Application of ultrasonography-elastography score to suspect porto-sinusoidal vascular disease in patients with portal vein thrombosis

Background:Porto-sinusoidal vascular disease(PSVd)and portal vein thrombosis(PVT)are causes of portal hypertension characterized respectively by an intrahepatic and a pre-hepatic obstacle to the flow in the portal *** PVT may be a consequence of PSVd,in PVT patients at presentation,a pre-existing PSVd should be *** these patients the identification of an underlying PSVd would have relevant implication regarding follow-up and therapeutic management,but it could be *** this setting ultrasonography may be valuable in differential *** aim of the study was to use ultrasonography to identify parameters to discriminate between PSVd and“pure”PVT and then to suspect PVT secondary to a pre-existing ***:Fifty-three patients with histologically proven PSVd and forty-eight patients affected by chronic PVT were enrolled and submitted to abdominal ultrasonography with elastography by acoustic radiation force impulse(ARFI).Results:ARFI was higher and superior mesenteric vein(SMV)diameter was wider in PSVd patients than in PVT ***,a prognostic score was obtained as linear combinations of the two parameters with a good discrimination capacity between PSVd and PVT(the area under the curve=0.780;95%confidence interval:0.690-0.869).Conclusions:A score based on ARFI and SMV diameter may be useful to suspect an underlying PSVd in patients with PVT and to identify a subgroup of patients to be submitted to liver biopsy.

Epidemicity indices and reproduction numbers from infectious disease data in connected human populations

We focus on distinctive data-driven measures of the fate of ongoing *** relevance of our pursuit is suggested by recent results proving that the short-term temporal evolution of infection spread is described by an epidemicity index related to the maximum instantaneous growth rate of new infections,echoing concepts and tools developed to study the reactivity of *** epidemicity indices can showcase the dynamics of infections,together with commonly employed effective reproduction numbers,especially when the latter assume values less than *** particular,epidemicity evaluates the short-term reactivity to perturbations of a disease-free ***,we show that sufficient epidemicity thresholds to prevent transient epidemic outbreaks in a spatially connected setting can be estimated by generalizing existing analogues derived when spatial effects are *** specifically account for the discrete nature,in both space and time,of surveillance data of the type typically employed to estimate effective reproduction numbers that formed the bulk of the communication of the state of the COVId-19 pandemic and its *** analyzing the effects of spatial heterogeneity on the considered prognostic indicators,we perform a short-and long-term analysis on the COVId-19 pandemic in Italy,showing that endemic conditions were maintained throughout the duration of our simulation despite stringent control *** method provides a portfolio of prognostic indices that are essential to pinpoint the ongoing pandemic in both a qualitative and quantitative manner,as our results *** base our conclusions on extended investigations of the effects of spatial fragmentation of communities of different sizes owing to connectivity by human mobility and contact scenarios,within real geographic contexts and synthetic setups designed to test our framework.

Classical Cosmology I. Anomalous Redshift for Galaxies in NEd-d

Three mechanisms for an alternative to the doppler effect as an explanation for the redshift are reviewed. A fourth mechanism is the attenuation of the light as given by the Beer-Lambert law. The average value of the Hubble constant is therefore derived by processing the galaxies of the NEd-d catalog in which the distances are independent of the redshift. The observed anisotropy of the Hubble constant is reproduced by adopting a rim model, a chord model, and both 2d and 3d Voronoi diagrams.

Two new solutions of the homogeneous diffusion equation in 1d are derived in the presence of losses and a trigonometric profile for a profile of density. A simulation for the ankle in the energy distribution of cosmic rays (CRs) is provided in the framework of the fine tuning of the involved parameters. A theoretical image for the overall diffusion of CRs in galactic coordinates is provided.

Realizing the full potential of ultrahigh-intensity lasers for particle and radiation generation will require multi-beam arrangements due to technology ***,we investigate how to optimize their coupling with solid ***,we show that overlapping two intense lasers in a mirror-like configuration onto a solid with a large preplasma can greatly improve the generation of hot electrons at the target front and ion acceleration at the target *** underlying mechanisms are analyzed through multidimensional particle-in-cell simulations,revealing that the self-induced magnetic fields driven by the two laser beams at the target front are susceptible to reconnection,which is one possible mechanism to boost electron *** addition,the resistive magnetic field generated during the transport of the hot electrons in the target bulk tends to improve their *** simulations also indicate that such effects can be further enhanced by overlapping more than two laser beams.
