Combined splenocaval or mesocaval C shunt and portoazygous devascularization in the treatment of portal hypertension: analysis of 150 cases

BACKGROUND: Portal hypertension is a common disease and its major surgical therapeutic approaches include devascularization and shunting. This study was undertaken to investigate the effects of combined splenocaval or mesocaval C shunt and portoazygous devascularization (combined procedures) on portal hypertension. METHODS: The clinical data of 150 patients with portal hypertension who had undergone combined procedures at the First Affiliated Hospital of Zhengzhou University from May1990 to May 2003 were analyzed retrospectively. RESULTS: The mean free portal pressure (FPP) was 25.6±1.83 mmHg, 18.0±2.07 mmHg and 18.4±2.19 mmHg before operation, after splenectomy plus splenocaval or mesocaval C shunt, and combined procedures, respectively. There was no operative death in all patients. The 1-7 year follow-up of 100 patients showed rebleeding in 3 patients, encephalopathy in 4, thrombosis of artificial vascular graft in 3, and dying from liver failure in 2. CONCLUSIONS: The combined procedures can not only decrease portal pressure but also preserve hepatic blood flow to some extent. It may be one of the best choices for treating portal hypertension in China.

19世纪欧美学界的原始法学思潮,对法律人类学学科的诞生有着直接影响。麦克伦南、摩尔根、梅因、巴霍芬等就原始法本质与内容展开了激烈争论,形成了“符号说”与“规范说”两种学说。萨维尼与梅因对法律的单线演进过程予以了描述,初步揭示了法律的进化逻辑。在对原始法的本质、内容及进化逻辑的探究中,相继涌现出了过程分析、文本分析与比较分析等研究方法。系统研究19世纪欧美学界原始法学思潮与法律人类学学科之间的历史关联,对我们思考当下中国法律人类学“向何处去”问题具有重要启示意义。

当下中国民族习惯法的研究中存在着一些固有的局限性,如论题选择的无命题化及参与观察方法的机械适用等。以西方法律人类学与历史法学的相关知识为基础,对中国民族习惯法研究中的固有局限性予以揭示,并提出未来中国民族习惯法研究的可能进路。中国民族习惯法学研究中面临着理论与方法上的诸多挑战,虽然西方的法律人类学与历史法学知识在一定程度上为我们提供了一个成熟的经验样本,但中国民族习惯法的研究还是离不开对于中国场域实践的充分关注。

Objective: To study the methods for diagnosis andtreatment of ***: Clinical data from 105 patients with insuli-noma who had been admitted to our hospital from Ju-ly 1966 to December 1999 were retrospectively ***: Fasting blood glucose values were less than2.75 mmol/L in all the patients. Fasting serum insulinvalues in 60 patients were higher than 25 mU/L, av-erage 65 mU/L. Before operation, carcinoma was de-tected in 2 of 45 patients by ultrasound scan, and in10 of 35 by CT. Enucleation of insulinoma was per-formed in 60 patients. Operations included insulinomaresection (35 patients), distal resection of the pancreas(8), and biopsy (2).Conclusion: Whipple's triad and the index of insulinrelease>0.3 are the major variables for diagno-sis Intraoperative exploration and ultrasound scan are themethods for the localization of insulinoma Enucleation ofbenign insulinoma is preferred, but proximal or distal re-sections of the pancreas are required only for large, deep ormultiple tumors

BACKGROUND:Budd-Chiari syndrome (BCS) is a disease caused by blood flow obstruction of the main hepatic veins (MHVs) and/or the outlet of the inferior vena cava (IVC), characterized by retrohepatic portal hypertension (PHT) and/or IVC hypertension. In the past decade, over 3000 cases of BCS have been reported in China. This study was to sum up our 20-year experience in surgical treatment of BCS and to investigate its pathological classification and principles of surgery.METHODS:The data from 1360 BCS patients were analyzed retrospectively.RESULTS:Four types (6 subtypes) were classified according to IVC angiography and hepatovenography: type Ia (594 patients), type Ib (123), type II (292), type IIIa (237), type IIIb (112), and type IV (2). Surgical procedures included: improved splenopneumopexy (265 cases), finger or balloon membranotomy (407), radical resection of membrane and thrombus (275), IVC bypass (88: cavocaval transflow 71 cases, and cavoatrial transflow 17 cases), mesocaval C-shape shunt (192), splenocaval shunt (32), splenoatrial shunt (23), splenojugular shunt (57), mesoatrial shunt (8), and combined methods (6), including plenal-cavoatrial shunt (4), and mesocavoatrial shunt (2), splenorenal shunt (4), mesojugular shunt (2), and other methods (1). The perioperative death rate and the complication rate after operation was 3.09% (42/1360) and 14.8% (201/1360) respectively. 885 cases were followed up from 9 months to 15 years (average 6.8+/-1.2 years. The 791 (89.4%) of 885 patients were successfully treated, 61 patients (6.89%) had a recurrence, and 33 died.CONCLUSION:Surgical treatment of BCS is dependent on a correct diagnosis and classification of the disease.

Multivariate time series forecasting iswidely used in traffic planning,weather forecasting,and energy *** decomposition algorithms can help models better understand the underlying patterns of the original series to improve the forecasting accuracy of multivariate time ***,the decomposition kernel of previous decomposition-based models is fixed,and these models have not considered the differences in frequency fluctuations between *** problems make it difficult to analyze the intricate temporal variations of real-world time *** this paper,we propose a series decomposition-based Mamba model,DecMamba,to obtain the intricate temporal dependencies and the dependencies among different variables of multivariate time series.A variable-level adaptive kernel combination search module is designed to interact with information on different trends and periods between *** backbone structures are proposed to emphasize the differences in frequency fluctuations of seasonal and trend *** with superior performance is used instead of a Transformer in backbone structures to capture the dependencies among different variables.A new embedding block is designed to capture the temporal features better,especially for the high-frequency seasonal component whose semantic information is difficult to acquire.A gating mechanism is introduced to the decoder in the seasonal backbone to improve the prediction accuracy.A comparison with ten state-of-the-art models on seven real-world datasets demonstrates that DecMamba can better model the temporal dependencies and the dependencies among different variables,guaranteeing better prediction performance for multivariate time series.

As a laser passes through a scattering medium,the light interacts with the irregular reflections within the medium,resulting in light scattering and the formation of *** this paper,an image sensor based on the combination of a coreless optical fiber and a digital camera is proposed for liquid refractive index sensing *** coreless fiber is used as a sensing unit,and the change in the speckle pattern is measured using the digital correlation method to detect the magnitude of the liquid's refractive *** experimental results indicate that the laser image sensing technique is capable of effectively distinguishing liquid samples with refractive indices ranging from 1.332 8 to1.390 8,with a sensing sensitivity of-1.306 ***,the laser image sensing technique,with its advantages of high experimental reproducibility,simple system design,remote over-control,holds great research significance and potential application in laser communication and sensor integration.

Seawater electrolysis is a promising approach for sustainable energy without relying on precious ***,the large-scale seawater electrolysis is hindered by low catalytic efficiency and severe anode corrosion caused by the harmful *** contrast to the oxygen evolution reaction （OER）and chlorin ion oxidation reaction （ClOR）,glycerol oxidation reaction （GOR） is more thermodynamically and kinetically favorable ***,a Ru doping cobalt phosphide （Ru-CoP2） is proposed as a robust bifunctional electrocatalyst for seawater electrolysis and GOR,for the concurrent productions of hydrogen and value-added *** in situ and ex situ characterization analyses demonstrated that Ru doping featured in the dynamic reconstruction process from Ru-CoP2to Ru-CoOOH,accounting for the superior GOR *** coupling GOR with hydrogen evolution was realized by employing Ru-CoP2as both anode and cathode,requiring only a low voltage of 1.43 V at 100 mA cm-2,which was 250 m V lower than that in alkaline *** work guides the design of bifunctional electrocatalysts for energy-efficient seawater electrolysis coupled with biomass resource upcycling.

Neurotoxic astrocytes are a promising therapeutic target for the attenuation of cerebral ischemia/reperfusion ***-density lipoprotein receptor,a classic cholesterol regulatory receptor,has been found to inhibit NLR family pyrin domain containing protein 3(NLRP3)inflammasome activation in neurons following ischemic stroke and to suppress the activation of microglia and astrocytes in individuals with Alzheimer’s ***,little is known about the effects of low-density lipoprotein receptor on astrocytic activation in ischemic *** address this issue in the present study,we examined the mechanisms by which low-density lipoprotein receptor regulates astrocytic polarization in ischemic stroke ***,we examined low-density lipoprotein receptor expression in astrocytes via immunofluorescence staining and western blotting *** observed significant downregulation of low-density lipoprotein receptor following middle cerebral artery occlusion reperfusion and oxygen-glucose deprivation/***,we induced the astrocyte-specific overexpression of low-density lipoprotein receptor using astrocyte-specific adeno-associated ***-density lipoprotein receptor overexpression in astrocytes improved neurological outcomes in middle cerebral artery occlusion mice and reversed neurotoxic astrocytes to create a neuroprotective ***,we found that the overexpression of low-density lipoprotein receptor inhibited NLRP3 inflammasome activation in oxygen-glucose deprivation/reoxygenation injured astrocytes and that the addition of nigericin,an NLRP3 agonist,restored the neurotoxic astrocyte *** findings suggest that low-density lipoprotein receptor could inhibit the NLRP3-meidiated neurotoxic polarization of astrocytes and that increasing low-density lipoprotein receptor in astrocytes might represent a novel strategy for treating cerebral ischemic stroke.
