The aim of this work was to study effects of ketotifen fumarate(KF)on prevention of tissue damage in testes of rats with experimental autoimmune orchitis(EAO)and on the contralateral testis in a model of prolonged testicular cord torsion(TCT).Rats with EAO or TCT were injected intraperitoneally once daily with KF or saline solution(vehicle group).Incidence and severity of testicular damage were evaluated by histopathology using an EAO score or a Johnsen *** cells(MC)were identified by histochemistry and *** EAO model,KF significantly reduced severity of histopathological testicular damage compared to rats in the vehicle *** also reduced the number of testicular MC compared to vehicle ***,in TCT model,multifocal damage of the contralateral testis was observed 30 days after testicular torsion characterized by sloughing of the germinal epithelium,seminiferous tubule atrophy,and interstitial *** signs of inflammation and fibrosis of seminiferous tubular walls were also *** contrast,sections of contralateral testis of rats injected with KF and killed 30 days after surgery showed normal histological features.A significant decrease in the number of MC was observed in rats treated with KF compared to untreated *** conclusion,we demonstrated that treatment with KF reduced testicular inflammatory process and MC infiltrates in both EAO and TCT *** results suggest a promising treatment for infertile male patients with testicular pathologies associated with inflammation and germ cell loss.

Association between vitamin D and hepatitis C virus infection: A meta-analysis

Multi-resistant bacteria in spontaneous bacterial peritonitis: A new step in management?

Spontaneous bacterial peritonitis(SBP) is the most typical infection observed in cirrhosis patients. SBP is responsible for an in-hospital mortality rate of approximately 32%. Recently, pattern changes in the bacterial flora of cirrhosis patients have been observed, and an increase in the prevalence of infections caused by multi-resistant bacteria has been noted. The wide-scale use of quinolones in the prophylaxis of SBP has promoted flora modifications and resulted in the development of bacterial resistance. The efficacy of traditionally recommended therapy has been low in nosocomial infections(up to 40%), and multi-resistance has been observed in up to 22% of isolated germs in nosocomial SBP. For this reason, the use of a broad empirical spectrum antibiotic has been suggested in these situations. The distinction between community-acquired infectious episodes, healthcare-associated infections, or nosocomial infections, and the identification of risk factors for multi-resistant germs can aid in the decision-making process regarding the empirical choice of antibiotic therapy. Broad-spectrum antimicrobial agents, such as carbapenems with or without glycopeptides or piperacillin-tazobactam, should be considered for the initial treatment not only of nosocomial infections but also of healthcare-associated infections when the risk factors or severity signs for multi-resistant bacteria are apparent. The use of cephalosporins should be restricted to community-acquired infections.

Expression of anti-Mullerian hormone receptor on the appendix testis in connection with urological disorders

The female internal sex organs develop from the paramesonephric （Mullerian） duct. In male embryos, the regression of the Mullerian duct is caused by the anti-Mullerian hormone （AMH）, which plays an important role in the process of testicular descent. The physiological remnant of the Mullerian duct in males is the appendix testis （AT）. In our previous study, we presented evidence for the decreased incidence of AT in cryptorchidism with intraoperative surgery. In this report, the expression of the anti-Mullerian hormone receptor type 2 （AMHR2）, the specific receptor of AMH, on the AT was investigated in connection with different urological disorders, such as hernia inguinalis, torsion of AT, cysta epididymis, varicocele, hydrocele testis and various forms of undescended testis. The correlation between the age of the patients and the expression of the AMHR2 was also examined. Reverse transcriptase-polymerase chain reaction （RT-PCR） and immunohistochemistry were used to detect the receptor＇s mRNA and protein levels, respectively. We demonstrate that AMHR2 is expressed in the ATs. Additionally, the presence of this receptor was proven at the mRNA and protein levels. The expression pattern of the receptor correlated with neither the examined urological disorders nor the age of the patients; therefore, the function of the AT remains obscure.

Integrated Scenario Planning and Multi-Criteria Decision Analysis Framework with Application to Forest Planning

This paper explores approaches concerning complex forest planning challenges, such as restoration after large-scale disturbances and under climate change. It introduces a new framework that integrates qualitative scenario planning with quantitative multi-criteria decision analysis. This framework allows stakeholders without background in forestry to express their preferences as a set of scenarios that are further assessed for specific forest management goals and activities using multi-criteria models. The assessment of the modelled scenarios created a common understanding for the stakeholders and experts to compare trade-offs between several management options and needed policy choices. The framework was applied in the case study of forest restoration following insect disturbance in British Columbia, Canada. The framework enabled structured stakeholder groups’ interactions such as industry, business associations, local and regional governments, and non-governmental organizations to identify potential restoration options. Different community futures were envisioned by two scenarios: one resembling current conditions and standard practices, while another promoting diversification of the forestry sector. The results indicated that each of the scenarios leads to different consequences for the community measured by levels of economic benefits, total harvest volumes and harvest flows over time. The results also show that the developed framework linking scenarios and multi-criteria decision analyses proved crucial to broaden the discussion on relevant species mixes and management practices, and their implications for the community and policy development.

New targeted therapies in pancreatic cancer

Patients with pancreatic cancer have a poor prognosis with a median survival of 4-6 mo and a 5-year survival of less than 5%. Despite therapy with gemcitabine, patient survival does not exceed 6 mo, likely due to natural resistance to gemcitabine. Therefore, it is hoped that more favorable results can be obtained by using guided immunotherapy against molecular targets. This review summarizes the new leading targeted therapies in pancreatic cancers, focusing on passive and specific immunotherapies. Passive immunotherapy may have a role for treatment in combination with radiochemotherapy, which otherwise destroys the immune system along with tumor cells. It includes mainly therapies targeting against kinases, including epidermal growth factor receptor, Ras/Raf/mitogenactivated protein kinase cascade, human epidermal growth factor receptor 2, insulin growth factor-1 receptor, phosphoinositide 3-kinase/Akt/m TOR and hepatocyte growth factor receptor. Therapies against DNA repair genes, histone deacetylases, micro RNA, and pancreatic tumor tissue stromal elements(stromal extracellular matric and stromal pathways) are also discussed. Specific immunotherapies, such as vaccines(whole cell recombinant, peptide, and dendritic cell vaccines), adoptive cell therapy and immunotherapy targeting tumor stem cells, have the role of activating antitumor immune responses. In the future, treatments will likely include personalized medicine, tailored for numerous molecular therapeutic targets of multiple pathogenetic pathways.
