GIS tools for preliminary debris-flow assessment at regional scale

The assessment of the areas endangered by debris flows is a major issue in the context of mountain watershed management. depending on the scale of analysis, different methods are required for the assessment of the areas exposed to debris *** 2-D numerical models are advised for detailed mapping of inundation areas on individual alluvial fans, preliminary recognition of hazard areas at the regional scale can be adequately performed by less data-demanding methods, which enable priority ranking of channels and alluvial fans at risk by debris flows. This contribution focuses on a simple and fast procedure that has been implemented for regionalscale identification of debris-flow prone channels and prioritization of the related alluvial fans. The methodology is based on the analysis of morphometric parameters derived from Digital Elevation Models(deMs). Potential initiation sites of debris flows are identified as the deM cells that exceed a threshold of slope-dependent contributing area. Channel reaches corresponding to debris flows propagation, deceleration and stopping conditions are derived from thresholds of local slope. An analysis of longitudinal profiles is used for the computation of the runout distance of debris flows. Information on erosion-resistant bedrock channels and sediment availability surveyed in the field are taken into account in the applications. A set of software tools was developed and made available(https://***/Hydrogeomorphology Tools) to facilitate the application of the procedure. This approach, which has been extensively validated by means of field checks, has been extensively applied in the eastern Italian Alps. This contribution discusses potential and limitations of the method in the frame of the management of small mountain watersheds.

Irritable bowel syndrome(IBS) and functional constipation(FC) are the most common functional gastrointestinal disorders. According to the Rome ⅢCriteria these two disorders should be theoretically separated mainly by the presence of abdominal pain or discomfort relieved by defecation(typical of IBS) and they should be mutually exclusive. However,many gastroenterologists have serious doubts as regards a clear separation. Both IBS-C and FC,often associated with many other functional digestive and non digestive disorders,are responsible for a low quality of life. The impact of the media on patients' perception of these topics is sometimes disruptive,often suggesting a distorted view of pathophysiology,diagnosis and therapy. These messages frequently overlap with previous subjective opinions and are further processed on the basis of the different culture and the previous experience of the constipated patients,often producing odd,useless or even dangerous behaviors. The aim of this review was to analyze the most common patients' beliefs about IBS-C and CC,helping physicians to understand where they should focus their attention when communicating with patients,detecting false opinions and misconceptions and correcting them on the basis of scientific evidence.

Esophageal complications caused by gastroesophageal reflux disease(GERD)include reflux esophagitis and Barrett’s esophagus(BE).BE is a premalignant condition with an increased risk of developing esophageal adeno-carcinoma(EAC).The carcinogenic sequence may progress through several steps,from normal esophageal mucosa through BE to EAC.A recent advent of functional esophageal testing(particularly multichannel intraluminal impedance and pH monitoring)has helped to improve our knowledge about GERD pathophysiology,including its *** findings(when properly confirmed)might help to predict BE neoplastic *** the last few decades,the incidence of EAC has continued to rise in Western ***,only a minority of BE patients develop EAC,opening the debate regarding the cost-effectiveness of current screening/surveillance ***,major efforts in clinical and research practice are focused on new methods for optimal risk assessment that can stratify BE patients at low or high risk of developing EAC,which should improve the cost effectiveness of screening/surveillance programs and consequently significantly affect health-care ***,the area of BE therapeutic management is rapidly *** eradication therapies have been shown to be effective,and new therapeutic options for BE and EAC have *** aim of the present review article is to highlight the status of screening/surveillance programs and the current progress of BE ***,we discuss the recent introduction of novel esophageal pathophysiological exams that have improved the knowledge of the mechanisms linking GERD to BE.

Gastroesophageal reflux disease(GERD) is a common disorder of the gastrointestinal tract. In the last few decades, new technologies have evolved and have been applied to the functional study of the esophagus, allowing for the improvement of our knowledge of the pathophysiology of GERD. High-resolution manometry(HRM) permits greater understanding of the function of the esophagogastric junction and the risks associated with hiatal hernia. Moreover, HRM has been found to be more reproducible and sensitive than conventional water-perfused manometry to detect the presence of transient lower esophageal sphincter relaxation. Esophageal 24-h p H-metry with or without combined impedance is usually performed in patients with negative endoscopy and reflux symptoms who have a poor response to anti-reflux medical therapy to assess esophageal acid exposure and symptom-reflux correlations. In particular, esophageal 24-h impedance and p H monitoring can detect acid and non-acid reflux events. Endo FLIP is a recent technique poorly applied in clinical practice, although it provides a large amount of information about the esophagogastric junction. In the coming years, laryngopharyngeal symptoms could be evaluated with up and coming non-invasive or minimally invasive techniques, such as pepsin detection in saliva or pharyngeal p H-metry. Future studies are required of these techniques to evaluate their diagnostic accuracy and usefulness, although the available data are promising.
