput （goodput）, and the increment of goodput is relative to the CER （cell error ratio） and the PDU length. The higher their values are, the greater the increment. For example, when the average PDU length values are 30 and 90, the improvement of goodput are respectively about 4% and 10%.

Single crystals of a bismuth-based coordination polymer(CP)with carboxyl-thiol ligands,[Bi(C_(8)H_(2)O_(4)S_(2))(C2H8N)]n(Bi-DSBDC-DMA,DMBDC=2,5-disulfur-1,4-dicarboxylate,DMA=dimethylamine),have been successfully synthesized.X-ray diffraction analysis reveals that Bi-DSBDC-DMA possesses a layered structure,with two-dimensional(2D)Bi-DSBDC networks alternating with layers composed of dimethylamine *** material demonstrates semiconducting properties,featuring an optical bandgap of 2.2 eV and an electrical conductivity of 2×10^(-8) S/***,electrodes based on this material exhibit a capacity of 250 mAh/g after 200 cycles for lithium-ion storage.

目的总结初诊为原发性肾炎型肾病综合征患儿的临床及病理特点,探讨国内原发性肾病综合征临床分型的应用价值及分型标准的可操作性。方法回顾性分析2014年1月至2020年12月郑州大学第一附属医院儿科收治的初诊为原发性肾炎型肾病综合征患儿的临床及病理资料。结果纳入研究初诊为原发性肾炎型肾病综合征患儿103例,分型标准:血尿(镜下血尿和肉眼血尿)占93.2%(96/103),高血压占13.6%(14/103),肾功能不全占9.7%(10/103),低补体血症占3.9%(4/103)。103例均行肾穿刺活检,10例行基因检测,最终诊断为原发性肾炎型肾病综合征27例(26.2%),IgA肾病71例(68.9%),Alport综合征3例(其中1例伴IgA肾病),单基因遗传激素耐药型肾病综合征1例(WT1基因变异),电子致密物沉积病(DDD)1例。27例原发性肾炎型肾病综合征确诊患儿表现为血尿者23例(85.2%),其中肉眼血尿5例(18.5%);病理类型:微小病变肾小球病(MCD)6例,局灶节段性肾小球硬化(FSGS)5例,不典型膜性肾病(AMN)5例,膜增生性肾小球肾炎(MPGN)4例,系膜增生性肾小球肾炎(MsPGN)3例,局灶增生性肾小球肾炎(FGN)3例,膜性肾病(MN)1例;追踪至2022年12月,失访5例,临床治愈和完全缓解共9例,蛋白尿反复3例,蛋白尿持续10例(慢性肾功能不全5例,其中3例接受透析,1例接受肾移植)。结论儿童原发性肾炎型肾病综合征的临床表现主要为合并血尿,现有分型标准不易规范和统一。初诊为原发性肾炎型肾病综合征患儿最终诊断绝大多数为IgA肾病,而非原发性肾炎型肾病综合征,早期明确肾脏病理及基因检测更为重要。

三阴性乳腺癌中SOX10表达的临床意义及与上皮间质转化的相关性

目的探讨Y框蛋白10基因(SRY‑related hmg‑box 10,SOX10)在三阴性乳腺癌(triple negative breast cancer,TNBC)组织中表达的临床意义及其与上皮间质转化(epithelial‑mesenchymal transition,EMT)的相关性。方法采用免疫组织化学方法检测80例TNBC及60例非TNBC组织SOX10的表达水平,同时检测TNBC组织中EMT关键标志物波形蛋白(vimentin)和上皮钙黏素(E‑cadherin)的表达水平,分析TNBC中SOX10、vimentin和E‑cadherin的表达与患者临床病理参数之间的关系及SOX10与EMT标志物的相关性。结果SOX10在TNBC中的表达明显高于非TNBC,差异有统计学意义(P<0.001)。SOX10的表达与TNBC患者的肿瘤分型、组织学分级、Ki67高增殖表达(Ki67≥30%)及淋巴结转移相关(P<0.05),与年龄、肿瘤最长径无关。SOX10的表达与vimentin表达呈正相关(r=0.398,P<0.001),与E‑cadherin表达呈负相关(r=-0.337,P<0.01)。结论SOX10在TNBC中特异性高表达,可作为TNBC鉴别诊断及预后的有效生物学标志物。SOX10可能通过调控EMT相关蛋白的表达促进肿瘤的侵袭和转移。

As an increasingly used alternative to perfluorooctanoic acid(PFOA),hexafluoropropylene oxide trimer acid(HFPO-TA)has been widely detected in global water ***,little is known regarding its toxic effects on cardiovascular ***,zebrafish embryos were treated with egg water containing 0,60,120,or 240 mg/L *** showed that HFPO-TA treatment led to a significant reduction in both larval survival percentage and heart ***,HFPO-TA exposure caused severe pericardial edema and elongation of the sinus venous to bulbus arteriosus distance(SV-BA)in Tg(myl7:GFP)transgenic larvae,disrupting the expression of genes involved in heart development and thus causing abnormal heart *** sprouting angiogenesis was observed in the 120 and 240 mg/L exposed Tg(fli:GFP)transgenic ***-TA treatment also impacted the mRNA levels of genes involved in the vascular endothelial growth factor(VEGF)pathway and embryonic vascular ***-TA exposure significantly decreased erythrocyte number in Tg(gata1:DsRed)transgenic embryos and influenced gene expression associated with the heme metabolism ***-TA also induced oxidative stress and altered the transcriptional levels of genes related to cell cycle and apoptosis,inhibiting cell proliferation while promoting ***,HFPO-TA exposure may induce abnormal development of the cardiovascular and hematopoietic systems in zebrafish embryos,suggesting it may not be a suitable or safe alternative for PFOA.
