Clinical research in febrile neutropenia in cancer patients: Past achievements and perspectives for the future

Febrile neutropenia(FN) is responsible for significant morbidity and mortality. It can also be the reason for delaying or changing potentially effective treatments and generates substantial costs. It has been recognized for more than 50 years that empirical administration of broad spectrum antibiotics to patients with FN was associated with much improved outcomes; that has become a paradigm of management. Increase in the incidence of microorganisms resistant to many antibiotics represents a challenge for the empirical antimicrobial treatment and is a reason why antibiotics should not be used for the prevention of neutropenia. Prevention of neutropenia is best performed with the use of granulocyte colonystimulating factors(G-CSFs). Prophylactic administration of G-CSFs significantly reduces the risk of developing FN and consequently the complications linked to that condition; moreover, the administration of G-CSF is associated with few complications, most of which are not severe. The most common reason for not using G-CSF as a prophylaxis of FN is the relatively high cost. If FN occurs, in spite of prophylaxis, empirical therapy with broad spectrum antibiotics is mandatory. However it should be adjusted to the risk of complications as established by reliable predictive instruments such as the Multinational Association for Supportive Care in Cancer. Patients predicted at a low level of risk of serious complications, can generally be treated with orally administered antibiotics and as out-patients. Patients with a high risk of complications should be hospitalized and treated intravenously. A short period of time between the onset of FN and beginning of empirical therapy is crucial in those patients. Persisting fever in spite of antimicrobial therapy in neutropenic patients requires a special diagnostic attention, since invasive fungal infection is a possible cause for it and might require the use of empirical antifungal therapy.

<i>Azospirillum</i>Growth Promotion Is Related to Changes in Ferulate and Dehydrodiferulate Contents in Cell Wall of Inoculated Cucumber Seedlings Hypocotyls

Azospirillum brasilense is a well known plant growth-promoting rhizobacterium. Plant cell growth is constrained by the primary cell wall (CW) which contains polysaccharide-bound hydroxycinammic acids (HCAs). They derive from phenylpropanoid metabolism, which first step is the reaction catalyzed by phenylalanine ammonia-lyase (PAL;EC 4.***.1.24). Also CW peroxidases (FAPOD;EC 1.***.1.7) play a key role in the stiffening of the CW, and in the cessation of cell elongation. Knowledge of the biochemical effects the bacteria could elicit into plant CW and how these responses could change the hypocotyl physiology still remains scarce. The objective of this work was to unravel the effects of A. brasilense Sp245 inoculation on HCAs of the primary CW in apical and basal segments of cucumber seedlings hypocotyls. Azospirillum inoculation increased hypocotyls’ length. Transferulate and p-coumarate were the major HCAs. Dimmers were detected only in the basal region of 13- and 15-d-old hypocotyls. The ratio t-ferulate/dimmer (an inverse index of CW cross-linking) was five times higher in inoculated seedlings. Enzyme activities determined were not directly involved in HCAs content changes. It was previously reported that the CW was a target for A. brasilense growth promotion. This work corroborates that the phenolics, FA, DFA, and p-coumarate bound to the CW are also involved in Azospirillum plant growth promoting strategies.

Validation of Novel Model for Identification of Thermal Conditions in the Low Corona

The electron density and temperature key properties of the neutral-magnetized plasma in the solar corona, which are predicted with a novel model, provide an interesting window along the whole solar cycle. In this work, we test the quantitative validity of the model and prove that the Coronal Density and Temperature (CODET) is reliable. Furthermore, this work contrasts the CODET model results with alternative observational remote and in-situ datasets during the simplest conditions of the quiescent corona near the solar minimum. This successful outcome/validation of the CODET model allowed a good qualitative density and temperature retrieval in the solar corona covering a large portion of time interval from solar cycles 23 and 24.
