Childhood constipation as an emerging public health problem

Functional constipation(FC) is a significant health problem in children and contrary to common belief, has serious ramifications on the lives of children and their families. It is defined by the Rome criteria which encourage the use of multiple clinical features for diagnosis. FC in children has a high prevalence(0.7%-29%) worldwide, both in developed and developing countries. Biopsychosocial risk factors such as psychological stress, poor dietary habits, obesity and child maltreatment are commonly identified predisposing factors for FC. FC poses a significant healthcare burden on the already overstretched health budgets of many countries in terms of out-patient care, in-patient care, expenditure for investigations and prescriptions. Complications are common and range from minor psychological disturbances, to lower health-related quality of life. FC in children also has a significant impact on families. Many paediatric clinical trials have poor methodological quality, and drugs proved to be useful in adults, are not effective in relieving symptoms in children. A significant proportion of inadequately treated children have similar symptoms as adults. These factors show that constipation is an increasing public health problem across the world with a significant medical, social and economic impact. This article highlights the potential public health impact of FC and the possibility of overcoming this problem by concentrating on modifiable risk factors rather than expending resources on high cost investigations and therapeutic modalities.

A Rainfall Intensity-Duration Threshold for Mass Movement in Badulla, Sri Lanka

Mass movement in Sri Lanka is mainly triggered by heavy rainfall. International literature is rich of works defining rainfall intensity-duration models to identify the rainfall threshold for various types of Mass movement. However, studies have not focused to establish a relationship between intensity and duration of rainfall in Sri Lanka. Therefore, this study focused to establish rainfall intensity-duration models to identify the rainfall threshold for mass movements in Badulla district in Sri Lanka, where forty four (44) rainfall events that resulted in same number of landslides during the last three decades were considered. Results indicate the rainfall threshold relationship fits to the log linear model of the exponential function, I = α·D-β. The constructed I-D curve revealed that short duration (54 mm/h) in rainfall events can potentially trigger the landslide. However, long-duration (>8 h) and low-intensity (<25 mm/h) in rainfall events may also trigger mass movements in Badulla. As per the results, most mass movements occur during northeast monsoons and inter-monsoons. In general, higher mean rainfall intensities trigger the debris flows, while long-duration rainfall events can trigger both landslides and debris flow. When compared to Sri Lankan mass movements triggering threshold intensities are fairly higher than the global threshold values. It confirms that within Badulla, mass movements are triggered by very high intense and/or long duration rainfalls events only. Further, time series analysis of the rainfall events shows an upward trend of extreme rainfall events, which increased landslide occurring frequency in last six (6) years.

Portal vein thrombosis in cirrhosis: Controversies and latest developments

Portal vein thrombosis(PVT) is encountered in livercirrhosis, particularly in advanced disease. It has been a feared complication of cirrhosis, attributed to significant worsening of liver disease, poorer clinical outcomes and potential inoperability at liver transplantation; also catastrophic events such as acute intestinal ischaemia. Optimal management of PVT has not yet been addressed in any consensus *** review current literature on PVT in cirrhosis; its prevalence, pathophysiology, diagnosis, impact on the natural history of cirrhosis and liver transplantation,and management. Studies were identified by a search strategy using MEDLINE and Google Scholar. The incidence of PVT increases with increasing severity of liver disease: less than 1% in well-compensated cirrhosis, 7.4%-16% in advanced cirrhosis. Prevalence in patients undergoing liver transplantation is 5%-16%.PVT frequently regresses instead of uniform thrombus progression. PVT is not associated with increased risk of mortality. Optimal management has not been addressed in any consensus publication. We propose areas for future research to address unresolved clinical questions.

Facial Index Based 2D Facial Composite Process for Forensic Investigation in Sri Lanka

The “facial composite” is one of the major fields in the forensic science that helps the criminal investigators to carry out their investigation process. The survey conducted by United States Law Enforcement Agencies confirms that 80% of the law enforcement agencies use computer automated composite systems whereas Sri Lanka is still far behind in the process of facial composite with lot of inefficiencies in the current manual process. Hence this research introduces a novel approach for the manual facial composite process, while eliminating the inefficiencies of the manual procedure in Sri Lanka. In order to overcome this situation, this study introduces an automated image processing based software solution with 2D facial feature templates targeting the Sri Lankan population. Thus, this was the first ever approach that creates the 2D facial feature templates by incorporating both medically defined indexes and relevant aesthetic aspects. Hence, this research study is comprised of two separate analyses on anthropometric indices and facial feature shapes which were carried out targeting the local population. Subsequently, several evaluation techniques were utilized to evaluate this methodology where we obtained an overall success rate as 70.19%. The ultimate goal of this research study is to provide a system to the law enforcement agencies in order to carry out an efficient and effective facial composite process which can lead to increase the success rate of suspect identification.

益肾蠲痹丸对类风湿性关节炎的药效学研究(英文)

目的:使用弗氏完全佐剂制作大鼠关节炎模型,评价益肾蠲痹丸对类风湿性关节炎的药效。方法:使用弗氏完全佐剂造模后,测量益肾蠲痹丸对对侧足爪肿胀和脏器指数的影响;ELISA法检测其对血清肿瘤坏死因子α(TNF-α),白介素1β(IL-1β),白介素-6(IL-6)的影响;分光光度法测量总一氧化氮合酶(tNOS)和诱导型一氧化氮合酶(iNOS)活性;并用Western blot法检测益肾蠲痹丸对凋亡蛋白Bcl-2表达的影响。结果:益肾蠲痹丸可有效抑制大鼠佐剂性关节炎所引起的足跖肿胀,且对胸腺脾脏指数无明显影响。益肾蠲痹丸可有效抑制血清中TNF-α,IL-1β,IL-6含量;抑制血清中iNOS活性;下调Bcl-2在滑膜组织中的表达。结论:益肾蠲痹丸的抗类风湿关节炎的作用与其调节主要炎症因子,下调炎症部位Bcl-2蛋白表达有关。

Demarcation of family,genus and species boundaries in the Diaporthales has been tentative due to uninformative illus-trations and descriptions,overlapping morphological characteristics,misplacement or poor condition of type specimens and shortage of molecular data from ex-type *** this study,we obtained the type specimens or other authentic specimens of diaporthalean taxa from worldwide *** examined,described and illustrated *** study is based on morphological characters from type or authentic specimens,details from protologue and original illustrations and molecular data obtained from *** analyses of nrITS,nrLSU,RPB2 and TEF1-a sequence data were used to construct the molecular ***,we provided separate phylogenetic trees for families when necessary to show the generic distribution within these families based on suitable gene *** on morphology and phylogeny,we treat 17 genera previously assigned to Diaporthales genera incertae sedis within several *** some genera we have designated new generic types as they are lacking type species or type species have affiliations with other *** exclude Anisomycopsis from Diaporthales and place it in Xylariomycetidae genera incertae ***,which was previously placed in Tirisporellales is placed in Diaporthales based on phylogeny and morphology.A new combi-nation,Dendrostoma leiphaemia propose for Amphiporthe leiphaemia(Fr.)*** on the morphological characters and molecular data we accept 27 families and 138 genera within Diaporthales,24 genera in Diaporthales genera incertae sedis and one genus in Xylariomycetidae genera incertae *** provide notes for genera in Diaporthales genera incertae sedis,and excluded and doubtful genera are listed with notes on their taxonomy and phylogeny.

Fungi have been often neglected,despite the fact that they provided penicillin,lovastatin and many other important *** are an understudied,but essential,fascinating and biotechnologically useful group of *** study of fungi in northern Thailand has been carried out by us since *** studies have been diverse,ranging from ecological aspects,phylogenetics with the incorportation of molecular dating,taxonomy(including morphology and chemotaxonomy)among a myriad of microfungi,to growing novel mushrooms,and DNA-based identification of plant *** this paper,advances in understanding the biodiversity of fungi in the region are discussed and compared with those further *** new species have been inventoried for the region,but many unknown species remain to be described and/or *** example,in the edible genus Agaricus,over 35 new species have been introduced from northern Thailand,and numerous other taxa await *** this relatively well known genus,93%of species novelty is *** the microfungi,which are relatively poorly studied,the percentage of novel species is,surprisingly,generally not as high(55–96%).As well as Thai fungi,fungi on several hosts from Europe have been also *** with the well studied European microfungi an astounding percentage of new taxa(32–76%)have been *** work is just a beginning and it will be a daunting task to document this astonishingly high apparent novelty among fungi.

Late post liver transplant protein losing enteropathy: Rare complication of incisional hernia

Development of oedema and hypoproteinaemia in a liver transplant recipient may be the first signs of graft dysfunction and should prompt a full assessment. We report the novel case of a patient who, years after liver transplantation developed a functional blind loop in an incisional hernia, which manifested as oedema and hypoproteinaemia secondary to protein losing enteropathy. After numerous investigations, the diagnosis was made by flurodeoxyglucose positron emmision tomography (FDG-PET) imaging. Surgical repair of the incisional hernia was followed several months later by resolution of the protein loss, and confirmed at a post operative FDG-PET scan at one year.
