Objectives: To assess the socio-professional determinants of burnout among the medical staff of Dakar University Hospitals. The aim was to propose effective means of prevention. Material and Methods: This was a cross-sectional, descriptive, and analytical study of physicians in permanent contact with patients at the Fann and Aristide Le Dantec teaching hospitals in Dakar, senegal. Results: A total of 159 doctors were included in the study, with a sex ratio (M/F) of 1.3 in favor of men. The prevalence of burnout was 91.8%. Burnout was significantly associated (p conclusion: Burnout is a threat to doctors at university hospitals in senegal. It has several determinants linked above all to work organization. It is essential to set up psychological support units in hospitals and integrate occupational medicine into various organizational processes.

脂溢性皮炎是一种慢性炎症性皮肤病,好发于头皮、面部和躯干等皮脂腺分布丰富的部位。头皮脂溢性皮炎主要表现为红斑和片状鳞屑,程度从轻度鳞屑到弥漫性大片黏着性鳞屑不等,伴有明显的瘙痒,并容易反复发作。目前认为脂溢性皮炎的发生与亲脂性马拉色菌定植增加以及遗传、环境和全身健康状况等因素相关,上述因素共同导致皮脂腺代谢活动异常、皮脂成分改变、表皮屏障功能障碍,最终加重炎症反应。

Genital cancers in women play an important role in cancer pathology in the developing countries. Objectives: This study aimed at clarifying the epidemiological and histopathological aspects of woman genital cancers in c?te d’Ivoire. Materials and Methods: This was a retrospective and descriptive study conducted in the anatomical pathology laboratories of the university hospitals in Abidjan. The study was carried out over a period of twenty-four years (1984-2007). The variables studied were: frequency, age, histological type and prognosis. Results: Genital cancers in women accounted for 41.28% (n = 2491) of cancer in women and 21% of cancers. The average age of patients was 46.65 years (range 2 - 88 years). The cervix (82.85%) was the main location followed by the ovary (8.6%). Histologically, carcinomas (92.93%) were the most common types of sarcomas. cervical cancers were diagnosed at an average age of 47.36 years (range 2 to 88 years). carcinomas were the predominant histological type (92.88%) with 57.4% (n = 450) diagnosed at pT2N0M0 stage. The average age of patients bearing ovarian cancer was 39.13 years (range 8 to 82 years).common malignant epithelial tumours were the most common histological type (57.48%) (n = 123) followed by non-Hodgkin lymphoma (14.95%). conclusion: Genital cancers in women are common and poorly prognosed in cote d’Ivoire. The adoption of a policy of routine screening is needed to improve the prognosis of these tumours.

OBJEcTIVEs: We determined whether caloric restriction(cR) has cardiac specific effects that attenuate the established aging-associated impairments in diastolic function(DF). BAcKGROUND: caloric restriction retards the aging process in small mammals; however, no information is available on the effects of long-term cR on human aging. In healthy individuals, Doppler echocardiography has established the pattern of aging-associated DF impairment, whereas little change is observed in systolic function(sF). METHODs: Diastolic function was assessed in 25 subjects(age 53± 12 years) practicing cR for 6.5± 4.6 years and 25 age- and gender-matched control subjects consuming Western diets. Diastolic function was quantified by transmitral flow, Doppler tissue imaging, and model-based image processing(MBIP) of E waves. c-reactive protein(cRP), tumor necrosis factor-alpha(TNF-α ), and transforming growth factor-beta1(TGF-β 1) were also measured. REsULTs: No difference in sF was observed between groups; however, standard transmitral Doppler flow DF indexes of the cR group were similar to those of younger individuals, and MBIP-based, flow-derived DF indexes, reflecting chamber viscoelasticity and stiffness, were significantly lower than in control subjects. Blood pressure, serum cRP, TNF-α , and TGF β 1 levels were significantly lower in the cR group(102± 10/61± 7 mm Hg, 0.3± 0.3 mg/l, 0.8± 0.5 pg/ml, 29.4± 6.9 ng/ml, respectively) compared with the Western diet group(131± 11/83± 6 mm Hg, 1.9± 2.8 mg/l, 1.5± 1.0 pg/ml, 35.4± 7.1 ng/ml, respectively). cONcLUsIONs: caloric restriction has cardiac- specific effects that ameliorate aging-associated changes in DF. These beneficial effects on cardiac function might be mediated by the effect of cR on blood pressure, systemic inflammation, and myocardial fibrosis.

Objective: To investigate intraocular penetration of moxi-floxacin hydrochloride after oral administration. Methods: Prospective study of 15 patients scheduled for vitrectomy between september and November 2004 at the Barnes Retina Institute,st louis,Mo. Aqueous,vitreous,and serum samples were analyzed from 15 patients after oral administration of 2 tablets containing 400 mg of moxifloxacin. Assays were performed using high-performanceliquid chromatography. Results: The mean± sD moxifloxacin concentrations in plasma (n=15),vitreous (n=13),and aqueous (n=13) samples were 3.56± 1.31 μ g/mL,1.34± 0.66 μ g/mL,and 1.58± 0.80 μ g/mL,respectively. Mean± sD sampling times after oral administration of the second moxifloxacin tablet for plasma,vitreous,and aqueous were 2.94± 0.81 hours,3.77± 0.92 hours,and 3.71± 0.89 hours,respectively. The percentages of plasma moxifloxacin concentration in the vitreous and aqueous were 37.6% and 44.3% ,respectively. Minimal inhibitory concentrations against 90% levels were exceeded against a wide spectrum of gram-positive and gram-negative pathogens in the vitreous and aqueous. conclusions: Moxifloxacin has a spectrum of coverage that encompasses the most common organisms in endophthalmitis. The pharmacokinetic findings of this investigation reveal that orally administered moxifloxacin achieves therapeutic levels in the noninflamed eye. Because of their broad spectrum of coverage,low minimal inhibitory con centration against 90% levels,good tolerability,and excellent oral bioavailability,fourth-generation fluoroquinolones may represent a major advance for managing posterior segment infections.
