Hepatitis c virus treatment with glecaprevir and pibrentasvir in patients co-prescribed carbamazepine:Three case reports

BAcKGROUND Highly effective and well-tolerated direct-acting antiviral(DAA)therapies have revolutionised the management of hepatitis c virus(HcV);however,niche populations face treatment *** co-prescribed with several firstgeneration anti-epileptic drugs(AEDs)are contraindicated due to drug-drug interactions.A common example is carbamazepine whereby steady-state carbamazepine reduces the maximum concentration and area under the curve of velpatasvir,glecaprevir and pibrentasvir due to potent cytochrome P450(cYP)3A4 *** also induces P-glycoprotein which reduces glecaprevir and pibrentasvir’s area under curve to infinite *** and glecaprevir-pibrentasvir are contraindicated in patients who are co-prescribed carbamazepine due to the risk of reduced DAA therapeutic effect and consequently,virological treatment *** presents a challenge for patients in whom carbamazepine substitution is medically unfeasible,impractical or ***,the properties of current generation DAA therapies,including high-potency non-structural protein 5A inhibitory effect,may be sufficient to overcome reduced bioavailability arising from carbamazepine related cYP 3A4 and P-glycoprotein *** SUMMARY We present a case series of three patients with non-cirrhotic,treatment-naïve,genotype 1a,1b,and 3a HcV who were treated with a 12 wk course of glecaprevir-pibrentasvir,while co-prescribed carbamazepine for seizure ***-pibrentasvir combination therapy was chosen due to its potent in vitro activity and low barrier to pan-genotypic resistance associated *** therapy was dose-separated from carbamazepine to maximise time to peak concentration,and taken with meals to improve *** virological response at 12 wk was achieved in each patient with no adverse *** DAA therapies,including glecaprevir-pibrentasvir,warrant consideration as a therapeutic agent in people with HcV w

Evaluation of Three <i>Helicobacter pylori</i>Eradication Protocols in a Digestive Endoscopy center in Dakar

Introduction: The treatment of Helicobacter pylori (H. pylori) requires the combination of antibiotic therapy with an antisecretory agent. Due to increasing antibiotic resistance, which varies from one geographical region to another, several eradication protocols exist. Objective: The objective is evaluate and compare the efficacy of three treatment regimens for H. pylori infection: 1) Arm A: Omeprazole + Amoxicillin + Clarithromycin for 10 days;2) Arm B: Omeprazole + Amoxicillin + Metronidazole for 10 days;3) Arm C: Sequential treatment = Omeprazole + Amoxicillin for 5 days then Omeprazole + Clarithromycin + Metronidazole for 5 days. Patients and Method: This was a prospective randomised study conducted from February 2016 to July 2016 and from April 2017 to September 2017 in the digestive endoscopy center of the gastroenterology and hepatology department of the University Hospital center Aristide Le Dantec. Our study population consisted of all patients aged 18 years or older referred for upper GI endoscopy. We included all patients whose indication for the examination was epigastralgia and/or dyspepsia and whose rapid urease test was positive. Patients were randomized to the different treatment arms. A 13c-urea breath test was performed at least 4 weeks after stopping antibiotics and 2 weeks after stopping proton pump inhibitor (PPI). Results: We included 120 patients. There were 95 women and 25 men. The mean age was 40 years. Epigastralgia and dyspepsia were present in 90% and 59% of cases, respectively. Upper G

An Unusual case of Submandibular Sialolithiasis in Early childhood: A case Report

Sialolithiasis is the most common condition of the salivary glands and it is characterized by the development of a calcified structure within a salivary duct. This condition can lead to inflammation, bacterial infection, and abscess. The different forms of management range from the direct massage of the duct to the complete removal of the gland. This report describes an unusual case of submandibular sialolithiasis in a four-year-old girl. The main complaints were pain when eating and swelling in the floor of the mouth. The diagnosis was sialolithiasis in the duct of the submandibular gland. Since the salivary stone was palpable and located in the anterior portion of the duct, a minimally invasive procedure was planned. An excisional biopsy was performed and a unilateral salivary stone was removed. The histopathological analysis confirmed the diagnosis. Although rare in children, understanding the diagnostic process and the different treatment modalities leads to a favorable prognosis.

Retention of <i>β</i>-carotene in cream-Fleshed Sweetpotato-Based complementary Food Stored in Different containers under Simulated Tropical Temperature and Humidity

The stability of β-carotene in cream-fleshed sweetpotato-based complementary food stored in three different containers under simulated tropical temperature of 32°c and 85% relative humidity in an environmental chamber for 24 weeks was investigated. The formulation stored in a metallised polyester film in a refrigerator (approximately 3.0°c and 95% relative humidity) retained about 81% of β-carotene at 24 weeks, with no significant change in the moisture content. The formulation stored in either a metallised polyester film or a translucent polypropylene container placed in the environmental chamber retained approximately half of the β-carotene at 24 weeks, with a slight increase in moisture content. However, the formulation held in a transparent low-density polyethylene bag had the lowest β-carotene retention (37%) and the highest moisture content at week 24. Within the limits of this study, it has been demonstrated that β-carotene in cream-fleshed sweetpotato-based complementary food was well retained when moisture uptake was minimised.

随着汉语在全球地位的不断上升,加上当今社会“移民热”现象的发生,在这种特殊背景下美国汉语学习的热潮也席卷而来,如同全球外籍人士学习汉语的热潮,许多美籍华裔华侨的子女也必然卷入了这场汉语热。华人比例在美国日益渐长,但大部分在美国出生生长的华人子女母语并不是汉语,他们可能通过父母的熏陶下只会最简单的日常交流。在基本没有语言背景的情况下,《快乐儿童华语》在美国的国际学校成为了最合适的教材,与此同时中国国家汉办出版的《快乐汉语》使用了第二语言教学法,这两本教材由于符合两种语言的实际教学,因此受到了广泛的赞誉,成为许多外籍学习者最受欢迎的汉语教材。本文将这两本教材进行对比分析,通过美国外语教学委员会制定的“5c标准”来分析二者的优势与不足,希望可以帮助对外汉语教材的编写以及修订有更大的进步。

The development of multilayer network techniques is a boon for researchers who wish to understand how different interaction layers might influence each other,and how these in turn might influence group ***,we investigate how integration between male and female grooming and aggression interaction networks influences male power trajectories in vervet monkeys chlorocebus *** previous analyses of this phenomenon used a monolayer approach,and our aim here is to extend these analyses using a dynamic multilayer *** do so,we constructed a temporal series of male and female interaction *** then used a multivariate multilevel autoregression model to compare cross-lagged associations between a male's centrality in the female grooming layer and changes in male Elo *** results confirmed our original findings:changes in male centrality within the female grooming network were weakly but positively tied to changes in their Elo ***,the multilayer network approach offered additional insights into this social process,identifying how changes in a male's centrality cascade through the other network *** dynamic view indicates that the changes in Elo ratings are likely to be short-lived,but that male centrality within the female network had a much stronger impact throughout the multilayer network as a whole,especially on reducing intermale aggression(i.e.,aggression directed by males toward other males).We suggest that multilayer social network approaches can take advantage of increased amounts of social data that are more commonly collected these days,using a variety of *** data are inherently multilevel and multilayered,and thus offer the ability to quantify more precisely the dynamics of animal social behaviors.

Hand rim wheelchair training: Effects of intensity and duration on physical capacity

The purpose of this study was to compare the effects of intensity and duration of training on physical capacity in a 7 weeks hand rim wheelchair training in able-bodied men. Thirty-six able-bodied men participated in three groups: a 30% heart rate reserve (HRR) 70 min training group (N = 14), a 70% HRR 30 minutes training group (N = 13) and a 30% HRR 30 minutes training group (N = 9). All groups trained 3 times per week for 7 weeks on a treadmill. Pre and post tests on a wheelchair ergometer comprised a submaximal test at 20% and 40% of the estimated peak power output, in which submaximal heart rate, oxygen uptake and mechanical efficiency were determined. In maximal exercise tests, maximal isometric strength, sprint power, peak power output and peak oxygen uptake were measured. No significant differences were found between the training groups on submaximal and maximal parameters. It can be concluded that, in persons new to wheelchair use, seven weeks of wheelchair endurance training at an intensity of 30% HRR for 30 minutes is as effective as a training at a higher intensity (70% HRR) or with a longer duration (70 min).

Persistently High Glycated Hemoglobin in a Subgroup of Type 2 Diabetic Patients Who Failed Usual Oral Antihyperglycemics and Insulin in côte d’Ivoire

Background: Type II diabetes mellitus is associated with multiple metabolic derangements which can cause secondary pathophysiological changes in multiple organ systems. This in turn can impose a heavy burden of morbidity and mortality from micro‑ and macro‑vascular complications. This study aimed to describe the metabolic and therapeutic profile of a subgroup of type 2 diabetic patients who have treatment failure with oral anti-hyperglycemic agents with persistent hyperglycemia despite insulin treatment. Methods: 60 type 2 diabetic patients in treatment failure with oral antidiabetics and under insulin treatment, aged 35 to 70 years, were recruited at the Diabetes clinic of the University Teaching Hospital of Treichville in Abidjan, côte d’Ivoire. Blood samples were collected in tubes containing Ethylenediaminetetraacetic Acid (EDTA) to determine glycated hemoglobin (HbA1c). Results: The average age of the population was 54 ± 9.38 years with a sex ratio (M/F) of 0.3, an average BMI of 30.25 ± 5 kg/m2, and an average HbA1c of 10.1% ± 1.6% for an average diabetes duration of 11.8 ± 5.8 years. The average insulin dose was 74.556 ± 16.21 UI/day, and the average duration of insulin treatment was 5.4 ± 3.1 years. The average HbA1c value was 10.1% ± 1.87% in men against 10.03% ± 1.53% in women with no significant difference (p = 0.1). The mean HbA1c values according to patient weight were 10.08% ± 2.05% for normal weight, 9.55% ± 2.26% for overweight, and 10.57% for obese, with no significant difference between the three groups of patients (p = 0.1). conclusion: This study showed a persistence increase in glycated hemoglobin regardless of the treatment regimen, duration, and dose of insulin treatment in the subpopulation of type 2 diabetic patients.

Introduction: Liver cirrhosis is a global public health issue. Our aim was to determine the epidemiological, clinical, paraclinical and therapeutic aspects of liver cirrhosis in the hepato-gastroenterology department of the hospital Aristide Le Dantec in Dakar. Patients and Methods: We conducted a retrospective study with a descriptive focus covering the period from January 1st, 2010 to December 31, 2020. We included the medical records of patients which presented body of clinical and paraclinical arguments which supported the diagnosis of cirrhosis. The data collected were related to age, gender, clinical, paraclinical, therapeutic and prognosis aspects of cirrhosis and were analyzed with the software Sphinx Plus. Results: Prevalence of cirrhosis was 6.2%. Sex ratio was 2.1%. The average age was 38 years. Asthenia (60.9%), altered performance status (60.1%), abdominal pain (37.2%), gastrointestinal bleeding (29.6%) and abdominal swelling (27.8%) were the most common motives of consultation. Physical examination revealed primarily portal hypertension (74.9%), liver failure (2.4%), hepatomegaly (28.2%) and anemia (13.7%). Viral hepatitis B was the most common etiology (81.9%). Abdominal screening (ultrasound/cT scan) showed hepatomegaly (80.6%), liver dysmorphia (87.7%), portal hypertension signs (85.3%) and portal vein thrombosis (18.2%). 55.1% Patients were classified as child Pugh A, 33.8% were child Pugh B and 11.1% were child Pugh c. Tenofovir Disoproxil Fumarate (TDF) was prescribed to all patients affected by hepatitis B virus only. Evolution was characterized by clinical improvement and prognosis reclassification. conclusion: Liver cirrhosis is a frequent pathology in the hepato-gastroenterology department of the hospital Aristide Le Dantec in Dakar. It affects young male adult. Hepatitis B virus is the leading cause. TDF is an effective treatment.
