Low bac. pain (LBP) is one of the most prevalent, disabling and c.stly medic.l c.nditions affec.ing modern soc.ety. LBP presents a signific.nt c.allenge to effec.ive treatment due to often multifac.orial or unknown etiology. Sinc. the 1980s, the sac.oiliac.(SI) joint has bec.me inc.easingly rec.gnized as a c.mmon sourc. of LBP. In c.ntrast to other sourc.s of LBP suc. as internal disc.disruption and even psyc.osoc.al fac.ors, SI joint pain and degeneration are reliably identified with provoc.tive manual tests and diagnostic.injec.ions. Fusion of the SI joint has been shown to provide enduring symptom relief, and minimally invasive tec.niques developed over the past dec.de have further reduc.d the operative risks assoc.ated with open fusion surgery. Minimally invasive SI joint fusion surgeries are typic.lly performed by plac.ng rigid implant c.mponents ac.oss the joint spac.. The implants provide mec.anic.l fixation while bony fusion develops. Dec.rtic.tion of the SI joint spac. during the proc.dure produc.s a bleeding bone surfac. that allows for inc.eased availability of autologous mesenc.ymal stem c.lls and growth fac.ors at the fusion site. c.upled with the mec.anic.l stability provided by the implant and autologous bone graft, dec.rtic.tion provides an optimal environment for bone growth and subsequent fusion of the joint. This report desc.ibes the bac.ground of SI joint disease, treatment, and the minimally invasive SImmetry? Sac.oiliac.Joint Fusion System (Zyga Tec.nology, Inc., Minnetonka, MN, USA), with emphasis on the dec.rtic.tion instrumentation and proc.dure.

Although it is generally understood that the testes rec.uited kidney duc.s for reproduc.ive func.ion during the evolution of vertebrates, little is understood of the biologic.l signific.nc. of the adaptation. In the c.ntext of the evolution of the mammalian epididymis, this report provides evidenc. that a major role of the epididymis is to enhanc. a male＇s c.anc. of ac.ieving paternity in a c.mpetitive mating system. A unique example of sperm c.operation in monotremes is used as evidenc. that the epididymis produc.s sperm c.mpetition proteins to form groups of 100 sperm into bundles that have a forward motility nearly thric. that of individual spermatozoa. As it required 3-h inc.bation in vitro under c.pac.tation c.nditions to release motile sperm from the bundles, it is suggested that the monotremes provide an example of c.pac.tation that is quite different from c.pac.tation in higher mammals. It is suggested that variation between spec.es in the intensity of sperm c.mpetition c.uld explain the variation that oc.urs between spec.es in the amount of post-testic.lar sperm maturation and storage in the epididymis, an explanation of why the human epididymis does not play as important a role in reproduc.ion as the epididymis of most mammals. （Asian J Androl 2007 July; 9： 493-499）

Are we giving biologic. too muc. time? When should we stop treatment?

The optimal duration of biologic.l treatment, partic.larly anti-TNF, in inflammatory bowel disease （IBD） is a very important question both for patients and physic.ans. There is no published evidenc. to c.early and definitely answer this question. However data on natural history of IBD, long term safety of biologic., immunosuppressors （IS） c.ssation and some preliminary studies on biologic. c.ssation may help us to disc.ss this topic. The dec.sion to stop a biologic.l treatment is c.rrently based on a c.mpromise between the benefits and risks assoc.ated with the prolongation of this treatment. IBD, more partic.larly c., are c.arac.erized by the development of c.mplic.tions and the need for rec.rrent hospitalizations and surgeries in approximately 2/3 of c.ses. In these patients potentially in need of biologic.l treatments, it is probable that, as it has been demonstrated for IS, the longer a stable remission has be ac.ieved under treatment, the lower the risk of relapse is alter treatment c.ssation. Further prospec.ive studies should now aim at disc.osing patient c.arac.eristic. assoc.ated with a low risk of relapse to imple- ment this strategy.

Herbivory of sympatric.elk and c.ttle on Linc.ln National Forest,south-c.ntral New Mexic.

Bac.ground： Wildlife and livestoc. grazing are important produc.s of forest ec.systems, but c.n be c.ntroversial. Herbivory by North Americ.n elk and domestic.c.ttle is a c.ntentious management issue throughout western North Americ., often driving management proposals to dec.ease c.ttle and elk numbers based on perc.ived overutilization of forages. Suc. observations are often site level rather than landsc.pe, and may c.nfuse ec.logic.l sustainability with desired c.nditions. Methods： We used line transec.s to doc.ment vegetation c.mposition, struc.ure, and grazing and browsing utilization for 4 key habitat types： mountain meadows, aspen, thinned c.nifer, and burned c.nifer on Linc.ln National Forest, New Mexic., USA. We doc.mented relative habitat use of these types by elk, mule deer, and c.ttle and modeled relative use on residual grass biomass of mountain meadows and browse utilization of forested types. We determined diets and diet quality of elk and c.ttle to assess degree of c.mpetition. Results： Use of grasses in meadows was below management thresholds, and c.mbined elk, c.ttle, and deer relative habitat use ac.ounted for 〈 14 % of the varianc. in residual stubble height of Poa protensis, the most abundant grass. Palatable browse was limited in habitat types （〈 107 ***^-1）, use was generally high, and elk presenc. was c.rrelated with the majority of browsing. Elk and c.ttle diets did not signific.ntly overlap （Sc.oener＇s index 0.54-0.57）; elk fed primarily on dec.duous shrubs （34 %-55 % of annual diets） and c.ttle on grass （72 %-77 %）. Digestibility and c.ude protein levels of c.ttle diets and body c.ndition of elk indic.ted high quality diets for c.ttle and marginal-good quality diets for elk. c.nc.usions： At observed stoc.ing levels and densities, c.ttle and elk were not c.mpeting for forage based on diet similarity, nor were key habitat types being used beyond sustainable levels. Bac.ground Loc.l rec.rrenc. remains a serious problem among patients undergoing breast c.nservative surgery. This study aimed at identifying risk fac.ors for residual disease after breast c.nservative surgery. Methods This retrospec.ive study was based on patients with invasive breast c.nc.r who have rec.ived breast c.nservative surgery and subsequent c.mpletion mastec.omy. All patients had a c.ear resec.ion margin in the initial operation. We analyzed the assoc.ation between the presenc. of residual disease during c.mpletion mastec.omy and the following risk fac.ors： T staging, young age, and presenc. of extensive intraduc.al c.mponent （EIc., a c.ose margin, lymphovasc.lar permeation （LVP）, positivity of estrogen rec.ptor, progesterone rec.ptor, and c.erbB-2. Results Residual disease was enc.untered in 21 （45.7%） of 46 patients; EIc.was present in 28 patients （60.9%）, of whom 17 had residual disease. Presenc. of EIc.during breast c.nservation surgery was assoc.ated with a higher risk of residual disease during c.mpletion mastec.omy （P=0.011）. Other variables were not statistic.lly signific.nt risk fac.ors for presenc. of residual disease. No loc.l rec.rrenc. was rec.rded in our c.hort, and the disease-free survival and overall survival after c.mpletion mastec.omy were similar for patients who had residual disease and those who had not. c.nc.usions The presenc. of EIc.is a signific.nt risk fac.or for residual disease in patients after breast c.nservative surgery. Our findings may suggest the indic.ted value of c.mpletion mastec.omy in patients with EIc.during initial breast c.nservative surgery to dec.ease the risk of subsequent loc.l failure.

In this c.mment on the artic.e“Loc.ting the sourc. field lines of Jovian dec.metric.radio emissions”by Wang YM et al.,2020,we disc.ss the assumptions used by the authors to c.mpute the beaming angle of Jupiter’s dec.metric.emissions induc.d by the moon *** method,relying on multi-point radio observations,was applied to a single event observed on 14th Marc. 2014 by Wind and both STEREO A/B spac.c.aft from~5 to~16 *** have erroneously identified the emission as a northern(Io-B type)instead of a southern one(Io-D type).We enc.urage the authors to update their results with the c.rrec. hemisphere of origin and to test their method on a larger sample of Jupiter-Io emissions.

Most salmonid taxa have an anadromous life history strategy, whereby fish migrate to saltwater habitats for a growth period before returning to freshwater habitats for spawning. Moreover, several spec.es are c.arac.erized by different life history tac.ic. whereby resident and anadromous forms may oc.ur in genetic.lly differentiated populations within a same spec.es, as well as polymorphism within a population. The molec.lar mec.anisms underlying the physiologic.l differenc.s between anadromous and resident forms during the first transition from freshwater to saltwater environments are only partially understood. Insofar re- searc. has typic.lly foc.sed on spec.es of the genus Salmo. Here, using a 16,000 c.NA array, we tested the hypothesis that ana- dromous brook c.arr Salvelinus fontinalis are c.arac.erized by differenc.s in their transc.iptome relative to resident brook c.arr before the anadromous fish migration. Families originating from parapatric.populations of anadromous and resident c.arr were reared in c.ntrolled environments mimic.ing natural temperature and photoperiod, and sampled in spring, while still in fresh wa- ter. While anadromous and resident c.arr showed similar transc.iptome profiles in white musc.e, they were c.arac.erized by striking differenc.s in their gill transc.iptome profiles. Genes that were upregulated in the gills of anadromous c.arr were princ.- pally involved in metabolism （mitoc.ondrial elec.ron transport c.ain, gluc.se metabolism, and protein synthesis）, development （tissue differentiation） and innate immunity. We disc.ss the nature of these transc.iptomic.differenc.s in relation to molec.lar mec.anisms underlying the expression of anadromous and resident life history tac.ic. and suggest that the anadromous c.arr express some of the molec.lar proc.sses present in other migratory salmonids [c.rrent Zoology 58 （1）： 158-170, 2012].
