The need for a flexible,dynamic,and decentralized energy market has rapidly grown in recent years.As a matter of fact,Industry 4.0 and Smart Grids are pursuing a path of automation of operations to insure all the steps among consumers and producers are getting closer.This leads towards solutions that exploit the paradigm of public blockchain,which represents the best platform to design flat and liquid markets for which providing trust and accountability to mutual interactions becomes crucial.On the other hand,one of the risks arising in this situation is that personal information is exposed to the network,with intolerable threats to privacy.In this paper,we propose a solution for energy trading,based on the blockchain Ethereum and Smart Contracts.The solution aims to be a concrete proposal to satisfy the needs of energy trading in smart grids,including the important feature that no information about the identity of the peers of the network is disclosed in advance.

MicroRNAs(miRNAs) are small non-coding RNAs that modulate gene expression at the post-transcriptional level by affecting both the stability and translation of complementary mRNAs. Several studies have shown that mi RNAs are important regulators in the conflicting efforts between the virus(to manipulate the host for its successful propagation) and the host(to inhibit the virus),culminating in either the elimination of the virus or its persistence. An increasing number of studies report a role of miRNAs in hepatitis B virus(HBV) replication and pathogenesis. In fact,HBV is able to modulate different host miRNAs,particularly through the transcriptional transactivator HBx protein and,conversely,different cellular miRNAs can regulate HBV gene expression and replication by a direct binding to HBV transcripts or indirectly targeting host factors. The present review will discuss the role of miRNAs in the pathogenesis of HBVrelated diseases and their role as a biomarker in the management of patients with HBV-related disease and as therapeutic targets.

Helical tomotherapy for duodenal adenocarcinoma in an elderly patient: A case report

To evaluate the efficacy and feasibility of external beam radiotherapy(EBRT) for duodenal adenocarcinoma in an 84-year-old female who underwent EBRT(2.2 Gy/d for a total dose of 46.2 Gy) using helical tomotherapy(HT). Toxicity was evaluated on the National Cancer Institute's common toxicity criteria(CTCAE 3.0). The patient completed the treatment without G3-G4 toxicity. After 22-mo follow-up, she is alive and well, in complete remission with no late side effects. HT seems to be feasible and effective for duodenal adenocarcinoma in old to very old patients.

The BALIN is a modern device,consisting of a battery with integrated battery charger,dedicated to powering the 24 V services of the carriages.In various Trenitalia rolling stock,used for the regional transport of travelers,there are old and obsolete battery charger devices and there are lead acid batteries that need frequent maintenance operations to be kept efficient.These components,due to their low reliability,are the source of serious disruptions to passenger transport,and the management of their maintenance involves high costs for Trenitalia.In order to solve these problems,Trenitalia has therefore decided to invest resources to develop a new system of integrated power supplies and batteries,therefore the BALIN has been developed.The main advantages of BALIN compared to the original components are the presence of a high energy efficiency battery charger designed to operate in a wide range of temperatures and the presence of sealed lead-acid batteries without maintenance with a life exceeding 6 years.The BALIN is able to disconnect its own batteries from the carriage electrical circuits.The BALIN battery charger also implements an intelligent battery management logic and thanks to the interface with the command and control signals present on the rolling stock,the batteries are isolated from the carriage electric load automatically when the train is off and the service it's over.Another important automatic feature of the BALIN is to protect its batteries from deep discharge,in fact the power supply from the batteries to the electric carriage loads is interrupted when the voltage reaches 21 V.The characteristics mentioned go towards the reduction of the charge/discharge cycles of batteries and contribute to increasing the reliability and durability of batteries throughout the entire life of the rolling stock.

During May 8-10,2019,the International Workshop on Efficient High-Order Time Discretization Methods for Partial Differential Equations took place in Villa Orlandi,Anacapri,Italy,a Congress Center of the University of Naples Federico II.About 40 senior researchers,young scholars,and Ph.D.students attended this workshop.The purpose of this event was to explore recent trends and directions in the area of time discretization for the numerical solution of evolutionary partial differential equations with particular application to high-order methods for hyperbolic systems with source and advection-diffusion-reaction equations,and with special emphasis on efficient time-stepping methods such as implicit-explicit(IMEX),semi-implicit and strong stability preserving(SSP)time discretization.The present focused section entitled“Efficient High-Order Time Discretization Methods for Partial Differential Equations”in Communications on Applied Mathematics and Computation(CAMC)consists of five regularly reviewed manuscripts,which were selected from submissions of works presented during the workshop.We thank all the authors of these contributions,and hope that the readers are interested in the topics,techniques and methods,and results of these papers.We also want to thank the CAMC journal editorial staff as well as all the referees for their contributions during the review and publication processes of this focused section.

Esophageal complications caused by gastroesophageal reflux disease(GERD)include reflux esophagitis and Barrett’s esophagus(BE).BE is a premalignant condition with an increased risk of developing esophageal adeno-carcinoma(EAC).The carcinogenic sequence may progress through several steps,from normal esophageal mucosa through BE to EAC.A recent advent of functional esophageal testing(particularly multichannel intraluminal impedance and pH monitoring)has helped to improve our knowledge about GERD pathophysiology,including its complications.Those findings(when properly confirmed)might help to predict BE neoplastic progression.Over the last few decades,the incidence of EAC has continued to rise in Western populations.However,only a minority of BE patients develop EAC,opening the debate regarding the cost-effectiveness of current screening/surveillance strategies.Thus,major efforts in clinical and research practice are focused on new methods for optimal risk assessment that can stratify BE patients at low or high risk of developing EAC,which should improve the cost effectiveness of screening/surveillance programs and consequently significantly affect health-care costs.Furthermore,the area of BE therapeutic management is rapidly evolving.Endoscopic eradication therapies have been shown to be effective,and new therapeutic options for BE and EAC have emerged.The aim of the present review article is to highlight the status of screening/surveillance programs and the current progress of BE therapy.Moreover,we discuss the recent introduction of novel esophageal pathophysiological exams that have improved the knowledge of the mechanisms linking GERD to BE.

Gastroesophageal reflux disease(GERD) is a common disorder of the gastrointestinal tract. In the last few decades, new technologies have evolved and have been applied to the functional study of the esophagus, allowing for the improvement of our knowledge of the pathophysiology of GERD. High-resolution manometry(HRM) permits greater understanding of the function of the esophagogastric junction and the risks associated with hiatal hernia. Moreover, HRM has been found to be more reproducible and sensitive than conventional water-perfused manometry to detect the presence of transient lower esophageal sphincter relaxation. Esophageal 24-h p H-metry with or without combined impedance is usually performed in patients with negative endoscopy and reflux symptoms who have a poor response to anti-reflux medical therapy to assess esophageal acid exposure and symptom-reflux correlations. In particular, esophageal 24-h impedance and p H monitoring can detect acid and non-acid reflux events. Endo FLIP is a recent technique poorly applied in clinical practice, although it provides a large amount of information about the esophagogastric junction. In the coming years, laryngopharyngeal symptoms could be evaluated with up and coming non-invasive or minimally invasive techniques, such as pepsin detection in saliva or pharyngeal p H-metry. Future studies are required of these techniques to evaluate their diagnostic accuracy and usefulness, although the available data are promising.

Esophageal achalasia is a chronic and progressive motility disorder characterized by absence of esophageal body peristalsis associated with an impaired relaxation of lower esophageal sphincter(LES) and usually with an elevated LES pressure, leading to an altered passage of bolus through the esophago-gastric junction. A definitive cure for achalasia is currently unavailable. Palliative treatment options provide only food and liquid bolus intake and relief of symptoms. Endoscopic therapy for achalasia aims to disrupt or weaken the lower esophageal sphincter. Intra-sphincteric injection of botulinum toxin is reserved for elderly or severely ill patients. Pneumatic dilation provides superior results than botulinum toxin injection and a similar mediumterm efficacy almost comparable to that attained after surgery. Per oral endoscopic myotomy is a promising option for treating achalasia, but it requires increased experience and further objective and long-term follow up. This article will review different endoscopic treatments in achalasia, and summarize the short-term and long-term outcomes.

AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis C virus(HCV) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HCV genotype 1. The HCV RNA testing was performed using COBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. CONCLUSION: In everyday practice, triple therapy is safe but has moderate efficacy, especially for patients over 65 years of age, with advanced fibrosis, nonresponders to peginterferon + ribavirin.
