Recuperation of severe tumoral calcinosis in a dialysis patient: A case report

BACkGROUND One of the common late sequela in patients with end-stage renal disease(ESRD)is the calcium phosphate disorder leading to chronic hypercalcemia and hyperphosphatemia causing the precipitation of calcium salt in soft *** calcinosis is an extremely rare clinical manifestation of cyst-like soft tissue deposits in different periarticular regions in patients with ESRD and is characterized by extensive calcium salt containing space-consuming painful *** treatment of ESRD patients with tumoral calcinosis manifestation involves an increase in or switching of renal replacement therapy regimes and the adjustment of oral medication with the goal of improved hypercalcemia and *** SUMMARY We describe a 40-year-old woman with ESRD secondary to IgA-nephritis and severe bilateral manifestation of tumoral calcinosis associated with hypercalcemia,hyperphosphatemia and tertiary *** patient was on continuous ambulatory peritoneal dialysis and treatment with vitamin D *** switching her to a daily hemodialysis schedule and adjusting the medical treatment,the patient experienced a significant dissolution of her soft tissue calcifications within a couple of *** remission was achieved 11 mo after the initial *** Reduced patient compliance and subsequent insufficiency of dialysis regime quality contribute to the aggravation of calcium phosphate disorder in a patient with ESRD leading to the manifestation of tumoral ***,the improvement of the treatment strategy and reinforcement of patient compliance enabled complete remission of this rare disease entity.

DNA modified nanoparticles(Au NPs) are an established and widely used type of nucleotide sensor. We sought to improve the design by applying short rigid DNA duplexes near the surface of the Au NPs forming a so called double-anchored Au NP sensor, and compared it with other conventional DNA modified Au NPs. The improved design exhibited higher assembly efficiency, and consequently increased its sensitivity to target DNA.

Gabapentin(Neurontin)is an anti-epileptic drug that has had wide off-label prescription use since market release due to presumed negligible abuse ***,trends in drug misuse have demonstrated that gabapentin misuse is occurring,particularly in those with a history of opioid *** is concerning,because although gabapentin has no direct ligand activity at opioid receptors,it does potentiate the analgesic effect of opioids,and concurrent use of gabapentin and opioids may increase the risk of respiratory depressive effects of *** study investigates the incidence of gabapentin detected in urine samples collected for clinical drug screening purposes in a local hospital emergency department and in postmortem samples submitted by medical examiners in the *** metropolitan *** prevalence of gabapentin and co-detected drugs in both populations is contrasted,compared,and *** study found that 30%of urine samples collected from patients with suspected drug intoxication presenting to SSM Health Saint Louis University Hospital,a quaternary care medical center,were positive for gabapentin,and nearly two thirds of those were also positive for *** a 6-month period,the incidence of gabapentin positive postmortem cases increased from 18%to 20%.Nearly all gabapentin positive postmortem cases were also positive for an opioid,the most significant being fentanyl,suggesting that gabapentin misuse may be due to its potentiating effect of opioid drug *** study also highlights the limited utility of immunoassay-based urine drug screens.

Acoustic Myography: Its Assessment of Ground Reaction Forces as Measured through Forelimb and Hind Limb of the Horse during Mild Exercise

Background: Fractures in the limbs of racehorses are common, resulting among other factors, as the result of repeated ground reaction forces on bones and joints, leading to catastrophic failure. Aim: To quantify ground impact transmission through the limb bones of un-shod healthy horses using the non-invasive technique of acoustic myography (AMG). Methods: Four sites were selected for AMG measurements at the walk and trot, hoof wall (site 1) and sites 2 - 4, metacarpal 3, carpals and the radius of the left forelimb of two healthy horses. Measurements were on both rubber and concrete. AMG of the equine hind limb suspensory system was made and analyzed (amplitude and timing) for the proximal suspensory ligament (PSL) and the SDFT/DDFT. Results: AMG signal amplitude at site 1 (1.5 ± 0.2 versus 1.1 ± 1.5) was not found to be significantly different at the trot compared to the walk;however, sites 2, 3 and 4 were all significantly different when compared between the two gaits;site 2 P = 0.008;site 3 P = 0.006;site 4 P = 0.005. AMG signals recorded on the rubber surface had smaller amplitude than the equivalent signal and site on the concrete surface. Ground Reaction Force (GRF) transmission in the equine forelimb was 22 m/sec, whilst that of the hind limb suspensory system was 25 m/sec. Conclusion: Findings indicate that GRFs are transmitted proximally along the limb at considerable speeds, that they are dampened by tissues and structures in the limb, and that the GRFs are present and detectable proximal to the fetlock joint.
