Non invasive evaluation of liver fibrosis in paediatric patients with nonalcoholic steatohepatitis

AIM: To identify the independent predictors of hepatic fibrosis in 69 children with nonalcoholic steatohepatitis (NAsH) due to nonalcoholic fatty liver disease (NAFLD). METHODs: All patients with clinically suspected NAsH underwent liver biopsy as a confirmatory test. The following clinical and biochemical variables at baseline were examined as likely predictors of fibrosis at histology: age, body mass index (BMI), systolic blood pressure (sBP), dyastolic blood pressure (DBP), fasting glucose, fasting insulin, homeostatic model assessment for insulin resistence (HOMA-IR), cholesterol, tryglicerides, alanine aminotransferase (ALT), aspartate aminotransferase (AsT), AsT/ALT ratio, gamma glutamil transferase (GT), platelet count, prothrombin time (PT). REsULTs: At histology 28 (40.6%) patients had no fibrosis and 41 (59.4%) had mild to bridging fibrosis. At multivariate analysis, BMI > 26.3 was the only independent predictor of fibrosis (OR = 5.85, 95% CI = 1.6-21). CONCLUsION: BMI helps identify children with NAsH who might have fibrotic deposition in the liver.

Lung cancer is the leading cause of cancer mortality worldwide. Its high mortality is due to the poor prognosis of the disease caused by a late disease presentation, tumor heterogeneities within histological subtypes, and the relatively limited understanding of tumor biology. Importantly, lung cancer histological subgroups respond differently to some chemotherapeutic substances and side effects of some therapies appear to vary between subgroups. Biomarkers able to stratify for the subtype of lung cancer, prognosticate the course of disease, or predict the response to treatment are in high demand. In the last decade, microR NAs(miR NAs), measured in resected tumor samples or in fine needle aspirate samples have emerged as biomarkers for tumor diagnosis, prognosis and prediction of response to treatment, due to the ease of their detection and in their extreme specificity. Moreover, miR NAs present in sputum, in plasma, in serum or in whole blood have increasingly been explored in the last five years as less invasive biomarkers for the early detection of cancers. In this review we cover the increasing amounts of datathat have accumulated in the last ten years on the use of miR NAs as lung cancer biomarkers.

Metabolic Requirements during six Minutes Walking Tests in Patients Affected by Chronic Obstructive Pulmonary Disease in Different stages

Backgrounds: In Chronic Obstructive Pulmonary Disease (COPD) a multi factorial effort limitation becomes pro- gressively relevant as the disease progresses in the consecutive stages. It is measured by both six minutes walking test (6MWT) and maximal cardiopulmonary incremental test (CPET). Aim: It is important to assess in each stage of dis-ease the metabolic load during 6MWT referring to the outcome of CPET and to ascertain whether there is a significant rela- tionship between the measures obtained by CPET and 6MWT. Methods: Four group of fifteen patients affected by COPD in stage I to IV underwent 6MWT and maximal CPET in the same day and results were compared to a group of healthy people. Airflow obstruction was measured by whole body plethysmography, blood gases by gas analysis, maxi- mal oxygen consumption and metabolic parameters by ergometer, lactic acid levels by analyzer. Results: Maximal oxygen consumption (V’O2max) and 6MWT are progressively impaired and related (V’O2max = 1.25 ? 0.26, 1.152 ? 0.4, 1.03 ? 0.44, .85 ? 0.2 l/m;6MWD = 452 ? 84, 446 ? 82, 381 ? 165, 200 ? 100 respectively in GOLD I to IV stage). Oxygen consumption (V’O2) during 6MWT becomes stable after 3 - 4 minutes and reached at the end of the test close to those measured at de-compensated metabolic acidosis anaerobic threshold (TDMA) (85 ± 0.4 l/m vs. 9 ? 0.4 l/m) in stage I to III, while in COPD there is no difference between V’O2max and V’O2 during 6MWT (0.85 ? 0.2 vs. 0.8 ? 0.23 l/m). 6MWT is more suitable to determine oxygen desaturation than CPET (dsaO2 ?4 ± 2% vs. ?2 ± 1%). 6MWD, the workload performed in 6MWT and V’O2max are significantly related. Conclusion: 6MWT looks as a suitable sub maximal test related CPET. Metabolic requirements under 6MWT are close to TDMA and are obtained in a suitable, self paced, usual exercise, close to everyday experience and thus related to activity daily levels. As the disease worsens the differences between V’O2 during 6MWT and V’O2max wane.

Cancer stem cells in colorectal cancer from pathogenesis to therapy: Controversies and perspectives

Colorectal cancer remains one of the most common and lethal malignancies worldwide despite the use of various therapeutic strategies. A better understanding of the mechanisms responsible for tumor initiation and progression is essential for the development of novel, more powerful therapies. The traditional, so-called “stochastic model” of tumor development, which assumes that each cancer cell is tumorigenic, has been deeply challenged during the past decade by the identification of cancer stem cells (CsCs), a biologically distinct subset of cells within the bulk of tumor mass. This discovery led to the development of the hierarchical model of tumorigenesis which assumes that only CsCs have the ability to initiate tumor growth, both at primary and metastatic sites. This model implies that the elimination of all CsCs is fundamental to eradicate tumors and that failure to do so might be responsible for the occurrence of relapses and/or metastases frequently observed in the clinical management of colorectal cancer patients. Identification and isolation of CsCs is essential for a better understanding of their role in the tumorigenetic process and for the development of CsC-specific therapies. several methods have been used for this purpose and many efforts have been focused on the identification of specific CsC-surface markers. This review provides an overview of the proposed roles of CsC in human colorectal tumorigenesis focusing on the most important molecules identified as CsC-specific markers in colorectal cancer and on the potential strategies for the development of CsC-targeted therapy.

Peliosis hepatis(PH) is a disease characterized by multiple and small,blood-filled cysts within the parenchymatous organs. PH is a very rare disease,more common in adults,and when it affects the liver,it comes to the surgeon's attention only in an extremely urgent situation after the lesion's rupture with the resulting hemoperitoneum. This report describes the case of a 29-year-old woman affected by recurring abdominal pain. Computed tomography scans showed a hepatic lesion formed by multiple hypodense areas,which showed an early acquisition of the contrast during the arterial phase. Furthermore,it remained isodense with the remaining parenchyma during the late venous phase. We decided on performing a liver resection of segment Ⅶ while avoiding a biopsy for safety reasons. The histopathologic examination confirmed the diagnosis of focal PH. PH should always be considered in the differential diagnosis of hepatic lesions. Clinicians should discuss the possible causes and issues related to the differential diagnosis in addition to the appropriate therapeutic approach. The fortuitous finding of a lesion,potentially compatible with PH,requires elective surgery with diagnostic and therapeutic intents. The main aim is to prevent the risk of a sudden bleeding that,in absence of properly equipped structures,may have a fatal outcome.

AIM:To evaluate long-term outcomes in a large series of patients who randomly received laparoscopic or open colorectal ***:From February 2000 to December 2004,six hundred sixty-two patients with colorectal disease were randomly assigned to laparoscopic(LPs,n = 330) or open(n = 332) colorectal *** patients were analyzed on an intention-to-treat ***-term follow-up was carried out every 6 mo by office *** 526 cancer patients five-year overall and disease-free survival were *** oncologic follow-up was 96 ***:Eight(4.2%) LPs group patients needed conversion to open *** long-term morbidity rate was 7.6%(25/330) in the LPs vs 11.1%(37/332) in the open group(P = 0.17).In cancer patients,fiveyear overall survival was 68.6% in the LPs group and 64.0% in the Open group(P = 0.27).Excluding stage Ⅳ patients,five-year local and distant recurrence rates were 32.5% in the LPs group and 36.8% in the Open group(P = 0.36).Further,no difference in recurrence rate was found when patients were stratified according to cancer ***:LPs colorectal resection was associated with a slightly lower incidence of long-term complications than open *** difference between groups was found in overall and disease-free survival rates.

In this paper, the problem of maintenance task scheduling is tackled with a twofold objective: meeting the performance criteria of a company and taking into account some operators’ requirements. The production manager makes sure that makespan is optimised while developing operators’ flexibility. The use of skill matrixes enables him to make pairs and to develop training in order to make trainees more autonomous. Operators’ requirements are in particular related to periods of unavailability and their wishes relating to their tasks. Given the complexity of the problem, an exact solution isn’t conceivable and our research focuses on a metaheuritic method giving us a solution that is considered satisfactory. A multi-criteria analysis of the results is performed in order to reach a compromise among conflicting goals.
