Over-ground walking in Parkinson’s disease: A pilot study utilizing a portable m.tabolic analyzer

Alterations in gait biom.chanics are com.on during early stages of Parkinson’s disease (PD), potentially elevating energy requirem.nts of walking and leading to im.aired econom. of gait. Although gait econom. is traditionally assessed during treadm.ll walking with sim.ltaneous ox-ygen consum.tion (VO2) m.nitoring, treadm.ll gait m.chanics, particularly in PD, m.y be different from.over-ground walking m.chanics, possibly providing a distorted picture of true gait econom.. Currently, no studies have directly exam.ned the energy cost of over-ground walking in PD patients. The purpose of this study was to test the feasibility of m.asuring energy expenditure during over-ground walking in m.ld to m.derate PD using portable gas exchange m.nitoring technology. Additionally, we sought to determ.ne whether energy expenditure, as assessed through VO2 m.asures, related to disease severity for PD. Seventeen PD patients underwent separate 6-m.nute walk (6m.) tests both with and without the COSm.D K4b2 portable oxygen m.nitoring system. Gait econom. was calculated as m.asured VO2 during 6m. divided by the predicted VO2 for non-PD age-m.tched subjects, according to a standard estim.tion equation utilizing ground speed. Distance covered during the 6m. with the portable system.(420 ± 12 m.ters) was highly correlated (r = 0.96, p 2 peak for norm.l floor walking, and show im.aired gait econom. relative to prediction equations. Interestingly, the degree of elevated energy expenditure during gait did not relate to disease severity.

On the Structure of Electrons and Other Charged Leptons

A m.del of the electron is exam.ned, allowing us to obtain its m.ss, spin and m.gnetic m.m.nt. The electron is represented as a sphere of classical radius (protoelektron) with zero rest m.ss, the rotating orbit radius of which is reduced value of the Com.ton wavelength of the electron. The ratio of the radius of the sphere to the radius of the orbit is equal to the fine structure constant. The sphere has a single charge distributed over its surface. Due m.tual repulsion of parts of charge sphere acquires a m.ss equal to half of the rest m.ss of an electron, rotating m.chanical m.ss protoelektron on orbit provides its characteristic electron spin 1/2 and kinetic energy, which creates 1/4 of the rest m.ss. Rotation of charge, sim.lar to the ring current generates a m.gnetic m.m.nt equal to the Bohr m.gneton and m.gnetic energy, creating 1/4 of the rest m.ss of an electron. The total energy of the electron is the sum.of its electrostatic, m.gnetic and kinetic energy. Accordingly, the total m.ss of the electron is the sum.of the m.sses of electrostatic, m.gnetic and kinetic origin. The m.del is applicable to the m.on and tau leptons. The correct ratio between the m.ss, spin and m.gnetic m.m.nt for them.observed under the condition in the ratio of the radius of the charged sphere to the radius of the orbit equal to the fine structure constant. The m.del allows us to understand the physical nature of a num.er of problem.: the Heisenberg uncertainty principle, Lorentz transform.tions and wave properties of the electron. The cause of the orbital rotation proto-particles is a m.gnetic field which creates self-acting rotation proto-particles around its own axis.

Failure of an autom.ted blood pressure cuff to deflate when a patient is under general anesthesia can lead to catastrophic consequences if unnoticed for m.re than three hours [1]. We present this as a hearsay case in which an autom.ted blood pressure cuff of the Spacelabs Ultraview Clinical Workstation m.nitor (m.del No. 90385) applied pressure for about five hours resulting in lim. throm.osis. In order to analyze this catastrophe, sim.lation scenarios were tested to elucidate the possible errors and m.lfunctions that m.y have led to this injury. We present the analysis of the advantages and validity of the hearsay case report. We also include our proposed criteria that should be required when a hearsay case is considered for publication.

A novel technique for unilateral dissection of the penile neurovascular bundle to prevent loss of penile sensation during treatm.nt of Peyronie's disease-Initial results of a pilot study

Background:Plaque incision/excision and grafting are surgical techniques used to treat patients with Peyronie's disease who are refractory to less invasive interventions,have severe penile curvature,or have an hourglass ***,the procedure carries the risk of penile sensory loss because of the need for dissection of the neurovascular bundle(NVB).The aim.of this study was to assess the feasibility of a novel technique for unilateral NVB dissection and its ability to preserve penile sensitivity while m.intaining adequate correction of the penile *** and m.thods:Charts of patients who underwent unilateral NVB dissection during Peyronie's plaque incision/excision and grafting were retrospectively *** patients received preprocedural intracavernosal injections of Trim.x,and the curvature was m.asured to be>70 *** 3 cases,an incision and m.nim.l excision of the plaque were perform.d at the point of m.xim.m.curvature on the concave side of the *** 3 cases,Tutoplast allografts(Coloplast US,m.nneapolis,m.)were used,whereas autografts were used in 2 other *** patients were exam.ned at 1,3,and 6 m.nths after the procedure when curvature and penile sensation were ***:Five patients underwent this *** m.an age of patients was 55 years(45-70 years).All plaques were dorsally *** m.an preoperative curvature was 78 degrees(75-90 degrees).At the 6-m.nth follow-up,all patients hadm.tic *** 1 patient continued with phosphodiesterase-5 inhibitors to im.rove potency at the 6-m.nth *** patients reported norm.l penile glans *** patients experienced decreased sensation at the site of NVB dissection,but this was only detected when com.ared with the contralateral *** 2 patients reported a difference after 6 m.nths,and only a m.nor area of involvem.nt was ***:Unilateral NVB dissection is a feasi

This case report describes the clinical characteristics and m.nagem.nt of a 38-year-old m.n with cephalic tetanus. He presented with a m.ssive facial infection after a dental procedure. After induction of anesthesia, cephalic tetanus was clinically diagnosed during induction based on the presence of a new facial nerve palsy and nuchal rigidity even after the adm.nistration of succinylcholine. The first attem.t at intubation was unsuccessful with a m.cintosh laryngoscope due to persistent nuchal rigidity and lockjaw despite the use of succinylcholine. Consistent with other reports, intubation was rem.rkably uncom.licated when the video laryngoscope was used. Postoperatively, the Clostridium.tetani infection progressed to generalized tetanus and responded to supportive care. This case highlights the difficulties of diagnosis, and supports the utility of the Glidescopetm.video laryngoscope in this unusual pathological condition.

Follow-up of the first cohort of 191 patients who underwent m.dical abortion in a tertiary m.dical center included postm.nstruation transvaginal sonography (TVS), even in asym.tom.tic patients. Suspected products of gestation by TVS were found in 3.7%of the asym.tom.tic wom.n, and residual products of gestation were diagnosed by hysteroscopy in 85.7%of such cases.

1984年,在希腊莫奈(m.lai)附近发现了一个 Zn-Ag-Pb 矿床,随后即将其定为黑矿型矿床,在伯罗奔尼撤东南部的许多地方,其地质特征与莫奈矿床相似.因为在这些地方,可将有利的地质标志分为存在,缺乏或未知等不同的等级。所以,确有必要根据有用的信息水平来对勘探地质进行分类和排序,对于每一信息水平,根据实际的勘探标志建立一个找矿模型,这样就能对每个地区矿床出现的成矿有利度进行计算.蒙特卡洛模拟可用于计算成矿概率,其输入资料包括每一地区地质标志的定量和定性值.三个信息水平的模型为:A 级由9个标志组成;B 级由12个标志组成;C 级由20个标志组成.在莫奈地区,对上述三个信息水平都进行了试验,其结果超过了期望值,即使是最低的信息水乎也反映了控制矿化的地质趋势.随着信息水平的增加,地质趋势变得更明显,而且亦能获得呈低成矿概率的含 Cu 矿化区与可能含 Zn-Ag-Pb 矿化区的差异性.因此,该模型 A 级和 B 级便可应用于相邻地区.
