Prevalence of and risk factors for erectile dysfunction in young nondiabetic obese men:results from a regional study

Erectile dysfunction(ED),a condition closely related to cardiovascular morbidity and mortality,is frequently associated with obesity.In this study,we aimed to determine the prevalence of ED and evaluate the associated risk factors in a cohort of 254 young(18–49 years)nondiabetic obese(body mass index[BMI]≥30 kg m−2)men from primary care.Erectile function(International Index of Erectile Function[IIEF-5]questionnaire),quality of life(Aging Males'symptoms[AMs scale]),and body composition analysis(Tanita MC-180MA)were determined.Total testosterone was determined using high-performance liquid chromatography–mass spectrometry.Multivariate logistic regression analysis was used to study the factors associated with ED.ED prevalence was 42.1%.subjects with ED presented higher BMI,waist circumference,number of components of the metabolic syndrome,AMs score,insulin resistance,and a more unfavorable body composition than those without ED.Multivariate logistic regression analysis showed that a pathological AMs score(odds ratio[OR]:4.238,Psity(BMI≥40 kg m−2,OR:2.602,P=0.005,compared with BMI 30–34.9 kg m−2),high-density lipoprotein(HDL)-cholesterol levels(OR:0.956,P=0.004),and age(OR:1.047,P=0.016)were factors independently associated with ED.In conclusion,we demonstrate that,in a primary care-based cohort of nondiabetic young obese men,ED affected>40%of subjects.A pathological AMs score,the degree of obesity,and age were positively associated with ED,while elevated HDL-cholesterol levels were inversely associated with the odds of presenting ED.Further prospective studies are needed to evaluate the long-term consequences of ED in this population.

Trends in oxycodone and oxycodone-containing analgesics administration for back pain in emergency departments in the UsA(2007–2018)

BACKGROUND:To describe trends in oxycodone and oxycodone-containing analgesic prescribing for the treatment of back pain among adults in emergency departments(EDs) in the UsA from 2007 to 2018.METHODs:Data were gathered from the National Hospital Ambulatory Medical Care survey(NHAMCs) from 2007 to 2018.The study population included individuals of all ages presenting to UsA EDs.The NHAMCs reasons for visit and oxycodone drug ID codes were used to isolate patients with back pain.The main outcome was the proportion of oxycodone and oxycodone-containing analgesics prescribed for back pain in the EDs over the specified time period.REsULTs:There was a relative decrease in the overall administration of oxycodone for back pain in the EDs by 62.3% from 2007(244,000 visits) to 2018(92,000 visits).The proportion of ED patients prescribed with oxycodone-containing analgesics for back pain increased among patients aged 45 years and older(from 43.8% to 57.6%),female patients(from 54.5% to 62.0%),black patients(from 22.5% to 30.4%),and Hispanic/Latino patients(from 9.4% to 19.6%).Oxycodone/acetaminophen was most prescribed and accounted for 90.2% of all oxycodone-containing analgesics in 2007,with a decrease to 68.5% in 2018.Pure oxycodone was the second most prescribed medication,accounting for 6.1% in 2007 and 31.5% in 2018.CONCLUsION:The overall number of oxycodone-containing analgesics decreased significantly from 2007 to 2018.However,that number trended upward in 45-year-old and older,female,black,or Hispanic/Latino patients from 2007 to 2018.The total amount of pure oxycodone increased significantly from 2007 to 2008.

survival effects of antibiotic exposure during the larval and adult stages in the West Nile virus vector Culex pipiens

The ability of mosquitoes to transmit a pathogen is affected,among other factors,by their survival rate,which is partly modulated by their microbiota.Mosquito microbiota is acquired during the larval phase and modified during their development and adult feeding behavior,being highly dependent on environmental factors.Pharmaceutical residues including antibiotics are widespread pollutants potentially being present in mosquito breeding waters likely affecting their microbiota.Here,we used Culex pipiens mosquitoes to assess the impact of antibiotic exposure during the larval and adult stages on the survival rate of adult mosquitoes.Wild-collected larvae were randomly assigned to two treatments:larvae maintained in water supplemented with antibiotics and control larvae.Emerged adults were subsequently assigned to each of two treatments,fed with sugar solution with antibiotics and fed only with sugar solution(controls).Larval exposure to antibiotics significantly increased the survival rate of adult females that received a control diet.In addition,the effect of adult exposure to antibiotics on the survival rate of both male and female mosquitoes depended on the number of days that larvae fed ad libitum in the laboratory before emergence.In particular,shorter larval ad libitum feeding periods reduced the survival rate of antibiotic-treated adult mosquitoes compared with those that emerged after a longer larval feeding period.These differences were not found in control adult mosquitoes.Our results extend the current understanding of the impact of antibiotic exposure of mosquitoes on a key component of vectorial capacity,that is the vector survival rate.

Remediation of amide pesticide-polluted soils by combined solarization and ozonation treatment

Agriculture has a close relationship with nature,but it can also be the source of negative and permanent environmental effects.The use of pesticides in modern agriculture is a common practice,but their side effects on the environment cannot be disregarded.In this study,we evaluated a combination of solarization and ozonation techniques for the elimination of six amide pesticides(boscalid,chlorantraniliprole,cyflufenamid,fluopyram,napropamide,and propyzamide)in soil.Initial experiments were performed with four different soils to assess the efficiency of this methodology at different soil temperatures and ozone dosages under laboratory conditions,and then a greenhouse pot experiment was conducted under controlled conditions during summer.Fifty days after the onset of the experiments,higher degradation percentages of amide pesticides were observed in ozonized soils than in other treated soils,particularly when ozone was applied at 10 cm soil depth.The results show that the utilization of ozonation,along with solarization,represents a valid method for degrading residues of the studied pesticides and suggest that this combined technology may be a promising tool for remediating pesticide-polluted soils.

When data privacy is imposed as a necessity,Federated learning(FL)emerges as a relevant artificial intelligence field for developing machine learning(ML)models in a distributed and decentralized environment.FL allows ML models to be trained on local devices without any need for centralized data transfer,thereby reducing both the exposure of sensitive data and the possibility of data interception by malicious third parties.This paradigm has gained momentum in the last few years,spurred by the plethora of real-world applications that have leveraged its ability to improve the efficiency of distributed learning and to accommodate numerous participants with their data sources.By virtue of FL,models can be learned from all such distributed data sources while preserving data privacy.The aim of this paper is to provide a practical tutorial on FL,including a short methodology and a systematic analysis of existing software frameworks.Furthermore,our tutorial provides exemplary cases of study from three complementary perspectives:i)Foundations of FL,describing the main components of FL,from key elements to FL categories;ii)Implementation guidelines and exemplary cases of study,by systematically examining the functionalities provided by existing software frameworks for FL deployment,devising a methodology to design a FL scenario,and providing exemplary cases of study with source code for different ML approaches;and iii)Trends,shortly reviewing a non-exhaustive list of research directions that are under active investigation in the current FL landscape.The ultimate purpose of this work is to establish itself as a referential work for researchers,developers,and data scientists willing to explore the capabilities of FL in practical applications.

The catalytic conversion of polystyrene (Ps) was studied in the presence of the materials type HZsM-5, CeOsub>2sub>, 10% CeOsub>2sub>/HZsM-5 and 20% CeOsub>2sub>/HZsM-5, which were characterized by scanning electron microscopy (sEM), X-ray diffraction (XRD) and nitrogen adsorption at 77K. The catalytic tests were performed via thermogravimetric analysis (TG) at heating rates of 5, 10 and 20˚C minsup>−1sup> in a temperature range from 30˚C to 900˚C. For the tests, a ratio of 30% by mass of each catalytic material mixed with Ps was used and the activation energy of the degradation process was determined by the Vyazovkin method. The obtained results showed that the addition of the catalyst to the Ps in general reduced its degradation temperature. The 10% CeOsub>2sub>/HZsM-5 catalyst showed greater efficiency, as it resulted in lower activation energy for Ps degradation. Thus, the combination of CeOsub>2sub> with HZsM-5 resulted in materials with potential for application in the catalytic degradation of polystyrene and the results indicate that the production of a composite material can be a good strategy to generate an increase in catalytic activity and a decrease in energy process activation.

Angiotensin II (Ang II) is the main mediator of the Renin-Angiotensin-system acting on ATsub>1sub> and other AT receptors. It is regarded as a pleiotropic agent that induces many actions, including functioning as a growth factor, and as a contractile hormone, among others. The aim of this work was to examine the impact of Ang II on the expression and function of αsub>1sub>-adrenergic receptors (αsub>1sub>-ARs) in cultured rat aorta, and aorta-derived smooth muscle cells. Isolated Wistar rat aorta was incubated for 24 h in DMEM at 37˚C, then subjected to isometric tension and to the action of added norepinephrine, in concentration-response curves. Ang II was added (1 × 10sup>−5sup> M), and in some experiments, 5-Methylurapidil (αsub>1Asub>-AR antagonist), AH11110A (αsub>1Bsub>-AR antagonist), or BMY-7378 (αsub>1Dsub>-AR antagonist), were used to identify the αsub>1sub>-AR involved in the response. Desensitization of the contractile response to norepinephrine was observed due to incubation time, and by the Ang II action. αsub>1Dsub>-AR was protected from desensitization by BMY-7378;while Rs-100329 and prazosin partially mitigated desensitization. In another set of experiments, isolated aorta-derived smooth muscle cells were exposed to Ang II and αsub>1sub>-ARs proteins were evaluated. αsub>1Dsub>-AR increased at 30 and 60 min post Ang II exposure, the αsub>1Asub>-AR diminished from 1 to 4 h, while αsub>1Bsub>-AR remained unchanged over 24 h of Ang II exposure. Ang II induced an increase of αsub>1Dsub>-AR at short times, and BMY-7378 protected αsub>1Dsub>-AR from desensitization.

Background:Knee examination guidelines in minors are intended to aid decisionmaking in the management of knee instability.Clinical question:A Delphi study was conducted with a formal consensus process using a validated methodology with sufficient scientific evidence.A group consensus meeting was held to develop recommendations and practical guidelines for use in the assessment of instability injuries in children.Key findings:there is a lack of evidence to analyse anterior cruciate ligament injuries in children and their subsequent surgical management if necessary.Diagnostic guidelines and clinical assessment of the patient based on a thorough examination of the knee are performed and a guide to anterior cruciate ligament exploration in children is developed.Clinical application:In the absence of a strong evidence base,these established guidelines are intended to assist in that decision-making process to help the clinician decide on the most optimal treatment with the aim of benefiting the patient as much as possible.Following this expert consensus,surgical treatment is advised when the patient has a subjective sensation of instability accompanied by a pivot shift test++,and may include an anterior drawer test+and a Lachman test+.If these conditions are not present,the conservative approach should be chosen,as the anatomical and functional development of children,together with a physiotherapy programme,may improve the evolution of the injury.

Hepatocellular carcinoma(HCC)is one of the most lethal malignant tumours worldwide.The mortality-to-incidence ratio is up to 91.6%in many countries,representing the third leading cause of cancer-related deaths.systemic drugs,including the multikinase inhibitors sorafenib and lenvatinib,are first-line drugs used in HCC treatment.Unfortunately,these therapies are ineffective in most cases due to late diagnosis and the development of tumour resistance.Thus,novel pharmacological alternatives are urgently needed.For instance,immune checkpoint inhibitors have provided new approaches targeting cells of the immune system.Furthermore,monoclonal antibodies against programmed cell death-1 have shown benefits in HCC patients.In addition,drug combinations,including first-line treatment and immunotherapy,as well as drug repurposing,are promising novel therapeutic alternatives.Here,we review the current and novel pharmacological approaches to fight HCC.Preclinical studies,as well as approved and ongoing clinical trials for liver cancer treatment,are discussed.The pharmacological opportunities analysed here should lead to significant improvement in HCC therapy.

After breeding,Black-necked Grebes(Podiceps nigricollis)perform a moult-migration to autumn hypersaline staging sites,where they moult the flight feathers and forage on superabundant brine shrimp(Artemia spp.)before leaving for wintering areas.During the stay in moulting sites,the grebes experience changes in organs and muscle size(atrophy,hypertrophy),and almost double their body mass,which has been suggested to act as an insurance against a collapse in prey availability in late autumn.During two years we collected blood samples from hundreds of individuals at one of the most important European moulting sites(the Odiel marshes,sW spain),which is a highly polluted area due to mining drainage and chemical industry.We assessed the potential effect of moulting stage,day of the year and body condition on 16 blood biochemical parameters.Because of the changes in prey availability and body composition of grebes,we expected some physiological adjustments during moult.Elevated levels of cholesterol suggested that birds in active moult increased foraging effort to face the costs of moulting.There was increased amount of lactate dehydrogenase,corresponding to periods of breast muscle atrophy.Birds in active moult augmented protein ingestion,likely to account for the requirements of feather growth.We also show that the probability of fasting due to low prey availability increased late in the moulting season,as demonstrated by an increase in plasmaβ-hydroxy-butyrate.
