Chronic pain is sustained by a phenomenon of hyper-activation of nociceptive neurons both at peripheral(peripheral sensitization)and central(central sensitization)*** onset and maintenance of pain,however,is to be found in the interaction among the various cell populations in the nervous tissue including neurons and glia(Nam et al.,2016).The pathogenesis of neuropathic pain is extremely complex depending on the primary cause of nerve damage,*** nerve injury is associated with a robust inflammatory response while chemotherapy-induced pain is characterized by a modest phlogistic component(di cesare mannelli et al.,2013).Despite these differences,a powerful common concept to explain the cellular mechanisms underlying pain is the activation of glia.

History and biology of mesenchymal stem cell (MSC): MSCs were firstly described around 1970s by Friedenstein and his collaborators as a subpopulation of colony forming, clonogenic, plastic adherent stromal bone marrow derived cells, with the capacity to regenerate other tissues (Murphy et al., 2013). Caplan (1991) proposed that these mesenchymal cells were “stem” because of their ability to differentiate to any lineage of mesodermal cells. The richest sources of MSCs are bone marrow and adipose tissue, but they can also be isolated from synovial fluid, periosteum, fetal organs, placenta, umbilical cord blood and amniotic fluid (Murphy et al., 2013;Sherman et al., 2015).

Abdominal pain is a common symptom associated with irritable bowel syndrome and inflammatory bowel diseases(IBDs),affecting about 20%of the global population(Grundy et al.,2019).Current pain therapies are poorly effective on visceral pain of intestinal origin and present several side effects,hence the need to identify novel molecular and cellular targets for drug development.

Peripheral neuropathies are chronic painful syndromes characterized by allodynia,hyperalgesia and altered nerve *** tissue degeneration represents the microanatomical correlate of peripheral *** to improve the therapeutic possibilities,this study investigated the hypersensitivity and the neuromorphological alterations related to the loose ligation of the sciatic nerve in *** elicited by treatment with acetyl-L-carnitine（ALCAR） in comparison to gabapentin were *** injury,reduction of myelin deposition and accumulation of inflammatory cells were detected in damaged nerve.A decrease of phosphorylated 200-k Da neurofilament（NFP） immunoreactivity and a redistribution in small clusters of myelin basic like-protein（MBP） were observed in ipsilateral *** with ALCAR（100 mg/kg intraperitoneally-i.p.） and gabapentin（70 mg/kg i.p.） administered bis in die for 14 days induced a significant pain relieving ***,but not gabapentin,significantly countered neuromorphological changes and increased axonal NFP *** findings indicate that both ALCAR and gabapentin significantly decreased the hypersensitivity related to neuropathic *** observation of the positive ALCAR effect on axonal and myelin sheath alterations in damaged nerve supports its use as neurorestorative agent against neuropathies through mechanism（s） consistent to those focused in this study.

In pat i e nt s,a s well as in animal mod e l s,hypersensitivity to external stimuli(hyperalgesia and allodynia)or spontaneous pain is often the first,and the most disabling,symptom of neuropathy(Davis et al.,2020).The increased activity of sensitive neurons drives pain development,making ion channel modulation a fundamental target for current pharmacotherapy as well as one of the most investigated by the R&D departments of pharmaceutical companies(Bennett et al.,2019).
