The aim of this study was to evaluate the level of protection of employees who are exposed to radiation in a level III hospital establishment. It was a descriptive cross-sectional survey of six months’ duration, involving eight level III Hospitals (Aristide Le Dantec, Fann, Hoggy, Hear, Abass Ndao, Pikine, Touba, and Thiès) in Senegal. Sixty-one of the one hundred questionnaires were recovered (overall response rate of 61%). The population of the study was mainly female (54.1%). The average age was 38.57 with extremes ranging from 23 to 65 years old. In the places where ionizing radiation sources are handled, only at the Aristide Le Dantec Hospital did we find a “competent person in radiation protection”. This explained the lack of a classification of employees and work areas. Forty out of sixty-one (73.77%) had no knowledge of the basic principles of radiation protection (justification, optimisation, dose limitation) and had not ever taken radiation holidays. For radiovigilance, exposure time limits to ionizing radiation concerned only 29/61 or 47.54% of the study population. The inverse square law of distance was known by only 40 workers, of whom 15 had no compliance. We found the presence of dosifilms in only 7/61 or 11.47% of the workers. On the other hand, the use of lead aprons was well established and concerned 57/61 workers, i.e., 93.44%. In sum, ionizing radiation causes adverse health effects. The absence of a good radiation protection culture in Senegal requires the presence of at least 4 to 5 competent persons in radiation protection for quality training of workers in radiobiology, radiopathology and radiation protection.

Research hospitals/academic medical centers have a threefold mission of patient care, research and education. The meaning of education is to train students with both clinical and administrative personnel in order to better suit themselves in the health care industry. Health administration programs are proliferating in China and the United States due to the influence of their recent health reform movements. In China, health administration programs are staffed with faculty from clinical medicine and management, mostly from their university affi liated medical centers. However, health administration program faculty in the United States have a diverse set of industry backgrounds and bring an invaluable wealth of experience into the classroom. The authors provide a literature background on the often-followed paths that a person in the U.S. might take while pursuing a full time teaching career in higher education. A health administration faculty typology has been well developed, and concentrating on the following professional roles: a healthcare operator, an academic/educational administrator, a consultant, a clinician and a researcher. Thus, the authors discuss anecdotal data that they have collected on the perceived positives and negatives associated for each type. The authors conclude with providing recommendations to both Chinese and American programs to produce a more collegial and collaborative academic environment.

Prevalence rates and cardiometabolic determinants of diabetes mellitus and pre-diabetes with projected coronary heart disease at bank site of Brazzaville

Background: Aim: Congolese Bank employees are often inactive without valid information on the burden of dysglycemia and cardiometabolic risk. This study aims to assess the prevalence rates of diabetes mellitus (DM) and pre-diabetes and to identify the environmental, genetic and cardiometabolic risk factors associated with Type 2 diabetes mellitus (T2DM) and pre-diabetes among Congolese bank employees. Methods: In representative 126 bank employees from Brazzaville, Congo, Central Africa, Abdominal obesity, dyslipidemia and metabolic syndrome (MetS) were defined by IDF for Europe, NCEP-ATPIII and IDF criteria modified for Central Africa. Projected high 10-year total risk of coronary heart disease (CHD) ≥20% was calculated using Framingham scores. Results: Out of the employees, 16% and 21.4% had DM and pre-diabetes, respectively. The rate of T2DM among diabetics was estimated 90%. Aging, high total cholesterol, high LDL-cholesterol, high conicity index and longer urban residence after migration were significantly associated with pre-diabetes. Physical inactivity, smoking, excessive alcohol intake, abdominal obesity, female gender, low HDL-C, hypertension, CHD, projected high 10-year total CHD risk, age ≥ 55 years, urban residence, Southern area residence, high socioeconomic status, non married status, MetS/NCEP, MetS/IDF for Europe and MetS/IDF for Africa were significantly associated with ***/IDF for Africa was the only independent determinant of T2DM. Conclusion: Urgent prevention and intervention programme are needed to curb the alarming increase in DM, T2DM, pre-diabetes.

Objective: Endotracheal intubation (ETI) is a life-saving emergency procedure, but it is a complex skill that is difficult to teach. Recent studies have shown that video laryngoscopy is effective in teaching ETI to learners at various levels of medical expertise;however, it has proven to be costly and provides images of inconsistent quality. In this educational proof of concept feasibility convenience sample pilot study, we aim to explore and compare the effectiveness of using modified Google Glass? (GG) and GoPro? (GP) technologies to visualize and teach ETI to critical care physicians in the austere medical environment of a low-income country. We propose, based on our findings, that this inexpensive technology could teach lifesaving ETI to pre-hospital providers in the austere medical environment, medical students, rural emergency physicians, critical care physicians in low-income countries, far forward military medical providers, and other learners. Methods: A case series of twenty-five patients, five in the United States (US) at Memorial Hospital in South Bend, IN and twenty at Saint Luc’s Hospital in Port Au Prince, Haiti, is presented. These patients were collected from November 1st 2015 through February 1st of 2016. The anesthesiologist and the emergency physicians in the United States utilized GG to intubate five patients in the US prior to the twenty patients intubated during two separate trips to Haiti. On the two separate trips to Haiti, the GG was trialed and modified to obtain better exposure. These adaptations resulted in the final collection of twenty patients studied with the adapted GG system and GP. Physicians graded airway visualization based on LEMON and Cormack-Lehane scores. Previously published parameters for the assessment of failed intubation risk and passage of the cords were used as data points for analysis using a Likert-Scale analysis for each parameter. Associated Factors with the Child’s Fully Immunized in the Tambacounda Health District (Senegal)

To improve vaccination coverage among children under one year of age in the Tambacounda health district, a household survey was carried out among mothers or babysitters. The objective was to study the factors related to child’s fully immunized in children aged 12 to 23 months. The cross-sectional, descriptive and analytical survey was carried out during the month of April 2019. A multistage cluster survey selected a sample of 657 mothers and babysitters. The data was collected using a questionnaire made from the World Health Organization reference guide. Data entry and analysis were done with Epi Info software and R. Among the women surveyed, biological mothers were the most representative (96.9%). In the series, 74.1% had a good knowledge of the age for initiating vaccination, 78.2% knew the number of contacts. The vaccination record of the children was available in 92.2%, and 71.0% of them had presented an adverse event. The proportion of children fully immunized was 41.0%. Complete childhood vaccination was positively associated with income-generating activity in women (0R = 2.4) and the short distance (<100 m) between home and place of vaccination (OR = 1.5). It was also improved by having a qualified health worker as a vaccinator (OR = 1.4) satisfaction in relation to visit (OR = 2.0), the advice given by the vaccinator (OR = 1.7) and the fixing of the date of the next vaccination appointment (OR = 2.5). The implementation of a good strategy for improving the quality of immunization services is an important element for strengthening immunization coverage in the Tambacounda health district.

Factors Associated with the Use of Cervical Cancer Screening in the Mbour Health District (Senegal)

Cervical cancer with an estimated incidence between 1700 and 2000 new cases per year and mortality around 70% to 80% has become the leading cancer in Senegal. The objectives of this present work were to study the factors associated with cervical cancer screening in the Mbour health district in 2015. It was a cross-sectional, descriptive and analytical study carried out on a sample of 599 women chosen according to a two-stage survey. The survey used a questionnaire where data on individual characteristics, knowledge, attitudes and practices are collected during an individual interview after informed consent. The data collected were entered in the Epi Info 3.5.3 software and analyzed with the R 3.1.2 software. The average age was 44.4 years with a standard deviation of 8.9. The proportion of women screened for cervical cancer was 19.4%. Screening for cervical cancer was positively related to the possession of an income-generating activity (Adjusted OR = 2.39 [1.4 - 4.2]), increase of education and knowledge of a person suffering from cancer in the environment (adjusted OR = 2.40 [1.3 - 4.2]). Knowledge of the methods of diagnosis of cervical cancer was positively associated with cervical cancer screening (adjusted OR = 20 [11.1 - 33.3]). The lack of awareness and the lack of financial resources and education for women were the main obstacles to cervical cancer screening for women in Mbour. The health system should focus on building the capacity of women to fight cervical cancer.

Determinants of Iron Consumption among Pregnant Women in Southern Senegal

The anemia by iron deficiency is a public health problem. To palliate the multiple maternal and fetal consequences, the WHO recommends the iron supplementation during at least 90 days to all pregnant women. The goal of our study is to study the determinants of this consumption in the Kolda area (Senegal). It’s the analytical cross-sectional study referred. We use the survey by clusters with 2 levels and it’s about all of the women who gave birth in Kolda area between February 2013 and January 2014. The dependent variable was iron consumption during at least 90 days and the independents variables were grouped on personal factors, knowledge and practices. Data were collected during a personal interview face to face. We used logistic regression to identify the determinants of this consumption. The average age of women surveyed in 1442 was 25.5 years. They had knowledge of iron consumption (93%) and the number of antennal consultation (ANC) (66%). The prevalence of pregnant women who consumed iron at least for 90 days was 51%. The factors associated with consumption were schooling (ORa = 2.49 [1.54 - 4.03]), health awareness (ORa = 1.61 [1.25 to 2.07]), knowledge about number of ANC (ORa = 1.54 [1.18 - 2.00]), councils on the benefits of iron (ORa = 2.66 [1.77 - 4.00]), the household wealth index (ORa = 1.83 [1.04 to 3.19]), number of ANC (ORa = 2.05 [1.56 - 2.69]), age pregnancy on the first ANC (ORa = 2 [1.47 - 2.7]) and iron prescription (ORa = 1.64 [1.25 - 2.16]). The prevalence of iron consumption during at least 90 days is low in Kolda area (51%);however, its determinants are identified;we can solve the problem by increasing communicate more about iron supplementation and antenatal consultation.

Knowledge Attitudes, Practices and Factors Associated with Blood Donation in the Fatick Health District in 2019 (Senegal)

Introduction: Timely availability of safe blood or blood products is essential for all health care facilities where transfusion occurs, but in many developing and transitional countries, there is still a considerable gap between the need for blood and the supply available. The overall objective was to study knowledge attitudes, practices, and factors influencing blood donation in the general population in Senegal. Methods: This was a cross-sectional descriptive and analytical study conducted among the Fatick health district population from April 1 to 15, 2019. Thus, the sample is based on a two-stage survey. Data collection was conducted through a questionnaire designed, pretested, and administered to 466 people by selected and trained enumerators. The data collected on the smartphone was analyzed using Epi Info 7.***.1.0 software. A descriptive and bivariate analysis was performed with a 5% risk of alpha error. Results: The average age of respondents was 35 years (±13), with 34 years (±12) for women and 38 years (±15) for men, and more than half of them between 20 and 40 years of age. A proportion of 87% of respondents lived in rural areas and had agriculture as their primary income source. More than half of the individuals (70%) were married, and 68% were educated. The level of knowledge of individuals about blood donation was low at 91.8%. On the other hand, more than half of them had good habits (53.65%). A proportion of 68.0% of individuals had an intention to donate in the future. However, only 24.68% of the population surveyed had already donated blood. Eighty percent of the donations were voluntary. However, 26.09% had repeated this practice. Spirulina Supplementation in Pregnant Women in the Dakar Region (Senegal)

The fight against iron deficiency anemia in pregnant women is a public health priority, especially in sub-Saharan Africa. Spirulina is an alga very rich in iron, used as a dietary supplement. This research proposes to study its effect on the hemogloblobinaemia of pregnant women. This was a blind randomized cohort study with a supplemented group of spirulina (1500 mg/d) and a second supplemented with iron and folic acid supplementation (IFAS). Included in this study were pregnant women attending four health centers in the Dakar region, with pregnancy at 28th amenorrhea, agreeing to participate and not having complications or twin pregnancies. Follow-up of the two groups was identical throughout the study from the 28th week of amenorrhea to the 42nd postnatal day. The study was conducted among 920 women, evenly distributed among the groups. IFAS allowed a statistically significant increase in hemoglobinemia with an average gain of 0.3 g/dl between the 28th and 32nd weeks of amenorrhea (p = 0.022) and 0.2 g/dl between the 28th week and the delivery (p = 0.043). Spirulina supplementation showed statistically higher gains than those observed with IFAS. These gains were 0.48 g/dl between the 28th and 32nd weeks of amenorrhea and 0.36 g/dl between the 28th week and the delivery, with p values less than 0.001. Spirulina, more accessible to developing countries, is a good option to strengthen the arsenal of iron deficiency anemia control, and therefore maternal mortality.
