1 病史摘要患者,女性,54岁,中国籍,因急性左下肢水肿就诊.既往有子宫肌瘤史2年.腹部及盆腔CT显示左髂总静脉血栓及子宫左下部肌瘤(直径6 cm).考虑静脉血栓系肌瘤压迫髂静脉所致,予抗凝治疗,为缩小子宫体积以减轻其对髂静脉的压力,皮下注射促性腺激素释放激素类似物(GnRHa)3个月.患者对GnRHa治疗无不良反应,治疗后闭经.4月后复查腹部及盆腔CT显示子宫明显缩小,但静脉血栓已扩展至下腔静脉,达左肾静脉水平.尽管已用抗凝剂并减轻了外来压力,但静脉血栓却迅速发展,结合患者有子宫肌瘤史,拟诊为静脉内平滑肌瘤病(intravenous leiomyomatosis,IVL),遂行全子宫及双附件切除术,并经腔静脉切开切除静脉内肿瘤.

BACKGROUND: Hepatectomy is the main curative treatment for hepatocellular carcinoma (HCC), but postoperative long- term survival is poor. Preoperative radiological features of HCC displayed by computed tomography or magnetic resonance imaging could serve as additional prognostic factors. This study aimed to identify preoperative radiological features of HCC that may be of prognostic significance in hepatectomy. METHODS: Ninety-two patients who underwent hepatectomy for HCC were included in this study. Preoperative radiological features including tumor number, size, location (peripheral, middle, central), portal vein invasion, hepatic vein invasion, and presence of pseudo-capsule were analyzed in relation to survival. RESULTS: With a median follow-up period of 41.7 months, the 1-, 3- and 5-year overall survival rates were 85%, 65% and 58%, respectively. Univariate analysis showed that portal vein invasion and absence of pseudo-capsule were significant prognostic factors for overall survival, while all the examined radiological features were prognostic factors for disease-free survival. Multivariate analysis for overall survival found no significant factor. On multivariate analysis for disease-free survival, patients who had tumors with portal vein invasion had poorer survival with a hazard ratio of 2.26 (95% CI, 1.05-4.91; P=0.038) and patients with single nodular HCC or pseudo-capsulated HCC had better survival with a hazard ratio of 0.50 (95% CI, 0.27-0.94; P=0.032) and 0.38 (95% CI, 0.14-0.99; P=0.048), respectively. CONCLUSIONS: Demonstrable pseudo-capsule of HCC and solitary HCC on imaging and absence of portal vein invasionare features associated with better disease-free survival after hepatectomy. These features may guide treatment planning for HCC.

In order to reduce the cost of the austenitic stainless steels(ASSs),the expensive austenite former(nickel) is often substituted by ***,manganese is generally seen to have a detrimental effect on the corrosion *** the present study,the feasibility of laser surface modification of a lean-alloyed ASS(FeCrMn) for enhancing pitting corrosion resistance was *** surface modification of FeCrMn was successfully achieved by a 2.3 kw high power diode laser(HPDL).Cyclic polarization tests for FeCrMn after laser surface modification in 3.5% NaCl solution at 25 ℃ were performed by using a *** pitting resistance of the laser-modified specimens was found to be significantly improved as reflected by the noble shift in pitting *** could be attributed to redistribution of manganese sulphide leading to a more homogenous and refined *** corrosion resistance of the laser-treated FeCrMn followed by subsequent citric acid passivation was found to be further improved as reflected by the noble shift in pitting potential to 0.18 V.

Preliminary Experimental Analysis of a CHP Hydromethane System

The aim of this paper is to present the preliminary experimental analysis results carried out on the commercial internal combustion engine set in a CHP （combined heat and power） mode, fueled by renewable hydrogen and methane mixtures. The hydrogen is produced by an alkaline electrolyser fed by a 5.8 kwp grid connected PV （photovoltaic） plant. The acceptance test conducted with hydrogen percentages ranging from 0%-10% has been carried out at partial load： 45 kw^l instead of the full power of 60 kwe~. In order to evaluate the CHP energy consumption and environmental performance （NOx and CO）, the analysis was conducted for 240 h, using a portable flue gas analyser and two mass flow meters for hydrogen and methane. Without engine parameters optimization--relative equivalence ratio （2） and spark advance--increasing hydrogen addition rate, a slight enhancement in electrical efficiency occurs. Furthermore, due to the engine control system and lower blends LHV （lower heating value）, the methane consumption decreases disproportionately to the hydrogen amount in the mixture. Finally, referring to standard operating condition, the environmental results show that using enrichment of 10%, running the engine with 18 degrees spark advance and 2 of 1.4, CO and NOx emissions are reduced by 6.3% and 27% respectively.

Engineering Design Study on an Innovative Hydrokinetic Turbine with on Shore Foundation

Tidal currents are a resource of great potentiality and not yet fully explored. Several efforts have been made to exploit these resources, but the costs associated to the deployment of tidal plants in marine environments are usually too high. The aim of this work is to present a system able to handle with the above mentioned problems, through the development of a particular hydrokinetic turbine design. In previous works, the authors described a basic turbine configuration achieving interesting performances, although some operational inconveniences were detected. Those problems have been existing so the solution should be optimized and redesigned. As a result, a new design of the turbine is proposed, consisting of a double rotor spinning in opposite directions in order to balance the induced mechanical torque. From preliminary evaluations related to the Messina Strait tidal cycles （Punta Pezzo site, RC, Italy）, a single 12 m diameter turbine can supply a power of about 500 kw with a peak current speed of 3 m/s and deliver up to 450 MWh/year.
