Malfunction of the thyroid gland is the second most common endocrine disorder encountered during pregnancy. It is well known that overt disease of the thyroid gland, either hyper or hypo can adversely affect pregnancy outcome. There is also an ongoing debate surrounding the issue of subclinical hypothyroidism and its effect on the cognitive development of the unborn child. The goal of this paper is to present a systematic review of the literature and the current recommendations for diagnosis and treatment of thyroid disease in pregnancy and postpartum.

AIM： To evaluate the utility of the hemoglobin A1C （HbA1C） at the frst prenatal visit as a triaging tool in patients at high risk for gestational diabetes （GDM）.MeTHODS： The HbA1C was obtained at the frst prenatal visit prior to 20 wk. Women with a HbA1C ≥ 6.5% （group one） were instructed on diet and daily self-monitoring of blood glucose. Women with a HbA1C between 5.7%-6.4% （group two） were offered testing or daily self-monitoring of blood glucose. Women with a HbA1C 〈 5.7% （group three） were tested at 24-28 wk. Patients were tested for GDM using the two step testing and Carpenter and Coustan values as cutoffs. Medication was started if patients failed to meet glycemic goals of fasting ≤ 95 mg/dL （5.3 mmol/L） and 2 h postprandial ≤ 120 mg/dL （6.7 mmol/L）.ReSULTS： In group one （n = 16）, 15/16 （95%） re-quired medication to achieve euglycemia. The mean gestational age at which medication was required was early at 14 ± 6 wk. Postpartum, 14/16 patients （87%） remained diabetic. Group two contained 82 patients. Sixty-sixpatients （80%） were given a diagnosis of GDM and 52 patients （64%） required medication. The mean gestational age at which medication was started in group two was 20 ± 7.8 wk. There were 205 patients in group three, 18 patients （8.7%） were diagnosed with GDM and 13 patients （6%） required medication. In comparison to group three, patients in group one were 220 times more likely to require medication （95%CI： 26.9- 〉 999, P 〈 0.0001）. Patients in group two were 26 times more likely to require medication （95%CI： 12.5-54.3, P 〈 0.0001）.

Objectives:A study was undertaken to:*** contaminating bacteria on a variety of spices purchased at retail market;*** if spice bacterial enrichments alter the phenotype of 13 bacterial foodborne and clinical pathogens and 1 probiotic organism;and *** if spices can alter antimicrobial activity of seven clinical antibiotics against 16 bacterial foodborne/clinical *** and Methods:Microbiological examination was undertaken employing 27 spice varieties with four antibiotics and 15 bacterial ***:Bacteriological contamination levels varied amongst spice varieties,ranging from Kasmin chilli powder(7.5×10^(6)cfu/g;log10^(6).88 cfu/g)to ginger(1.5×10^(4)cfu/g;log_(10)4.18 cfu/g);mean contamination was 1.38×10^(6)cfu/g(log_(10)6.14 cfu/g).Four species within the genus Bacillus were identified(Bacillus megaterium,Bacillus subtilis,Bacillus licheniformis,and Bacillus cereus).There was no phenotypic difference with the 14 bacteria,with bacterial colony growth/proliferation,pigment production,or with adhesin and mucoid *** of the spice cultures inhibited any of the 14 bacterial species *** the case of doxycycline,amoxicillin,colistin,erythromycin,and piperacillin/tazobactam,the zone of inhibition increased with the inclusion of the 26 spice varieties,suggesting that the spices were interacting synergistically with the antibiotic,thus making the antibiotic more potent against the bacteria ***:This study demonstrates a positive interaction between spices and conventional *** the burden of antimicrobial resistance(AMR)worldwide,but particularly in South Asian countries(India and Pakistan),any food-related innovation that can help maximize the potency of existing antibiotics is to be encouraged and *** specific mechanism as to how spices increase the potency of antibiotics needs to be elucidated,as well as novel food(spice)delivery modalities including novel medicinal foo

119 Karst Springs, all with electric conductivity of less than 500 um/cm, discharging two carbonatic aquifers in Zagrosides of Iran are chosen and studied. The results indicate that the lithology of each Unit is well reflected in the chemistry of springs and the overall mineralogical composition of the aquifer can successfully be reconstructed from the hydrochemistry of the springs using a mass balance approach. The comparison of the chemistry of springs reveals that the significant difference are due to the dolomitic nature of one of the studied aquifers.

The Denver Basin bedrock aquifer system consists of Tertiary and Cretaceous age sedimentary rocks known as the Dawson, Denver, Arapahoe and Laramie-Fox Hills aquifers. The number of bedrock wells has increased from 12,000 in 1985 to over 33,700 in 2001 and the withdrawal of groundwater has caused water level declines in excess of 75 meters. Water level declines now range from 3 to 12 meters per year in the critical Arapahoe Aquifer. The groundwater supplies were once thought to be sufficient for 100 years but there is concern that they may be depleted in 10 to 15 years in areas on the west side of the basin. Groundwater is being mined from the aquifer system because the withdrawal through wells exceeds the rate of recharge. Increased groundwater withdrawal will cause further water level declines, increased costs to pump groundwater, and reduced yield from existing wells. In the Denver Basin, hydrologists have some capability to monitor declines in water levels for the Arapaho Aquifer, but generally have a limited ability to monitor water use. More complete and accurate water use data are needed to predict groundwater longevity for the Arapahoe Aquifer. The life of the Arapahoe Aquifer can be extended with artificial recharge using imported surface water, water reuse, restrictions on lawn watering, well permit restrictions and other conservation measures. Availability of groundwater may limit growth in the Denver Basin over the next 20 years unless residents are willing to pay for additional new sources of supply.

Investigating the impact of preoperative corneal astigmatism orientation on the postoperative spherical equivalent refraction following intraocular lens implantation

Background:To investigate the impact of the orientation of preoperative corneal astigmatism on achieving the postoperative target refraction following monofocal intraocular lens(IOL)***:This study enrolled 339 eyes who had uneventful cataract surgery or refractive lens exchange(RLe)with subsequent monofocal IOL *** were initially categorized dependent upon axial length and then on the orientation of preoperative anterior corneal *** 1 had against-the-rule(ATR)anterior corneal astigmatism,group 2 had with-the-rule(WTR)anterior corneal astigmatism,and group 3 had oblique(OB)anterior corneal *** preoperative corneal astigmatism was determined by the IOLMaster(Carl Zeiss Meditec AG).Postoperative refraction was completed for all eyes,and the results were calculated and compared for the separate ***:In eyes with axial lengths greater than 22.0 mm and less than 25.0 mm there was a significant difference between the magnitude of preoperative corneal astigmatism between groups 2 and 3 with 0.827±0.376 D in group 2,and 0.677±0.387 D in group *** mean postoperative spherical equivalent(Se)prediction error was−0.132±0.475 D in group 1,0.026±0.497 D in group 2,and−0.130±0.477 D in group *** was a significant difference between groups 1 and *** was no significant difference in the magnitude of preoperative corneal astigmatism and postoperative Se prediction error between the anterior corneal astigmatism orientation groups in eyes with axial lengths of less than or equal to 22.0 mm and greater than or equal to 25.0 ***:The orientation of preoperative anterior corneal astigmatism significantly affected the postoperative biometry prediction error in eyes with astigmatism of 1.75 D or less in eyes with the axial length between 22.0 mm and 25.0 ***,the results were not clinically significant.

Coordinated investigations into the interactions between biologically mimicking(biomimetic)material constructs and stem cells advance the potential for the regeneration and possible direct replacement of diseased cells and *** clinically relevant therapies will require the development and optimization of methods that mass produce fully functional cells and *** advances in the design and synthesis of biomaterial scaffolds,one of the biggest obstacles facing tissue engineering is understanding how specific extracellular cues produced by biomaterial scaffolds influence the proliferation and differentiation of various cell *** elasticity is one such tailorable property of synthetic scaffolds that is known to differ between ***,we investigate the interactions between an elastically tailorable polyethylene glycol(PeG)-based hydrogel platform and human bone marrow-derived mesenchymal stem cells(hMSCs).For these studies,two different hydrogel compositions with elastic moduli in the ranges of 50-60 kPa and 8-10 kPa were *** findings demonstrate that the different elasticities in this platform can produce changes in hMSC morphology and proliferation,indicating that the platform can be implemented to produce changes in hMSC behavior and cell state for a broad range of tissue engineering and regenerative ***,we show that the platform’s different elasticities influence stem cell differentiation potential,particularly when promoting stem cell differentiation toward cell types from tissues with stiffer *** findings add to the evolving and expanding library of information on stem cell-biomaterial interactions and opens the door for continued exploration into PeG-based hydrogel scaffolds for tissue engineering and regenerative medicine applications.

Consequences of bullying victimization in childhood and adolescence:A systematic review and meta-analysis

AIM To identify health and psychosocial problems associated with bullying victimization and conduct a meta-analysis summarizing the causal *** A systematic review was conducted using Pub Med, eMBASe, eRIC and Psyc INFO electronic databases up to 28 February 2015. The study included published longitudinal and cross-sectional articles that examined health and psychosocial consequences of bullying victimization. All meta-analyses were based on qualityeffects models. evidence for causality was assessed using Bradford Hill criteria and the grading system developed by the World Cancer Research *** Out of 317 articles assessed for eligibility, 165 satisfied the predetermined inclusion criteria for *** significant associations were observed between bullying victimization and a wide range of adverse health and psychosocial problems. The evidence was strongest for causal associations between bullying victimization and mental health problems such as depression, anxiety, poor general health and suicidal ideation and behaviours. Probable causal associations existed between bullying victimization and tobacco and illicit drug use. CONCLUSION Strong evidence exists for a causal relationship between bullying victimization, mental health problems and substance use. evidence also exists for associations between bullying victimization and other adverse health and psychosocial problems, however, there is insufficient evidence to conclude causality. The strong evidence that bullying victimization is causative of mental illness highlights the need for schools to implement effective interventions to address bullying behaviours.
