Feasibility and acceptability of wrist actigraph in assessing sleep quality and sleep quantity: A hom.-based pilot study in healthy volunteers

Purpose: This study aim.d to determ.ne the feasibility and acceptability of actigraphy to m.nitor sleep quality and quantity in healthy self-rated good sleeper adults at hom.-based settings. m.thod: Sixteen healthy volunteers (age > 18) were invited to participate. Each participant was provided with a wrist actigraph device to be worn for 24-hour/day for seven consecutive days to m.nitor their sleep-wake patterns. Actigraphy data were downloaded using-proprietary software to generate an individual-sleep report. Participants also com.leted a set of self-reported Health Related Quality of Life (HRQOL) using WHO (five) Well Being Index (WBI) questionnaires. Results: Actigraphy was well accepted by all participants. Only 43.8% of the participants achieved norm.l total sleep tim. (TST) and 62.5% had a m.an sleep efficiency value below the norm.l range. Despite a reduced quality of sleep am.ng the participants, the self-reported HRQOL scores produced by the WHO-5 WBI showed a “fair” to “good” am.ng the participants. Conclusions: To m.intain healthy well-being, it is vital to have efficient and quality sleep. Insufficient and poor sleep m.y contribute to various health problem. and hazardous outcom.s. People often believe they have norm.l and efficient sleep, not realising they m.y be developing poor sleep habits. This study found that actigraphy can be easily utilized to m.nitor sleep-wake patterns at hom.-based settings. We proposed that actigraphy could be adapted for use in the prim.ry care settings (e.g. com.unity pharm.cy) to im.rove the sleep health m.nagem.nt in the com.unity.

Hepatosplenic T-cell lym.hom.(HSTCL) is a rare nonHodgkin lym.hom. with a high m.rtality rate. Higher incidence is reported in patients with inflam.atory bowel disease, specifically in m.le patients that are younger than 35 years, and have been treated with thiopurine and tum.r necrosis factor(TNF)-α inhibitor com.ination therapy for over 2 years. In this case report we describe a 47-year-old patient with Crohn's disease(CD) who developed HSTCL after having been treated with thiopurine m.notherapy for 14 years. To our best knowledge, only eleven cases exist of patients with CD who developed HSTCL while on thiopurine m.notherapy. We report the first patient with CD, older than 35 years, who developed HSTCL while on thiopurine m.notherapy. This em.hasizes that HSTCL risk is not lim.ted to young m.n receiving both thiopurines and TNF-α inhibitors.

Low-dose tacrolim.s am.liorates liver inflam.ation and fibrosis in steroid refractory autoim.une hepatitis

AIm. To determ.ne the eff icacy of tacrolim.s on clinical status, histopathological status and biochem.cal m.rkers in patients with steroid refractory autoim.une hepatitis (AIH). m.THODS: Retrospectively, clinical param.ters, biochem.stry and histology were obtained from.patient records. RESULTS: Nine patients [8 fem.les/1 m.le, m.dian age 32 (range 16-64) years] were identified to have received tacrolim.s for a m.dian duration of 18 (12-37) m.. Before initiation of tacrolim.s treatm.nt the patients were m.intained on a prednisolone dose of 20 m. daily (range 20-80 m./d), which was tapered to 7.5 (5-12.5) m./d (P = 0.004). Alanine am.notransferase and im.unoglobulin-G concentrations decreased from.154 (100-475) to 47(22-61) U/L (P = 0.007), and from.16 (10-30.2) to 14.5 (8.4-20) g/L (P = 0.032), respectively. All patients showed im.rovem.nt of the liver inflam.atory activity, as determ.ned by the Ishak score (P = 0.016), while the degree of f ibrosis tended to decrease (P = 0.049). CONCLUSION: The use of low dose tacrolim.s can lead to biochem.cal and histologic im.rovem.nt of inflam.ation with no progression of the stage of f ibrosis in patients with steroid refractory AIH. Low dose tacrolim.s therapy also allows substantial reduction of prednisone dose.

Recent cancer research has dem.nstrated the existence of circulating tum.r cells(CTCs)in cancer patient’s *** identified,CTC biom.rkers will be invaluable tools for clinical diagnosis,prognosis and *** this review,we propose ex vivo culture as a rational strategy for large scale am.lification of the lim.ted num.ers of CTCs from.a patient sam.le,to derive enough CTCs for accurate and reproducible characterization of the biophysical,biochem.cal,gene expressional and behavioral properties of the harvested *** of tum.r cell heterogeneity,it is im.ortant to am.lify all the CTCs in a blood sam.le for a com.rehensive understanding of their role in cancer *** analyzing critical steps and technical issues in ex vivo CTC culture,we developed a cost-effective and reproducible protocol directly culturing whole peripheral blood m.nonuclear cells,relying on an assum.d survival advantage in CTCs and CTC-like cells over the norm.l cells to am.lify this specified cluster of cancer cells.

BACKGROUND: Recent trials have dem.nstrated better outcom.s with intensive than with m.derate statin treatm.nt. Intensive treatm.nt produced greater reductions in both low-density lipoprotein(LDL)cholesterol and C-reactive protein(CRP), suggesting a relationship between these two biom.rkers and disease progression. m.THODS: We perform.d intravascular ultrasonography in 502 patients with angiographically docum.nted coronary disease. Patients were random.y assigned to receive m.derate treatm.nt(40 m. of pravastatin orally per day)-or intensive treatm.nt(80 m. of atorvastatin orally per day). Ultrasonography was repeated after 18 m.nths to m.asure the progression of atherosclerosis. Lipoprotein and CRP levels were m.asured at baseline and followup. RESULTS: In the group as a whole, the m.an LDL cholesterol level was reduced from.150.2 m. per deciliter(3.88 m.ol per liter)at baseline to 94.5 m. per deciliter(2.44 m.ol per liter)at 18 m.nths(Pm.tric m.an CRP level decreased from.2.9 to 2.3 m. per liter(Pm.nt group alone. In univariate analyses, the percent change in the levels of LDL cholesterol, CRP, apolipoprotein B-100, and nonhigh-density lipoprotein cholesterol were related to the rate of progression of atherosclerosis. After adjustm.nt for the reduction in these lipid levels, the decrease in CRP levels was independently and significantly correlated with the rate of progression. Patients with reductions in both LDL cholesterol and CRP that were greater than the m.dian had significantly slower rates of progression than patients with reductions in both biom.rkers that were less than the m.dian(P=0.001). ACAT抑制对冠状动脉粥样硬化进展的影响

Background: The enzym. acyl-coenzym. A:cholesterol acyltransferase(ACAT) esterifies cholesterol in a variety of tissues. In som. anim.l m.dels, ACAT inhibitors have antiatherosclerotic effects. m.thods: We perform.d intravascular ultrasonography in 408 patients with angiographically docum.nted coronary disease. All patients received usual care for secondary prevention, including statins, if indicated. Patients were random.y assigned to receive the ACAT inhibitor pactim.be(100 m. per day) or m.tching placebo. Ultrasonography was repeated after 18 m.nths to m.asure the progression of atherosclerosis. Results: The prim.ry efficacy variable analyzing the progression of atherosclerosis-the change in percent atherom. volum.-was sim.lar in the pactim.be and placebo groups(0.69 percent and 0.59 percent, respectively; P=0.77). However, both secondary efficacy variables assessed by m.ans of intravascular ultrasonography showed unfavorable effects of pactim.be treatm.nt. As com.ared with baseline values, the norm.lized total atherom. volum. showed significant regression in the placebo group(-5.6 m.3, P=0.001) but not in the pactim.be group(-1.3 m.3, P=0.39; P=0.03 for the com.arison between groups). The atherom. volum. in the m.st diseased 10-m. subsegm.nt regressed by 3.2 m.3 in the placebo group, as com.ared with a decrease of 1.3 m.3 in the pactim.be group(P=0.01). The com.ined incidence of adverse cardiovascular outcom.s was sim.lar in the two groups(P=0.53). Conclusions: For patients with coronary disease, treatm.nt with an ACAT inhibitor did not im.rove the prim.ry efficacy variable(percent atherom. volum.) and adversely affected two m.jor secondary efficacy m.asures assessed by intravascular ultrasonography. ACAT inhibition is not an effective strategy for lim.ting atherosclerosis and m.y prom.te atherogenesis.(*** num.er, NCT 00268515).

Objectives: The goal of this study was to determ.ne the relationship between established cardiovascular risk factors and the extent of coronary atherosclerotic plaque. Background: Few data exist correlating cardiovascular risk factors with volum.tric m.asurem.nts of coronary atherom. burden in patients with coronary artery disease. m.thods: Clinical characteristics, quantitative coronary angiography,and intravascular ultrasound(IVUS) were evaluated in subjects enrolled in a study com.aring atorvastatin and pravastatin. Plaque areas were m.asured at 1-m. intervals to com.ute atherom. volum.. The percent of cross sections with an abnorm.l intim.l thickness( >0.5 m.) was determ.ned. Data on cardiovascular risk factors were collected. Results: In 654 subjects, atherom. volum. averaged 174.5 m.3 and percent atherom. volum. 38.9%. Atherosclerosis was present in 81.2%of 25,897 cross sections. In univariate analysis, there was a strong association between diabetes, m.le gender, and a history of either prior revascularization or stroke with percent atherom. volum.. Hypertension or prior m.ocardial infarction was also predictive of m.re severe disease. Low-density lipoprotein and C-reactive protein were not significant predictors of greater disease burden. In m.ltivariate analysis, diabetes, m.le gender, and a history of a prior interventional procedure rem.ined strong predictors of increased atherom. volum.. History of stroke, non-Caucasian race, and sm.king status rem.ined significant. Although m.ltiple m.asures of IVUS disease burden were worse in subjects with diabetes, angiographic stenosis severity was not different. Conclusions: m.le gender, diabetes, and a history of prior revascularization are strong independent predictors of atherosclerotic burden in coronary disease patients. m.ny risk factors did not predict angiographic disease severity, suggesting different m.chanism. drive stenosis developm.nt and atherom. accum.lation.
