Introduction: Since the times of our ancestors, traditional medicine has existed to cure all diseases [1]. The objective of this study was to assess the complications of traditional fracture treatment. Patients and Methods: This was a prospective study extending from January 01, 2018 to June 30, 2018, duration of six months. It concerned patients who had traditionally been treated beforehand for traumas with bone lesion in a traditional therapist and who had developed a complication, the definitive management of which was made in the department. Results: This study involved 102 patients, including 71 men (69.6%), with a sex ratio of 2.2. 33 patients or 32.4% were under 15 years old. The average of our patients’ age was 29.61 years with extremes of 2 and 89 years. Pupils/students (44 pupils and 5 students) were the most represented with 49 cases or 48.1% of the cases. The majority of patients (59.8% of cases) were educated or had a basic level. The majority of patients (36.3%) came as motif for a painful swelling. Gangrene was the most common complication with 37 cases or 36.3% of cases. Conclusion: At the end of our study, we have noticed that the traditional preliminary treatment of fractures was based mainly on trial and error. Practicing with inadequate means and measures without respecting the anatomical structures, is a real source of disabling handicaps. We have recorded 38, among the 102 patients who came for a complication of traditional treatment: that is 45.2% amputation, which is deplorable.

Understanding metal accumulation at organ level in roots, leaves and seeds in O. glaberrima (OG) is crucial for improving physiological and metabolic aspects in growing Asian and African rice in salted areas. The micro-analytical imaging techniques are required to reveal its accumulation and distribution within plant tissues. PIXE studies have been performed to determine different elements in rice plants. The existing microbeam analytical technique at the iThemba LABS will be applied for the 2D image mapping of fresh rice tissues to perform a concentration of low atomic mass elements (such as Al, Si, P, S, Cl, Ca, Ti, Mn, Fe, Cu, Br, Zn and K) with detection limits of typically 1-10 μg/g. Comparison of the distribution of the elements between leaves, root and seed samples using uptake and distribution of elements in particular environmental conditions with potential amount of salt in water have been performed. We are also expecting to indicate metal exclusion as salt tolerance strategies from leaves, root, and seed compartments using matrix correlation between samples and between elements on rice species.

Management of Subtalar Fractures-Dislocations at Ségou Hospital in Mali: A 7 Cases Series

Introduction: Subtalar or peritalian dislocation is rare;it represents 15% of peritalian injuries. The aim of this study was to describe the radioclinical and therapeutic characteristics and to assess the functional result. Patients and Methods: This study is about a continuous descriptive and prospective study over a period of 5 years. It has been conducted from March 2013 to February 2018 at the Ségou Hospital, a second referral hospital in Mali. The functional results were evaluated by the score of the American Orthopedic Foot and Ankle Society (AOFAS). Results: Seven cases of subtalar dislocation fractures were diagnosed in six male patients with an average age of 38 years (36 to 47 years old). In five cases the etiology of the trauma was a motorcycle accident and a fall from height. The lesion was bilateral in one case. The average time to care was 3 hours (1 to 9 hours). There was one case of open dislocation fracture with talus enucleation. The dislocation was medial in 6 cases and lateral in one case. It was pure in three cases. The treatment was orthopedic in 2 cases and surgical in 5 cases. The post-operative complications were complicated by an infection of the operative site in one case. The average length of hospital stay was 4 days. The functional result was excellent in 2 cases;good in 2 cases, fair in 2 cases and poor for 1 case. Conclusion: Subtalar dislocation is a rare and serious lesion of the posterior tarsus. The long-term prognosis depends on the earliness of treatment and the severity of the associated injuries.

Epidemiological and Diagnostic Features of Non-Traumatic Gonalgia in Adults in Bouake (Ivory Coast): About 140 Patients

Background: Non traumatic gonalgia is the second most common reason for rheumatology consultations and accounted for 15.6% of rheumatological disorders in Abidjan. Objective: The aim of this work was to study the epidemiological and diagnostic features of non-traumatic gonalgia (NTG) in a black adult population from sub-Saharan Africa. Patients and Methods: This was a five-year retrospective descriptive study (January 2014 to December 2018) of black sub-Saharan African adult patients with non traumatic gonalgia seen in the rheumatology department of the Bouake University Hospital in Côte d’Ivoire. Results: Of 2198 patients seen for rheumatological conditions, 140 had non traumatic gonalgia (6.4%). The mean age of patients was 56.13 ± 13.3 years (extremes 20 and 87 years), and the sex ratio was 0.43 (42 men and 98 women). Gonalgia without swelling was observed in 33.6% of cases. Symptoms were predominantly progressive (97.1%), chronic (82.1%) and bilateral (50%). The causes of TNM were gonarthrosis (85%), septic arthritis (7.1%), reactive arthritis (2.9%), gout (2.9%), rheumatoid arthritis (1.4%) and chondrocalcinosis (0.7%). Gonarthrosis (119/140) was bilateral in 65 patients, bi- or tri-compartmental in 83, radiographic stage (Kellgren and Lawrence) III in 80 and IV in 15. A germ was isolated in two cases of septic arthritis (methicillin-sensitive Staphylococcus aureus and Streptococcus pneumoniae). Conclusion: GNT is common in rheumatology practice in Bouake, and mainly affects women in their fifties. It is most often a chronic bilateral gonalgia, with gonarthrosis as the main etiology.

Objective: To study the epidemiological and diagnostic features of chronic rheumatic disease (CRD) in a black population in sub-Saharan Africa. Patients and Methods: Retrospective descriptive study over eight years (January 2005 to December 2012) of patients seen for CRD in rheumatology hospitalization at the CHU de Cocody in Abidjan, Côte d’Ivoire. Results: Of 3147 hospitalized patients, 92 had CRD, a frequency of 2.9%. The mean age of patients was 43.50 ± 15.6 years (extremes: 10 to 79 years). The sex ratio was 0.08 (7 men and 85 women). The socio-economic level was low in 44 patients (47.8%), medium in 44 patients (47.8%), and high in 4 patients (4.4%). The reasons for consultation were polyarthritis (64.1%), polyarthralgia (30.4%), and oligoarthritis (5.4%). The mean duration of symptomatology was 19 months (range: 3 to 20 years). The mean length of hospital stay was 14.2 ± 7.7 days (range: 2 to 36 days). The CRD observed were: rheumatoid arthritis (59.8%), systemic lupus erythematosus (23.9%), mixed connective tissue disease (6.5%), undifferentiated connective tissue disease (3.3%), polymyositis (3.3%), systemic scleroderma (2.2%), and systemic vasculitis (1%). Conclusion: CRD is not uncommon in rheumatology hospitals in Abidjan. Diagnostic delays are long, and rheumatoid arthritis and systemic lupus erythematosus are the most frequently encountered conditions.

Circumcision refers, in its most widespread form, to the total or partial removal of the foreskin, leaving the glans exposed. It is a ritual practice imposed by Mosaic law for Israelites, a tradition for Muslims, a rite of passage to adulthood for many people of Africa and the East. Circumcision is also a widely used therapeutic surgical procedure for a number of balanopreputial affections. The aim of this study was to list cases of circumcision complications received in a urological hospital (CHR Saint louis) and to analyze their epidemiological, anatomo-clinical and therapeutic aspects. Patients and methods: This was a retrospective, descriptive study of circumcision complications, carried out in the urology department of the Saint louis regional hospital in Senegal over a 5-year period (January 1, 2013 to December 31, 2017). Results: Thirty-three (33) complications of circumcision were collated. The mean age of patients was 7.63 years [9 months - 16 years]. Duration of time for Consultation varied according to the type of complication, with an average of 1.6 years [1 day - 10 years]. Penile granuloma accounted for 36.3% of the complications (n = 12), local infection for 30.3% (n = 10) and urethro-cutaneous fistula for 18.2% (n = 6). For six patients with urethro-cutaneous fistula, we performed urethro-cutaneous splitting in 5 patients, while the same complication necessitated urethroplasty in another patient. We noted five cases of verge plasty for verge granuloma, one glan reimplantation and one glan plasty for the two patients with glans amputation. Conclusion: Circumcision is a medical procedure widely practised throughout the world for medical, cultural and aesthetic reasons. In Africa, the lack of health coverage and the shortage of doctors explain why this surgical procedure is so common. In Senegal, circumcision is still carried out in the majority of cases by paramedical staff, which explains the high rate of complications.

Lumbar Hernia, a Rare Cause of Intestinal Occlusion: About a Case

Defects of the posterolateral abdominal wall of the abdomen, lumbar hernias are rare. They represent 2% to 3% of hernias of the abdominal wall. This rarity explains the large number of short series and the absence of a well-defined therapeutic modality in the publications. Grynfeltt’s hernia at the top and Jean louis Petit’s at the bottom are the two entities. The old lumbar hernias are bulky with a rich history and easy diagnosis. Though young, they are discreet and reveal most often by their strangulation. We report the case of a Grynfeltt hernia revealed by colonic occlusion in a 77-year-old woman who had a superior right posteriolateral tumefaction for 15 years. The hernia cure was prosthetic made by the sandwich technique. No recurrence was observed after 25 months of follow-up. Through a review of the literature, we reveal the diagnostic and therapeutic difficulties.

Background: Hypospadias is a congenital malformation resulting from hypoplasia of the tissues forming the ventral face of the penis. It is associated to variable degrees with an ectopic urethral meatus, a penis curvature and a sapper’s apron foreskin [1]. The incidence of this malformation varies from country to country. It is estimated at 1/300 male births in France and 0.26/ 1000 in Mexico [1]. In Senegal, the prevalence of this malformation is unknown. Hypospadias surgery has improved significantly in recent years due to a better understanding of the anatomy on the one hand and the improvement of the instruments used during surgery on the other (suture material, surgical magnification, urethral catheter of suitable size). In sub-Saharan Africa and particularly in Senegal, hypospadias surgery remains a challenge due to the unavailability of adequate equipment (lack of microsurgical instruments and surgical loupes). Objective: To evaluate the outcomes of the treatment of hypospadias by the different surgical techniques used in our center as well as their sexual function in adulthood. Patients and Methods: This is a retrospective study, including all patients operated for hypospadias between January 2009 and December 2017 in Urology-Andrology department of Aristide Le Dantec hospital. The studied parameters were: frequency, age, clinical and therapeutic aspects, and their sexual function in adulthood after treatment. The outcomes of the treatment were judged good or poor depending of the quality of penile straightening, the aesthetic appearance of the penis, the position of the urethral neo-meatus, the permeability of the urethra and the existence or not of fistula. The sexual function was assessed by the International Index of Erectile Function 15 (IIEF) score. Results: Fifty-five patients were included. The median age was 6 years (P25 = 2.8 and P75 = 13 year
