Evaluation of current surgeon practice for patients undergoing lumbar spinal fusion surgery in the United Kingdom

AIM: To ascertain current surgeon practice in the United Kingdom National Health Service for the management of patients undergoing lumbar spinal fusion surgery. METHODS: Descriptive survey methodology utilised an online questionnaire administered through SurveyMonkey. Eligible participants were all surgeons currently carrying out lumbar spinal fusion surgery in the National Health Service. Two previous surveys and a recent systematic review informed questions. Statistical analyses included responder characteristics and pre-planned descriptive analyses. Open question data were interpreted using thematic ***: The response rate was 73.8%. Most surgeons(84%) were orthopaedic surgeons. Range of surgeon experience(1-15 years), number of operations performed in the previous 12 mo(4-250), and range of information used to predict outcome was broad. There was some consistency of practice: most patients were seen preoperatively; all surgeons ensured patients are mobile within 3 d of surgery; and there was agreement for the value of post-operative physiotherapy. However, there was considerable variability of practice: variability of protocols, duration of hospital stay, use of discharge criteria, frequency and timing of outpatient follow up, use of written patient information and outcome measures. Much variability was explained through patient-centred care, for example, 62% surgeons tailored functional advice to individual patients. CONCLUSION: Current United Kingdom surgeon practice for lumbar spinal fusion is described. The surgical procedure and patient population is diverse, and it is therefore understandable that management varies. It is evident that care should be patient-centred. However with high costs and documented patient dissatisfactionit is important that further research evaluates optimal management.

Genetics and Characteristics of a Pigmentation Defective Laboratory Strain of the Lady Beetle, Coleomegilla maculata

Beetles in the family Coccinellidae, commonly known as ladybugs, lady beetles, or ladybirds, are easily identifiable and popular beneficial insects. Current research aims to support conservation efforts of beneficial insects in agroecosystems by exploring genetic processes related to nutrition. As a part of this research, colonies of Coleomegilla maculata have been maintained in culture and inbred over many generations since 2009. One result of this inbreeding has been the discovery of novel morphological phenotypes unique to laboratory strains or present in wild populations at such low levels that they have not yet been described. One such phenotype is described here. The strain described here, ye (yellow elytra and eyes) was characterized with classical Mendelian breeding and digital image analysis. This phenotype differs from wild populations by possessing yellow pigment in the elytra and pale grey to white eyes. In contrast, wild populations of C. maculata possess pink or red pigmented elytra with black spots, and black eyes. C. maculata is not known to exhibit polymorphism in the field. Inheritance is autosomal and recessive. This species was not previously known to exhibit the dramatic variation of color described here. The strain is stable in the homozygous recessive form, and retains laboratory rearing characteristics similar to the wild type laboratory strain.

Graft loss among renal-transplant recipients with early reduction of immunosuppression for BK viremia

AIM To review the incidence of graft loss and acute rejection among renal transplant recipients with early reduction of immunosuppression for BK *** We performed a retrospective analysis of consecutive de-novo kidney-only transplants from January 2009 to December 2012 to evaluate the incidence of Polyomavirus associated nephropathy(PyV AN). Recipient plasma was screened for BKV DNA via quantitative polymerase chain reaction(PCR) at months 1,3,6,9 and 12 post-transplant and on worsening graft *** was reduced at ≥ 3-log copies/mL. Those with viremia of ≥ 4-log copies/mL(presumptive PyV AN) underwent renal transplant biopsy. Presumptive Py VAN(PP) and definitive Py VAN(DP; biopsy-proven) were treated by immunosuppression reduction(IR) only. RESULTS Among 319 kidney transplant recipients,the median age was 53 years(range 19-83),65.8% were male,and 58.9% were white. Biopsy-proven acute rejection was found in 18.5% within 0-168 wk. Death-censored graft loss occurred in 5.3%(n = 17) and graft loss attributable to PyV AN was 0.6%(n = 2). Forty-seven patients were diagnosed with PP(14.7%) and 18(5.6%) with DP. Graft loss among participants with PyV AN(8.5%) and those without(4.8%) was not significantly different. Deceased donor kidney transplantation(OR = 2.3,95%CI = 1.1-4.6) and AR(OR = 2.3,95%CI = 1.2-4.7) were associated with Py VAN in the multivariate analysis. BK viremia between 3 and 4-log copies/mL occurred in 27 patients,all of whom underwent IR. Of these,16(59%) never developed PyV AN while 11(41%) developed PyV AN(4 DP,7 PP) within a range of 11-39 wk. CONCLUSION Instituting an early reduction of immunosuppression,in the absence of adjunctive antivirals,is effective at preventing PyV AN and may be associated with a lower incidence of graft-loss without a reciprocal increase in the incidence of acute rejection.
